슬관절 주위에 발생한 골종양 환자에서 종양 대치물을 이용한 재건술 후 기능적 평가 및 보행 분석의 유용성

Purpose: This study attempts to know functional results and gait analysis usefulness in patients with bone tumor around knee joint tumors who underwent prosthesis knee joint reconstruction. Materials and Methods: Retrospective study was conducted with 7 patients out of 30 patients who underwent prosthesis knee joint reconstruction after wide marginal excision for bone tumor around knee in orthopedics of this hospital from 2001 to 2010. Functional assessment and gait analysis were perforemed. Results: For the SF-36 score, while 'role physical' and 'role emotional' items showed 100% (100 points) high scores individually, general health, physical function, vitality, and social function showed low scores. The mean score of MSTS was 88.1% (23.8 points [17-27]), indicating a relatively high score. For the gait analysis, mean gait velocity was 97.2 m/s, mean cadence was 105.6 step/min, mean stride length was 111.3 m, mean step length was 61.5 cm, swing phase was 39.8%cycle, stance phase was 60.1%cycle, mean single limb support was 37.1%cycle, mean double limb support was 13.0%cycle, and mean push off was 60.7%cycle. Conclusion: It is expected that prosthesis reconstruction after wide marginal excision for bone tumor around knee has relatively good functional results. Gait analysis was considered one of method which showed gait phase and assessed functional ability objectively by quantitative assessment post operative patient condition. It might help treatment and post operative rehabilitation planning with the functional assessment. 목적: 슬관절 주위에 발생한 골종양 환자에서 종양 대치물을 이용한 재건술의 결과와 보행 분석의 유용성을 알아보고자 하였다. 대상 및 방법: 2001년부터 2010년까지 슬관절 주위에 발생한 골종양에 대하여 광범위 절제술 후 종양 대치물을 이용한 재건술을 시행 받은 30명 중 7명을 대상으로 하였으며 기능적 결과 및 보행 분석 검사를 평가하였다. 결과: SF-36 점수는 신체적, 정신적 역할제한 항목에서 각각 100% (100점)으로 높은 점수를 보였으며 일반 건강상태, 신체적 기능 정도, 활력, 사회적 기능에서 낮은 점수를 나타냈다. 또한 MSTS 평가의 종합 평균 점수는 88.1% (23.8점[17-27])였다. 보행 분석 검사상 평균 보행속도 97.2 m/s, 평균 분속수 105.6 step/min, 평균 활보장 111.3 m, 평균 보장 61.5 cm, 유각기 39.8%cycle, 입각기 60.1%cycle, 평균 단하지 지지기 37.1%cycle, 평균 양하지 지지기 13.0%cycle, 평균 발 들림시기 60.7%cycle였다. 결론: 슬관절 주위 종양에 대한 광범위 절제술 후 종양 대치물을 이용한 재건술은 좋은 기능적결과를 기대할 수 있으며, 보행 분석 검사는 술후 환자의 상태를 정량적으로 평가함으로써 보행 모습 및 기능을 객관화 할 수 있는 한 방법으로 사료되며 기능적 평가 방법과 함께 치료 및 술 후 재활 계획을 세우는데 도움이 되리라 생각된다.

본 연구에서 시험부 크기 30㎜×35.6㎜, Run Time 20초인 소형-저가의 초음속 풍동 을 개발하였다. 개발된 교육용 초음속 풍동은 단속적 불어내기식형이며, 노즐 블록 교체 형으로 설계되었다. 본 연구를 통하여 초음속 용동에 사용되는 대형-고가의 기계장비를 기존 소형의 상용 제품으로 대체하여 제작 단가를 낮추었다. 그리고 초음속 풍동의 핵심 기술인 노즐 설계를 비롯한 전체적인 초음속 풍동 설계 기술을 확보하였다. 풍동 시험부에서의 초음속 유동장 형성 및 유동 안정성에 대한 성능검증 실험을 수행하였다. A small size - low priced supersonic wind tunnel of which test section size is 30㎜ by 35.6㎜ and run time is 20sec is developed. This educational supersonic wind tunnel is an intermittent blowdown type with an exchangeable nozzle block. In this study, the proper sized and low priced commercial parts are used to reduce the total cost of supersonic wind tunnel. A nozzle design and small supersonic wind tunnel design process has been established, and it is confirmed that a given supersonic flow field has been obtained and proved by experiment.

While a RAID 5 disk arrays offer advantages of performance and reliability on wide variety of applications, they suffer from the performance degradation with write jobs. To reduce the effect of this problem, parity logging scheme was suggested. This scheme reduces disk busy time by transforming many small random accesses into large sequential accesses. This paper presents a new parity logging technique which prevents parity update image from redundant storing in the disk buffer and reduces parity maintenance overhead. A analytical model and several simulation results of our proposed scheme are presented.

Familial Mediterranean fever (FMF) is an autosomal recessive disease characterized by recurrent episodes of fever and serosal, synovial, or cutaneous inflammation, caused by a dysfunction of pyrin as a result of mutation within the MEFV gene. It occurs mainly among Mediterranean and Middle Eastern populations, including Jews, Arabs, and Turks. However, FMF cases have been reported outside the Mediterranean and Middle Eastern countries in recent years. Although FMF has been relatively rare in Korea until now, proper recognition of FMF might lead to more frequent diagnoses of FMF. We experienced an interesting case, a 31-year-old Korean man who presented with recurrent abdominal pain with fever and urticarial eruption for 10 years. DNA analysis showed complex mutations (p.Leu110Pro, p.Glu148Gln) in the MEFV gene. To date, three cases have been reported, and this case of FMF with skin conditions is the first case in Korea. This paper investigates the line dynamic forces connecting tugs and a floater, where the planar motion of the floater is due to the weather criteria during the position keeping of the offshore structure by tugs. The analysis situation consists of the position keeping states for the seabed mooring line connection work of the offshore structure at the offshore site. Specifically, the decision about the tug power capacity for the position keeping is essential and depends on the weather criteria, line characteristics, length of line, etc. The planar motion of the structure is constrained by the interference of the installation vessels operational range, behavior of the underwater fairlead, and other surrounding structures. In this paper, the tug line forces and planar motion of an offshore structure are summarized dependent on the tug line length and line material characteristics in the states for a given floater draft and weather criteria. The tug line dynamic forces and planar motion evaluated here will be used to determine the proper tug power and clearance of an offshore structure.

Growth factor [basic fibroblast growth factor (bFGF) or epidermal growth factor (EGF)]-immobilized polycaprolactone (PCL)/Pluronic F127 microspheres were fabricated to investigate their potential use as an injectable bioactive filler for enhancing soft tissue augmentation. It was expected that the microspheres may stimulate the regeneration of soft tissues by the sustained release of the growth factors as well as provide a bulking effect by the injected volume of the microspheres, and thus allow more effective long-term filling effect. The PCL/F127 microspheres were fabricated by an isolated particle-melting method, and the growth factors were easily immobilized onto the surfaces of the PCL/F127 microspheres via heparin binding. From an in vivo animal study, it was observed that the mixture of both bFGF-immobilized and EGF-immobilized microspheres lead to effective soft tissue augmentation.

최근 방 탈출 게임이 두뇌 활성화에 도움이 된다고 알려지면서 유행이 되었다. 하지만 PC, 모바일의 경우 몰입도가 떨어지고, 현실의 경우 시간과 장소의 제약이 생긴다. 이러한 제약 요소를 해소하기 위해 기존의 방 탈출 게임과 VR기술을 접목시킨 몰입형 미션 시뮬레이션 게임을 기획하게 되었다. 본 논문에서는 VR 방 탈출 시뮬레이션 게임을 제작하기 위해 사용된 S/W와 H/W 구성도, 기능 및 시스템 시나리오 그리고 게임을 통해 얻을 수 있는 기대효과를 4가지 유형으로 기술하였다.
