        Acupuncture for ventilator-dependent patients at a hospital-based respiratory care center: A randomized controlled trial

        Background: In intensive care units, mechanical ventilation is an important therapy to help patients with dysp- nea. However, long-term ventilator dependence would consume huge medical resources and increase the risk of morbidity and mortality. The aim of the study was to examine the efficacy of the acupuncture combined with western medical care on ventilator parameters in ventilator-dependent patients. Methods: In this clinical trial, 80 ventilator-dependent patients aged 20 to 80 years old were randomly assigned to acupuncture group and control group in the respiratory care center (RCC) of Changhua Christian Hospital. Besides regular medical care and therapy, participants in the acupuncture group received acupuncture therapy at the same 17 acu-points for 20 minutes once a day, a total of 12 sessions. The ventilator parameters were recorded to evaluate the respiratory efficiency for all participants. The primary outcome was rapid shallow breathing index (RSBI), and secondary outcomes were respiratory rate (RR), tidal volume (TV) and ventilation per minute (MV). Results: Though there was no significant difference in the parameter between the acupuncture group and the control group, we found the trend of decreasing RSBI in the acupuncture group. In subgroup analyses, the mean of RSBI significantly decreased 16.02 (with the SD in 60.84) in acupuncture group, while it increased 17.84 (with the SD in 39.38) in control group (p = 0.036) after 12 sessions. Conclusion: Acupuncture treatment can improve breathing ability of patients with respirator dependence in respiratory care center.

        An adaptive pilot-symbol assignment scheme is proposed for interpolation-based channel estimation in wireless orthogonal frequency division multiplexing (OFDM) communication systems. Computer simulations are conducted to demonstrate both system performance and convergence/tracking superiority and to provide a comparative evaluation of the proposed scheme with other conventional fixed pilot-symbol schemes.

        Microstructure and Abrasive Wear Properties of Fe-Cr-C Hardfacing Alloy Cladding Manufactured by Gas Tungsten Arc Welding (GTAW)

        A series of Fe-Cr-C hardfacing alloys is deposited by gas tungsten arc welding and subjected to abrasive wear testing. Pure Fe with various amounts of CrC (Cr:C=4:1) powders are mixed as the fillers and used to deposit hardfacing alloys on low carbon steel. Depending on the various CrC additions to the alloy fillers,the claddings mainly contain hypoeutectic, near eutectic, or hypereutectic microstructures of austenite γ-Fe phase and (Cr,Fe)7C3 carbides on hardfacing alloys, respectively. When 30% CrC is added to the filler, the finest microstructure is achieved, which corresponds to the γ-Fe+(Cr,Fe)7C3 eutectic structure. With the addition of 35% and 40% CrC to the fillers, the results show that the cladding consists of the massive primary (Cr,Fe)7C3 as the reinforcing phase and interdendritic Fe+(Cr,Fe)7C3 eutectics as the matrix. The (Cr,Fe)7C3 carbide-reinforced claddings have high hardness and excellent wear resistance under abrasive wear test conditions. Concerning the abrasive wear feature observable on the worn surface, the formation and fraction of massive primary (Cr,Fe)7C3 carbides predominates the wear resistance of hardfacing alloys. Abrasive particles result in continuous plastic grooves when the cladding has primary γ-Fe phase in a hypoeutectic structure.

        Predictive maintenance of abnormal wind turbine events by using machine learning based on condition monitoring for anomaly detection

        The predictive maintenance of wind turbines has become a critical issue with the rapid development of wind power generation. The early detection of abnormal operation conditions can prevent failure status, which takes a long time to recover. Energy waste can also be reduced while maintenance efficiency can be improved by using a supervisory control and data acquisition (SCADA) system to monitor the operation status of wind turbines. Massive data are generated from different sensors during wind turbine operation, and SCADA can be used to gather reports about hundreds of possible abnormal conditions. The popular maintenance methods have been mostly designed on the basis of statistical analysis and data mining. However, such schemes need not only big data but also sophisticated processing techniques. This study addresses the aforementioned challenges by proposing a deep learning model with comprehensive data preprocessing and hyperparameter tuning on batch size to achieve abnormal early detection. The necessary data preprocessing is initially conducted besides the conventional data cleaning and normalization steps, and time-series data windowing and label settings are also performed. Then, the imbalanced classes in the records are addressed by adopting an augmentation scheme called the synthetic minority oversampling technique. Principal component analysis is also used to enhance the training. Finally, the proposed deep learning method with fine-tuning is compared with three machine learning models for early anomaly event detection. Experimental results show that the proposed scheme can identify potential faults 72 hours before they occur, and the precision rate exceeds 90 %.

      • Nonlinear vibration analysis of an embedded multi-walled carbon nanotube

        Based on the Reissner mixed variational theorem (RMVT), the authors present a nonlocal Timoshenko beam theory (TBT) for the nonlinear free vibration analysis of multi-walled carbon nanotubes (MWCNT) embedded in an elastic medium. In this formulation, four different edge conditions of the embedded MWCNT are considered, two different models with regard to the van der Waals interaction between each pair of walls constituting the MWCNT are considered, and the interaction between the MWCNT and its surrounding medium is simulated using the Pasternak-type foundation. The motion equations of an individual wall and the associated boundary conditions are derived using Hamilton's principle, in which the von $K{\acute{a}}rm{\acute{a}}n$ geometrical nonlinearity is considered. Eringen's nonlocal elasticity theory is used to account for the effects of the small length scale. Variations of the lowest frequency parameters with the maximum modal deflection of the embedded MWCNT are obtained using the differential quadrature method in conjunction with a direct iterative approach.

        Malignant transformation of ovarian mature cystic teratoma into squamous cell carcinoma: a Taiwanese Gynecologic Oncology Group (TGOG) study

        Objective: The malignant transformation (MT) of ovarian mature cystic teratoma (MCT)to squamous cell carcinoma (SCC) is very rare. This study analyzed cases from multiplemedical centers in Taiwan to investigate the clinicopathologic characteristics, treatment, andprognostic factors of this disease and reviewed related literature. Methods: Pathological reports of 16,001 patients with primary ovarian cancer who weretreated at Taiwan medical centers from 1990 to 2011 were reviewed. In total, 52 patients withMT of MCT to SCC were identified. Results: Among all ovarian MCTs, the incidence of MT to SCC is 0.2%. The median age ofpatients was 52 years (range, 29–89 years), and the mean tumor size was 10.5 cm (range, 1–40cm). We analyzed the patients in our study and those in the literature and determined thatearly identification and complete surgical resection of the tumor are essential for long-termsurvival. In addition, adjuvant chemotherapy or concurrent chemoradiotherapy can be usedto treat this malignancy. Old age, large tumor size (≥15.0 cm), and solid components in MCTsare suitable indicators predicting the risk of MT of MCT to SCC. Conclusion: Similar to general epithelial ovarian cancers, the early detection of MT of MCTto SCC is critical to long-term survival. Therefore, older patients with a large tumor or those with a tumor containing a solid component in a clinically diagnosed MCT should beevaluated to exclude potential MT to SCC.

        Three methods for studying coupled vibration in a multi flexible disk rotor system

        This paper improved and developed Shaft-disk-blade (SDB) rotor system based on the previous studies of the authors in the last decade from Yang and Huang (2005) to Chiu et al. (2017). This paper also explored blade-bending, disk-transverse, and shaft-torsion coupling vibration of a multi flexible disk rotor system. Unlike the previous studies of the authors, this paper adopted three methods: (a) Assumed mode method (AMM), (b) Finite element method (FEM), and (c) experimental method. The first approach is the main method, and the two other methods are complementary. Results generated from the three methods were then compared and analyzed. Based on the previous definition of the authors, a flexible disk rotor system displays three types of coupling vibrations: Inter-blade, SDB, and diskblade modes. The system changes the rules of natural frequencies and mode shapes. This paper presents several interesting results. First, the author determined the change rules of the mode shapes and natural frequencies using the AMM, FEM (including three kinds of software), and the experimental method. Second, numerical calculation results also revealed that two phenomena regarding the distance of disk and flexible disk would be affected by the natural frequencies. Third, the experimental results would be explored in this paper. Last, the flexible disk could affect the system instability in the case study of rotation effects.

        Negative effects on medical students’ scores for clinical performance during the COVID-19 pandemic in Taiwan: a comparative study

        Purpose Coronavirus disease 2019 (COVID-19) has heavily impacted medical clinical education in Taiwan. Medical curricula have been altered to minimize exposure and limit transmission. This study investigated the effect of COVID-19 on Taiwanese medical students’ clinical performance using online standardized evaluation systems and explored the factors influencing medical education during the pandemic. Methods Medical students were scored from 0 to 100 based on their clinical performance from 1/1/2018 to 6/31/2021. The students were placed into pre-COVID-19 (before 2/1/2020) and midst-COVID-19 (on and after 2/1/2020) groups. Each group was further categorized into COVID-19-affected specialties (pulmonary, infectious, and emergency medicine) and other specialties. Generalized estimating equations (GEEs) were used to compare and examine the effects of relevant variables on student performance. Results In total, 16,944 clinical scores were obtained for COVID-19-affected specialties and other specialties. For the COVID-19-affected specialties, the midst-COVID-19 score (88.51–3.52) was significantly lower than the pre-COVID-19 score (90.14–3.55) (P<0.0001). For the other specialties, the midst-COVID-19 score (88.32–3.68) was also significantly lower than the pre-COVID-19 score (90.06–3.58) (P<0.0001). There were 1,322 students (837 males and 485 females). Male students had significantly lower scores than female students (89.33–3.68 vs. 89.99–3.66, P=0.0017). GEE analysis revealed that the COVID-19 pandemic (unstandardized beta coefficient=-1.99, standard error [SE]=0.13, P<0.0001), COVID-19-affected specialties (B=0.26, SE=0.11, P=0.0184), female students (B=1.10, SE=0.20, P<0.0001), and female attending physicians (B=-0.19, SE=0.08, P=0.0145) were independently associated with students’ scores. Conclusion COVID-19 negatively impacted medical students' clinical performance, regardless of their specialty. Female students outperformed male students, irrespective of the pandemic.
