        Sleep and Anesthesia

        Since both anesthesia and sleep depress consciousness, bidirectional relationship between them has been further studied. Earlier findings have shown that they share electroencephalographic features and brain regions that are activated in both state of unconsciousness. Despite these similarities, medication-induced sedation provokes different outcome from natural sleep. Enlisting commonly used analgesic drugs, such as benzodiazepines, intravenous agents, benzodiazepine antagonists, opioids, and other adjuvants, the study is comprised of assorted case studies that are clinically applicable or comparable. Acknowledging potential of analgesic drugs on sleep disorders including sleep deprivation, narcolepsy, circadian rhythm disorder, periodic limb movement disorder, and obstructive sleep apnea, the study underscores the clinical importance of studying both fields, sleep and anesthesia. In conclusion, the aim of this review is explaining the consequences of analgesic agents or sedatives on sleep and sleep disorders.

      • The Effects of Immune Checkpoint Modulators on the Clinical Course of Patients with Resectable Hepatocellular Carcinoma

        Aims: Immune checkpoint proteins regulating T-cell mediated anti-tumor immunity have been reported to affect clinical outcomes in multiple malignancies. This study aimed to investigate the prognostic effect of histological expression of immune checkpoint proteins in patients with resected hepatocellular carcinoma (HCC). Methods: A total of 221 patients with HCC who underwent curative resection were included. Expression of Programmed-cell death ligand- 1 in tumor cells (tPD-L1) and tumor infiltrating mononuclear cells (TIMCs) (iPD-L1), Programmed-cell death-1 in TIMCs (iPD-1), and cytotoxic T lymphocyte antigen-4 in TIMCs (iCTLA-4) were measured immunohistochemically. Results: Among the 221 patients, histo-positivity for iCTLA-4, iPD-1, iPD-L1, and tPD-L1 was 32.1% (n=71), 42.5% (n=94), 35.3% (n=78), and 14.9% (n=33), respectively. Multivariate logistic analyses revealed that male sex and tumor >5cm were variables related to iCTLA-4 positivity (odds ratios [ORs] 0.46 and 1.94 respectively; Ps<0.05). Poor differentiation was related to PD-L1 expression in both tumor cells and TIMCs (ORs 2.88 and 3.46, respectively; Ps<0.05). Microvascular invasion was significantly associated only with iPD-L1, whereas tPD-L1 was positively correlated with baseline elevation of serum alpha-fetoprotein (≥200 ng/ml) (ORs 2.24 and 2.45; Ps<0.05). In time-dependent outcome analyses, expression of immune checkpoint proteins in TIMCs (i.e., iCTLA-4, iPD-1, and iPD-L1) was significantly related to longer overall survival and non-cancer-related survival (all Ps< 0.05), but not to time-to-recurrence or cancer-specific deaths (all Ps >0.05). Concurrent activation of the PD-1:PD-L1 and CTLA-4 pathways predicted improved outcomes in terms of overall survival and non-cancer related survival (P=0.06 and P=0.03, respectively). Conclusions: Immune checkpoint proteins upregulated in TIMCs in HCC tissues have individual and additive effects in prolonging the survival of patients, specifically in terms of survival not related to cancer recurrence.

        Nocturnal Sleep Onset Rapid Eye Movement Sleep Periods as a Predictor of the Severity of Narcolepsy in Korea

        Background and Objective To investigate the severity of narcolepsy based on the presence of nocturnal sleep onset rapid eye movement sleep period (nSOREMP). Methods Subjects included 167 narcolepsy patients diagnosed at the St. Vincent Hospital, the Catholic University of Korea. They underwent polysomnography (PSG) and Multiple Sleep Latency Test (MSLT). The standardized face to face interview and Epworth Sleepiness Scale were used to inquire about daytime sleepiness of patients. Overall retrospective chart review was performed on their sleep health data. Results The presence of nSOREMP was highly correlated with short mean sleep latency and high number of SOREMPs in MSLT. Subjects with nSOREMP also demonstrated higher percentage of N1 sleep, lower percentage of N2 sleep, and more frequent arousals in PSG. They also showed higher prevalence of cataplexy and human leukocyte antigen DQB1*06:02 positivity. Conclusion Subjects with nSOREMP showed more excessive daytime sleepiness and lower quality of sleep compared to subjects without nSOREMP. Our results suggest that nSOREMP might be a severity marker of narcolepsy.

      • 대형승용차 충돌특성 및 변형양상에 관한 실험적 연구

        The purpose of this study is to conduct various crash tests of large passenger cars and measure the deformation, occupant injuries and body decelerations of vehicles. A relationship between impact speed and vehicle damage data such as deformation, body decelerations and etc are analyzed. The characteristics of domestic cars in crash tests are obtained and will be used for developing a precise accident reconstruction program by using a data which is recording at Automatic Accident Recording System(AARS). In this study, the test results of the full frontal barrier crash and side impact are reviewed and examined. The results show that the decelerations of large passenger cars are decreased on the increment of the vehicle weight. But duration of decelerations are increased on the increment of the vehicle weight.

      • 공간마케팅 관점에서 본 외식공간의 디자인 사례 분석

        The purpose of this study is to analyze how the restaurants located in the non-commercial areas are designed and to propose the basic data for space planning. More people establish a business these days and their most preferred business is restaurant. However, the strong competition is raising the risk. So it is essential to reduce investment costs for the better risk management. In this paper, a non-commercial area is considered as an alternative location for a restaurant, and a survey is conducted to analyze the uniqueness and curiosity of the restaurants. These are worth studying as constant basic needs of human beings. The survey engaged 121 people between 20’s and 50’s who can be potential customers of the restaurants and they were asked to answer about the degree of uniqueness and curiosity, and also the spacial factors of selected examples. The results are as follows. Firstly, the case is a restaurant successfully operating in a non-commercial areas over three years, and the restaurants are specialized in focusing on the design aspect. Secondly, the respondents said positively about the degree of uniqueness and curiosity of the restaurant. Those characteristics tend to be recognized internally rather than externally. Thirdly, the exterior of the restaurant is regarded as the most factor to induce the uniqueness and curiosity of the space, and the formative elements such as interior layout, ceiling, furniture and object are selected more than color. Sign is more relevant to curiosity. Therefore, the formative elements should be actively utilized to cause the uniqueness and curiosity of the space. The study can be a useful basic resource for the spacial marketing to supplement the disadvantages of being in a non-commercial area.

        비(非)상권 외식공간의 디자인 특성에 관한 사례 연구

        The purpose of this study is to identify design elements for restaurant space in a non-commercial area and propose them for profit improvements of founders with small capital. As the barriers for market entry declines, new players in restaurant business has stiffen the competition and increased risk. So, it is crucial to reduce the investment costs for better risk management. In this paper, a non-commercial area is considered as an alternative location for a restaurant, and a survey is conducted to analyze the uniqueness and curiosity of the restaurants. Uniqueness and curiosity are worth studying as they are important factors of decision making. The survey was conducted on 121 individuals between 20’s and 40’s who are target customers of the restaurants. The survey measures the level of uniqueness and curiosity via images of the restaurant: how likely you will visit it, what are design elements that make it unique and curious. The results are as follows: Firstly, the respondents said positively about the degree of uniqueness and curiosity of the restaurants. The interior design elements are perceived to be more important than exterior design elements. Secondly, uniqueness and curiosity had a positive corelation with number of visits. Uniqueness in the interior space and curiosity in the exterior space have a higher influence on customers. Therefore, it is necessary to enhance effects by expressing curiosity on the exterior and uniqueness on the interior. Thirdly, the exterior shape of the restaurant is regarded as the most factor to induce the uniqueness and curiosity of the space. And the elements of spacial design related to shape such as layout, ceiling, sign are found to be more important than color. Therefore, it is necessary to actively utilize the formative factors to induce uniqueness and curiosity. The study can be a useful basic resource for the spacial marketing to supplement the disadvantages in a non-commercial area.

        플랫폼 독립적인 웹서비스를 이용한 그리드 네트워크 성능 관리 시스템에 대한 연구

        슈퍼 컴퓨터의 등장으로 기존에는 풀지 못했던 과학적, 학술적인 문제들이 대부분 해결되었지만 슈퍼 컴퓨터 자체의 막대한 비용 문제는 여전히 풀지 못한 숙제로 남아 있다. 이에 일반 사용자의 컴퓨터의 유휴 자원을 통합하여 협업하는 그리드 컴퓨팅이라는 개념이 등장하였다. 한편 분산되어 있는 이기종의 시스템의 자원을 통합하여 관리하기 위해서 연결 지향적인 프로토콜들이 사용되었지만, 다른 프로토콜끼리는 호환성이 없는 등의 문제가 존재한다. 본 논문에서는 플랫폼 독립적인 XML 기반의 SOAP을 사용하는 웹 서비스를 이용하여 그리드 네트워크의 성능 정보를 관리해주는 시스템을 제안한다. The advent of supercomputers contribute greatly in overcoming scientific and academic problems that were previously difficult to solve. However, the supercomputer itself suffers from the problem of being considerable cost. In response, the concept of grid computing, to use the resources of distribute computers connected with each other, was created. This system uses connection oriented protocols to integrate and manage the resources of different types of distributed systems, yet it has the problem of compatibility between protocols of other types. In this paper, a system to manage grid network performance through XML-based SOAP web service which is platform-independent, is proposed.

      • 탑승자 보호성능을 위한 교량용 방호울타리에 관한 실험적 연구

        Bridge rails are usually installed on the curb with a specified height and width. The variance of the height and the width of the curd could affect the safety for the passengers and the behavior of the vehicles. In this study, the impact severities of passengers and the behaviors of vehicles are investigated according to the variable location of the bridge rails on the curb with the specified height and width. The full scale crash test results show that the impact severities such as the THIV and the maximum PHD are minimized in the case of the correspondence of the face of bridge rails and the curb. Also, the results show that the behaviors of the vehicles are stable.

      • 블랙박스 카메라 기반 차량 정밀 측위

        This paper proposes a indoor-positioning method for vehicles based on image processing technology. The method obtains an absolute coordinate through QR-code scanning, and an relative coordinate of a black-box camera through the tilt and roll angle correction using affine transformation, scale transformation, and trigonometric function. Using these information of an absolute coordinate and an relative one, the precise position of a car is estimated. As a result, average error of 13.79cm is achieved and it corresponds to just 27.6% of the error of 50cm in indoor-positioning techniques based on wireless networks.
