      • Endogenous Cushing`s syndrome in a patient with systemic lupus erythematosus

        Introduction: Systemic lupus erythematosus is one of the autoimmune disease which glucocorticoids are mainstay of treatment. Cushing’s syndrome caused by glucocorticoid excess which could be exogenous or endogenous. Although iatrogenic Cushing’s syndrome is most common form especially in the patient with glucocorticoid treatment, endogenous glucocorticoid excess should be under consideration because of different treatment strategy. Case Report: We describe a 51-year old woman had had a longstanding history of SLE. She was treated with steroid and cytoxan pulse therapy and plasmapheresis. Her lupus activity had remained stable for 7 years with low-dose glucocorticoid treatment. She showed excessive weight gain, easy bruising, moon face, truncal obesity, acne, and menstrual disorder. With history of a steroid therapy, iatrogenic Cushing’s syndrome was considered by priority. However endogenous Cushing’s syndrome was suspected in respect that the feature of Cushing’s syndrome had been aggravated with the long term use of minimal dose glucocorticoid. After evaluation under the impression of Cushing’s syndrome we diagnosed her as concomitant endogenous Cushing’s syndrome due to a left adrenal adenoma.(table1)The patient underwent laparoscopic left adrenalectomy. SLE was controlled with transient apply of low-dose glucocorticoid treatment and her lupus activity remains stable without glucocorticoid treatment until now. Discussions: This is the first reported case of concomitant endogenous Cushing’s syndrome onset with preexisting SLE in Korea. This case shows the importance of differential diagnosis of exogenous Cushing’s syndrome and endogenous Cushing’s syndrome in autoimmune disease patients with glucocorticoid therapy.

        여성독거노인의 건강관련 삶의 질과 보완대체의학 이용 및 의도와의 관련성

        Purpose: This study aimed to identify association between the health-related quality of life and complementary and alternative medicine (CAM) in elderly women living alone. Method: A face-to-face survey with each elderly who lives alone in Gwangju, was proceed for two months from October to September, 2013, and 478 questionnaires were analyzed for this study. General characteristics, complementary and alternative medicine (CAM) related factors and health related quality of life (HRQoL: EQ-5D) were collected. Chi-squre tests and multiple logistic regression were tested to evaluate association between HRQoL and CAM use. We performed multiple logistic regression to identify whether factor of life quality was independently meaningful variables to CAM use after controlling the confounding variables. Result: A total of 478 subjects, and 39.7% were more than 80 years. About 51% elderly women lived alone for more than 15 years. The pain/discomfort dimension was the highest reported problem: 85.6% for elderly women. The proportion of CAM use was significantly higher among elderly with problem in mobility compared to elderly without problem (OR=2.74, 95% CI=1.45-5.18). The proportion of CAM use intention was significantly higher among elderly with problem in mobility (OR=1.97, 95% CI=1.06-3.66) and pain/discomfort (OR=2.21, 95% CI=1.07-4.55). Conclusion: The HRQoL was significanlty associated with CAM use in elderly women living alone. Further study is needed to investigate the role of CAM in elderly considering elderly's medical conditions. 본 연구는 광주지역의 65세 이상 여성독거노인에서 건강관련 삶의 질의 영역과 보완대체의학 이용과의 관련성을 알아보고자 하였다. 2013년 9월부터 10월까지 광주지역 독거노인을 대상으로 각 구당 100명씩 총 500명을 대상으로 1:1면접조사를 시행하였으며, 그 중 여성 독거노인 478명을 대상으로 하였다. 조사원은 사전 집체교육을 통해 표준화된 방식으로 조사를 진행할 수 있도록 하였으며, 설문도구를 활용하여 조사하였고, 빈도분석, 카이제곱 검정, 다중로지스틱 회귀분석을 이용하여 분석하였다. 연구결과를 살펴보면, 대상자의 39.7%가 80세 이상이었으며, 약 51%는 독거기간이 15년 이상이었다. 여성독거노인의 85.6%는 통증/불편에 지장이 있었으며, 운동능력에 지장이 있는 경우 보완대체의학 이용여부(OR=2.74, 95% CI=1.45-5.18)와 이용의도(OR=1.97, 95% CI=1.06-3.66)가 유의하게 증가하였고, 통증/불편에 지장이 있는 경우 보완대체의학 이용여부가 유의하게 증가하였다(OR=2.21, 95% CI=1.07-4.55). 결론적으로 광주지역 여성독거노인에서 건강관련 삶의 질과 보완대체의학이용여부 또는 이용의도와의 관련성을 파악할 수 있었다. 향후 의학적 처치를 고려한 보완대체의학의 적절한 사용에 대한 연구가 필요하리라 판단된다.

        Polymer Film-Based Screening and Isolation of Polylactic Acid (PLA)- Degrading Microorganisms

        Polylactic acid (PLA) has been highlighted as an alternative renewable polymer for the replacement of petroleum-based plastic materials, and is considered to be biodegradable. On the other hand, the biodegradation of PLA by terminal degraders, such as microorganisms, requires a lengthy period in the natural environment, and its mechanism is not completely understood. PLA biodegradation studies have been conducted using mainly undefined mixed cultures, but only a few bacterial strains have been isolated and examined. For further characterization of PLA biodegradation, in this study, the PLA-degrading bacteria from digester sludge were isolated and identified using a polymer film-based screening method. The enrichment of sludge on PLA granules was conducted with the serial transference of a subculture into fresh media for 40 days, and the attached biofilm was inoculated on a PLA film on an agar plate. 3D optical microscopy showed that the isolates physically degraded the PLA film due to bacterial degradation. 16S rRNA gene sequencing identified the microbial colonies to be Pseudomonas sp. MYK1 and Bacillus sp. MYK2. The two isolates exhibited significantly higher specific gas production rates from PLA biodegradation compared with that of the initial sludge inoculum

        감염을 진단하기 위한 내시경 생검의 최적 위치

        Background/Aims: Antrum has been proposed for the best biopsy site to diagnose Helicobacter pylori (H. pylori) infection. The aim of this study was to determine the best biopsy site for maximum diagnostic yield. Methods: H. pylori infection was determined by rapid urease test and histology from the biopsy specimens obtained from 992 patients (652 gastritis, 130 gastric ulcer, 148 duodenal ulcer, and 62 gastric adenocarcinoma). Three biopsy specimens were obtained from each of three biopsy sites (the antral greater curvature, mid-body greater curvature, and fundus). Results: H. pylori positivity was significantly higher in mid-body than in antrum or fundus (52.8% vs. 40.0%, 40.3%). The diagnostic yield from the mid-body alone was not significantly different from those of any combinations of other two biopsy sites. The results of rapid urease test were very similar to those of histologic diagnosis. H. pylori positivity in the patients with gastric ulcer and gastric cancer was significantly higher in mid-body than in antrum (55.4%, 45.2% vs. 26.9%, 27.4%). In duodenal ulcer patients, the positivity was significantly higher in mid-body than in antrum or in fundus (68.9% vs. 54.7%, 56.2%). Conclusions: The greater curvature of mid-body, not the antrum, is the best site to diagnose H. pylori infection irrespective of the kinds of gastroduodenal diseases.         Effects of Physically Effective Neutral Detergent Fiber Content on Intake, Digestibility, and Chewing Activity in Fattening Heifer Fed Total Mixed Ration

        The objective of this study was to determine the effects of physically effective neutral detergent fiber (peNDF) content in total mixed ration (TMR) on dry matter intake, digestibility, and chewing activity in fattening Hanwoo (Bos taurus coreanae) heifers. The experiment was designed as a replicated $3{\times}3$ Latin square using 12 heifers. Fattening heifers were offered one of three diets [high (T1), medium (T2), and low (T3) peNDF] obtained by different mixing times (3, 10, and 25 min) for the same TMR feed. The peNDF content of TMR was determined by multiplying the proportion of dry matter retained by a 1.18 mm-screen in a Penn State Particle Separator by the dietary NDF content. The $peNDF_{1.18}$ content was 30.36%, 29.20%, and 27.50% for the T1, T2, and T3 diets, respectively (p<0.05). Dry matter intake was not affected by peNDF content in TMR. Total weight gain in T1 group was significantly higher (p<0.05) than in T2 and T3 groups. However, weight gain did not differ between T2 and T3 groups. The feed conversion ratio decreased with an increase in the peNDF content (T1: 12.18, T2: 14.17, and T3: 14.01 g/g). An increase in the peNDF content of TMR was associated with a linear increase in the digestibility of dry matter, crude protein, crude fiber, neutral detergent fiber, and acid detergent fiber (p<0.05). Also, an increase in peNDF content of the TMR resulted in a linear increase in the number of chews in eating and ruminating (p<0.05), and consequently in the number of total chews (p<0.05). These results indicate that peNDF content affects digestibility and chewing activity. Consequently, the peNDF content of TMR should be considered for improving feed efficiency, digestibility, body weight gain, and performance in fattening heifers.

        번식생리 및 내분비 : 제주흑우 동결정액 제조 시 Amide 계열의 동결보호제가 동결 융해 후 정자의 성상에 미치는 영향

        본 연구는 희소 한우인 제주흑우의 정액 동결을 위하여 동결보호제로서 glycerol과 amide 계열의 동결보호제인 5%의 DMA, DMF 그리고 7%의 MF를 이용하여 동결 융해 후 정자의 운동성, 생존율, 정자막 온전성 및 정자의 첨체 양상의 변화 조사를 위하여 수행하였다. 제주흑우의 정액을 동결 한 결과 glycerol과 DMF를 사용하였을 때 운동성은 64.00±9.62와 59.00±5.48로 DMA와 MF에 비해 유의적으로 높았으며(p<0.05), 생존율 역시 glycero과 DMF 처리구에서 58.25±6.63과 53.05±3.77로 유의적으로 높게 나타났다(p<0.05). 그러나 glycerol과 DMF 처리구 간의 유의적 차이는 나타나지 않았다. 정자막 온전성 검사(HOST)에 있어서도 역시 glycerol과 DMF 처리에서 정자 미부의 팽창 비율 역시 45.12±25.08과 44.95±8.58로 유의적으로 높았다(p<0.05). 정자의 첨체막 양상 변화에 있어 F pattern의 비율이 MF 처리시 다른 처리구에 비하여 유의적으로 낮았으며(p<0.05), B pattern의 비율은 DMA, DMF 및 MF의 사용시 glycerol에 비하여 유의적으로 증가하였다(p<0.05). AR pattern의 비율에 있어서는 DMF를 제외한 amide의 사용은 F pattern의 감소와 조기 첨체반응에 의한 유의적인 AR pattern의 증가를 볼 수 있었다(p<0.0.5). 그러나 DMF의 사용은 glycerol 보다 AR pattern의 비율이 유의적으로 낮았다(p<0.05). 동결 융해 후 시간에 따른 정자의 생존율과 정자 미부 팽창 비율에 있어서 융해 초기에는 glycerol에서 DMF 보다 높은 생존율과 정자 미부 팽창율을 나타냈으나, 1시간 이후부터는 DMF에서 glycerol 보다 높은 생존율과 정자 미부 팽창율을 나타내었다. 이러한 결과는 희소 가축의 생식세포 보존 및 유전자원 확립을 위한 중요한 자료가 될 것이며, 제주흑우 및 희소 가축의 안정적이고 효율적인 동결 정액 제조 연구에 있어 중요한 정보를 제공할 것으로 평가된다. The objective of this study was to examine the effect of amides as a cryoprotectant for semen cryopreservation in Korean Jeju Black Bull. The semen was cryopreserved with extenders containing 5% dimethyl acetamide(DMA), 5% dimethyl formamide (DMF), 5% methyl formamide(MF) or 7% glycerol. Post-thawed sperm were evaluated for sperm motility, viability, acrosome integrity and membrane integrity. Post-thawed sperm motility was significantly higher(p<0.05) in glycerol and DMF(64.00%± 9.62 and 59.00%±5.48, respectively) than DMA and MF(50.00%±3.24 and 44.00%±4.18, respectively). Sperm viability wassignificantly higher(p<0.05) in glycerol and DMF(58.25%±7.35 and 53.05%±3.77, respectively) than others. However, for sperm motility and viability, there were no differences among glycerol and DMF. Also, swelling sperm ratio by hypo-osmetic selling test(HOST) was significantly increased(p<0.05) in glycerol and DMF treatments(45.12%±25.08 and 44.95%±8.58, respectively). The percentage of capacitated sperm assessed by CTC staining, F pattern was lower(p<0.05) in DMF than others. B pattern was increased(p<0.05) in DMA, DMF and MF when compared with glycerol. AR pattern ratio was decreased(p<0.05) in glycerol and DMF when compared with DMA and MF. These results suggested that amides performed better and could be used as a cryoprotectant for semen freezing of Korean Jeju Black Bull.

        남부지역 시설채소 재배 농가의 농약 사용실태

        This study was undertaken to survey the pesticides used under the greenhouse cultivation of 9 vegetable crops including cucumber, tomato, strawberry, eggplant, hot pepper, paprika, lettuce, perilla leaves and green onion in the Southern area during the period of 2003-2004. The total number of farmers investigated was 202 which consisted of 124 conventional growers and 78 environment-friendly growers. The farmers were requested to record with respect to the crop's name, cultivation acreage, growing practices (conventional/ environment-friendly), use purpose, pesticide kinds, usage quantity and application time of pesticides. The average usage quantity of pesticides for the 9 crops was 3.30 ㎏ ai/㏊ in conventional growing and 0.47 ㎏ ai/㏊ in environment-friendly growing. The average application time of pesticides was 11 times in conventional and 2 times in environment-friendly growing, with more than 90% cases as a foliar application. The number of active ingredients of pesticides was 34 in conventional and 10 in environment-friendly growing on the average per crop. The purpose of pesticide application was as a fungicide in 56% cases and as an insecticide in 43% cases.
