26 cases(9 males and 17 females) with catatonia features on admission were analyzed in terms of its clinical characteristics. The results obtained were as follows : 1) In the diagnostic distribution, more than a half of the cases were schizophrenic disorders(57.5%) and the others were major depressions(26.9%) and schizophreniform disorders(15.4%). 2) The catatonia signs were nonspecific and evenly distributed among a variety of clinical diagnostic entities. But the signs of negativism, mutism, and posturing appeared frequently in all cases. 3) Acute onset was prevalent(65.4%), ECT was more frequently applied(61.5%) and treatment response was better(70.9%) in the catatonia group than non-catatonia. Although the definition of catatonia is not clear yet, above results support the opinions of previous studies that catatonia should be understood as a heterogenous symptom complex. And not only schizophrenia but also affective disorder are associated with catatonia features. This suggests that we have to re-evaluate the concept of Kahlbaum's old description.
Neuromuscular Characteristics and Physical Function in Participants with Parkinson’s Disease

PURPOSE: This study aimed to investigate the level of physical function, lower body strength, and muscle activation during various types of muscle contraction in participants with and without Parkinson’s disease (PD). METHODS: Twelve participants with PD (mean age=63.17±6.24 years) and 12 age- and sex-matched healthy adults (mean age = 58.67±6.39 years) were recruited. An isokinetic dynamometer was used to measure the length- and velocity-dependent maximum voluntary force and the rate of torque development (RTD) of the knee extensor muscles. Muscle activation of the vastus lateralis (VL), vastus medialis (VM), and rectus femoris (RF) muscles of both legs was examined using surface electromyography. The 6-minute walk test, chair stand test, timed up-and-go test, sit-and-reach test, and back-scratch test were performed to assess physical function. RESULTS: Compared to healthy individuals, participants with PD showed significantly lower maximum voluntary force and RTD (p<.05), performed fewer repetitions in the chair stand test (11.64±1.75 vs. 17.08±2.27, p<.001), were slower in the timed up-and-go test (8.36±1.42 vs. 5.65±1.07, p<.001), and walked shorter distances in the 6-minute walk test (424.17±65.97 vs. 539.47±63.18, p<.001). However, activation of the three different muscles during isometric and isokinetic muscle contraction was not different between participants with and without PD. CONCLUSIONS: Preserved muscle activation and significantly lower muscle strength during various types of muscle contractions may suggest lower muscle strength and efficiency. The lower physical function seen in participants with mild PD could be due to disease and low physical activity-related muscle atrophy rather than lower muscle activation.

Tolperisone, a muscle relaxant used for post-stroke spasticity, is metabolized to its main metabolite by CYP2D6 and to a lesser extent by CYP2C19 and CYP1A2. We investigated the effects of CYP2D6 and CYP2C19 genetic polymorphisms and cigarette smoking on tolperisone pharmacokinetics. A 150 mg oral dose of tolperisone was given to 184 healthy Korean subjects and plasma concentrations of tolperisone were measured by liquid chromatography-tandem mass spectrometry (LC-MS/MS). A 3.14-fold significant increase in AUC0–∞ was observed in the CYP2D6*10/*10 group compared with the CYP2D6*wt/*wt group, whereas a 3.59-fold increase in AUC0–∞ was observed in CYP2C19PMs compared to CYP2C19EMs. Smokers had a 38.5% decrease in AUC0–∞ when compared to non-smokers. When these effects were combined, CYP2D6*10/*10-CYP2C19PM-Non-smokers had a 25.9-fold increase in AUC0–∞ compared to CYP2D6*wt/*wt-CYP2C19EM-Smokers. Genetic polymorphisms of CYP2D6 and CYP2C19 and cigarette smoking independently and significantly affected tolperisone pharmacokinetics and these effects combined resulted in a much greater impact on tolperisone pharmacokinetics.
소아청소년암 생존자의 운동 경험과 운동 제약에 대한 질적 사례연구

본 연구는 아동·청소년 시기에 암을 진단받은 소아청소년암 생존자의 개별적인 운동 경험과 운동 제약을 파악하고, 공통적으로 나타나는 이슈와 실천적 함의에 대해 심층적으로 탐색하고자 하였다. 소아청소년암을 진단받고 치료를 완료한 9명의 청소년을 연구 참여자로 선정하였으며, 개인별 특수성을 이해하기 위해 질적 사례연구로 진행되었다. 개방형 질문을 사용하여 심층 면담을 1대1로 실시하였다. 운동 경험은 ‘발병 전 운동 경험’, ‘발병 후 운동 경험’으로 분류되었다. 참여자들은 암 발병 전에는 일반 청소년들과 다름없는 운동 경험을 가지고 있었지만, 진단 이후 치료 기간에 급격하게 신체활동량이 감소하였으며, 암 치료가 종료된 이후에도 이전과 같이 충분한 운동 참여를 하지 못하고 있었다. 참여자가 경험하는 운동 제약은 함께 운동할 친구 혹은 장소 부족과 같은 ‘환경적 제약 요인’과 낮은 체력 혹은 치료 후유증과 같은 ‘신체적 제약 요인’으로 분류되었다. 이러한 운동 제약으로 말미암아, 참여자들은 신체 활동 권고량을 충족하지 못하고 있었으며, 비활동적인 생활 습관을 가지고 있었다. 향후 소아청소년암 생존자의 운동 경험과 제약을 깊이 있게 이해하고, 이들이 운동에 활발하게 참여할 수 있도록 지역 사회와 학교에서 다양한 방안을 모색해야 할 것이다. The purpose of this study was to identify the exercise experience and perceived exercise barriers of childhood cancer survivors diagnosed with cancer in childhood and adolescence, and to explore common issues and practical implications in depth. Nine adolescents who were diagnosed with childhood cancer and completed treatment were selected as study participants, and a qualitative case study was conducted to understand individual characteristics. In-depth interviews were conducted one-on-one using open-ended questions. Our results showed that the exercise participation experience was classified as ‘exercise experience before diagnosis cancer’ and ‘exercise experience after diagnosis cancer’. Before cancer diagnosis, participants of the study experienced similar exercise and physical activity (PA) participation as children and adolescents without cancer. However, during cancer treatment, their PA participation decreased dramatically, even after cancer treatment, participants do not recover their pre-diagnosis PA levels. Exercise barriers which they experienced were classified into ‘environmental constraint factors’ such as lack of exercise partner and facilities, and ‘physical constraint factors’ such as such as low fitness levels and side effect of cancer treatment. Due to these exercise barriers, participants did not meet the recommended amount of PA levels for adolescents and had an inactive lifestyle. Based on the in-depth understanding of the exercise participation experience and exercise barriers of childhood cancer survivors, various methods will be proposed about participation in regular exercise in local communities and schools for childhood cancer survivors.

Background: Tumor necrosis factor antagonists are increasingly used to treat patients with immune-mediated infi ammatory disease including rheumatoid arthritis, ankylosing spondylitis, Crohn`s disease, and ulcerative colitis. Because TNF-alpha is essential for the prevention of latent tuberculosis recrudescence, patients treated with TNF-alpha inhibitors are at increased risk of developing active TB. The time of initiating anti-TNF therapy following the induction of LTBI treatment is different depending on each nation`s guideline. We aimed to investigate the treatment outcome of patients with immune-mediated infi ammatory disease who received anti-tumor necrosis factor therapy within 3 weeks of latent tuberculosis infection treatment. Methods: A total of 411 patients received LTBI treatment before commencing TNF antagonist between June 2004 and October 2013 at a tertiary referral center in South Korea. Their medical records were retrospectively reviewed. Results: The mean age of the 411 study subjects was 44.5 years and 261(63.5%) were male. The underlying IMID was ankylosing spondylitis in 203(49.4%), rheumatoid arthritisin 136(33.3%) and infi ammatory bowel diseases in 57(13.9%) patients. Anti-TNF agent was initiated in 61 patients(14.8%) within 3 weeks after chemoprophylaxis for LTBI, whereas 3 weeks later in remaining 350 patients(85.2%). These two groups were comparable in terms of baseline characteristics, treatment regimens, anti-TNF agents used and follow-up duration except signifi cant difference of the mean duration of LTBI treatment before starting anti-TNF therapy(8 vs. 30 days, p < 0.001). A total of 6 patients developed TB during follow-up period. All of these patients received anti-TNF agents 3 weeks after initiation of LTBI treatment. None in the patients who commenced TNF antagonist within 3 weeks of LTBI treatment developed TB. Conclusions: The present study suggested that TNF antagonist may be initiated within 3 weeks of LTBI treatment in patients with IMID.

<P><B>Abstract</B></P><P>The crystallization behavior of Cu<SUB>43</SUB>Zr<SUB>43</SUB>Al<SUB>7</SUB>Ag<SUB>7</SUB> (numbers indicate at.%) bulk metallic glass was investigated using the isothermal electrical resistivity measurements at 450°C in the supercooled liquid region. The crystallization process is a single step phase transformation. To analyze the electrical resistivity reduction, microstructure evolutions were analyzed using differential scanning calorimetry, X-ray diffraction, transmission electron microscopy and small-angle X-ray scattering. The Avrami parameter of the electrical resistivity reduction step was 1.73, indicating that the crystallization process is a diffusion-controlled growth of intermetallic compounds with decreasing nucleation rate.</P>
