      • Is Surgical Resection Justified for Hepatocellular Carcinoma with Portal Vein Tumor Thrombus? (A Systematic Review and Meta-Analysis)

        Aims: The prognosis of hepatocellular carcinoma (HCC) with portal vein tumor thrombus (PVTT) is very poor. According to the BCLC treatment recommendations, sorafenib or other palliative treatment (PT) is recommended as the first-line therapy when it happens. In real world, however, a significant number of selected patients with HCC and PVTT suffered from surgical resection (SR). Methods: PubMed, Embase, Medline and Cochrane library were searched for studies comparing SR with PT (including TACE, sorafenib, etc.) for HCC with PVTT, which were published before September 2017. Results: 4,810 patients from 7 studies were enrolled in this meta-analysis, which divided into the SR group (n = 2,344) and the PT group (n = 2476). When compared with the PT group, the pooled hazard ratio (HR) for the 1, 3 and 5-year OS rates of the SR group were 0.56 (95% CI 0.52-0.60, P=0.03), 0.56 (95% CI 0.53-0.59, P<0.001) and 0.55 (95% CI 0.54-0.57, P<0.001). For subgroup analysis, when compared with the mere TACE group, the pooled HR for the 1, 3 and 5-year OS rates of the SR group were 0.54 (95% CI 0.43-0.67, P=0.81), 0.75 (95% CI 0.65-0.87, P=0.25) and 0.76 (95% CI 0.67-0.88, P=0.25). Conclusions: This meta-analysis demonstrated SR had better OS than TACE or other palliative therapy for HCC with PVTT. SR may be suitable as the first-line treatment for selected patients with resectable HCC and removable PVTT.

        Reinforced high-strength concrete square columnsconfined by aramid FRP jackets. part II: modeling

        Based on the experimental data presented in part I of these companion papers, a semi-empirical model is proposed for axial stress-strain curves of reinforced high-strength concrete square columns confined by aramid fiber reinforced polymer (FRP) jackets. Additionally, a three-dimensional finite element model is developed to simulate the mechanical behaviors of the columns. In the finite element model, both material nonlinear and contact nonlinear are taken into account. Moreover, the influence of contact nonlinear (i.e., the end friction on the contact surface between test machines and specimens) is investigated deeply. Predictions from both the semi-empirical model and the finite element model agree with the experimental results, and it is also demonstrated that the friction coefficient of end friction notably affect the properties of columns when it ranges from 0.00 to 0.25.

        Experimental study on reinforced high-strength concrete short columns confined with AFRP sheets

        This paper is aiming to study the performances of reinforced high-strength concrete (HSC) short columns confined with aramid fibre-reinforced polymer (AFRP) sheets. An experimental program, which involved 45 confined columns and nine unconfined columns, was carried out in this study. All the columns were circular in cross section and tested under axial compressive load. The considered parameters included the concrete strength, amount of AFRP layers, and ratio of hoop reinforcements. Based on the experimental results, a prediction model for the axial stress-strain curves of the confined columns was proposed. It was observed from the experiment that there was a great increment in the compressive strength of the columns when the amount of AFRP layers increases, similar as the ultimate strain. However, these increments were reduced as the concrete strength increasing. Comparisons with other existing prediction models present that the proposed model can provide more accurate predictions

        Multiscale modeling for compressive strength of concrete columns with circular cross-section

        In order to construct a multiscale model for the compressive strength of plain concrete columns with circular cross section subjected to central longitudinal compressive load, a column failure mechanism is proposed based on the theory of internal instability. Based on an energy analysis, the multiscale model is developed to describe the failure process and predict the column’s compressive strength. Comparisons of the predicted results with experimental data show that the proposed multiscale model can accurately represent both the compressive strength of the concrete columns with circular cross section, and the effect of column size on its strength.

        Nomogram for predicting overall survival in children with neuroblastoma based on SEER database

        Purpose: This study was performed to establish and validate a nomogram for predicting the overall survival in children with neuroblastoma. Methods: The latest clinical data of neuroblastoma in Surveillance, Epidemiology, and End Results (SEER) database was extracted from 2000 to 2016. The cases included were randomly divided into training and validation cohorts. The survival curves were drawn with a Kaplan-Meier estimator to investigate the influences of certain single factors on overall survival. Also, least absolute shrinkage and selection operator regression was applied to further select the prognostic variables for neuroblastoma. Additionally, receiver operating characteristic (ROC) curves and calibration curves were used to evaluate the accuracy of the nomogram. Results: In total, 1,262 patients were collected and 8 independent prognostic factors were achieved, including patients’ age, sex, race, tumor grade, radiotherapy, chemotherapy, tumor site, and tumor size. Then we constructed a nomogram by using the data of the training cohort with 886 cases. Subsequently, the nomogram was validated internally and externally with 886 and 376 cases, respectively. The internal validation revealed that the area under the curves (AUC) of ROC curves of 1-, 3-, and 5-year overall survival were 0.69, 0.78, and 0.81, respectively. Accordingly, the external validation also showed that the AUC of 1-, 3-, and 5-year overall survival were all ≥0.69. Both methods of validation demonstrated that the predictive calibration curves were consistent with standard curves. Conclusion: The nomogram possess the potential to be a new tool in predicting the survival rate of neuroblastoma patients.

        Computation Controllable Mode Decision and Motion Estimation for Scalable Video Coding

        This paper proposes an efficient computation-aware mode decision and search point (SP) allocation algorithm for spatial and quality scalabilities in Scalable Video Coding. In our proposal, a linear model is derived to allocate the computation for macroblocks in enhancement layers by using the rate distortion costs of the base layer. In addition, an adaptive SP decision algorithm is proposed to decide the number of SPs for motion estimation under the constraint of the allocated computation. Experiment results demonstrate that the proposed algorithm allocates the computation resource efficiently and outperforms other works in rate distortion performance under the same computational availability constraint.

        The miR-98-3p/JAG1/Notch1 axis mediates the multigenerational inheritance of osteopenia caused by maternal dexamethasone exposure in female rat offspring

        As a synthetic glucocorticoid, dexamethasone is widely used to treat potential premature delivery and related diseases. Our previous studies have shown that prenatal dexamethasone exposure (PDE) can cause bone dysplasia and susceptibility to osteoporosis in female rat offspring. However, whether the effect of PDE on bone development can be extended to the third generation (F3 generation) and its multigenerational mechanism of inheritance have not been reported. In this study, we found that PDE delayed fetal bone development and reduced adult bone mass in female rat offspring of the F1 generation, and this effect of low bone mass caused by PDE even continued to the F2 and F3 generations. Furthermore, we found that PDE increases the expression of miR-98-3p but decreases JAG1/Notch1 signaling in the bone tissue of female fetal rats. Moreover, the expression changes of miR-98-3p/JAG1/Notch1 caused by PDE continued from the F1 to F3 adult offspring. Furthermore, the expression levels of miR-98-3p in oocytes of the F1 and F2 generations were increased. We also confirmed that dexamethasone upregulates the expression of miR-98-3p in vitro and shows targeted inhibition of JAG1/Notch1 signaling, leading to poor osteogenic differentiation of bone marrow mesenchymal stem cells. In conclusion, maternal dexamethasone exposure caused low bone mass in female rat offspring with a multigenerational inheritance effect, the mechanism of which is related to the inhibition of JAG1/Notch1 signaling caused by the continuous upregulation of miR-98-3p expression in bone tissues transmitted by F2 and F3 oocytes.

        Investigation of muscle-specific beef color stability at different ultimate pHs

        Objective: This study was aimed to investigate the muscle-specific beef color stability at normal and high ultimate pHs. Methods: The impact of muscle (Longissimus lumborum [LL] vs psoas major [PM]) and pH (normal ultimate pH [Np] vs high pH dark cutting beef [Hp]) on color stability, indicated by basic color traits, metmyoglobin reducing activity (MRA) and oxygen consumption (OC), as well as the lipid oxidation, were determined over 7 days of display at 4°C. Results: Hp-LL had the highest pH (6.92), followed by Hp-PM (6.01), Np-PM (5.76), and Np-LL (5.52). Hp-LL had increased (p<0.05) a*, chroma and % oxymyoglobin during display. Hp-LL also had the highest metmyoglobin (MMb) reducing activity and OC among all the samples, thus, the greatest color stability, although very dark throughout storage, with lowest values for lightness (L*) and yellowness (b*). Np-LL also exhibited relatively high color stability, as a result of its lower % MMb and OC and higher MRA than psoas muscle samples. The 0.2 unit difference of the pH between Hp and Np psoas muscle, resulted in the difference of the color intensity, not the color stability. Interestingly, high pH psoas muscle (Hp-PM) did not have better color stability than Np-PM, and in fact had lower color stability than even Np-LL. The similar level of OC and lipid oxidation cannot explain the difference in color stability between Hp-PM and Np-LL. Conclusion: The Hp does not always show better color stability compared with Np beef, which depends on the muscle type. The balance of MRA and OC is important to keep the color in great intensity and stability in the meantime.

        Role of Selenium in Alteration of Erythrocyte Parameters in Bovine Fluorosis

        Signs of dental discolouration, difficulty in mastication, bony exostosis and debility were observed in cattle from Qingtongxia Ningxia, China where fluoride concentration in drinking water, soil, fodder, serum, bone, teeth, haircoat and urine were significantly higher than the corresponding health site. The problem of fluorosis in beef cattle is attributable to water containing toxic levels of fluoride. The objective of this paper was therefore to evaluate the influence of fluoride on erythrocyte parameters in cattle under high fluoride and low selenium conditions, as well as the protective efficacy of selenium exposure in feedstuff for bovine endemic fluorosis. Sixteen 6 to 7 year-old high fluoride beef cattle were randomly allotted into two groups each with eight cows: high fluoride control group, and supplemented with 0.25 mg/kg selenium per day for 83 days respectively. In addition, eight 6 to 7 year-old normal control beef cattle were selected from a non-high fluoride site. Blood samples were collected on day 0, 30 and 83 for erythrocyte parameters analysis and scanning electronic microscopy. The results indicated that erythrocytes, hemoglobin, packed cell volume values and $Na^+-K^+$ ATPase activity from affected cattle on the high fluoride site were significantly reduced during the period as compared with the corresponding samples of normal control cattle, a great number of echinocytes were present in peripheral blood, and subsequent anaemia. However, affected cattle exposed to selenium revealed increasable erythrocyte parameters, the extent of elevation in these values being dependent on the duration of supplementation with selenium. These findings suggest that fluoride exposure can cause erythrocyte damage, whereas selenium supplementation can antagonize fluoride-induced generation of free radicals and cumulative effects of lipid peroxidation in erythrocytes. Selenium supplementation may help to alleviate the possible hazards associated with bovine endemic fluorosis.
