Effects of Early Operation in Patients Who Diagnosed Complicated Parapneumonic Effusion

Purpose Thoracic empyema is defined as active suppuration within the pleural space and its treatment is controversial. Recently treatment of empyema is changing observation of empyema formation to early invasive treatment with the operation. This study compared the outcomes of early surgery and delayed surgery Method We retrospectively reviewed 40 patients who underwent surgery for complicated parapneumonic effusion from January of 2013 to May of 2020. The early group was defined as operation with empyema diagnosed within 1 week. Results Among 40 patients, 22 patients received early surgery, and 18 patients received delayed surgery. Stage III empyema was prevalent in the delayed group (27.3% vs 76.9%, p=0.004). Operative time was longer in the delayed group (87.5 vs 132.5 min, p=0.004). Intraoperative blood loss was more in the delayed group (200ml vs 750ml, p=0.009). The hospital stay was longer in the delayed group (18.5 days vs 30.0 days, p<0.001). The complication rate was not different between groups (31.8% vs 33.3%, p=0.919). The mortality rate was no significance between the two groups (4.5% vs 5.6%, p=0.884). Conclusion Delayed surgical treatment make operation difficult. Although inactive infected phase, early surgical treatment did not increase postoperative morbidity and mortality.

Multivariate Process Control Chart for Controlling the False Discovery Rate

With the development of computer storage and the rapidly growing ability to process large amounts of data, the multivariate control charts have received an increasing attention. The existing univariate and multivariate control charts are a single hypothesis testing approach to process mean or variance by using a single statistic plot. This paper proposes a multiple hypothesis approach to developing a new multivariate control scheme. Plotted Hotelling’s T2 statistics are used for computing the corresponding p-values and the procedure for controlling the false discovery rate in multiple hypothesis testing is applied to the proposed control scheme. Some numerical simulations were carried out to compare the performance of the proposed control scheme with the ordinary multivariate Shewhart chart in terms of the average run length. The results show that the proposed control scheme outperforms the existing multivariate Shewhart chart for all mean shifts

This paper presents the design and analysis of gate-recessed AlGaN/GaN Fin-type Field-Effect Transistor (FinFET). The three-dimensional (3-D) technology computer-aided design (TCAD) simulations were performed to analyze the direct-current (DC) and radio-frequency (RF) characteristics for AlGaN/GaN FinFETs. The fin width ($W_{fin}$) and the height of GaN layer ($H_{GaN}$) are the design parameters used to improve the electrical performances of gate-recessed AlGaN/GaN FinFET.

Objective: Transcutaneous electrical nerve stimulation (TENS) is a treatment method for pain, and it can be divided into conventional TENS (C-TENS) and acupuncture-like TENS (A-TENS). More recently, high power lasers have increasingly been used to reduce pain caused by arthritis, residual neuralgia, and musculoskeletal disorders. The aim of this study was to compare the laser needle with C-TENS and A-TENS in terms of pain and blood flow in healthy people, as well as to confirm that the laser needle can replace TENS to treat pain. Design: A randomized controlled trial. Methods: The selected participants were divided using Minimize computer software into a laser group (n=13), a C-TENS group (n=13), and an A-TENS group (n=14); they underwent a pre-test for blood flow and pain in their forearm. The three groups received their respective interventions; they then underwent a second pain and blood flow test on the same spot. Results: No significant differences were observed in the A-TENS group between the pre- and post-tests, and a comparison among the three groups revealed no significant differences between the laser needle group and the C-TENS group in terms of pain. Regarding blood flow, no significant differences were found between the pre- and post-tests in the laser needle group; a comparison among the three groups only revealed a significant between the laser needle and A-TENS groups. Conclusions: This study confirmed that the laser needle can be used to treat pain when it is necessary to control blood flow.
