EU coordination of welfare states after the crisis: further interconnecting soft and hard law

After the economic shock in 2009, the European Union (EU) changed its economic governance system considerably. The question is how social policies fare within this new system. Does stricter economic governance limit the sovereignty of states to deal with social risks? To answer this question, the article determines the nature of the EU’s stricter economic governance package and explains how the interconnection between coordination mechanisms brings employment and social policy areas within the range of strengthened economic coordination. The article combines an analysis of new regulations with a study of the actual interaction between the EU and seven member states in 2011. It concludes that stricter economic governance gives the EU options to also strengthen employment and social policy coordination. Yet, the policy response of seven member states shows that there is leeway still in reacting to new EU demands.

Objective: This study was conducted to evaluate single cell protein (SCP), produced from Methylococcus species, as a protein source on the growth performance, carcass traits and gut health of broiler chickens. Methods: Ten iso-nitrogenous and iso-caloric diets containing 0 (Control), 2.5%, 5%, 7.5%, and 10% SCP replacing either soybean meal (T1 to T5) or fish meal (T6 to T10) were formulated. Each diet prepared for starter (0 to 14 days), grower (15 to 21 days), and finisher (22 to 42 days) phases was offered to four replicates of 10 chicks each (n = 400). Growth performance at different phases and carcass characteristics and intestinal morphology on 42nd day of trial were measured. Results: Body weight gain in groups fed 2.5% and 5% SCP diets were comparable to control during different phases and cumulatively, however lower (p<0.01) in 7.5% and 10% SCP diets. Feed conversion ratio was better (p<0.01) in 2.5% and 5% SCP diets. Dressing percentage, abdominal fat percentage and meat:bone ratio were not affected (p>0.05) by SCP inclusion in the diets. However, breast percentage was higher (p<0.01) in 2.5% and 5% SCP groups and thigh percentage higher in 7.5% and 10% SCP groups. Total microbial count in duodenum, jejunum and ileum were not affected (p>0.05) by SCP inclusion up to 10% in diets. Duodenal villi length and crypt depth were highest (p<0.01) in group fed 5% SCP diets and lowest in group fed 10% SCP diets. Jejunal villi length and crypt depth as well as ileal villi length were lowest (p<0.01) in group fed 10% SCP diets. Body weight gain, feed consumption, feed conversion ratio and gut health were better (p<0.01) in broilers fed fish meal based diets compared to soybean meal based diets. Conclusion: It was concluded that inclusion of SCP up to 5% replacing soybean meal in broiler diets is beneficial in improving growth rate, breast yield and gut health status. Neuronal stimulation improves physiological responses to infection and trauma, but the clinical potential of this strategy is unknown. We hypothesized that transdermal neural stimulation through low-frequency electroacupuncture might control the immune responses to surgical trauma and expedite the postoperative recovery. However, the efficiency of electroacupuncture is questioned due to the placebo effect. Here, electroacupuncture was performed on anesthetized patients to avoid any placebo. This is a prospective double-blinded pilot trial to determine whether intraoperative electroacupuncture on anesthetized patients improves postoperative recovery. Patients with electroacupuncture required 60% less postoperative analgesic, even they had pain scores similar to those in the control patients. Electroacupuncture prevented postoperative hyperglycemia and attenuated serum adrenocorticotropic hormone in the older and heavier group of patients. From an immunological perspective, electroacupuncture did not affect the protective immune responses to surgical trauma, including the induction of interleukin-6 and interleukin-10. The most significant immunological effect of electroacupuncture was enhancing transforming growth factor-β1 production during surgery in the older and lighter group of patients. These results suggest that intraoperative electroacupuncture on anesthetized patients can reduce postoperative use of analgesics and improve immune and stress responses to surgery.

Objective: The aim of this study was to assess the SLN detection rate in presumed early stage, low- and intermediate-risk endometrial cancers, the incidence of SLN metastases, and the negative predictive value of SLN mapping performed with indocyanine green (ICG). Methods: A systematic review with meta-analyses was conducted. Study inclusion criteria were A) low- and intermediate-risk endometrial cancer, B) the use of ICG per cervical injection; C) a minimum of twenty included patients per study. To assess the negative predictive value of SLN mapping, D) a subsequent lymphadenectomy was an additional inclusion criterion. Results: Fourteen studies were selected, involving 2,117 patients. The overall and bilateral SLN detection rates were 95.6% (95% confidence interval [CI]=92.4%–97.9%) and 76.5% (95% CI=68.1%–84.0%), respectively. The incidence of SLN metastases was 9.6% (95% CI=5.1%–15.2%) in patients with grade 1–2 endometrial cancer and 11.8% (95% CI=8.1%–16.1%) in patients with grade 1–3 endometrial cancer. The negative predictive value of SLN mapping was 100% (95% CI=98.8%–100%) in studies that included grade 1–2 endometrial cancer and 99.2% (95% CI=97.9%–99.9%) in studies that also included grade 3. Conclusion: SLN mapping with ICG is feasible with a high detection rate and negative predictive value in low- and intermediate-risk endometrial cancers. Given the incidence of SLN metastases is approximately 10% in those patients, SLN mapping may lead to stage shifting with potential therapeutic consequences. Given the high negative predictive value with SLN mapping, routine lymphadenectomy should be omitted in low- and intermediate- risk endometrial cancer.
