Background and Objectives: In 2021, biologics were approved for treating chronic rhinosinusitis with nasal polyps (CRSwNP) in Korea. However, CRS is a heterogeneous disease, and its characteristics are thought to differ between Western and Korean populations. This study aimed to evaluate the experiences of members of the Korean Rhinologic Society during the first year of biologic usage for the treatment of nasal polyps.Methods: An anonymous survey consisting of 15 items was conducted from November to December 2021. The survey included questions about participant demographics, use of biologics for treating CRSwNP, and expectations regarding the effectiveness of biologics for treating CRSwNP.Results: In total, 44 members participated in the survey. Approximately half of the respondents were in their 40s (50.0%) and had 5–9 years of clinical experience as otorhinolaryngologists (47.7%). The majority of participants held academic positions (95.4%). About half of them worked in Gyeonggi Province. The utilization of biologics did not differ significantly based on clinical experience (p=0.192). When asked about the factors considered for prescribing biologics, the most common reason was recurrence of polyps after surgery (87.2%). The most frequent reason for discontinuing biologics was cost (48.6%). When asked about the extent to which they expected that the availability of biologics for CRSwNP treatment would reduce endoscopic sinus surgery (ESS), 45.5% of members expected a reduction of approximately 10%–29%. In addition, 20.5% expected a reduction of 50% or more. However, 61.4% expected a reduction of less than 10% in primary ESS. In addition, most respondents (93.2%) agreed with the need for Korea-specific guidelines for biologic treatment.Conclusion: There are discrepancies between the current guidelines for biologic treatment of CRSwNP and the reality of the situation, highlighting the need for the development of Korea-specific guidelines.

Purpose: This study investigated the clinical implications of neutrophil extracellular trap (NET) formation (NETosis) and eosinophil extracellular trap (EET) formation (EETosis) regarding refractoriness in chronic rhinosinusitis (CRS) with nasal polyps (CRSwNP). Methods: Nasal polyp specimens were obtained from 117 patients with CRSwNP who received endoscopic sinus surgery. Disease control status at postoperative 1 year was assessed. Refractory cases were defined as partly controlled or uncontrolled cases according to the EPOS 2020 guidelines. NETosis and EETosis were evaluated through immunofluorescence staining (citrullinated histone H3-human neutrophil elastase and citrullinated histone-galectin-10, respectively) followed by manual counting. The z-score of NET and EET counts was used to define the following four groups: low extracellular trap formation (ETosis), NETosis-predominant, EETosis-predominant, and high-ETosis. Results: The refractory and non-refractory groups showed significant differences in the tissue eosinophil count (P = 0.005) and EET count (P = 0.029). The tissue neutrophil count and the NET/neutrophil ratio were significantly different between the refractory and non-refractory groups of patients with neutrophilic CRS (P = 0.045, 0.031, respectively). Refractoriness significantly differed among the low-ETosis (30.77%), NETosis-predominant (47.83%), EETosis-predominant (56.67%), and high-ETosis (83.33%) groups (P = 0.005). Conclusions: The results of this study suggest that tissue Eosinophilia and EETosis may play a prognostic role, primarily in CRSwNP and thattissue neutrophilia and NETosis can play as prognostic biomarkers in neutrophilic CRSwNP.

Objectives : To investigate the progress, characteristics, and reasons of CV12 moxibustion that King Sukjong of Joseon Dynasty had experienced 31 years since 1684. Methods : This study mainly extracted the records and related records about CV12 moxibustion of King Sukjong and relevant records from Seungjeongwon Ilgi, and analyzed them. Aside from that, medical books wrote at that time and relevant records were additionally investigated. Results : King Sukjong cauterized CV12 or other CV12 related acupoints with moxa 21 years out of his 31 years from 1684 to 1714. The cautery period was usually between late January and mid February. He had 100 pieces of moxa for cautery. On the last day of his moxibustion, he cauterized ST36 with moxa. The moxibustion process consisted of the following steps: suggestion or requirement of moxibustion, decision of date and time, and moxa cautery. In 1684, the reason why King Sukjong cauterized CV12 with moxa was to treat his joint pain and nauseous symptoms. The effect can be found in Donguibogam, many other Korean medical books, and other general books at that time. In 1685, he began to cauterize CV12 with moxa to prevent diseases. Since then, most moxa cautery on CV12 had been performed in the same context. Conclusions : For 21 years, King Sukjong repeatedly cauterized CV12 with moxa. The moxa therapy was conducted to prevent diseases mostly. And King Sukjong had a lot of interest in the therapy. For the reasons, the therapy had repeatedly been performed so long.

Objectives: The purposes of the study were to analyze the temporal variation of carbon dioxide (CO 2 ) and particulate matter (PM) in daycare centers and evaluate the appropriateness of the official test method of onetime measurement. Methods: Indoor air quality in 46 daycare centers in the Seoul Metropolitan Area was measured as specified in the official test method of Indoor Air Quality Management law. In addition, indoor air quality in the 46 daycare centers was measured over 37 days using a real-time monitor (AirGuard K). Results: The daily means of CO 2 and PM in the 46 daycare centers were 1042.74 ± 134.45 ppm and 67.60 ± 18.25 µg/m 3 , respectively. Indoor air quality in the daycare centers showed significant temporal fluctuation. Measurements for single days were significantly different from the 37-day average exposure. Relative error of short term exposure decreased with an increase in the number of sampling days. The noncompliance rate for CO 2 using the official testing method was 2.17%, and none exceeded the PM 10 standard of 100 µg/m 3 . With monitoring over 37 days, the daily noncompliance rate for CO 2 was 50.4% and the daily noncompliance rate for PM was 13.8%. Conclusions: When the official test method evaluates the indoor air at daycare centers one day per year, the results may not represent actual indoor air quality over a longer period of time. Real-time monitoring devices could be an alternative for managing indoor air quality.

Background and Purpose The purpose of this study was to confirm that what symptoms are adequated indicator in the Gi-Deficiency patients. Methods In the time period July. 2005 to Sep. 2006, 136 patients with a first-ever stroke admitted in the department of Internal Medicine of Daejeon University Oriental Medical Hospital in Daejeon city, Wonkwang Oriental Medical Hospital in Iksan, Jeonju city were included. Patients were hospitalized within 3 months after the onset of stroke. Stroke patients had been interviewed by resident who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. Gi-deficiency patients was confirmed by medical specialist diagnosis, resident diagnosis, case report form analysis without a dissenting voice. Results Gi deficiency group included 23 case, Non Gi deficiency group 47 case out of 136 patients. Fatigue, weakness purse, somnolence, low voice, difficulty of uprise, pale face, pale tongue were higher among Gi deficiency group. Gi deficiency and Non Gi deficiency patients do not significantly differ in white coating tongue, light-red tongue, poor appetite, frequent sweating, teeth printed tongue. Conclusions This study was insufficiency because sample size is very small. More data from prospective cohort studies will help to Korean Standard Differentiation of the Symptoms and Signs for the Stroke.
