알레르기 ; 알레르기 환자에서 한방 이외의 대체 요법의 사용 현황에 대한 다기관 조사

목적: 한국에서 알레르기 질환 치료를 위한 한방 치료뿐만 아니라 대체 요법이 많이 이용되고 있으며, 그 현황에 대하여 다기관 조사를 시행하였다. 방법: 10개의 대학병원 외래로 내원한 647명의 알레르기 환자를 대상으로 설문조사를 시행하였으며, 이 중 510명이 한방 이외의 대체 요법에 응답하였다. 이환되어 있는 알레르기 질환은 천식(50.0%), 알레르기 비염(36.0%), 아토피 피부염(36.0%), 그리고 두드러기(9.3%)였다. 각각 환자들에게 12 문항의 설문조사를 시행하였으며, 한방 이외의 대체 요법의 사용 빈도 및 종류, 대체 요법에 의지하게 된 이유, 비용, 그리고 치료효과에 대한 의견을 물어 보았다. 결과: 16.7%의 알레르기 환자에서 한방 이외의 다른 대체 요법을 받았으며, 평균 1.6종의 한방 이외의 대체 요법을 받았다. 가장 많이 의지하는 대체 요법은 민간 식이요법 (48.6%)이었고, 그 다음이 아로마 요법(23.8%), 지압(14.3%), 건강 호흡법(7.6%), 마인드 컨트롤(5.7%), 목욕요법(5.7%) 순이었다. 대체 요법을 받은 환자 중 마인드 컨트롤(50%), 식이요법(50%), 그리고 건강 호흡법(75%)의 경우 치료 효과가 만족스럽다는 응답이 높았으며, 목욕요법, 아로마 요법, 그리고 지압은 만족도가 각각 33.3%, 28.0%, 26.7%로 저조하였다. 대체 요법 사용자의 63.6%가 5회 미만의 대체 요법을 받았고, 91.8%가 1년 이내로 치료받았으며, 1인당 평균 연 37 만원을 지출하였다. 대체 요법을 선택한 가장 큰 이유는 체질이 개선될 수 있을까 해서(44.2%)였으며, 대체 요법이 몸에 부담이 없어서(31.4%), 면역기능이 강화되어서(22.9%) 순으로 나타났고, 15.7%는 양약치료가 도움이 되지 않아서 대체 요법을 선택하였다고 응답하였다. 결론: 한국에서 알레르기 환자의 상당수가 한방뿐만 아니라 여러 가지의 대체 요법을 사용하고 있음을 알 수 있었다. 대체 요법의 사용 빈도가 증가하고 그 종류가 다양함을 고려할 때, 의료진은 대체 요법의 현황에 대하여 정확히 인지하고 이에 대해 관심을 가지고 환자를 대하여야 하며 올바른 알레르기 질환 치료법에 대해 환자뿐만 아니라 의료인에게 교육하는 것이 필요하다. Background/Aims: The use of unproven complementary/alternative medicine (CAM) to treat allergies is popular in Korea. We conducted a multicenter survey of the current use of CAM other than herbal medication in Korean allergy patients. Methods: This study enrolled 510 adults with allergic diseases, including asthma, allergic rhinitis, atopic dermatitis, and chronic urticaria, from ten hospitals. They underwent a structured questionnaire interview and clinical assessment of the prevalence, motivation, costs, and subjective assessment of CAM. Results: Of the patients, 16.7% used at least one type of CAM to treat their allergic diseases. Common types of CAM were diet (48.6%), aroma therapy (23.8%), massage (14.3%), breathing exercises (7.6%), baths (5.7%), and mind control (5.7%). The therapeutic effects of CAM included improved in patients using mind control (50%), diet (50%), breathing exercises (75%), and baths (33.3%), but worse in patients using aroma therapy (28%), and massage (26.7%). About 36.4% of the patients used CAM more than four times per year, and the average cost per CAM user was 370,000 won/year. The main reason for trying CAM was `hope to improve my constitution`. Conclusions: CAM is used widely for treating allergic diseases in Korea. Detailed knowledge of CAM and patient education are important. Further studies of the clinical efficacy of CAM are needed. Purpose The impact of BCG scar on the development of nontuberculous mycobacterial pulmonary disease (NTM-PD) ant its clinical outcomes remains unclear. The aim of this study is to clarify the impact of BCG scar on NTM-PD occurrence and its clinical outcomes. Method NTM-PD patients (cases), who participated in Seoul National University Hospital (SNUH)’s prospective NTM-PD cohort and healthy volunteers (Controls), who were recruited for SNUH healthcare system Gangnam center, were used for case-control study. Controls were matched to cases us according to age, sex and smoking habit to investigate the association between BCG scar and NTM-PD development. The presence of BCG scar was measured by independent examiners. The impact of BCG scar on clinical outcomes of NTM-PD were measured in NTM-PD cohort using multivariable logistic regression analysis. Results A total of 499 patients (409 with BCG scar and 90 without BCG scar) participated in the prospective cohort. and 369 patients were matched to controls. According to the case control study, low body mass index (adjusted odds ratio [aOR] 0.75, 95% confidence interval [CI] 0.67-0.85) and history of tuberculosis (aOR 12.8, 95% CI 4.21-39.1) were associated with NTM-PD, while BCG scar was not associated with the development of NTM-PD (P=0.064). Anti-mycobacterial treatment was more frequently adopted in patients with BCG scar (P=0.028) However, BCG scar was not associated with treatment initiation (aOR= 1.61, 95% CI 0.92-2.81, P=0.094) or microbiological cure (aOR= 0.69, 95% CI 0.23-2.11, P=0.521) Conclusion BCG vaccination is not associated with the development of NTM-PD and its clinical outcomes.

Aims: Acute variceal bleeding is a fatal complication for cirrhotic patients and endoscopic band ligation (EVL) is recommended for treatment. But there are sporadic data for mortality in patients underwent EVL of acute variceal bleeding. We developed a novel bed-side scoring model to predict 30 days’ mortality for cirrhotic patients who underwent EVL of acute variceal bleeding. Methods: From a Cox-regression model for 30 days’ mortality, variables were developed among baseline characteristics, history of complication, the presence of hepatocellular carcinoma, ascites and endoscopic finding from the derivation cohort (n=1372) underwent EVL for acute variceal bleeding. The model’s prognostic performance was compared with CTP and MELD score and assessed in validation cohort. (n=200). Results: Among 1372 patients, rebleeding or death within 5 days was observed in 51 patients (3.7%) and 72 patients (5.2%), respectively. The validation cohort showed more aggressive clinical outcome than derivation cohort in term of endoscopic active bleeding, shock at presentation and treatment failure.From a multivariate Cox-regression model, four objective variables (the presence of HCC, CTP class (A/B vs. C), the shock at presentation and the history of hepatic encephalopathy) were developed and scored to generate an 11-point risk prediction model. The scoring model was able to stratify patient 30 days’ mortality as low (4.25%), medium (20.9%) and high (42.3%) risk group. (P<0.001) The time-dependent area under receiver-operating characteristics curves (AUROCs) for 30days mortality were higher in risk prediction model than CPT score. (derivation cohort: 0.79 vs 0.73, validation cohort:0.82 vs 0.75, P<0.001). Even in the group of same MELD (<15 or ≥15), new scoring model also stratified 30 days’ mortality to three groups with statistically significant in derivation and validation cohort. Conclusions: New simplified scoring system was useful in estimating 30days mortality for patients who underwent EVL for acute variceal bleeding. This score can be useful in planning and guiding further strategies after EVL which warrants prospective validation.

<P>Synthetic biopolymers are commonly used for the repair and regeneration of damaged tissues. Specifically targeting bone, the composite approach of utilizing inorganic components is considered promising in terms of improving mechanical and biological properties. We developed gelatin-apatite co-precipitates which mimic the native bone matrix composition within poly(lactide-<I>co</I>-caprolactone) (PLCL). Ionic reaction of calcium and phosphate with gelatin molecules enabled the co-precipitate formation of gelatin-apatite nanocrystals at varying ratios. The gelatin-apatite precipitates formed were carbonated apatite in nature, and were homogeneously distributed within the gelatin matrix. The incorporation of gelatin-apatite significantly improved the mechanical properties, including tensile strength, elastic modulus and elongation at break, and the improvement was more pronounced as the apatite content increased. Of note, the tensile strength increased to as high as 45 MPa (a four-fold increase vs. PLCL), the elastic modulus was increased up to 1500 MPa (a five-fold increase vs. PLCL), and the elongation rate was ∼240% (twice vs. PLCL). These results support the strengthening role of the gelatin-apatite precipitates within PLCL. The gelatin-apatite addition considerably enhanced the water affinity and the acellular mineral-forming ability in vitro in simulated body fluid; moreover, it stimulated cell proliferation and osteogenic differentiation. Taken together, the GAp-PLCL nanocomposite composition is considered to have excellent mechanical and biological properties, which hold great potential for use as bone regenerative matrices.</P>

<P>Ionization channel formation is observed in an atmospheric-pressure plasma jet. A radial electric field is applied between coaxial electrodes. Helium flows in the axial direction through an inner metal tube electrode. In this device configuration, time-resolved images show that a plasma bullet can propagate only when the inner electrode is an anode. This phenomenon is experimental evidence that the bullet head is a positive space charge region. Moreover, it supports the hypothesis that the mechanism of bullet propagation is similar to that of a positive streamer. A comet-tail-like feature appears during the bullet propagation. This interesting shape occurs probably because of the Penning ionization near the helium-air boundary.</P>
Effects of 4-Nonlyphenol Exposure on $P.$ $olivaceus$ and $S.$ $schlegeli$ Vitellogenesis

The effects of the estrogenic compound 4-nonylphenol (4-NP) on vitellogenesis in juvenile olive flounder ($Paralichthys$ $olivaceus$) and rockfish ($Sebastes$ $schlegeli$) exposed continuously at 10, 50 and $100{\mu}g{\ell}^{-1}$ levels for 7 days were compared. The expression of VTG mRNA level and protein using specific probes were examined. The levels of plasma estradiol-17 ${\beta}(E_2)$ and testosterone (T) were assessed by radioimmunoassay (RIA). Plasma $E_2$ concentrations increased significantly in two female fish species exposed to $100{\mu}g{\ell}^{-1}$ of 4-NP over concentrations in control fish. Plasma T concentrations increased in $P.$ $olivaceus$. Four days after exposure, the level of VTG mRNA expression increased in $P.$ $olivaceus$ and $S.$ $schlegeli$ exposed to $20{\mu}g{\ell}^{-1}$ of 4-NP. In addition, plasma VTG protein expression was seen in $P.$ $olivaceus$ and $S.$ $schlegeli$. In $S.$ $schlegeli$ and $P.$ $olivaceus$ exposed to 4-NP, the changes were noticed mainly in hepatocytic vaculation after 7 days of exposure. Thus, 4-NP may disrupt vitellogenesis in immature fish both directly and indirectly via disrupted steroidogenesis and liver pathology. Immature $S.$ $schlegeli$ were the most sensitive to 4-NP exposure in vitellogenesis.
Imatinib Mesylate plus Hydroxyurea Chemotherapy for Cerebellar Meningioma in a Belgian Malinois Dog

<P><B>ABSTRACT</B></P><P>An 8-year-old intact male Belgian Malinois, weighing 37.2 kg, was referred for evaluation due to right side facial paresis, ataxia and a 2-month history of decreased cognitive ability. Physical and neurological examinations revealed mild depression, left-sided head tilt, right-sided facial paresis and ataxia. A well-demarcated, broad-based cerebellar mass and hyperostosis were found on CT imaging of the brain. Based on these CT findings, a cerebellar meningioma was strongly suspected. Hydroxyurea and prednisolone were administered; after 4 weeks, there was reduction in mass size as compared to initial CT results. However, the mass size was found to have grown 6 weeks after hydroxyurea treatment. We then prescribed a combination of imatinib mesylate and hydroxyurea. Two weeks following combination treatment, the mass size had reduced significantly. The mass continuously decreased in size until the patient died during anesthesia. Cerebellar transitional meningioma was confirmed by histopathologic examination. To the author’s knowledge, this is the first reported case of imatinib mesylate plus hydroxyurea therapy for the treatment of meningioma in veterinary medicine.</P>
