Pyrantel Pamoate(Combantrin)에 依한 蛔蟲症의 減量療法

The mass treatment trial with low dosage of pyrantel pamoate against Ascaris lumbricoides infection was conducted to school children in rural areas of Korea. A comparative trial in the use of piperazine salt, santonin-kainic acid complex in the treatment of ascariasis was also attempted to evaluate the efficacy of' pyrantel pamoate. A total of 804 Ascaris infected school children selected out of 822-positives, who were screened from the total 1, 699 collected specimens, were classified into 7 groups. One of the three kinds of anthelmintics, i.e., pyrantel pamoate, piperazine adipate and santonin-kainic acid complex was administered to each group according to the planned regimen. For the evaluation, the egg negative conversion rate(cure rate) and egg reduction rate were calculated 4 weeks after treatment on 623 follow-up cases with the obtained results from the stool examinations (Cellophane Thick Smear Method and Stoll's Egg Counting Technique). The results obtained were; 1. The egg negative conversion rate was the highest in pyrantel treated groups (Group 1, 2, 3), santonin-kainic acid complex treated group (Group 7) coming next, and the lowest in piperazine treated groups (Group 4, 5, 6). 2. The reduction rates were also obtained just as the same order of the above results. 3. Among the pyrantel pamoate treated groups, the low dosage groups (Group 2 8: 3) showed 90.6% and 85.7% of negative conversion rate, and 99. 2% and 98. 2% of reduction rate respectively, while 94.1% of negative conversion rate and 97.9% of reduction rate were obtained in the conventional dosage group (Group 1). 4. On the analysis of efficacy in the groups treated with piperazine adipate, the best results were. shown in 2 or 3 consecutively treated groups (Group 4 &.- 6) under the supervision of qualified doctors, and the worst was in the group treated with 2 consecutive administration by teachers without supervision (Group 5). 5. The side reactions were so negligible in all the trial groups that none of the treated cases complained about the known side effect of those anthelmintics. From the present study, it can be suggested that the pyrantel pamoate is the most effective among three drugs, and also be recommended to apply the low dosage schedule of pyrantel pamoate in the masstreatment of ascariasis to reduce the expense of masstreatment.

N/A Background/Aims: The neurogenic responses of gastrointestinal smooth muscles to electrical field stimulation(EFS) is biphasic, consisting firstly of cholinergic contraction, followed by a transient relaxation. It is well known that a transient relaxation does involve the inhibitory non- adrenergic non-cholinergic(NANC) systems. However, the exact nature of the neurotransmitter released by these neurons is controversial. This study was done to investigate the relaxing agents and their action mechanisms by the use of organ bath with platinum electrodes. Methods: In 10 guinea-pigs, muscle strips of fundus, 10mm x 2mm, were prepared in a circular axis. Isometric tension study was performed in 30ml organ baths containing Krebs bicarbonate buffer, tnaintatined at 37 'C and gassed with 5% CO and 95% Oz. Results: The fundic relaxation by EFS was suppressed by NG-nitro-L-arginine methyl ester(L-NAME) and nitric oxide(NO) synthase inhibitor, in a dose-dependent manner and these effects were reversed by L-arginine, precurssar of NO. Nitroprusside, NO-donor, suppressed the fundic basal tension largely. Methylene blue, inhibitor of guanylate cyclase, suppressed the fundic basal tension and decreased EFS-induced relaxations. Forskolin and isoprenaline, which are activators of adenylate cyclase, suppressed the fundic basal tension as rnuch as nitroprusside. Intracellularly permeable cGMP and cAMP pro- duced relaxations in the precontracted fundic muscle with noradrenaline. TEA, non-specific K' channel blocker, and aparnin, Ca ' activated K' channel blocker, increased the fundic basal tension and EFS-induced relaxations. Conclusions: Above results indicate that NO is released upon stimulation of the NANC nerves in the guinea-pig fundic muscle and play a major role in the receptive relaxation of the fundus region. The release of NO is related with K' channel, and some other inhibitory agents may be involved in transient relaxation through modulative effects on cAMP or cGMP. The complement acts as a part of immune system to protect the individual from microbial infection by mediating a variety biologic reaction; opsonization, chemotaxis of leukocytes, increased vascular permeability, and cytolysis of target organisms. The complement system is consisted about 18 proteins, among them, The C3 and C4 are the highest concentrates in serum. These proteins are said to be in "classic pathway" or "alternative pathway" names. Activation of the complement proteins result in loss of their precursors, native activities and, or their accelerated clearence from p1asma. Progressing hypercatabolism without compensation by increased synthesis of complements, the determination of depressed levels of them may provide the evidence of the comp1ement system activation. For the evaluation of the patient with autoimmune disease such as autoimmune hemolytic anemia, SLE, rheumatoid arihritis and poststreptococcal glomerulonephritis, the determination of C3 and C4 concentration are both of the most informative method. Low concentration of C4 indicated that classic pathway activation has occurred, since C4 is extremely sensitive to C1 Depressed level of C3 suggests that rather intense activation of ether pathway. Decreased C3 associated with normal C4 levels indicate exclusive activation of alternative pathway. During the past decade, a number of invastigators have developed the techniques for measurement of complement. Since 1929, Michael Heidleberger has developed immunoprecipitation analysis of complement. And also Boyden et al (1947) demonstrated turbidimetric absorbance using the basis of antigen quantitation. The radial immunodiffusion (RID) technique has proved to be one of the most useful methods in clinical laboratories during the past few years, but it require rather long processes from 18 to 48 hrs in order to complete precipitation ring formation. So we attempt to introduce and establish more rapid completion of complement quantitation based on turbidimetric technique using a centrifugal analyzer.
Analysis of the Midface, Focusing on the Nose: An Anthropometric Study in Young Koreans

ABSTRACT: Improvements in both living standards and incomes have meant that many more people are now able to undergo elective plastic surgery for facial improvement. The nose and midface are the most frequent surgical target in Korea. However, the lack of reliable and precise anthropological data is a hindrance for effective surgical applications in this area. In the current study, 21 different lengths and angles of the midface and nose were measured in 2065 volunteers. The data were analyzed to establish sex and racial differences. Most of the measurements obtained from male volunteers were 5% to 10% greater than for the females, exceptions being the nasofrontal, nasolabial, and nasal tip angles. The biggest sex difference was found for depth of pupil, and the smallest difference was for intercanthal distance. For each measurement, the ratio of proportional differences was similar to that for whites. Many Koreans exhibited a columella protrusion of 4 to 5 mm; that parameter measures 2 to 3 mm in whites. The prevention of a hanging columella is important in nasal tip surgery. The intercanthal distance was also relatively large compared with the facial width. In females, the intercanthal distance was similar to the length of the nasal dorsum and the width of the nose. Surgeons may find this analysis useful for surgical planning. The present study provides basic clues for planning and practice in reconstructive and aesthetic facial surgery.
청피(靑皮)가 뇌혈류와 복강내 Macrophage에 미치는 영향

Citri Reticulatae Viride Pericarpium is being used to regulate the flow of gi. However, the mechanism of its pharmacological actions is not well understood. The purpose of this research was to investigate effects of Citri Reticulatae Viride Pericarpium on peritoneal macrophages in mice and blood pressure, regional cerebral blood flow in rats. Nitric oxide production was tested using a Griess reagents, and a phagocytic activity of mouse peritoneal macrophage was tested using a luminometer. Blood pressure and regional cerebral blood flow was tested using a phygiograph, MacLab and Macintosh computer with data acquisition system. 1. Blood pressure was increased by Citri Reticulatae Viride Pericarpium in a dose-dependent manner. 2. rCBF was increased by Citri Reticulatae Viride Pericarpium in a dose-dependent manner. 3. Pretreatment with propranolol significantly inhibited Citri Reticulatae Viride Pericarpium induced increase in rCBF. 4. Citri Reticulatae Viride Pericarpium inhibited nitric oxide production from peritoneal macrophages in mice. 5. Citri Reticulatae Viride Pericarpium accelerated phagocytic activity of peritoneal macrophage in mice These results suggest that Citri Reticulatae Viride Pericarpium causes a diverse response of blood pressure, regional cerebral blood flow(rCBF), nitric oxide production and phagocytic activity. The increase in rCBF is also mediated by adrenergic βrecepter.
