      • Impact of Interferon-Based Treatment on Quality of Life and Work Related Productivity from the Korean Cohort in the MOSAIC Study

        Aims: Chronic Hepatitis C Virus (HCV) infection increases the risk for progressive liver disease, hepatocellular carcinoma and negatively impacts the patient’s quality of life. HCV treatment is evolving with direct acting antivirals but IFN based therapy has been the standard of care for many years and remains available in some countries. The MOSAIC study aims to characterize patients with chronic HCV infection and assess the impact of IFN-containing treatment on health-related quality of life, work related productivity and health care utilization. Methods: MOSAIC is an international prospective multicenter observational study that has been conducted in 20 countries. Consecutive patients with chronic HCV infection were enrolled and those who initiated an IFN based regimen were prospectively followed for 48 weeks. We report results from the Korean cohort Results: 100 patients were enrolled: 86 were treatment naïve and 14 were treatment experienced. 33 patients initiated an IFN based regimen: 6 patients started IFN + RBV, 26 patients started Peg-IFN + RBV, none started Peg-IFN + RBV + DAA and 1 patient received other treatment. Among the treated cohort, demographic and disease characteristics were the following: the mean age was 54.5 years; 14 patients were male. 14 had minimal or no fibrosis, 2 portal fibrosis, 3 bridging fibrosis and 6 patients suffered from cirrhosis. HCV Genotype distribution was as follows: genotype 1: 11; genotype 2: 19 and genotype 3: 3. Table 1 describes the results at baseline and changes over 4, 12 and 48 weeks and end-of-treatment (EOT) for the quality of life and work productivity outcome measures (EQ-5D-5L, HCV-PRO and WPAI). Conclusions: Results from the Korean cohort of the MOSAIC study show a moderate trend for deterioration of health-related quality of life and work productivity associated with IFN based treatment for patients with chronic HCV infection during treatment period.         A Capacitance Estimation of Film Capacitors in an LCL-Filter of Grid-Connected PWM Converters

        A capacitor deterioration of LCL-filter grid-connected PWM converters is progressed by the self-healing mechanism. It leads to the degradation of the filter performance and drop of power factor. Thus, it is required to diagnose fault-point of capacitors and determine the replacement time. Typically, the fault of capacitors is determined when the capacitance is reduced up to 80% from initial value. This paper proposes algorithm to the determine capacitor replacement time of an LCL filter. The algorithm takes the advantage of change of the response on the injected resonant frequency corresponding to 80% value from the initial capacitance. The results of the algorithm are demonstrated through simulations and experiments.

        Association of CDKAL1 Polymorphisms with Early-Onset Atopic Dermatitis in Koreans

        Background: Atopic dermatitis (AD) has increased in frequency to rates as high as 20% for children in developed countries. AD is one of the most common childhood diseases and has a complex etiology involving genetic and environmental factors. Thus, a broad understanding of genetic background is needed for early diagnosis of AD. Objective: Identification of candidate functional genetic variants associated with early-onset AD in Koreans. Methods: Whole-exome sequencing (WES) was performed in three families. Sanger sequencing was used to validate detected variants in 112 AD patients and 61 controls. Results: Functional variants were filtered by WES, and then variants related to allergic immune diseases were selected through a literature search. Two candidate non-synonymous single-nucleotide polymorphisms of CDKAL1 (rs77152992) and ERBB2 (rs1058808) were identified, c.1226C>T, p.Pro409Leu, c.3463C>G, and p. Pro1170Ala respectively. A case-control study was performed to determine whether rs77152992 and rs1058808 are candidate risk factors for early-onset AD. rs77152992 was significantly associated with early-onset AD (odds ratio [OR], 0.42; 95% confidence interval [CI], 0.21∼0.83; p=0.0133) in allele frequencies. The CC genotype of CDKAL1 had significantly increased risk of AD (OR, 2.16; 95% CI, 1.0∼4.6; p=0.0475). rs1058808 had no correlation with AD. Total eosinophil count was significantly increased in AD patients with the CC genotype of CDKAL1 (rs77152992). Conclusion: CDKAL1 (rs77152992) and ERBB2 (rs1058808) were deemed functionally interesting based on WES. Our case-control study suggests that the CC genotype of rs77152992 may be associated with increased eosinophil counts. It may enhance the risk of early-onset AD. (Ann Dermatol 30(3) 276∼283, 2018)

        Birt-Hogg-Dubé Syndrome, a Rare Case in Korea Confirmed by Genetic Analysis

        Simple benign tumors can present as part of a syndrome with substantial mortality. Fibrofolliculomas are benign skin tumors most often associated with the Birt-Hogg-Dubésyndrome (BHDS). The most life-threatening complication of this syndrome is renal cancer and other major features include multiple lung cysts and spontaneous pneumothorax. We present the case of a 54 year-old man with multiple flesh-colored papules on his face confirmed histologically as fibrofolliculomas. He had a history of recurrent pneumothorax and chest computed tomography showed multiple lung cysts. To confirm the diagnosis of BHDS, we conducted gene analysis that revealed a single nucleotide duplication in the folliculin (FLCN) gene (Exon 11, C.1285dupC). BHDS confirmed by the FLCN gene mutation is rarely reported in Korea. Appropriate investigation is recommended whenever a patient with benign skin tumors is encountered.       • Enhanced Expression of the ORF2 Protein of Porcine Circovir Type2 using Baculovirus

        Porcine Circovirus Type2 (PCV2), a single-stranded DNA virus associated with Postweaning multisystemic wasting syndrome(PMWS) of swine, has two major open reading frames, ORF1 and ORF2. The genomic size and molecular weight of ORF2 is respectively 699bp, 28kDa. ORF2 encodes the capsid protein (structural protein) that has type-specific epitopes and is very immunogenic and associated with the induction of neutralizing antibodies, suggesting its potential use in diagnostic assays as well as vaccine development. For efficient production of the capsid proteins, we expressed the PCV2 ORF2 gene with baculovirus in the insect cells. In this study, PCV2 ORF2 was appropriately ligated into the baculovirus transfer vector, pBacPAK9 and pB9-Acpol19-110-EK. Sf21 cells were transfected with a mixture of the purified recombinant transfer vector and bAcGOZA. We generated and purified recombinant viruses containing PCV2 ORF2, and named rAc-B9-PCV2ORF2 and rAc-B9-19-110-EK-PCV2ORF2, respectively. Expression levels of capsid fusion proteins with a partial polyhedrin region of AcNPV more increased than recombinant proteins from non-fusion expressed. Also, expression efficiency increased over time and differed at MOI. As a results, fusion expression of porcine circovirus type2 ORF2 using baculovirus could be utilized as an alternative expression method to produce recombinant antigen against PCV2 infection and is worthy of further investigation.

        Sulforaphane inhibits the Th2 immune response in ovalbumin-induced asthma

        Sulforaphane (1-isothiocyanato-4-(methylsulfinyl)-butane), belonging to a family of natural compounds that are abundant in broccoli, has received significant therapeutic interest in recent years. However, the molecular basis of its effects remains to be elucidated. In this study, we attempt to determine whether sulforaphane regulates the inflammatory response in an ovalbumin (OVA)-induced murine asthma model. Mice were sensitized with OVA, treated with sulforaphane, and then challenged with OVA. Sulforaphane administration significantly alleviated the OVA-induced airway hyperresponsiveness to inhaled methacholine. Additionally, sulforaphane suppressed the increase in the levels of SOCS-3 and GATA-3 and IL-4 expression in the OVA-challenged mice. Collectively, our results demonstrate that sulforaphane regulates Th2 immune responses. This sutdy provides novel insights into the regulatory role of sulforaphane in allergen- induced Th2 inflammation and airway responses, which indicates its therapeutic potential for asthma and other allergic diseases.       • Enhanced Biocompatibility of Polyimide Film by Anti-Inflammatory Drug Loading

        <P>Polyimide (PI) has been used as a substructure in the electronics industry for several decades due to its excellent thermal, mechanical, and chemical properties as well as its relatively low dielectric constant. Micro-fabricated neural prosthetic devices, which are used for chronic recording and stimulation of nervous system tissues, are in the early stages of design and development. The interface between neural microelectrodes and neural tissue plays an important role in chronic in vivo recording. However, when these devices are implanted into brain tissue for long-term recording, they lose electrical connectivity due to immune response. To enhance biocompatibility of polyimide-based neural electrodes, we performed anti-inflammatory drug loading by soaking the PI in drug loaded solvent. The drug loaded PI films were subcutaneously implanted in a rat model. The biocompatibility of the PI film was analyzed using cytotoxicity assays, RT-PCR, and H&E staining. The solvent-extracted and drug loaded PI film shows better short term biocompatibility as compared to control and non-treated PI film. This anti-inflammatory drug loading method can be used to increase the biocompatibility of neural electrodes.</P>

        Influence of Transmucosal Designs of Dental Implant on Tissue Regeneration in Beagle Dogs

        This study was conducted to evaluate the influence of concave machined and concave microgrooved profiles of the transmucosal portion of implants on early peri-implant tissue regeneration. We assessed implants with 3 different transmucosal profiles: the bevel group that had only a bevel in the transmucosal part, bioseal group and bioseal/ groove group that had a micro-groove in the S-shaped portion of the implant. A total of 36 implants (12 of each type) were placed in the edentulous mandibular ridges of 6 beagle dogs. We used radiographs to investigate the mesio-distal change of the marginal bone. Bucco-lingual bone resorption and soft tissue reactions were evaluated histologically. Radiographic and histological analysis did not show any difference in mesio-distal or bucco-lingual marginal bone resorption among any of the 3 groups (p > 0.05). The bioseal and bioseal/groove groups had more rigid connective tissue attachment than that in the bevel group. However, the bevel group had significantly long junctional epithelium attachment and the bioseal and bioseal/groove groups had long connective tissue attachment (p < 0.05). Implants with S-shaped concavity of the transmucosal portion resulted in a firmer connective tissue barrier, and thus, a better soft tissue regeneration, than implants with straight bevel.
