Persistent Staphylococcus aureus bacteremia is frequently defined as bacteremia persisting for ≥7 days despite proper antibiotic therapy. Its treatment includes removal of all infection foci and proper antibiotic therapy. Vancomycin remains the antibiotic of choice in MRSA bacteremia. Alternative agents, linezolid or daptomycin, are available, but a consensus regarding management of persistent MRSA bacteremia on vancomycin failure is still lacking. We report a case of a 60-year-old male who received thoracoabdominal aorta replacement operation due to dissecting aneurysm of the ascending and descending aorta. Surgical site infection and bacteremia caused by MRSA occured, and wound debridement operations were performed. The patient was treated with vancomycin in therapeutic doses but MRSA bacteremia persisted for 168 days in a row. Although the inserted aortic graft was the most probable source of persistent bacteremia, surgical removal was impossible. Linezolid was administered as an alternative antibiotic but had to be discontinued from time to time due to thrombocytopenia induced by this agent. In the end, MRSA bacteremia was successfully managed by alternating vancomycin-linezolid therapy.

<P>Ion exchange using aerosol OT (AOT) offers dye adsorption twice as fast as known methods. Moreover, it suppresses the dye-agglomeration that may cause insufficient dye-coverage on the photoelectrode surface. <P>The sea urchin TiO<SUB>2</SUB> (SU TiO<SUB>2</SUB>) particles composed of radially aligned rutile TiO<SUB>2</SUB> nanowires are successfully synthesized through the simple solvothermal process. SU TiO<SUB>2</SUB> was incorporated into the TiO<SUB>2</SUB> nanoparticle (NP) network to construct the SU–NP composite film, and applied to the CdS/CdSe/ZnS quantum-dot-sensitized solar cells (QDSSCs). A conversion efficiency of 4.2% was achieved with a short-circuit photocurrent density of 18.2 mA cm<SUP>−2</SUP> and an open-circuit voltage of 531 mV, which corresponds to ∼20% improvement as compared with the values obtained from the reference cell made of the NP film. 최근 현대인들은 식습관이 불규칙하고 서구화되면서, 건강상의 많은 문제를 겪고 있다. 이와 더불어 1인 가구의 증가와 간단한 구매 방법 등으로 인해 온라인 몰 사용자가 늘어나고 있다. 본 프로젝트는 이러한 추세를 바탕으로, 사용자가 자주 사용하는 온라인 몰에 축적된 데이터를 기반으로 사용자의 식습관을 분석한다. 뿐만 아니라, 이를 바탕으로 구매 패턴을 분석하여 사용자의 영양 상태를 개선시킬 수 있는 상품 추천 서비스를 제공한다. 사용자는 자주 사용하는 온라인 쇼핑몰에서 상품 구매를 함과 동시에 구매한 상품에 대해 시각화된 영양소 분석 결과와 구매 패턴 분석 결과를 제공받을 수 있다. 본 논문에서는 개발한 API를 통해 사용자는 부족한 영양소를 쉽게 파악하여 효율적으로 건강관리를 할 수 있게 된다. 더 나아가, 자신의 구매 패턴을 파악할 수 있게 되어 현명한 소비 습관을 만드는 데에 기여할 수 있다.

OBJECTIVE:: This study was performed to determine whether neoadjuvant treatment increases survival in patients with BRPC. SUMMARY BACKGROUND DATA:: Despite many promising retrospective data on the effect of neoadjuvant treatment for borderline resectable pancreatic cancer (BRPC), no high-level evidence exists to support the role of such treatment. METHODS:: This phase 2/3 multicenter randomized controlled trial was designed to enroll 110 patients with BRPC who were randomly assigned to gemcitabine-based neoadjuvant chemoradiation treatment (54 Gray external beam radiation) followed by surgery or upfront surgery followed by chemoradiation treatment from four large-volume centers in Korea. The primary endpoint was the 2-year survival rate (2-YSR). Interim analysis was planned at the time of 50% case enrollment. RESULTS:: After excluding the patients who withdrew consent (n = 8) from the 58 enrolled patients, 27 patients were allocated to neoadjuvant treatment and 23 to upfront surgery groups. The overall 2-YSR was 34.0% with a median survival of 16 months. In the intention-to-treat analysis, the 2-YSR and median survival were significantly better in the neoadjuvant chemoradiation than the upfront surgery group [40.7%, 21 months vs 26.1%, 12 months, hazard ratio 1.495 (95% confidence interval 0.66–3.36), P = 0.028]. R0 resection rate was also significantly higher in the neoadjuvant chemoradiation group than upfront surgery (n = 14, 51.8% vs n = 6, 26.1%, P = 0.004). The safety monitoring committee decided on early termination of the study on the basis of the statistical significance of neoadjuvant treatment efficacy. CONCLUSION:: This is the first prospective randomized controlled trial on the oncological benefits of neoadjuvant treatment in BRPC. Compared to upfront surgery, neoadjuvant chemoradiation provides oncological benefits in patients with BRPC.
