      • Antegrade Continence Enema를 이용한 소아 배변장애의 치료

        Fecal incontinence is not rare in children who have been operated upon for anorectal malformation, Hirschsprung's disease, and meningomyelocele. Incontinence can negatively impact on the emotional and social development of these children. Antegrade continence enema (ACE) was utilized in 9 patients with meningomyelocele from January 1998 to June 1999. Cleanliness and post-operative complications were evaluated. Three operative techniques were employed; reversed appendicocecostomy, in situ appendicostomy and neoappendicostomy with cecal flap. All of the patients achieved complete fecal cleanliness. Four patients had post-operative complications; two with stoma leakage, one with abdominal pain on irrigation and one with stoma stenosis. The leakage was minimal and the pain on irrigation improved spontaneously. The stoma stenosis required revision 6th month postoperatively. The ACE has been shown to be safe and highly effective.

        배추흰나비 (Pieris rapae L.)와 누에나방(Bombyx mori L.) 의 변태(變態)에 따른 지방체(脂肪體)의 미세구조(微細構造) 변화(變化)에 관한 연구(硏究)

        The fat bodies of cabbage worm (Pieris rapae) and silk worm (Bomyx mori) during metamorphosis was comparatively studied by electron microscope. 1. Cell oranelles: Golgi apparatus were not observed in both species. It is observed that RER of cabbage worms initiate to degenerate in prepupa stage with complete degeneration at adult stage, while that of silk worms shows similar degenerative pattern. However, mitochondria of cabbage worms are transformed into autophagic vacuole from prepupa stage until adult stage whereas those of silk worm shows a decrease in number in prepupa stage but maintains a certain level until adult stage. 2. Storage substance in cell: Lipid droplets in cabbage worms were observed to increase in numbers during larval stage but afterward decrease in number with an enlargement in size. However immediately after their pupal stage, they almost disappear. On the contrary lipid droplets in silk worms show rather increase in number until adult stage. Protein storage granules in bothspecies were arised from autophagic vacuoles(lysosome) . Fat cells of cabbage worm in adult stage turn out to be residual bodies which last until final stage, but those of silk worm rapidly decrease. Glycogen particles in both species reach maximum at last larval instar and thee gradually decrease thereafter. 3. Fat body sheath: The average width of fat body sheath was measured to be $0.2{\mu}m$ and $0.6{\mu}m$ and surface of fat cells adjacent to fat body sheath in silk worm is heavily infolded.

        식재용 블록을 이용한 옹벽 녹화 기법에 관한 연구(I) - Eco-Stone의 시공 사례를 중심으로 -

        The retaining wall is a structure which was made for changing land form in many construction. The first role of the retaining wall is to maintain the slope stability. But recently, the amount of retaining wall have been increasing because of the expansion of construction works and the amenity of urban environment have been decreasing because of environmental destruction and the scenic heterogeneity. So we should consider the slope stability and ecological stability at the same time. The purpose of this study is to develop the retaining wall revegetation technology using the Eco-Stone, the structure of co-satisfying which included the slope stability and the revegetation effect. Eco-Stone is a structure which has high stability for earth pressure, settlement and drainage. And cost and term of construction works also have been decreased. Eco-Stone structure is one of factors composing the ecological network which is harmonize with surrounding environment. In this way, it is expected that the ecological habitats of various species would be restored.

        난소제거 바퀴에서 알라타체의 이상비대화에 관한 연구

        The present study is undertaken to investigate the hypertrophical changes of the corpus allatum (corpora allata, CA) after the ovariectomy in Blattella germanica. In particular, the ultrastructural aspects of the normal and ovariectomized conditions, and induced factors of the hypertrophic phenomenon are focused. Ultrastructure of the CA from an immediately emergent adult is similar to that of the last larval stage that has stopped secreting juvenile hormone. The CA is composed of undifferentiated cells, exhibiting an electron-lucent matrix, a few mitochondria and less smooth endoplasmic reticulum. The karyoplasm occupy most of the cytoplasm. Electron-dense materials are filled with the intercellular spaces and gap junctions are also found. Almost no ultrastructural changes have been noticed during seven days until the oviposition. However, considerable changes in structure have been detected soon after the oviposition. Mitochondria are increased dramatically in number and cristae, and changed to the filamentous form with a high electron density. In addition, Golgi complexes, microtubules, and polysomes are also increased. After an oviposition, the total volume of the CA are decreased again. The volume of the CA are increased continuously, hypertrophy, after the ovariectomy. Morphological aspects of the CA in an early stage after the removal are similar to the structure of the secondary egg maturation. Large and electron dense globules are observed in the ovariectomized CA cytoplasms and they are present in those cells for a long period of time. Yet such a hypertrophical phenomenon occur only in the specific cells. The hypertrophy are caused by hollowing the part of the CA cells and later filling such site with polysomes. In 42 days after the ovariectomy, the nuclear membranes disappear in the CA cells, thus, exhibiting the prokaryotic-like features. Some results of the current study will contribute to the establishment of the model that explain unusual changes accompanied by certain treatment in insects and/or further in animals.

        팽대부 종양의 해부학과 수술적 치료

        Ampulla is a common channel where biliary and pancreatic duct join. The reason why it is called ampulla is that this area is enlarged where it penetrates the duodenal wall. As it protrudes into the duodenal lumen, it is also called papilla. Ampulla is surrounded by sphincter muscles which control the secretion of bile and pancreatic juice. Ampullary tumor includes broad spectrum of benign and malignant tumors which need different treatment options - Endoscopic papillectomy (EP), surgical ampullectomy (SA), and pylorus-preserving pancreatoduodenectomy (PPPD). There is a consensus that benign tumors need EP and malignant tumors need PPPD. However, there are controversies regarding how to treat high-grade dysplasia (HGD), carcinoma in-situ, or T1 cancer. Surgical treatment includes SA and PPPD. The indication of SA is usually benign tumors which extend to bile duct or pancreatic duct or too big tumors to snare completely by endoscopy. Compared to EP, SA is less likely to leave remnant tumor which results in re-resection. In some limited cases of early carcinoma, SA can be attempted. Oncologically, however, SA is not recommended because there might be incomplete resection, lymph node metastasis, lymphovascular invasion, or perineural invasion. As PPPD still has high morbidity, prolonged length of stay, and relatively high post-op mortality, SA can be recommended in old patients with co-morbidity that can be exacerbated after surgery. Technically important points of SA are as follows; first, resecting the whole layer of duodenum, second, reimplanting the bile duct and pancreatic duct with duodenal mucosa to keep the mucosal continuity. PPPD is a standard operation of malignant tumor because it can completely remove the tumor as well as regional lymph nodes.

      • 급성 외상 환자의 상부경추 검사에서 변형된 open mouth view의 유용성

        목 적 : 급성 외상 환자의 상부경추 검사 시 환자의 움직임이나 자세 잡이의 어려움으로 재검사가 많은데, 이러한 재검율을 최소화 하기 위하여 변형된 open mouth view를 적용한 검사의 유용성을 평가하고자 하였다.  대상 및 방법 : 성인 팬텀을 대상으로 안와외이공선을 검출기면에 수직이 되도록 하고, 초점과 피사체 간 거리를 100cm으로 고정한 후, X선관의 각도를 머리쪽으로 각각 0°, 5°, 10°, 15°, 20°, 25° 를 설정하고 제 1경추와 제 2경추의 높이인 상치열(teeth incisor)을 중심 X-선으로 촬영하였다. 영상평가는 5년차 이상 방사선사 10명이 PACS의 모니터 영상을 통하여  ‘Excellent’ 5점, ‘Good’ 4점, ‘Fair’ 3점, ‘Poor’ 2점, ‘Very poor’ 1점으로 부여하여 육안적 평가를 하였으며,  Kruskal wallis test와 Mann-Whitney로 통계적 분석을 하였다. 결 과 : 응답 점수의 평균은 15°에서 5.0점으로 가장 높게 평가 되었다. Kruskal wallis test 검정 결과에서는, 15°에서 55.5점으로 가장 높게 나타났고 20°에서 40.6점으로 나타났다. Mann-Whitney 사후 검정 결과 0°와 5°, 10°와 20°를 제외한 방법들은 통계적으로 유의하였다(p<0.05). 결 론 : 급성 외상 환자의 상부경추 검사 시 환자의 움직임 없이 중심 X-선의 각도만을 변화시킨 변형된 open mouth view는 진단적 가치가 우수한 유용한 검사법이라고 사료된다.   Purpose : This study is to evaluate the modified open mouth radiography which is valuable and minimizes the reexamination rate in case of acute trauma patient. Materials and Methods : The modified open mouth view was taken changing x- ray angles of 0°, 5°, 10°, 15°, 20°, 25° toward to the head with orbitmeatal line of the phantom perpendicular to the detector, and the focus-object distance was 100cm. The center of x-ray was teeth incisor that is the same anatomical level as the atlas(C1) and axis(C2). All the images were evaluated by ten radiological technologists who have over five-year work experiences and 5-point Lickertis scale throughout visual evaluation in PACS system was used. Statistical significance was analyzed by Kruskal wallis test and Mann-whitney test Result :  In response to mean score of Likertis scale, the image of 15° was highly evaluated as 5.0 points among the group. The result of the Kruskal wallis test, the image of 15° was ranked first as 55.50 points and the image of 20° was scored 40.60 points secondly. All the results of the Mann-whitney post-hoc test, except between 0° and 5°, 10° and 20°, indicated statistical significant differences(p<0.05). Conclusion : In case of patient with acute upper cervical spine trauma, the modified open mouth radiography can minimize patient s movement and can make them more comfortable and bringing satisfaction. In addition, it can get a high quality image without specific positioning. As a result, the modified open mouth radiography would be useful and applicable to the clinical practice especially for acute trauma patients.
