Background: Most febrile neutropenic patients are treated in an aggressive manner. However, identification of low-risk patients may enable clinicians to administer risk-based treatment. The object of this study is to certify the factors associated with increased risk at the time of visiting the emergency department. Methods: This is a retrospective study. We reviewed the medical records of 101 febrile neutropenic patients who had visited the emergency department of Seoul National University Hospital from January 1998 to August 1999. We assumed 22 risk prediction factors that could be assessed at admission to the emergency department and 5 factors that could be assessed during treatment course. To find independent risk-prediction factors, we analyzed these factors respectively by using multiple regression analysis. Results: Tachycardia(aOR=136.5), altered mentality(aOR=28.8), decreased renal function(aOR=20.1), and significant comorbidity(aOR=17,2) are the independent factors associated with higher mortality. Altered mentality(aOR=31.6) and decreased renal function(CCr < 75㎖/min, aOR=5.4) are those associated with a higher incidence of septic shock. Independent factors associated with persistent(more than 3 days) fever are the early(within 10 days) onset of fever after last chemotherapy(aOR=8.8) and the existence of new pulmonary infiltrates on a simple chest X-ray(aOR=4.3). Conclusion: The stability of vital signs, the change of mentality, the renal function, the existence of significant comorbidity, the existence of new pulmonary infiltrates, and the rate of neutropenia are clinically useful risk-prediction factors in febrile neutropenia at the time of visiting the emergency department.
태권도 수련생 학부모들의 도장 선택 시 고려하는 요인에 관한 분석

이 연구의 목적은 태권도수련생 학부모들이 자녀들의 태권도장 입관을 위해 고려하는 요인들을 규명하기 위하여 관련 선행 연구들을 중심으로 가설을 설정하여 이를 규명하는데 주된 목적이 있다. 이 연구에 사용한 자료분석 기법은 기술분석(descriptive analysis), t-test, 일원변량분석(one-way ANOVA), 상관분석이었으며 이 연구에서 얻어진 결론은 다음과 같다. 첫째, 학부모들이 태권도장 선택 시 가장 많이 고려하는 요인으로는 지도자 요인은 상담 시 지도자의 친절도, 시설 요인은 태권도 수련장의 청결상태, 위치 요인은 집에서 도보이동이 가까운 위치, 프로그램 요인은 태권도장의 인성교육 프로그램 등을 태권도장 선택 시 많이 고려하고 있는 것으로 나타났다. 둘째, 학부모들의 인구통계학적 특성에 따른 도장선택 시 고려하는 요인으로 성별은 지도자요인, 위치요인에서, 연령은 지도자요인에서, 학력은 위치요인에서, 주거환경은 위치요인, 프로그램요인에서 통계적으로 유의한 차이가 있는 것으로 나타났으며, 수입에서는 모든 변인에서 집단간 통계적으로 유의한 차이가 없는 것으로 나타났다. 셋째, 학부모들의 인구통계학적 특성과 도장 선택 시 고려한 요인과의 관계를 살펴본 결과 성별은 지도자요인, 위치요인에서 월소득은 시설요인에서 주거환경은 위치요인에서 통계적으로 유의한 상관관계가 있는 것으로 나타났다.

Background : Multi-organ failure from sepsis is very lethal disease entity, which is suspected to be caused by activated inflammatory cells. Inflammatory cells activated by endotoxins generate oxidants and cytokines such as TNF-α and IL-6, which in turn stimulate macrophages and neutrophils. Augmented inflammation makes an organ-injury deteriorate into an organ-failure, which may progress to multi-organ failure. This study is designed to evaluate the therapeutic effects of vitamin-C, a scavenger of oxidants, in sepsis. Methods : Male Sprague-Dawley rats were divided into 3 groups : a control group, a group injected intrapetoneally with LPS(lipopolysaccharide), and a group injected intraperitoneally with LPS and vitamin-C. Each eight rats were sacrified 24 hours and 48 hours after injection, and samples of the blood, the liver and the lung were obtained. Biochemical assays of TNF-α level in the blood and malondialdehyde(MDA)level, catalase activity and nitric oxide synthase(NOS)activity in the liver and the lung tissues were performed. Results : Serum TNF-α level, tissue lipid peroxidation and tissue i-NOS activity were dramatically increased, and tissue catalase activity was exhausted rapidly in sepsis. High dose vitamin-C administration decreased serum TNF-α level, tissue lipid peroxidation and tissue i-NOS induction, and protected against catalase exhaustion. Conclusion : high dose vitamin-C therapy was proved to have definite antioxidant effect in septic condition.

신장의 기능을 정량분석 할 수 있는 <SUP>99m</SUP>Tc-DMSA 평면 영상은 단층영상에 비해 병소의 위치 정보에 한계가 있다. 따라서 광자 신호에 민감한 범용성 collimator로 교체하여 SPECT 방식으로 임상실험을 시행한 후, 진단의 정확도와 검사 소요시간을 분석하여 임상적용 가능성을 평가하였다. 10명의 실험대상에게 방사성의약품(1.0-1.2 MBq/kg)을 정맥주사하고, 이중 검출기 감마카메라를 이용하여 planar scan (high resolution (HR)-mode, 256×256, 50 kcts/view, 4 image)과 SPECT (HR / high sensitive (HS)-mode, 128×128, step and shoot, 180°, variable sec/angle, total 64 frame, OSEM reconstruction)를 시행하였다. 획득한 데이터를 분석 프로그램을 이용하여 비교한 결과, 실험 방법에 따라서 total counts는 high sensitive-mode SPECT가 대략 1.8-5.6 배 정도 많았고, counts를 이용한 상대신장기능은 모든 실험에서 유의한 수준 범위에서 차이가 없었으며(p=0.96), 검사 소요시간은 39%정도 단축되었다. 그러므로 고감도 범용성 collimator를 이용한 <SUP>99m</SUP>Tc-DMSA renal SPECT는 planar scan에 비하여 신장의 기능을 정량분석 할 수 있고, 병변의 위치에 대한 정보를 보다 정확하게 진단할 수 있으며, 동시에 검사 소요시간을 단축시킬 수 있으므로 임상적으로 유용하게 활용할 수 있을 것으로 사료된다. <SUP>99m</SUP>Tc-DMSA planar scan that can analyze the functions of kidney quantitatively provides less information on a lesion than tomography scanning. Therefore, this study applied a high sensitivity all-purpose collimator that is sensitive to photonic signals to <SUP>99m</SUP>Tc-DMSA and carried out a clinical scan with single photon emission computed tomography (SPECT). And diagnostic accuracy and time requirement of were analyzed to know the clinical usefulness of the applied scanning method. 10 subjects were intravenously injected with radiopharmaceutical product (1.0-1.2 MBq/kg) and scanned by a gamma camera with planar scanner (high resolution (HR)-mode, 256×256, 50 kcts/view, 4 image) and SPECT (HR / high sensitive (HS)-mode, 128×128, step and shoot, 180°, variable sec/angle, total 64 frame, OSEM reconstruction), respectively. The collected data was compared with an analysis program. The results showed that HS-mode SPECT detected total counts 1.8-5.6 times more than planar scan. Relative renal function evaluated based on the counts was not significantly different by two scanning methods (p=0.96) and it turned out that test time was shortened by 39% when HS-mode SPECT was used. Therefore, SPECT using HR, HS-mode collimator could analyze renal function more quantitatively than using planar scan and the former could diagnose the location information of a lesion more accurately than the latter as well as shortened test time requirement, which demonstrated the clinical usefulness of <SUP>99m</SUP>Tc-DMSA renal SPECT using high sensitivity all purpose collimator.
Evaluation of the Liver Cancer Diagnosis Function of PET-MRI Based on Decision Matrix Analysis

최근 임상에서 활용하고 있는 일체형 PET-MRI의 간암 진단능력을 평가하기 위해 2×2 판정행렬을 이용하여 유용성을 평가하였다. 실험대상의 PET-MRI 검사 결과를 통해 간암 판정 여부 즉 비정상과 정상판정을 받은 경우는 각각 98건, 51건 이었으며, 세포병리학적 결과가 양성과 음성 판정을 받은 경우는 각각 103건, 62건으로 나타났다. 이 중 두가지 검사에서 진양성의 경우는 95건, 위양성은 3건으로 나타났으며, 진음성은 62건, 위음성의 경우는 5건으로 분석되었다. 실험결과 PET-MRI 검사의 예민도는 95.00%, 특이도는 95.38%, 위음성률은 0.05%, 위양성률은 0.05%, 정확도는 95.15%로 분석되었다. 따라서 간암의 진단에 있어 수술 전 병기 결정이나 치료 후 재발 및 원격전이의 발견, 불분명한 원발 림프절 전이 등의 평가등에 활용 가능성이 높을 것으로 판단되며, 특히 병리학적 검사와의 복합적 진단 및 추적검사를 통해 간암 진단을 위한 PET-MRI 임상적 유용성은 충분할 것으로 사료된다. To evaluate the capability of integrated PET-MRI, which has recently been utilized in the clinical practices, on the diagnosis of liver cancer, its utility was assessed by 2×2 decision matrix. The numbers of abnormal and normal decisions on the liver cancer were 98 and 51 cases, respectively, upon PET-MRI scan results of the subjects, and the numbers of positive and negative decisions were 103 and 62, respectively, upon cytopathologic results. Out of the two tests, 95 cases were shown as true-positive and 3 were false positive, while 62 were true negative and 5 were false negative. Upon the results of PET-MRI test, its sensitivity, specificity, false negative rate, and false positive rate were 95.00%, 95.38%, 0.05%, and 95.15%, respectively. Therefore, it is considered to have the high potential to use the determination of the stage before the surgery, detections of recurrence and remote metastasis, assessment of uncertain remote lymph node metastasis, and so on in the diagnosis of the liver cancer, and also for the clinical utility of PET-MRI to be sufficient by integrated diagnosis and follow up scan with pathological studies.
