Cutaneous Nontuberculous Mycobacterial Infection: A Clinicopathological Study of 7 Cases

Nontuberculous mycobacteria (NTM) are human opportunistic pathogens with an environmental source of infection. The reports of cutaneous NTM infections has increased, and NTM have been regarded as important pathogens in recent years. This study aimed to identify characteristic clinical and histological features of cutaneous NTM infections. We evaluated the medical records and histopathologic slides of 7 cases of NTM infections that were confirmed by polymerase chain reaction between 2003 and 2007. The results showed that cutaneous NTM infections occurred in various aged people independent of their immune states and were associated with fish-related jobs or intramuscular medicinal injection. The main clinical feature was a painful erythematous nodule. Histopathologically, the most common findings were diffuse infiltration of mixed inflammatory cells and small vessel proliferation in the dermis (100%). Epidermal proliferation (71%) and dermal granuloma (71%) were also very common. Suppurative granuloma was found in 43% of the cases, and eosinophil infiltration was uncommon (14%). The lesions disappeared after a mean of 7 months (range, 1.5-12 months) with treatment by oral clarithromycin alone or in combination with other antimycobacterial agents. These clinical and histopathological data should assist clinicians in the diagnosis of NTM.

Objective: The purpose of this study was to compare the pregnancy outcomes of history-indicated cerclage (HIC) and transabdominal cerclage (TAC) in twin pregnancy. Methods: Retrospective chart review of twin pregnancies with HIC or TAC was performed. The study population underwent cerclage at Hallym University Kangnam Sacred Heart Hospital from 2007 to 2018. We compared the pregnancy outcomes between HIC and TAC group; The cerclage-to-delivery interval, the mean gestational age (GA) at cerclage, the mean GA at delivery, and birth weight were analyzed; The incidence of preterm birth (PTB) less than 28 weeks and 34 weeks, neonatal mortality were also compared between two groups. Results: Fifty-two patients were included; 19 patients underwent HIC, and 32 underwent TAC. There was no difference in the mean GA at cerclage between two groups (HIC, 13.73 weeks vs. TAC, 13.83 weeks, p=0.574). The mean GA at delivery in HIC group was 31.77 weeks, and that in TAC group was 35.85 weeks (p=0.003). There was statistically significant difference in the mean cerclage-to-delivery interval between two groups (HIC, 131.21days vs. TAC, 154days, p= 0.032) There were significant difference in neonatal birthweight between two groups (1st baby, 1846.84g vs. 2417.27g (p=0.004); 2nd baby 1732.11g vs. 2353.33, (p=0.001)), and neonatal mortality (HIC 4/19(21.1%) vs. TAC 1/33(3.0%), p=0.054). TAC group showed lower incidence of PTB < 28 weeks compared to HIC group (HIC 4/19 (21.1%) vs. TAC 1/33(3.0%), p=0.054). There was statistical difference in the incidence of PTB < 34weeks (HIC group, 8/19(42.1 %) vs. TAC group, 2/33(6.1 %), p=0.003). Conclusion: Both HIC and TAC were beneficial in prolonging pregnancy in twin pregnancy. TAC in twin pregnancy showed more favorable obstetric outcomes when compared to HIC.

This paper proposes an efficient way of compressing parameters needed for hidden Markov model (HMM)-based statistical parametric speech synthesis (SPSS) systems. By analyzing the statistical characteristics of HMM model parameters, this paper proposes various quantization methods to minimize the footprint of HMM model parameters. Subjective and objective test results verify that the proposed algorithm not only reduces the memory size, but also maintains the perceptual quality of synthesized speech.
Optimal Power Allocation Scheme for Energy Efficiency Maximization in Distributed Antenna Systems

<P>In this paper, we present a power allocation method for a distributed antenna system (DAS) to maximize energy efficiency (EE), which is defined as the ratio of the transmission rate to the total consumed power. Different from conventional EE maximization schemes that require iterative numerical methods, we derive the optimal solution as a closed form by solving Karush-Kuhn-Tucker conditions. The obtained closed-form expression is applicable to DAS with an arbitrary number of distributed antenna (DA) ports and general per-DA port power constraints and is also guaranteed to be globally optimum. Then, we provide several interesting observations on the proposed EE maximizing power allocation scheme. Based on these results, we propose a simplified practical power allocation method that employs the DA port selection and computes the power level in a distributed manner. Through Monte Carlo simulations, we show that the proposed optimal power allocation method produces the EE identical to exhaustive search with significantly reduced computational complexity. In addition, it is shown that the proposed simplified power allocation method based on the DA port selection exhibits little performance loss compared to the optimal algorithm with a remarkable reduction in the system overhead.</P>

BACKGROUND/OBJECTIVES: Citrus flavonoids have a variety of physiological properties such as anti-oxidant, anti-inflammation, anti-cancer, and anti-obesity. We investigated whether bioconversion of Citrus unshiu with cytolase (CU-C) ameliorates the anti-adipogenic effects by modulation of adipocyte differentiation and lipid metabolism in 3T3-L1 cells. MATERIALS/METHODS: Glycoside forms of Citrus unshiu (CU) were converted into aglycoside forms with cytolase treatment. Cell viability of CU and CU-C was measured at various concentrations in 3T3L-1 cells. The anti-adipogenic and lipolytic effects were examined using Oil red O staining and free glycerol assay, respectively. We performed real time-polymerase chain reaction and western immunoblotting assay to detect mRNA and protein expression of adipogenic transcription factors, respectively. RESULTS: Treatment with cytolase decreased flavanone rutinoside forms (narirutin and hesperidin) and instead, increased flavanone aglycoside forms (naringenin and hesperetin). During adipocyte differentiation, 3T3-L1 cells were treated with CU or CU-C at a dose of 0.5 mg/ml. Adipocyte differentiation was inhibited in CU-C group, but not in CU group. CU-C markedly suppressed the insulin-induced protein expression of CCAAT/enhancer-binding protein α (C/EBPα) and peroxisome proliferator-activated receptor gamma (PPARγ) as well as the mRNA levels of CEBPα, PPARγ, and sterol regulatory element binding protein 1c (SREBP1c). Both CU and CU-C groups significantly increased the adipolytic activity with the higher release of free glycerol than those of control group in differentiated 3T3-L1 adipocytes. CU-C is particularly superior in suppression of adipogenesis, whereas CU-C has similar effect to CU on stimulation of lipolysis. CONCLUSIONS: These results suggest that bioconversion of Citrus unshiu peel extracts with cytolase enhances aglycoside flavonoids and improves the anti-adipogenic metabolism via both inhibition of key adipogenic transcription factors and induction of adipolytic activity.

As the demand on ubiquitous communication increases, global roaming and vertical handover will be prevailing in the near future. Since this environment is accompanied by the frequent handovers at remote sites, a scalable and fast authentication becomes prerequisite for ubiquitous communication. In this paper, we suggest a framework for scalable and fast authentication, using hierarchical caching based on general trust relationship among domains. At the end, we show that the proposed scheme achieves reduced authentication delay and network overhead through an analytic method with fluid flow model. 국제 로밍 및 이기종 망간 핸드오버가 활성화된 환경에서 사용자에게 부드러운 통신 서비스를 지원하기 위해서는 보다 빠르고 확장성이 뛰어난 인증 기법이 필요하다. 본 논문은 빠른 로밍을 지원하기 위한 확장성 높은 인증 프레임워크에 관한 것으로, 직간접적으로 구성된 도메인간의 일반적인 신뢰 관계를 바탕으로 계층적 캐슁을 구성한다. 계층적 인증 캐슁을 이용할 경우 확장성을 보장함은 물론 인증 지연 및 인증으로 인한 망의 부하를 줄일 수 있으며, 이를 수학적으로 분석하였다.

In triplet pregnancy, preterm delivery is a major cause of neonatal morbidity and mortality. If mother was suffering from cervical insufficiency, the effectiveness and safety of cervical cerclage remains controversial. The authors experienced the case of a 29-year-old patient with triplet pregnancy who had dilated cervix and bulging fetal membranes at 20+4 weeks of gestation. The patient underwent emergency cervical cerclage and carried pregnancy until 26th weeks of gestation. The babies were delivered by cesarean section in the 26th week of gestation and they were discharged home at the 40 weeks of corrected age. In triplet gestation as well as single or twin gestation, cervical cerclage in the patients with cervical dilatation and bulging fetal membrane could be successful if obstetricians and neonatologists collaborate.
