정상류 환기법(Constant flow ventilation, CFV)중 체위 변화와 환기량 변화가 Gas 교환에 미치는 영향

In 7 apneic dogs, pulmonary gas exchange was compared between the supine and prone position during constant flow ventilantion (CFV) with room air or 100% oxygen at the different flow rate (5, 10, 15, 20 L/min). CFV was performed through two catheter (I.D. 2.0 mm) inserted into both main stem bronchi. The body position of the dogs was changed alternately and the order of flow rate selection was randomized. After 10 min. of each run of CFV, arterial blood gases were measured and alveolar arterial oxygen partial pressure difference (AaDO₂) was calculated. AaDO₂ with room air under the prone position was significantly smaller than that under the supine position at the flow rate of 5, 15 and 20 L/min. with increasing flow rate, AaDO₂ with room air increased significantly regardless body position. No significant difference of AaDO₂ with 100% oxygen were observed between the flow rate. The present study shows that ventilation perfusion (V/Q) mismatching during CFV in apneic dogs improved by changing the supine position to the prone position and deteriorated by increasing flow rates, which fact suggests that the impaired oxygenation during CFV is associated with the changes of the status of ventilation perfusion relationship, airway geometry and relative position of intrathoracic organ.

<P><B>Abstract</B></P><P>To prepare hydroxyapatite (HA)/poly(vinyl alcohol) (PVA) composite fibers with high processibility applicable to various biomedical fields, HA/PVA composite powders were synthesized through a coprecipitation method and then mixed with pure PVA in dimethyl sulfoxide at 90°C for 2 h to prepare wet‐spinning dopes. HA/PVA solutions with HA contents of 0, 1, 3, 5, 10, and 15 wt % were analyzed with a rheometer to elucidate the effect of HA particles on the shear viscosities of the spinning dopes. The spinning dopes with pure PVA and an HA/PVA concentration of 12 wt % showed non‐Newtonian flow behaviors, and as the HA content increased from 1 to 15 wt %, the shear viscosities increased simultaneously because of the complex formation between PVA chains, citric acid, and HA particles acting as crosslinkers. The increased viscosities of the spinning dopes diminished the breakage of PVA fibers and enhanced the spinnability. Through the addition of citric acid interacting with both PVA and HA particles, the aggregation of HA particles was suppressed, and it was possible to prepare HA/PVA composite fibers with smooth surfaces. An adequate amount of HA in the PVA fibers improved the thermal and mechanical properties of the fibers because uniformly dispersed HA particles interacting with PVA chains through hydrogen bonding in the PVA matrix complemented the structural defects. © 2007 Wiley Periodicals, Inc. Platelet-activating factor receptor (PAFR) plays an important role in bacterial infection and inflammation. We examined the effect of the bacterial cell wall components lipopolysaccharide (LPS) and lipoteichoic acid (LTA) from Lactobacillus plantarum (pLTA) and Staphylococcus aureus (aLTA) on PAFR expression in THP-1, a monocyte-like cell line. LPS and aLTA, but not pLTA, significantly increased PAFR expression, whereas priming with pLTA inhibited LPSmediated or aLTA-mediated PAFR expression. Expression of Toll-like receptor (TLR) 2 and 4, and CD14 increased with LPS and aLTA treatments, but was inhibited by pLTA pretreatment. Neutralizing antibodies against TLR2, TLR4, and CD14 showed that these receptors were important in LPS-mediated or aLTA-mediated PAFR expression. PAFR expression is mainly regulated by the nuclear factor kappa B signaling pathway. Blocking PAF binding to PAFR using a PAFR inhibitor indicated that LPS-mediated or aLTA-mediated PAF expression affected TNF-α production. In the mouse small intestine, pLTA inhibited PAFR, TLR2, and TLR4 expression that was induced by heat-labile toxin. Our data suggested that pLTA has an anti-inflammatory effect by inhibiting the expression of PAFR that was induced by pathogenic ligands.
마취 회복시 R-R 간격 변화의 Power Spectrum 의 변화에 관한 연구 : Preliminary Report Preliminary report

Changes of power spectrum of R-R interval variabilty during recovery from N₂O-O₂ - halothane anesthesia have been studied in 20 patients. Power spectral analysis of R-R intervals during awake period and recovery period have been performed with newly developed R-R interval variability, we compared the power of each frequency range(low: 0.04-0.08 Hz, middle 0.10-0.15 Hz, high: above 0.3 Hz) between awake period and recovery period. There was no difference between awake period and recovery period in the power of low and high frequency ranges, but there was remarkable difference in power of midfrequency range. During awake period the power of midfrequency range changed with cyclic pattern, but there was no cyclic change during recovery period even after the regain of consciounss(30±3.2 min.). The power of midfrequency range during recovery period began to change with cyclic pattern only after 45 minutes. It is concluded that the power of midfrequency range is useful index of recovery from N₂O-O₂ -halothane aesia.
R-R 간격 변화의 On-line Power Spectral Analysis(PSA)를 위한 Program 의 개발

Analysis of R-R interal variability is a useful method of obtaining many information about cardiovascular control mechanisms. Among the many methods of analyzing R-R variability, power spectral analysis(PSA) is said to be the most powerful and acurate tool. Although application of computer techniques in analysing R-R interval variability were introduced, as most of the studies on the R-R interval variability have been off-line and invasively, they were not suitable for practical use. So we prepared a program for non-invasive on-line perioperative assessment of R-R interval variability. We used Turbo C^(++)(ver 1.0, Borland International, USA) and Turbo Assembler(ver 2.0, Borland International, USA) for programing. Data were collected by A/D converter(PCL 718,, Taiwan) by interrupt mode and transferred to TBM386 compatibie computer with VGA color monitor. By applying PSA to R-R interval variation, we can get an almost as confident information about the cardiovascular system as analysis of arterial waveform, In addition to this we can get data that requires very reduced momory size, and get them non-invasively. So analysing R- R interval variability may be the most suitable method for on-line continuous assessment of cardiac parameters.

<P>In this study, the stimulatory effects of different lactic acid bacteria strains, and their subcellular fractions, on the THP-1 cell line were evaluated. Lactobacillus plantarum was found in particular to induce high levels of IL-23p19 mRNA, but it moderately induced TNF-alpha production. IL-10 production was not entirely affected by L. plantarum stimulation. When subcellular fractions of L. plantarum were used to treat THP-1 cells, IL-23p19 mRNA expression was enhanced in a dose-responsive manner, specifically by lipoteichoic acid (LTA). The cotreatment of THP-1 cells by both L. plantarum and Staphylococcus aureus LTA resulted in decreased IL-10 production when compared with cells treated by S. aureus LTA alone. Taken together, these data suggest that LTA isolated from L. plantarum elicits stimulatory effects upon the expression of IL-23p19 and inhibitory effects on pathogen-mediated IL-10 production.</P>
