CD64 Expression Is Increased in Patients with Severe Acute Pancreatitis: Clinical Significance

Background/Aims: Upregulated CD64 expression on neutrophils is the most useful marker for acute bacterial infections and systemic inflammation. However, it is unknown whether CD64 is involved in the pathogenesis of acute pancreatitis (AP). This study was designed to determine whether CD64 is implicated in severe acute pancreatitis (SAP), and thus, is a suitable marker for SAP. Methods: SAP was induced in rats with an intraperitoneal injection of L-arginine. CD64 expression in the rat pancreas was determined by quantitative real-time polymerase chain reaction (qRT-PCR) and immunohistochemistry. Additionally, the CD64 mRNA expression in peripheral blood leukocytes from 21 patients with mild acute pancreatitis (MAP) and 10 patients with SAP was investigated at the time of admission and during remission by qRT-PCR. Results: CD64 mRNA and protein expression in the pancreas was significantly higher in rats with SAP, compared to the controls. The CD64 expression was higher in the patients with SAP than in the patients with MAP. During remission, CD64 mRNA decreased in both the MAP and SAP patients. The area under the curve of CD64 expression for the detection of SAP was superior to both the Ranson and the Acute Physiology and Chronic Health Evaluation II scores. Conclusions: The CD64 level was significantly increased in correlation with the disease severity in SAP and may act as a useful marker for predicting the development of SAP. A new androstane derivative, $4{\beta}$-methyl-15-oxa-$14{\beta}$-androstane-7-ene-$4{\alpha}$-carboxylic acid (1) and a known one $4{\beta}$-methyl-15-oxa-$14{\beta}$-androstane-7-ene-$4{\alpha}$-hydroxyl (2) were isolated from the EtOAc extract of the cultures of the fungus Marasmiellus ramealis (Bull.) Singer. Their structures were elucidated on the basis of 1D and 2D NMR as well as MS spectroscopic data analysis. The inhibitory activity of two isolates against acetylcholinesterase (AChE) revealed that compound 1 exhibited definitely inhibitory activity.

<P>Non-cryogenic protein structures determined at ambient temperature may disclose significant information about protein activity. Chloride-pumping rhodopsin (ClR) exhibits a trend to hyperactivity induced by a change in the photoreaction rate because of a gradual decrease in temperature. Here, to track the structural changes that explain the differences in CIR activity resulting from these temperature changes, we used serial femtosecond crystallography (SFX) with an X-ray free electron laser (XFEL) to determine the non-cryogenic structure of ClR at a resolution of 1.85 Å, and compared this structure with a cryogenic ClR structure obtained with synchrotron X-ray crystallography. The XFEL-derived ClR structure revealed that the all-<I>trans</I> retinal (ATR) region and positions of two coordinated chloride ions slightly differed from those of the synchrotron-derived structure. Moreover, the XFEL structure enabled identification of one additional water molecule forming a hydrogen bond network with a chloride ion. Analysis of the channel cavity and a difference distance matrix plot (DDMP) clearly revealed additional structural differences. B-factor information obtained from the non-cryogenic structure supported a motility change on the residual main and side chains as well as of chloride and water molecules because of temperature effects. Our results indicate that non-cryogenic structures and time-resolved XFEL experiments could contribute to a better understanding of the chloride-pumping mechanism of ClR and other ion pumps.</P>
Research on Discontinuous Pulse Width Modulation Algorithm for Single-phase Voltage Source Rectifier

Single phase voltage source converter (VSC) is an important power electronic converter (PEC), including single-phase voltage source inverter (VSI), single-phase voltage source rectifier (VSR), single-phase active power filter (APF) and single-phase grid-connection inverter (GCI). As the fundamental part of large scale PECs, single-phase VSC has a wide range of applications. In the paper, as first, on the basis of the concept of the discontinuous pulse-width modulation (DPWM) for three-phase VSC, a new DPWM of single-phase VSR is presented by means of zero-sequence component injection. Then, the transformation from stationary frame (abc) to rotating frame (dq) is designed after reconstructing the other orthogonal current by means of one order all-pass filter. Finally, the presented DPWM based single-phase VSR is established analyzed and simulated by means of MATLAB/SIMULINK. In addition, the DPWMs presented by D. Grahame Holmes and Thomas Lipo are discussed and simulated in brief. Obviously, the presented DPWM can also be used for single-phase VSI, GCI and APF. The simulation results show the validation of the above modulation algorithm, and the DPWM based single-phase VSR has reduced power loss and increased efficiency.

Purpose We sought to investigate the effectiveness and safety of dabrafenib in children with BRAFV600E-mutated Langerhans cell histiocytosis (LCH). Materials and Methods A retrospective analysis was performed on 20 children with BRAFV600E-mutated LCH who were treated with dabrafenib. Results The median age at which the patients started taking dabrafenib was 2.3 years old (range, 0.6 to 6.5 years). The ratio of boys to girls was 2.3:1. The median follow-up time was 30.8 months (range, 18.9 to 43.6 months). There were 14 patients (70%) in the risk organ (RO)+ group and six patients (30%) in the RO– group. All patients were initially treated with traditional chemotherapy and then shifted to targeted therapy due to poor control of LCH or intolerance to chemotherapy. The overall objective response rate and the overall disease control rate were 65% and 75%, respectively. During treatment, circulating levels of cell-free BRAFV600E (cfBRAFV600E) became negative in 60% of the patients within a median period of 3.0 months (range, 1.0 to 9.0 months). Grade 2 or 3 adverse effects occurred in five patients. Conclusion Some children with BRAFV600E-mutated LCH may benefit from monotherapy with dabrafenib, especially high-risk patients with concomitant hemophagocytic lymphohistiocytosis and intolerance to chemotherapy. The safety of dabrafenib is notable. A prospective study with a larger sample size is required to determine the optimal dosage and treatment duration.
Sleep Duration and Cancer Risk: a Systematic Review and Meta-analysis of Prospective Studies

To assess the risk of cancers associated with sleep duration using meta-analysis of published cohort studies, we performed a comprehensive search using PubMed, Embase and Web of Science through October 2013. We combined hazard ratios (HRs) from individual studies using meta-analysis approaches. A random effect dose-response analysis was used to evaluate the relationship between sleep duration and cancer risk. Subgroup analyses and sensitivity analyses were also performed. Publication bias was evaluated using Funnel plots and Begg's test. A total of 13 cohorts from 12 studies were included in this meta-analysis, which included 723, 337 participants with 15, 156 reported cancer outcomes during a follow-up period ranging from 7.5 to 22 years. The pooled adjusted HRs were 1.06 (95% CI: 0.92, 1.23; P for heterogeneity =0.003) for short sleep duration, 0.91 (95% CI: 0.78, 1.07; P for heterogeneity <0.0001) for long sleep duration. In subgroup analyses stratified by cancer type, long duration of sleep showed an inverse relation with hormone-related cancer (HR=0.79; 95% CI: 0.65, 0.97; P for heterogeneity =0.009) and a greater risk of colorectal cancer (HR=1.29; 95% CI: 1.09, 1.52; P for heterogeneity =0.346). Further meta-analysis on dose-response relationships showed that the relative risks of cancer were 1.00 (95% CI: 0.99, 1.01; P for linear trend=0.9151) for one hour of sleep increment per day, and 1.00 (95% CI: 0.98, 1.01; P for linear trend=0.7749) for one hour of sleep increment per night. No significant dose-response relationship between sleep duration and cancer was found on non-linearity testing (P=0.5053). Our meta-analysis suggests a positive association between long sleep duration and colorectal cancer, and an inverse association with incidence of hormone related cancers like those in the breast. Studies with larger sample size, longer follow-up times, more cancer types and detailed measure of sleep duration are warranted to confirm these results.
Design and Implementation of a USN Middleware for Context-Aware and Sensor Stream Mining

Recently, with the advances in sensor techniques and net work computing, Ubiquitous Sensor Network (USN) has been received a lot of attentions from various communities. The sensor nodes distributed in the sensor network tend to continuously generate a large amount of data, which is called stream data. Sensor stream data arrives in an online manner so that it is characterized as high-speed, real-time and unbounded and it requires fast data processing to get the up-to-date results. The data stream has many application domains such as traffic analysis, physical distribution, U-healthcare and so on. Therefore, there is an overwhelming need of a USN middleware for processing such online stream data to provide corresponding services to diverse applications. In this paper, we propose a novel USN middleware which can provide users both context-aware service and meaningful sequential patterns. Our proposed USN middleware is mainly focused on location based applications which use stream location data. We also show the implementation of our proposed USN middleware. By using the proposed USN middleware, we can save the developing cost of providing context aware services and stream sequential patterns mainly in location based applications.
Dielectric and Conductivity Properties of Poly(L-lactide) and Poly(L-lactide)/Ionic Liquid Blends

Dielectric relaxation spectroscopy (DRS) was employed to study the dielectric and conductivity propertiesof poly(L-lactide) and poly(L-lactide)/ionic liquid (IL) blends. The experimental dielectric data were analyzedwithin the formalism of complex permittivity. The results were discussed in terms of AC conductivity, αN relaxationcorresponding to the longest normal mode motion, α relaxation originating from the segmental mode motion, andDC conductivity. The results revealed that the motion of the polymer chains governs the charge carrier transport. The temperature dependence of the relaxation times follows the Vogel-Tammann-Fulcher (VTF) equation. Theincorporation of IL into the matrix accelerates the segmental (α) and normal mode (αN) motions and the conductivity,and increases the charge carrier movement of IL following the longest αN motion of chains, which led to an increasein the relaxation strength of the αN relaxation.
