N/A Gallstone is the most common disease in the biliary system, and the incidence is increasing gradually. Furthermore, the composition of stones in Korea seems to be changed to approach that of stones formed in the west. Factors responsible for this change may include rapidly pro- ceeding urbanization and changing food habits. The method of medical therapies for gall- stones is also increasing and results in a number of clinical studies for pathogenesis of gall- stones such as stasis, supersaturation, and crystal formation of bile juice. We studied for 40 subjects, healthy controls were 20 (11 male, 9 female: mean age, 44.45 years, mean body mass index, 24.07), and gallstone patients were 20 (1 1 male, 9 female: mean age, 54.7 years, mean body mass index, 22.78). The gallbladder volumes were measured by real time ultrasonography using the ellipsoidal method. And then, we calculated ejection fraction (EF) by these results. The results were expressed as mean + standard deviation, and differ- ence in the mean values of each group were evaluated by the Students t test. Statistical sig- nificance was assigned if P was <0.05. There was no significant difference between gallstone patients and healthy controls in age, body mass index, and biochernical laboratories. Both fasting gallbladder volume and ejection fraction were not different between obese and non obese group. However, ejection fraction in gallstone patients was significantly lower than normal controls in obese group. Ejection frac- tion(EF, %) was significantly greater in controls than gallstone patients. No significant differ- ences were observed in fasting gallbladder volumes between gallstone patients and healthy controls, but gallbladder volume after fatty meal was significantly larger in gallstone patients than control subjects. Fasting gallbladder volumes were significantly reduced after fatty meal in both groups. The gallbladder motility rnay play a role on gallstone formation, and the estimation of its motility may be an important factor in deterrnining the response to medical therapy.(Korean J Gastroenterol 1994; 26: 143 150)

Background/Airns: To evaluate whether gastric resection is a predisposing condition in the formation of gallstone. Methods: Gallbladder motility after gastrectomy was studied by means of measuring fasting and postprandial gallbladder volume using real time ultrasonography 46 gastrectomized patients and 37 healthy controls were selected as study subjects. Results: Mean fasting and postprandial gallbladder volume was significantly increased in gastrectomized subjects(FV 30.20±13.86 mL, PV 17.215.82 mL) than healthy control group(FV 12.58±6.53mL PV 4.28±3.29 mL, p (0.01). The ejection fraction of gallbladder in gastrectomized subjects(56.93±13.0%) was significantly smaller than healthy control group(75.53±16.1%, p0.01). Gallbladder ejection fraction was slightly increased correlated with post operative date(r=0.232). Conclusions: Gastrectorny may increase the risk of gallstone disease by inducing gallbladder dysmotility and bile stasis. Gallbladder motility evaluation in high risk group of gallstone disease would be helpful for prevention and understanding gallstone formation.

Idiopathic Hemochromatosis 의 1예

We have seen a case of hemachromatosis, which is rare in this country. A 47-year-old man, who had been a heavy drinker, was admitted to our clinic with the complaints of general weakness, indigestion, pain on right upper quadrant and discoloration of the skin. Physical examination disclosed typically bronze-colored skin particularly on the face and hands, mild icteric sclera and palmer erythema. Liver was palpable 4 F.B. below the right costal margin but abdaminal distention and ascites were not marked. Neither past history of blood transfusion and an ingestion of iron-preparation nor family history was noted. The serum iron level and total iron binding capacity were increased up to 330㎍/dl and 394 ㎍/dl respectively. There were mild hyperglycemia with diabetic curve following standard oral glucose tolerance test and an evidence of hemosiderin pigments on the specimens of urine, skin, bone marrow and liver. Supportive treatment for the control of liver cirrhosis was given with some improvement.

Background/Aims: The aim of this study was to assess canine gastric contraction by surface electrogastrography (EGG). Methods: Serosal EGG, surface EGG and measurement of gastric contraction by strain gauge were performed simultaneously in 20 unconscious dogs during the motor quiescence period and the contraction period induced by bethanechol. Amplitude, wave form, frequency and vector analysis of EGG were performed. Results: During motor quiescence period, normal smooth sine wave was maintained. During gastric contraction, the amplitude of slow wave was increased and the wave form of it changed into a late peak and descending arm in cutaneous wave was dominated. On the frequency analysis of the motor quiescence period, a single dominant wave of three cycles per minute (3 cpm) was observed. During the contraction, the dominant frequency was 3.5 cpm, and a marked increment was observed in first harmonic compared to motor quiescence period. There was rough correlation between the power of first harmonic and gastric contractions. In the vector analysis of motor quiescence period, the average axis of EGG vector was 62.7, y=0.00512+1.935x and the coefficient of determination was 0.476. During gastric contraction, the average axis of EGG vector was 12.6, y=0.00377+0.228x, and the coefficient of determination was 0.00281 (p$lt;0.05). Conclusions: Gastric contractions could be assessed by surface EGG in terms of such parameters.
