자궁내 태아 사망시 Prostaglandin F2a의 양막외 주입법에 의한 중기임신중절

1977년 2월부터 1978년 5월까지 대구동산기독병원 산부인과에 입원하여 PGF2α의 양모외주입법에 의한 자궁내태아사망시의 중임 기신중절술을 받았던 17예를 임상적으로 조사분석하여 다음과 같은 결과를 얻었다. 1. 년령은 21세에서 41세까지 였으며, 21세에서 25세까지가 가장 많았다. 2. 분만회수는 초년부가 8명, 경산부가 6명이었으며 임신주수는 16주에서 34주까지 였으며, 23주이상이 전체의 70.5%로 가장 많 았다. 3. 48시간내 유산성공율은 초임부에서는 8명에서 8명으로 100%, 경산부에서는 9명에서 8명으로 88.9%로 평균 94.1%이었다. 4. 평균유산소요시간은 14.47시간이었으며, 초임부에서는 16.34시간, 경산부에서는 12.51시간이었다. 5. 평균 PGF2α주입량은 4.507mg이었으며, 초임부에서는 4,458mg, 경산부에서는 4,556mg이었다. 6. 폴리씨도뇨관 자연배출후 유산까지의 소요시간은 평균 6.27시간 이었으며, 초임부에서 7.36시간, 경산부에서 5.27시간이었 다. 7. 부작용은 17예중 2예로 악심, 구토등 소화기 계통이었다. 8. 합병증으로는 불완전유산으로 소파를 시행한 예가 1예, 수혈을 요하는 출혈이 1예 있었다. Various methods of termination at midtrimester pregnancy were individually used until today, but no one method was satisfactory. Midtrimester abortion in fetal death in utero, by a extra-amniotic administration of prostaglandin F2, alpha every 2 hours via the foley catheter was successfully in duced in 16 of 17 patients. Among the 16 successful abortion, 13 patients aborted within 24 hours. The one patient failed to abort after 48 hours of instillation of prostaglandin F2 alpha and supplementary intravenous dripping of pitocin and aborted by curettage. The mean abortion time was 14.47 hours and average total dose used was 4,507mg The abortion time appeared to decrease in multiparous than nulliparous patients and had no relationship to gestational age and parity. The most frequent side effects was gastro-intestinal disturbances-nausea and vomiting in 2 of 17 patients. The main complication in this series was incomplete abortion and excessive bleeding over 500ml, which required a curettage and transfusion, respectively. Extra-amniotic administration of PGE2 alpha in inducing midtrimester abortion was found to have a high success rate, a rapid abortion time and fewer side effects and complications than any other method of prostaglandin administration. But the drawback of the extra=amiotic method is the inconvenience of intermittant, tedoius, reported instillations and the necessity for an indwelling catheter, putting the patient at risk for developing an intrauterine infection.
