스트레스 골절은 정상 골이 반복되는 부하를 견디지 못하여 발생되는 골절을 말하며 주로 활동적인 사람에서 발생한다. 슬개골의 스트레스 골절은 아직 국내에서 보고된 바가 없을 정도로 매우 드물다. 우리는 운동선수가 아닌 9세 여아에서 달리기를 한 후 발생한 슬개골의 스트레스 골절을 경험하였기에 문헌 고찰과 함께 보고하고자 한다. Stress fracture usually occurs in physically active individuals as a result of repetitive strenuous muscle and tendon forces acting on bones that have not adapted to such forces. To the best of our knowledge, the literature contains few reports of stress fracture of the patella. We report herein on a case of a nine-year-old female who developed a stress fracture in the patella after running.

This paper describes the shaft vibration phenomena measured on a pump-turbine of a pumped storage power plant. The pump-turbine runs at a rotational speed of 450 rpm (7.5 Hz). The power output (load) of the pump-turbine is varied from 100 to 300 MW in the generating mode. The magnitude of the shaft vibration highly depends on the power load. The vibration magnitude of the shaft is very high in the middle load zone from 170 to 210 MW, elsewhere the vibration is low. From nitration spectra, it is shown that the frequency of major nitration in that load zone is 2.5 Hz which is approximately $34\%$ of the shaft rotating speed in Hz. This frequency component does not occur below and above that load zone. This subsynchronous vibration is caused by the flow induced disturbance due to spiral vortex flow downstream of the pump-turbine runner. Furthermore, the shaft vibration is highly decreased due to an increased bearing preload.
Lindholm 방법을 이용한 진구성 아킬레스 건 파열의 치료

Purpose: This retrospective study was designed to evaluate the treatment results of chronic Achilles tendon rupture by Lindholm method. Materials and Methods: Between 2002 and 2006, we performed the reconstruction of the Achilles tendon by using of the gatrocnemius-sloeus fascia known as Lindholm method. Ten cases of ten patients were enrolled in this study (8 men and 2 women). The mean age of the patients at the time of operation was 49 years (range, 32${\sim}$66 years). The mean follow-up duration was 15.2 months (range, 12${\sim}$19 months). The retrospective review of the clinical history, physical examination, the American Orthopedic Foot and Ankle Society (AOFAS) score were conducted. Results: The mean AOFAS score before surgery was 74.10${\pm}$2.56 and that of the latest follow-up was 90.60${\pm}$5.72. The excellent results were six and good results were four patients. Eight patients were normal triceps power and the others were good. The average of heel to floor distance was 4.5 mm less in the operated legs than the contralateral ones in each patient, but there was no significant difference (p>0.05). The average of calf muscle circumference in the mid-leg was 7.5 mm less in the operated legs than the contralateral ones and there was significant difference (p<0.05). The active range of motion of the ankles, mean plantarflexion was 40 degrees and dorsiflexion was 16.8 degrees in operated side. The contralateral side was 43 degrees in plantarflexion and 19 degrees in dorsiflexion. No case showed rerupture of the reconstructed Achilles tendon. One patient had the superficial wound infection which was treated successfully by antibiotic therapy. Conclusion: It was suggested that the overall results of Lindholm method for the chronic Achilles tendon rupture indicated satisfactory outcomes.
슬개하 지방체에 발생한 관절 주위 연골종 - 증례 보고 -

주변골과 연결성을 가지지 않는 것을 특징으로 하는 골격계 외의 연골종에는 활액막성 연골종증, 관절 내 및 관절 주위 연골종, 연부 조직성 연골종 등이 있으며, 후자의 두 경우는 매우 드물다. 본 증례는 20세 군인이 심한 훈련을 한 후 발생한 슬관절의 압통, 굴곡시의 통증과 충만감, 운동 제한을 주소로 내원하여 시행한 x-ray와 MRI 검사에서 발견한 종괴에 대한 것으로, 이 종괴에 대해 절제술이 시행되었으며, $5.5{\times}4{\times}3$ cm 크기의 단단한 고립성 종괴였다. 현미경 소견 상 종괴는 소엽상을 이루는 초자 연골이 바깥쪽의 섬유성 피막에 의해 싸여져 있었으며, 이에 관절 주위 연골종으로 진단하였다. 저자들은 슬관절 슬개하 지방체에서 발생한 관절 주위 연골종 1 예를 경험하여 임상적, 방사선학적, 조직학적 소견에 대해 문헌 고찰과 함께 보고하고자 한다. Extra-skeletal chondroma, characterized by its lack of connection with the adjacent bone, includes synovial chondromatosis, intra-articular and para-articular chondroma, and soft tissue chondroma. Among them, the last two lesions are extremely rare. This is the case about 20-year-old soldier, who had complained of tenderness of the knee, pain and fullness during knee flexion and limitation of motion after a hard military training. We found a mass in the x-ray and MRI and resect the mass surgically, which was a $5.5{\times}4{\times}3$ cm size hard solitary mass. On microscopic finding, it was consisted of lobulated hyaline cartilage and outer fibrous capsule, and we ascertained it as para-articular chondroma. We experienced a case of para-articular chondroma in the infrapatellar fat pad of the knee joint and present its clinical, radiologic and pathologic findings with literature review.
