        분변 감입을 동반한 무증상의 심한 소아변비 치험례

        Objectives The purpose of this study is to report the case of fecal impaction in a child without any abdominal symptoms treated by taking herbal medicine. Methods We examined a 7-year-old girl who had normal bowel movement and defecation per day, and had no particular abdominal symptoms. Abdominal radiography was taken, and unexpected severe fecal impaction was observed throughout the abdominal cavity, filled with intestinal gas and feces. According to the subject's parents, she had no generalized symptoms, such as abdominal pain or distension, and had on a regular diet and normal bowel movement daily. She was treated with herbal medicine (Daeseunggi-tang) for 23 days. While she was on the therapy, numbers, doses, bowel movements, and radiography were checked and recorded. Results During the treatment, her stool was softened, and fecal impaction was relieved as showed by abdominal radiography. Conclusions We have identified that there are cases where subjects have no symptoms of abdominal pain, despite presence of severe fecal impaction. In addition, it was found that Daeseunggi-tang is effective in fecal impaction in childhood.

        Hereditary tyrosinemia type 1 (HT-1) is a metabolic disorder caused by biallelic pathogenic variants in the fumarylacetoacetate hydrolase (FAH) gene, which impairs the function of the FAH enzyme, resulting in the accumulation of tyrosine's toxic metabolites in hepatocytes and renal tubular cells. As a consequence, individuals with HT-1 exhibit symptomatic manifestations. Rapid diagnosis and treatment of HT-1 can prevent short-term death and long-term complications. A 15-day-old boy presented to the outpatient department with elevated levels of tyrosine on his newborn screening tests conducted at the age of 3 and 10 days, respectively. Further blood tests revealed increased levels of alpha-fetoprotein and amino acids including tyrosine and threonine. Urine organic acid tests indicated a significant elevation in tyrosine metabolites, as well as the presence of succinylacetone (SA), which led to the diagnosis of HT-1. Two pathogenic and likely pathogenic variants of FAH compatible with HT-1 were also detected. He began a tyrosine-restricted diet at one month old and received nitisinone (NTBC) at two months old. With continued treatment, the patient's initially elevated AFP level, detection of SA in the urine, and mild hepatomegaly showed improvement. During four years and seven months of treatment, there were no exceptional complications apart from an increase in tyrosine levels and a delay in speech. We report a case of tyrosinemia type 1 detected through newborn screening, treated with dietary restriction and NTBC, with a good prognosis.

        위 내 이물로 오인한 석회화 섬유 종양 1예

        평소 복부 팽만을 보인 2세 남아가 내원 3일 전부터 간헐적인 구토를 동반하여 시행한 복부 방사선 사진에서 좌상복부에서 석회화 소견을 관찰하여 상부 위장관내시경과 복부 전산화 단층 사진을 시행하였다. 술 후 병리 검사에서 상당히 빈도가 드문 석회화 섬유 종양을 발견하였기에 보고하는 바이다. Calcifying fibrous tumors (CFTs) are unusual benign tumors of childhood, located primarily in soft tissues, pleura, and peritoneum. The cause and pathogenesis are unclear. We report a rare case of a CFT in a 2-year-old boy who presented with vomiting and abdominal distension. An abdominal X-ray showed an elliptical, calcific shadow in the LUQ area mimicking a foreign body. An internally protruding mass along the lesser curvature of the gastric body was an incidental finding during upper endoscopy, biopsies of which were negative. Abdominal CT showed a 4.5${\times}$3.2 cm soft tissue mass of the gastric wall with calcifications. A diagnosis of gastric submucosal mass was suspected and a wedge resection of the stomach was performed. On microscopic examination, the tumor was composed of whorls of dense hyalinized collagen bundles with a few fibroblasts. There were also amorphous dystrophic calcifications and nodular aggregates of mononuclear inflammatory cells. Immunohistochemically, spindle cells did not stain for anaplastic lymphoma kinase-1 (ALK-1), CK, smooth muscle actin (SMA), or desmin. Taken together, the mass was compatible with a CFT of the gastric wall. This is the first reported case of CFT in a Korean child.

        이러닝 환경에서 중학생의 디지털 리터러시가 이러닝 학습태도에 미치는 영향: 자기조절 학습역량과 이러닝 수용의도의 매개효과

        본 연구는 이러닝 환경에서 중학생의 디지털 리터러시가 자기조절 학습역량과 이러닝 수용의도를 매개로 하여 이러닝 학습태도에 어떠한 영향을 미치는지에 대해서 탐색하였다. 이를 위해, 부산광역시 소재 A중학교의 1학년 128명을 대상으로 설문조사를 실시하였으며, 수집된 자료는 위계적 회귀분석(Hierarchical regression analysis)과 소벨 테스트(Sobel-test)를 활용하여 분석하였다. 주요 연구 결과는 다음과 같이 나타났다. 첫째, 이러닝 환경에서 중학생의 디지털 리터러시가 자기조절 학습역량을 매개로 하여 이러닝 학습태도에 미치는 영향은 유의미한 것으로 나타났다. 둘째, 이러닝 환경에서 중학생의 디지털 리터러시가 이러닝 수용의도를 매개로 하여 이러닝 학습태도에 미치는 영향은 유의미한 것으로 나타났다. 본 연구 결과는 중학생의 이러닝에 대한 긍정적 학습태도의 향상을 위해, 학습자의 디지털 리터러시, 자기조절 학습역량 및 이러닝 수용의도를 고려한 수업설계 및 수업전략의 개발과 운영이 필요함을 함의하고 있다. 또한, 본 연구 결과는 포스트 코로나 시대에 적합한 중학교 이러닝 환경 구축 및 효과적인 교수·학습전략 설계 시 활용할 수 있는 기초 자료로서 의의가 있다. This study investigated the effects of middle school students’ digital literacy on their e-learning attitude mediated by self-regulated learning competency and e-learning acceptance intention in e-learning environments. For this purpose, a survey was conducted with 128 the first grade students in an a middle school, Busan city, and the collected data was analyzed using by Hierarchical regression analysis and Sobel-test. The main results of this study were as follows. First, There were significant effects of middle school students’ digital literacy on their e-learning attitude mediated by self-regulated learning competency in e-learning environments. Second, there were significant effects of middle school students’ digital literacy on their e-learning attitude mediated by e-learning acceptance intention in e-learning environments. Based on these results, these main results imply that instructional designs and instructional strategies should be developed and implemented by considering middle school students’ digital literacy, self-regulated learning competency and e-learning acceptance intention to improve learners’ e-learning attitude. Also, the results of this study are meaningful as basic data which can be used to construct suitable middle school e-learning environments for the Post-Covid-19 era and design effective teaching-learning strategies.

        딥러닝 기반 피사체 시선 추적을 통한 자동 주석 생성 시스템

        피사체의 시선 추적(Gaze Following)은 단일 이미지에서 피사체의 시선이 응시하는 지점을 탐지한다. 딥러닝 기반의 기존 연구는 단순히 시선의 각도를 추정하거나, 스마트폰과 같은 기기 스크린 내부의 응시점을 추정하므로 어떤 객체를 보는지에 대한 정보를 얻을 수 없다는 한계가 있다. 본 논문에서는 최초로 딥러닝 모델을 활용하여 피사체의 시선을 추적하고 ‘J가 넥타이를 본다.’와 같이 자동으로 주석을 생성하는 시스템을 제안한다. 시스템은 전처리 모듈, 시선 추적 모듈, 후처리 모듈로 구성되며, 전처리 모듈에서 인물을 인식하고 딥러닝 모델의 입력을 생성한다. 시선 추적 모듈에서는 딥러닝 모델이 피사체의 응시 지점이 표시된 히트맵(heatmap)을 생성한다. 후처리 모듈에서는 우리가 제안하는 객체 선택 알고리즘에 의해 응시 지점에 있는 객체를 인식하고 주석을 생성한다. 제안된 시스템은 리테일링 및 학술 목적의 대규모 메타데이터를 효율적으로 생성하는 데 활용될 수 있다. Gaze following is the task of detecting the point of attention in a single image at which a subject"s gaze is staring. The existing deep learning methods have a limitation in that they cannot determine which specific object is the target of the gaze angle, because these methods simply estimate the gaze angle, or estimate the gaze point within the screen of a device, such as a smart phone. In this paper, we propose a novel system that infers where a subject is looking by using a deep learning model and automatically generating annotations such as "J is looking at the tie." The proposed system consists of a pre-processing module, a gaze following module, and a post-processing module. In the pre-processing module, our system recognizes faces and generates inputs for the deep learning model. In the gaze following module, the deep neural network generates a heatmap for the point at which the subject is looking. In the post-processing module, the proposed object selection algorithm determines the object of the gaze point, and annotations are then generated. The proposed system can be used to generate large-scale metadata for retailing and academic purposes.

        반복되는 쌕쌕거림이 있는 소아에서 IgG 아형 결핍의 진단과 치료 1예

        Differential diagnosis of recurrent wheezing in school-age children includes not only asthma but also immunodeficiency. Immunoglobulin G (IgG) subclass deficiency is deficiency of 1 or more IgG subclasses (<2 standard deviations below the mean for age). IgG subclass deficiency in children may be associated with frequent respiratory tract infection and wheezing. An 8-year-old female patient had recurrent respiratory tract infection and wheezing despite the use of inhaled corticosteroids after the first wheezing around 100 days of age. The results of chest radiography, high resolution computed tomography of the chest, and spirometry were normal. In methacholine challenge test, provocation concentration causing a 20% fall in forced expiratory volume in 1 second was 18.29 mg/mL. Her total IgG was within the normal limit on the immunologic evaluation, but the IgG3 and IgG4 levels were decreased. She was diagnosed with IgG3 and IgG4 subclass deficiency. After intravenous immunoglobulin G (IVIG) infusion every 4-6 weeks, the number of respiratory tract infection and wheezing, the doses of systemic corticosteroids, and duration of antibiotic therapy were reduced. This case suggests that even in pediatric patients, if there are frequent respiratory tract infection and poorly controlled asthma, IgG subclass deficiency is suspected and clinical symptoms can be alleviated through IVIG treatment.       • 시판중인 우유 및 발효유제품의 이화학적 및 기능적 특성

        The present study was performed to assess the intake, consumption pattern, and functional protein components of milk and fermented dairy products. Total 8 commercial processed dairy products from supermarkets were surveyed. The fermented dairy products were classified according to the health-promoting effects on stomach, intestine, and Liver. In main nutrition sources of information about fermented dairy products have the property of containing living microorganisms such as Bifidobacterium, Lactobacillus and Streptococcus featuring sensory and nutritional qualities. The pH values of the products for stomach protection were 4.30~4.52, for intestinal improvement were 4.26~4.27, and for liver protection were 4.20~4.26. The SDS-PAGE results indicated that main bands of fermented dairy products had Ig heavy chain, lactoferrin, BSA, caseins and whey proteins, immunoglobulin (Ig) and bovine serum albumin (BSA). There were no remarkable differences of protein fractions and peptides between products.
