        수면내시경 검사에서 Midazolam 전처치와 Flumazenil 길항작용에 대한 연구

        Background/Aims : Midazolam is utilized as a premedication for uppoer gastrointestinal endoscopy. Midazolam has a more rapid onset of reaction than that of diazepam and its duration is shorter. But the Consciousness of premedicated patients has not been regained sooner. The Purpose of this study was to examine the effectiveness of flumazenil against midazolam as premedication for upper gastrointesinal endoscopy. Methods : Sixty patients underwent upper gastrointestinal endoscopy. These patients were divided to three groups: Group I included twenty patients without premedication; Group Ⅱ Included twenty patients with premedication of midazolam and then were not given an antisedative agent excluign of normal saline; and Group Ⅲ included the others with midazolam and flumazenil as an antisedative agent. Results : There was no change in vital signs after midazolam and flumazenil as an antisedative agent. Results : There was no change in vital signs after midazolam injection, compared with presedation value. Modied Steward Coma Scale showed a significant increase after flumazenil injection as an antagonist of midazolam. The assessment of the endoscopist and the comfort of patients were satisfactory. When the 40 patients were asked about their willingness to undergo the same procedure in the future, thirty-four patients responded favorably. Conclusion : Midazolam was safe and effective for sedation for upper gastrointestinal endoscopy. There was rapid regaining of consciousness with flumazenil indection after midazolam, so the use of flumazenil against midazolam injection also appeared to be effective.

        IMF체제 하의 한국경제는 새로운 GDP시대에 맞는 생산과 유통 그리고 소비의 기반을 구축하고 새로운 기관차역할을 할 수 있는 기업군을 육성해야 한다. 그것은 바로 중소기업이 독자적으로 성공하여 하나의 전문생산업체가 된 독립기업이나 전문기업, 기술혁신을 기반으로 성장한 기술기업, 무역으로 성공한 수출기업, 경영의 창의성과 투명성이 있는 우량기업, 미래형 사업을 꿈꾸는 벤쳐기업 등 중견기업으로서 이에 대한 국가적인 지원이 요청된다. 중견기업은 중소기업으로 출발하고 성공한 우량전문기업이기 때문에 많은 중소기업의 성장목표가 되어 희망을 주고 대기업으로 하여금 그 존립의 생명력의 모범을 보여야 한다. 따라서 중견기업은 IMF체제 하의 기업구조조정에 있어서 그 모범사례로서 산업구조의 계란형 안정모델 형성의 토대가 되어야 한다.IMF체제를 극복하기 위해서는 중견기업이 다음과 같은 일곱가지의 역할을 해야 하며 그 역할을 다할 수 있도록 정부와 금융기관은 이를 적극 지원해야 할 것이다 : 첫째, 중견기업은 IMF체제 하의 경제위기를 극복하는데 주체적인 역할을 해야 한다. 둘째, 기업의 구조조정과 경제개혁에 있어서 주도적인 역할을 해야 한다. 셋째, 기업의 기술혁신에 있어서 첨단적인 역할을 해야 한다. 넷째, 중소기업과 대기업간의 허리역할을 해야 한다. 다섯째, 한국경제의 세계화와 정보화를 위해서 선도적인 역할을 해야 한다. 여섯째, 수출증대의 기관차 역할을 해야 한다. 일곱째, 21세기 선진한국의 경제기반으로서의 역할을 해야 한다. 중견기업이 이러한 일곱가지 역할을 다할 때 IMF시대는 조기에 끝나게 될 것이다.

        비인두암의 한의학적 진료 지침 개발을 위한 기초 연구

        Objectives: The purpose of this study is to present clinical practice guidelines for nasopharyngeal cancer (NPC).Methods: Data related to western and oriental medical treatment of NPC were collected using various search engines such as Google Scholar, KIOM OASIS, PUBMED, and the library of Woosuk University.Results: In recent studies, applying combined oriental and western medicine has been shown to improve survival, quality of life, and immune function and to decrease side effects with respect to NPC. However, there still is no objective and systematic clinical guideline for NPC, so we have proposed one. This study will be meaningful in establishing clinical practice guidelines of Korean medicine for NPC.Conclusion: Further studies related to Korean medicine are needed to develop more advanced clinical practice NPC guidelines.

        Na 원자의 2광자 공명 3광자 이온화

        Na 원자 증기에 대한 2광자 공명 3광자 이온화 실험을 실시하여 이온신호의 스펙트럼, 선폭, AC Stark 이동 및 이온화율의 레이저 강도 의존성 등을 측정하였으며, 이때 2광자 공명준위는 4D 준위[과정 I]와 5S 준위[과정 II] 였다. 과정 I과 II에서 이온신호 스펙트럼의 반치폭은 레이저의 강도(I)에 비례하였으며, 이온화율은 이론적으로 알려진 바와 같이 낮은 레이저 강도 영역에서 I3에 비례하여 증가하였다. 과정 I의 이온신호는 과정 II에 비해 20~25배 정도 컸으며, 3S$\longrightarrow$4D 전이의 Ac Stark 이동계수는 $174\pm60MHz/MW/textrm{cm}^2$로 측정되었다. We studied two-photon-resonant three-photon ionization processes via 4D[channel #1] and 5S(channel #2] intermediate state of Na atom in a quartz ionization cell. For each channel. the bandwidth of ionization spectrum increased linearly with laser intensity and the ionization signal followed J3 dependence at low intensity. Compared with channel # 1 . ionization signal of channel #2 was enhanced by a factor of 20 - 25. The measured AC Stark shift factor of 3S - 4I) transition was $174\pm60MHz/MW/textrm{cm}^2$..

        중소형 무인기 브레이크 시스템용 복합형 지능재료펌프 설계

        In this study, the design of compound smart materials hydraulic pump that can be applied to a small-medium size UAV having a limited space envelope and weight has been conducted. Compound Smart Material Pump(CSMP) proposed in this paper is composed of a pressurize pump and a flow pump for supplying the high pressure and fluid displacement to overcome the disadvantages of the piezoelectric actuator which has a small strain. Though this compound smart material pump has been designed as small size and lightweight as possible, it can sequentially supply the sufficient large flow rate and pressure required for the brake operation. For the design of CSMP, about 2,700 kg (6,000 lb) class fixed wing manned aircraft was selected. Based on the established requirements, the design of the CSMP have been done by strength, vibration, and fluid flow analysis.

        일본 뇌염바이러스 감염에 대한 마크로파지의 역할

        Macrophage, scavenger cell against foreign materials, may play an important role cn hcst defense mechanism, maintenance of homeostasis function, and various immune resporses. However, it is not certain wheather or not those function of macrophages derived from patients who were treated with immuncsuppressive drugs may be kept well as in normal persens. To investigate the effect of immuncsuppressive drugs (prednisolone, cyclophcsphamide and vincristine sulfate) on mcuse peritoneal macrophages against Japanese encephalitis virus(JE virus) infecticn, each drug was dissolved and adjusted with physiological saline at the proper concentration(prednisolone 20mg/kg B.W./day, 200mg/kg B.W./day; cyclcsphamide 10mg/kg B.W./day, 100mg/kg B.W./day; vincristine sulfate 0.05mg/kg B.W. /day, 0.5mg/kg B.W./day). Then, were injected intraperitoneally with each drug solution for three corsecutive days. In 24 hours after last three groups of mice injection, peritoneal macrophages were isolated. And then macrophage layers were formed both the inner surface of Leightcn tube and on the coversligs of Leighten tube. Thereafter, JE virus was inoculated on macrophage monolayer ar.d cultured at 37 C. At apprcpriate time interval, macrophage cultvres were sempled. JE virus antigrs retained in the macrophage were detected by immuncfluorescence technique, and viral infectivity was measured by the plaque-forming methad on the monolayer of chick embryo fibroblasts. Peritoneal macrophages obtained from untreated nomal mice were used as the control. The results were summarized as follows: 1. On the cultures of normal peritoneal macrophage phagccytized Japarese cncephslitis virus, mezximal rate of macrophages retaining JE virus antigens was reachcd at 6 hours after virus adsorption. Thereafter, the rate of immunofluorescent cells was rapidly decreased, but low rate was still observed at 96 hours after virus inoculation. In contrast, on the cultures of peritoneal macrophages obtained from mice that were pretreated with immunosuppressive drug, such as prednisolone, cyclophosphamide and vincristine sulfate, the rate of macrophage retaining JE virus antigens was higher than that of untreated control macrophages. Duration necessary for reaching to the maximal rate was required approxinately 2-6 hours more than that of the control macrophages.2. Within 24 hours after the virus inoculation, virus infectivity was rapidly inactivated in cell-free medium, but inact!vation of virus infectivity in tbe macrophage cultures was more rapid than that of cell-free medium. However, virus infectivity recovered from macrophage cultures, even though low in titer, was continuously maintained throughout 96 hours. Recovery rate of the infectivity, which was observed from macrophage cultures from immunosuppressant pretreated mice, was always bigher than that of the untreated control mcrophage cultures.
