      • Disorders of Liver : The Role Of Germinative Layer In Inducing Hydatid Cyst

        Background/Aims: In spite of use of protoseololeidal agents during surgery, a notable rate of recurrence of disease in post-operation patients is still observed. The question exists is that alive scolices would cause any recurrence or is due to the presence of the germinative layer. The aim of this study was to evaluate the effect of scolicidal agents on this layer as well as its ability of the layer to produce any hydatid disease in an in vivo condition. Methods: Germinative layer of hydatid cyst was separated under sterile condition, was divided into 0.5 cm parts, exposed to various protoseolicidal agents for 2 minutes, and implanted in peritoneal cavity of 62 BALB-C mice. After 9 months, the peritoneum was evaluated macroscopically and microscopically for the presence of any hydatid disease. Results: After 9 months, no hydatid disease was visible in the peritoneum of all the sixty two mice. Conclusions: In this study, the germinative layer could not cause any hydatid disease but positive results of Tarakanovet al. with cultures of protoseolices and the germinative layer cells in the peritoneal cavity mice indicate to a need of another study of implantation of culture cells of the germinated layer lacking protoscolices in a larger animal study to clarify the differences.

      • Disorders of Liver : In Vivo Protoscolicidal Effect Of Hypertonic Glucose On Protoscolices Of Hydatid Cyst

        Background/Aims: The treatment of choice for Hydatidosis is surgery, but before evacuation of a cyst, protoscolicidal agents are needed to be injected to prevent a secondary cyst formation and the spreading of the disease. This study was performed to determine the protoseolicidal effect of various concentrations of hypertonic glucose in an animal model infected to hydatid cyst. Methods: Sheep liver hydatid cyst surface were sterilized by heat and the cyst contents were evacuated completely and transferred into falcon tubes, where protoscolices were precipitated and separated. The viability of protoseolices was determined by using eosin stain method. A total of 1500 protoscolices were exposed to 0.5% cetrimide, 0.5% silver nitrate, 0.9% and 20% sodium chloride and 10%, 15%, 25%, and 50% concentrations of glucose with exposure times of one, 2 and 5 minutes and were injected into the peritoneal cavity of 5 BALB/c mice for each agent. The animals were kept for 9 months and then were autopsied. Blood sample for countercurrent immunoelectrophoresis (CCIEP) was provided and the animals were examined for presence of any cyst in peritoneal cavity and recorded. Results: A total of 40 BALB/C mice were used in all exposure times while 24, 21 and 32 mice survived when exposed to different protoscilicidal agents after 1, 2 and 5 minutes respectively. None of the mice were infected to cysts after exposure to 50% hypertonic glucose solution. Identical to 50% glucose solution no infection was noticed for 0.5% cetrimide and 20% sodium chloride solutions too. The CCIEP results of animals indicating to cyst formation confirmed the presence of antibody in the circulation. Conclusions: Similarity of 50% glucose solution and cetrimide and 20% sodium chloride in complete killing of protoscolices present it as potent protoscolicidal agent and the side effects of cetrinide and hypertonic sodium chloride indicate to further studies on probable side effects.

        A systematic review and meta-analysis of the genetic characterization of human echinococcosis in Iran, an endemic country

        Human echinococcosis is an infectious disease caused by tapeworms belonging to the species Echinococcus. This parasite has a worldwide distribution and is considered a neglected tropical disease by the World Health Organization. Due to the diversity of Echinococcus spp. hosts, as well as variation in geographical, climatic, and socio-ethnic conditions, the question of the strains or genotypes of Echinococcus spp. that are involved in human infections is important. The aim of this study was to provide a summary of the available data on genotypes of Echinococcus obtained from the Iranian population. Four international databases (PubMed, Scopus, Science Direct, and Web of Science) and 4 Persian databases (Magiran, Scientific Information Database, Iran Medex, and IranDoc) were searched for cross-sectional studies that reported the genotypes of Echinococcus spp. in human echinococcosis cases using molecular methods in Iran through July 2018. The Newcastle-Ottawa Scale was used to assess the quality of the selected studies. A total of 559 cases of human cystic echinococcosis were reported in the 21 included articles. The majority of cases belonged to genotype G1 (89.2%; 95% confidence interval [CI], 80.1 to 95.8), genotype G6 (8.2%; 95% CI, 2.8 to 15.9), and genotype G3 (2.3%; 95% CI, 1.1 to 3.9). Since genotype G1 of Echinococcus appears to be the most prevalent genotype affecting humans in Iran, disease control initiatives aimed at sheep intermediate hosts may be the most beneficial. In addition, educational programs and serological screening in individuals may help reduce the national impact of the disease.

        Improved immunogenicity of Newcastle disease virus inactivated vaccine following DNA vaccination using Newcastle disease virus hemagglutinin-neuraminidase and fusion protein genes

        The present study describes the development of DNA vaccines using the hemagglutinin-neuraminidase (HN) and fusion (F) genes from AF2240 Newcastle disease virus strain, namely pIRES/HN, pIRES/F and pIRES-F/HN. Transient expression analysis of the constructs in Vero cells revealed the successful expression of gene inserts in vitro. Moreover, in vivo experiments showed that single vaccination with the constructed plasmid DNA (pDNA) followed by a boost with inactivated vaccine induced a significant difference in enzyme-linked immunosorbent assay antibody levels (p < 0.05) elicited by either pIRES/F, pIRES/F+ pIRES/HN or pIRES-F/HN at one week after the booster in specific pathogen free chickens when compared with the inactivated vaccine alone. Taken together, these results indicated that recombinant pDNA could be used to increase the efficacy of the inactivated vaccine immunization procedure.

        Optimal Strain Gauge Placement in Instrumented Wheelset for Measuring Wheel-Rail Contact Forces

        Concurrent measurement of the wheel-rail contact forces is always a crucial factor in design of safe railway vehicles. Commonly, instrumented wheelset has been developed towards this goal. A novel technique for precise design of the instrumented wheelset is presented in this paper. Influences of different factors such as the wheel rotation, the temperature or the centrifugal force are eliminated by cancelation of the higher harmonics. The accuracy of estimated forces is dependent on the placement of gauges on the instrumented wheelset. A solution procedure in terms of mean square error is implemented to determine the optimal radial locations of the strain gauge to achieve the most precise force measurement. Validation of the proposed method through numerical simulation is also presented. We show that the effectiveness and robustness of the technique are preserved even in the presence of noise in strain signal.

        Diet intervention to improve cardiovascular risk factors among Iranian postmenopausal women

        Cardiovascular disease is a leading cause of death and disability and remains so in the future. The aim of this study was to detect the impact of a 6-month diet intervention on cardiovascular risk factors in postmenopausal Iranian women. It was a randomized controlled trial that carried out in the East Health Clinic in Ahvaz- Iran. This study started on June 2007 and was completed on May 2008. A total 64 healthy postmenopausal women recruited and randomly assigned to the intervention group (35) and control (29). Over the six months follow-up, the intervention group received five educational sessions (two face to face and three lecture discussion classes with slide demonstration) at the first month. These sessions were about menopause, cardiovascular disease and healthy diet. Every participant in the intervention group received one face to face education session at the 3<SUP>rd</SUP> month, and also received a telephone call at the end of each month starting with the second month to remain on the diet. Pre-intervention and post-intervention anthropometric measurements, blood pressure, blood lipids and dietary intake were assessed. Data analyzed using the SPSS version 15. Descriptive (means and SD), univariate (Paired-t test, Chi-Square and Independent T-test) and multivariate (GLM Repeated Measure) statistics were used. Participants in the diet group had significantly lower weight (-0.9 ㎏), body mass index (-0.4 ㎏/㎡), and fasting blood sugar (-4.5 ㎎/dl). The diet group significantly increased their daily intake of fiber (+2.3 g, P = 0.05), decreased their intake of sodium (-28 ㎎, P = 0.04), and consumption of fruit and vegetable ≥ 5 serving a day (80%, P = 0.03) compared to the control group. Post menopausal women are at a greater risk for cardiovascular disease. Healthy diet using educational intervention can be an effective means of reducing cardiovascular risk in postmenopausal women.

        Congenital toxoplasmosis among Iranian neonates: a systematic review and meta-analysis

        Toxoplasmosis is a serious zoonotic disease that can lead to abortion and congenital disorders and has a widespread global distribution in humans and animals. The objective of this review was to investigate the incidence of toxoplasmosis in Iranian neonates in order to obtain a comprehensive assessment of the overall situation of the disease for use in developing future interventions. Original studies investigating the incidence of Toxoplasma gondii infections in Iranian neonates were systematically searched in a number of English-language and Persian-language electronic databases. The search process resulted in the inclusion of a total of 11 studies in the systematic review, 10 of which were entered into the meta-analysis. The reviewed articles included 2,230 Iranian neonates investigated through January 1, 2018. Based on the retrieved studies, the overall weighted incidence rates of toxoplasmosis in the Iranian neonatal population and neonates with suspected congenital toxoplasmosis were estimated to be 0.64% (95% confidence interval [CI], 0.31 to 1.09) and 4.10% (95% CI, 2.68 to 5.77), respectively, using a fixed-effects model. The findings of the reviewed studies demonstrate that the incidence of toxoplasmosis is high in Iranian neonates. Accordingly, it can be concluded that toxoplasmosis is a serious public health concern that has been ignored by the Ministry of Health. Therefore, it is essential to perform further studies, in addition to implementing screening and detection programs, using standardized methods to estimate the incidence of toxoplasmosis in Iran and to determine its associated risk factors.
