Background: The presence of a urinary catheter, postoperative pain, and postoperative nausea and vomiting are risk factors for emergence agitation (EA). Antimuscarinic agents are primary agents used in the prevention and treatment of urinary catheter-related bladder discomfort. Chlorpheniramine has antimuscarinic, antinociceptive, and antiemetic effects. This retrospective study investigated the role of chlorpheniramine in EA prevention following ureteroscopic stone surgery.Methods: Of 110 adult patients who underwent ureteroscopic stone surgery under general anesthesia between January and December 2019, the medical records of 93 patients were analyzed retrospectively. The patients were divided into control (n = 52) and chlorpheniramine (n = 41) groups according to the receipt of intravenous chlorpheniramine before the induction of anesthesia. The incidence and severity of EA were compared between the groups as primary and secondary endpoints, respectively. The effects of chlorpheniramine on the requirement for inhalation anesthetic (desflurane) during surgery, changes in mean blood pressure and heart rate during emergence, and adverse events were also compared. Results: The incidence (21.2% in the control group, 24.4% in the chlorpheniramine group) and severity of EA did not differ between groups. The intraoperative requirement for desflurane, changes in mean blood pressure and heart rate during emergence, and adverse events were also similar between groups. Conclusion: Chlorpheniramine was not associated with a decrease in EA incidence or severity in patients who underwent ureteroscopic stone surgery.
Engineering Heme Biosynthesis Pathway for Phycobilin Biosynthesis in Corynebacterium glutamicum

Phycobilin, a linear tetrapyrrole derived from heme, has gained attention in medical fields including research on antiviral agents, inflammatory regulators and antioxidant agents. Corynebacterium glutamicum has been studied as the main producer for heme and tetrapyrrole derivatives, and its heme biosynthesis pathway has been well engineered. First, 5-Aminolevulinic acid (ALA) has been produced by flux redistribution of the TCA cycle and introduction of heterogenous ALA transporter. Second, combinatorial expression of hemAm and hemL genes with dtxR gene has been placed to enhance porphyrin biosynthesis. Third, heme-secreting C. glutamicum strain has been developed through knock out of genes by CRISPR/Cas12a-based recombineering and membrane engineering. Then, for biliverdin production, electron reduction reaction has been placed to pull the carbon flow to biliverdin. Last, phycobilin can be efficiently produced in engineered C. glutamicum by expression of key enzymes that are involved in the conversion of biliverdin to phycobilin. Thus, enhanced phycobilin production by these metabolic strategies will provide cost-effective therapeutic potential for remediation of health problems.
자동차용 선형 솔레노이드 밸브 구동을 위한 전류제어 IC 개발

This paper presents a developed current control IC to drive solenoid valve in automotive brake system. We have developed a current control IC to drive solenoid valve linearly in automotive braking system. Our experiments show that developed IC is accurate and has the linear performance. Developed IC has 25㎃ deviation at the range of 0 < target current <250㎃, 10% deviation at the range of 250㎃ ≤ target current < 400㎃ and 6% deviation at the range of 400㎃ ≤ target current < 2.25A.

Purpose: This study aimed to determine the incidence of central line-associated bloodstream infection (CLABSI) in the neonatal intensive care unit (NICU), evaluate the patients' clinical characteristics, and identify the etiologic agents for guidance in prevention and treatment. Methods: A retrospective chart review study of infants classified as having CLABSI was conducted at the NICU of Seoul National University Bundang Hospital from January 2016 to December 2020. Results: Of the 45 infants, 53 had CLABSIs within a follow-up period of 18,622 catheter days. The incidence of CLABSIs was 2.85 per 1,000 catheter days. The most common catheter type was a peripherally inserted central catheter (n=47, 81%). A total of 57 pathogens were isolated, of which 57.9% (n=33) were Gram-positive bacteria, 36.8% (n=21) were Gram-negative bacteria, and 5.3% (n=3) were Candida spp. The most common pathogens were Staphylococcus aureus (n=12, 21%) and coagulase-negative staphylococci (n=12, 21%), followed by Klebsiella aerogenes (n=8, 14%). The median duration of bacteremia was 2 days, and 19 episodes showed bacteremia for 3 days or more. The mortality rate of infants within 14 days of CLABSI was 13.3% (n=6). Conclusions: This study analyzed the incidence of CLABSI and the distribution of pathogens in the NICU. Continuous monitoring of CLABSI based on active surveillance serves as guidance for empiric antibiotic use and also serves as a tool to assess the necessity for implementation of prevention strategies and their impact.

국내 최초로 ABS/ESC를 국산화한 만도는 2007년 1세대 주문형반도체(ASIC) 개발에 이어 2세대 ASIC 개발에 성공하였다. 자동차 산업의 국가간 경계가 허물어짐에 따라 특정업체의 공급자뿐만 아니라 국내 외 다양한 고객의 요구사항을 충족하기 위하여 Functional Safety에 대한 대비를 하는 한편, 유럽 고객의 요구에 대응하기 위하여 휠센서의 새로운 VDA프로토콜 디코딩 방식을 적용하여 ABS 및 차량자세제어장치(ESC)의 안전을 강화하고 있다. 뿐만 아니라 ESC의 정밀 제어 및 효율 향상을 고려하여 기존의 PWM 제어 방식에서 8채널 전류제어 방식의 ASIC 개발을 통하여 ABS/ESC의 성능 및 가격경쟁력 향상에 일조하고 있다. ASIC은 ABS/ESC 시스템의 기능안전을 향상시키기 위해서 개발초기부터 System Requirement 뿐만 아니라 32bit lockstep MCU와의 매칭을 고려하여 개발되었다. 온도, 클럭, 전압 모니터링의 Redundancy를 확보하여 ECU H/W의 fault가 system failure로 확장되지 않도록 다양한 safety mechansim을 도입하였다. Anistropic Magneto-Resistance (AMR) Effect를 이용한 휠센서에 적합하도록 새로운 VDA프로토콜을 채용하여 ABS/ESC의 안전 및 편의 기능을 향상시킬 수 있었다. Manchester 디코딩이 가능하여 휠속, 회선방향, 에어갭 등 휠센서의 안전정보를 시스템에 부가로 전달하여 차량자세제어장치의 안전 뿐 아니라 다양한 고객의 요구사항을 만족하도록 대비하고 있다. ASIC 내부에 ADC, Power FET, PI 제어 회로, current mirror를 집적하여 솔레노이드 밸브 전류제어를 H/W로 구현하였다. 따라서 온도와 전압의 영향을 최소화하면서도 밸브 소음에 대한 고객의 요구사항에 맞추어 PWM Frequency 조정하여 성능향상에 기여하였다.

Background: Older patients are more vulnerable to inadvertent perioperative hypothermia. Prewarming contributes to the prevention of inadvertent perioperative hypothermia in patients under general or neuraxial anesthesia. However, the effects of brachial plexus block (BPB) on thermoregulation and the efficacy of prewarming in the prevention of hypothermia in older patients undergoing surgery with BPB remain unclear. This study evaluated the effects of BPB on thermoregulation and the efficacy of prewarming during BPB in older patients. Methods: Patients aged ≥65 years were randomly allocated to receive either standard preoperative insulation (control group, n=20) or preanesthetic forced-air warming for 20 minutes before BPB (prewarming group, n=20). During the perioperative period, tympanic temperatures were measured. Thermal comfort scores and shivering grades were also obtained. Results: The tympanic temperatures at the end of surgery did not differ between the groups (36.9°C±0.5°C and 37.0°C±0.4°C in the control and prewarming groups, respectively; p=0.252). The maximum temperature change was significantly lower in the prewarming group compared to the control group (0.36°C±0.4°C and 0.65°C±0.3°C, respectively; p=0.013). The hypothermia incidence and severity, thermal comfort scores, and shivering grades did not differ between the groups. Conclusion: Regardless of the application of prewarming, BPB did not cause a clinically significant impairment of thermoregulation. Moreover, the efficacy of prewarming appeared to be low; thus, it may not be routinely required in patients undergoing orthopedic hand surgery under BPB.
Enhanced Secretion of Heme via Membrane Modification of Engineered Corynebacterium glutamicum

Heme is applied as an organic iron supplement and main ingredient in the food industry as well as pharmaceutical in medical fields. Here, we developed heme-secreting Corynebacterium glutamicum at high level by modifying cell membrane components. First, CRISPR-based recombineering was performed for the disruption of genes encoding heme binding membrane proteins. Membrane-bound heme and binding/unbinding rate were 1.20- and 1.26-fold lower in the engineered strain knocking out three heme-binding proteins (KOP strain) than the wild-type C. glutamicum, respectively. When these proteins were knocked out in the heme-overproducing strain, resulting in the KOPHM strain, heme secretion was also increased 2.22-fold. Second, reducing fatty acid content on the membrane was performed by treating the cell wall inhibitor in the cultivation. Interestingly, ETB-added cells displayed decreased binding/unbinding rate and membrane-bound heme. The KOP strain treated by ETB showed the lowest binding/unbinding rate and membrane-bound heme. Finally, The ETB-treated cultivation of the KOPHM strain showed a 1.75-fold increase in heme secretion compared to the normal cultivation condition. Thus, these biotechnological strategies enable the efficient heme secretion in C. glutamicum.
