        Comparison of the Recurrence and Surgical Outcome of Spinal Hemangioblastoma in Sporadic and Von Hippel-Lindau Diseases: A Subanalysis of a Nationwide Study by the Neurospinal Society of Japan

        Objective: This study aimed to clarify the relationship between recurrence and the extent of resection in surgery for intramedullary spinal hemangioblastoma (sHB) and its impact on von Hippel-Lindau (vHL) disease. Methods: Data on sHB cases followed up for at least 6 months after surgery were extracted from a nationwide registry of 1,033 consecutive spinal intramedullary tumors surgically treated between 2009 and 2020, and were retrospectively categorized into a sporadic or vHL group. The diagnosis of vHL disease was made at each institution based on clinical findings. Results: A total of 168 patients (sporadic group, 101; vHL group, 67) were included in the study. Compared with the sporadic group, the vHL group had a younger onset (45.4 ± 16.8 years vs. 39.6 ± 14.1 years, p = 0.02), more preoperative motor (47.5% vs. 68.7%, p < 0.01) and gait (37.6% vs. 61.2%, p < 0.01) impairments, and more patients with worsening neurological symptoms at discharge (p = 0.02). The gross total resection (GTR) rates and the recurrence rates were not statistically different between the sporadic and the vHL groups. GTR significantly improved recurrence-free survival compared to non-GTR in all patient analysis (p < 0.01) but this trend was not observed in the sporadic group. Physical functional improvement from discharge to 6 months after surgery was observed in the sporadic group (p < 0.01) but not in the vHL group. Conclusion: A high GTR rate may sufficiently decrease susceptibility to recurrence, especially in patients with sHB with vHL. In sporadic sHB, postoperative functional improvement can be expected, and the long-term functional prognosis is favorable.

      • Monitoring of Hydrogen Sulfide from an Industrial Waste Landfill in Japan

        Landfill management in Japan was developed more than Centuries. At industrial rapid growth era, numbers of waste were disposed with less control, such as co-disposal of Construction and Demolition waste (C&D waste) and organic Municipal Solid Wastes (MSW). Generally, Hydrogen sulfide (H<sub>2</sub>S) is detectable in ppm by human as odor, but more seriously it causes toxic effect in hundreds ppm. H<sub>2</sub>S is often generated in landfill containing sulfate source (such as gypsum board) and organic waste thorough the activity of the Sulfate Reducing Bacteria (SRB). This study was carried out to investigate the emission behavior and environmental parameters related to H<sub>2</sub>S generation in a landfill under aftercare phase, which has been disposed of C&D waste, and to develop the methodology to assess the environmental situation in landfills to be monitored. This survey was also involved in the project that aim to develop the methodology of appropriate management of the landfills. The objected landfill was located nearby the river and was operated by private sector. After the “standard of landfill disposal” regulation was enforced in Japan, this landfill was closed and started to monitoring. Figure 1 shows schematic of geological section of landfill, the area of dumping was mountainous, North and Eastern side of landfill was mounted to the hill called ‘Upper Zone’. South and Western side called ‘Lower Zone’. The slope of landfill surface was from upper to lower part. The approximate landfill surface is 4000 m<sup>2</sup>. Landfill investigation was conducted on October 2017. The location for measurement of surface gas emission by static chamber method was selected by grid placing. Soil gas concentration was measured by a gas analyzer and gas detecting tube. Water quality from landfill layer in monitoring well was analyzed in laboratory. The results show H<sub>2</sub>S gas was detected 5 out of 29 grid points from 0.7 m beneath cover soil in range 0.2-800 ppm. These H<sub>2</sub>S gas concentrations were related to surface H<sub>2</sub>S emissions which emitted in range 0.3-37.7 l/m<sup>2</sup>/d. The detected H<sub>2</sub>S emitted points were located at the lower zone of this landfill. In the same area, CH<sub>4</sub> emission were found in 10 out of 29 points in range 0.1-62.7 l/m<sup>2</sup>/d. H<sub>2</sub>S emission and CH<sub>4</sub> emission were negatively correlated. In contrast to the lower zone, CH<sub>4</sub> emission was detected at few points and no H<sub>2</sub>S gas was found at the upper zone. It is widely known that SRB and Methane Generating Bacteria inhabited or competitive of carbon source under anaerobic condition. Landfill gas results indicated that anaerobic degradation was comparably active in lower zone due to the existing of organic matter inside waste body. To compare biogas generation from lower and upper zone, CH<sub>4</sub> and CO<sub>2</sub> emissions from lower zone was about 10.1 and 3.2 times higher than that from upper zone, respectively. The higher degradation rate explain by the different amount of organic matter from upper to lower zone. Quality of inner water of landfill layer in the monitoring wells which located in lower zone were analyzed. The SO<sub>4</sub> concentration were in range 110-710 mg/l. Low concentration of dissolved oxygen (0.68 mg/l) and remaining of organic carbon (33.7 mg/l) were confirmed in inner water of landfill. These would possibly provide condition of H<sub>2</sub>S generation. From the result, landfill gas emission still active even the landfill already close for several years. It could suggest that the intensive and log-term monitoring program for this landfill must be necessary. It was also shown that the integration of monitoring of behavior of gas emission and dissolved component must be necessary to identify the practical situation of landfills, and to apply for future investigation in terms of improving the accuracy and simplification of methodology.

        ORiginal Article : Visceral Obesity as a Risk Factor for Left-Sided Diverticulitis in Japan: A Multicenter Retrospective Study

        Background/Aims: Left-sided diverticulitis is increasing in Japan, and many studies report that left-sided diverticulitis is more likely to be severe. Therefore, it is important to identify the features and risk factors for left-sided diverticulitis. We hypothesized that left-sided diverticulitis in Japan is related to obesity and conducted a study of the features and risk factors for this disorder in Japan. Methods: Right-sided di-verticulitis and left-sided diverticulitis patients (total of 215) were compared with respect to background, particularly obesity-related factors to identify risk factors for diverticulitis. Results: There were 166 (77.2%) right-sided diverticulitis patients and 49 (22.8%) left-sided diverticulitis patients. The proportions of obese patients (body mass index ≥25 kg/m2, p=0.0349), viscerally obese patients (visceral fat area ≥100 cm2, p=0.0019), patients of mean age (p=0.0003), and el-derly patients (age ≥65 years, p=0.0177) were significantly higher in the left-sided-diverticulitis group than in the right-sided-diverticulitis group. The proportion of viscerally obese patients was significantly higher in the left-sided-diverticulitis group than in the left-sided-diverticulosis group (p=0.0390). Conclusions: This study showed that obesity, particularly visceral obesity, was a risk factor for left-sided diverticulitis in Japan. (Gut Liver 2013;7:532-538)

        En Bloc Spondylectomy for Spinal Metastases: Detailed Oncological Outcomes at a Minimum of 2 Years after Surgery

        Study Design: Retrospective case series. Purpose: To investigate the oncological outcomes, including distant relapse, after en bloc spondylectomy (EBS) for spinal metastases in patients with a minimum of 2-year follow-up. Overview of Literature: Although EBS has been reported to be locally curative and extend survival in select patients with spinal metastases, detailed reports regarding the control of distant relapse after EBS are lacking. Methods: We conducted a retrospective review of 18 consecutive patients (median age at EBS, 62 years; range, 40–77 years) who underwent EBS for spinal metastases between 1991 and 2015. The primary cancer sites included the kidney (n=7), thyroid (n=4), liver (n=3), and other locations (n=4). Survival rates were estimated using the Kaplan–Meier method, and groups were compared using the log-rank method. Results: The median operative time and intraoperative blood loss were 767.5 minutes and 2,375 g, respectively. Twelve patients (66.7%) experienced perioperative complications. Five patients (27.8%) experienced local recurrence of the tumor at a median of 12.5 months after EBS, four of which had a positive resection margin status. Thirteen patients (72.2%) experienced distant relapse at a median of 21 months after EBS. The estimated median survival period after distant relapse was 20 months (95% confidence interval, 0.71–39.29 months). No association was found between resection margin status and distant relapse. Overall, the 2-year, 5-year, and 10-year survival rates after EBS were 72.2%, 48.8%, and 27.1%, respectively. Importantly, the era in which EBS was performed did not impact the oncological outcomes. Conclusions: Our results suggest that EBS by itself, even if margin-free, cannot prevent further dissemination, which occurred in >70% of patients at a median of 21 months after EBS. These results should be considered and conveyed to patients for clinical decision-making.

        Change of Gastric Emptying With Chewing Gum: Evaluation Using a Continuous Real-Time 13C Breath Test (BreathID System)

        Background/Aims There are few reports on the correlation between chewing gum and the gastrointestinal functions. But previous report showed use of chewing gum to be an effective method for controlling gastrointestinal symptoms. The aim of this study was to determine the correlation between chewing gum and gastric emptying using the continuous real time 13C breath test (BreathID system). Methods Ten healthy male volunteers participated in this randomized, 2-way crossover study. The subjects fasted overnight and were randomly assigned to chewing gum (Xylish, 2-3/1 tablet) for an hour following intake of a test meal (200 kcal/200 mL) or intake of the test meal alone. Gastric emptying was monitored for 4 hours after administration of the test meal by the 13C-acetic acid breath test performed continually using the BreathID system. Results No significant differences in the calculated parameters, namely, T1/2 (median, 111.82 vs 109.26 minutes; P = 0.575), Tlag (median, 53.28 vs 56.53 minutes; P = 0.333), gastric emptying coefficient (median, 3.58 vs 3.65; P = 0.285), regression-estimated constant β (median, 1.85 vs 1.80; P = 0.575) and regression-estimated constant κ (median, 0.61 vs 0.62; P = 0.959) were observed between the test meal alone group and the test meal and chewing gum group. Conclusions This study showed that chewing gum had no effect on the rate of gastric emptying. Therefore, since chewing gum did not enhance the speed of gastric emptying, it may ameliorate gastrointestinal symptoms through other mechanisms, such as saliva and autonomic nervous system.         Early Effect of Single-dose Sitagliptin Administration on Gastric Emptying; Crossover Study Using the 13C Breath Test

        The gastrointestinal motility effects of endogenous incretin hormones enhanced by dipeptidyl peptidase-IV (DPP-IV) inhibitors have not yet been sufficiently investigated. The aim of this study was to determine whether single pre-prandial sitagliptin, the DPP-IV inhibitor, administration might have an effect on the rate of liquid gastric emptying using the 13C-acetic acid breath test. Methods Ten healthy male volunteers participated in this randomized, two-way crossover study. The subjects fasted for overnight and were randomly assigned to receive 50 mg sitagliptin 2 hours before ingestion of the liquid test meal (200 kcal per 200 mL, containing 100 mg 13C-acetate) or the test meal alone. Under both conditions, breath samples were collected for 150 minutes following the meal. Liquid gastric emptying was estimated by the values of the following parameters: the time required for 50% emptying of the labeled meal (T1/2), the analog to the scintigraphy lag time for 10% emptying of the labeled meal (Tlag), the gastric emptying coefficient and the regression-estimated constants (β and k), calculated by using the 13CO2 breath excretion curve using the conventional formulae. The parameters between the 2 test conditions were compared statistically. Results No significant differences in the calculated parameters, including T1/2, Tlag, gastric emptying coefficient or β and k, were observed between the 2 test conditions. Conclusions The present study revealed that single-dose sitagliptin intake had no significant influence on the rate of liquid gastric emptying in asymptomatic volunteers.
