소아 다발성 외상 환자의 중증도에 영향을 미치는 위험인자에 관한 분석

Purpose: The purpose of this study is, first, to analyze the risk factors that influence the severity of injury in pediatric multiple trauma patients and, second, to present solutions for the problems related to the treatment of such patients. Our living situations are so complicated that the danger of accidents is truly open to children who are not prepared. We need to draw attention to the increased numbers of various accidents involving children. Methods: We studied patients who visited the Emergency Medical Center at Chosun University Hospital from January 1, 2006, to December 31, 2008. Using medical records, we evaluated the general characteristics: the mechanism of injury, the vital signs, the revised trauma score (RTS), the injury severity score (ISS), and the hemoglibin value, which was checked at the time of visit, and the presence or the absence of emergent on regular surgery. We divided the level of injury as follows: light level (1-8 points), mid level (9-15 points), and serious level (above 16 points). We analyzed the medical data by using SPSS 17.0 for windows. Results: The average age of the patients examined was 8.6 years. The number of 6- to 11-year-old patients was 96, which was the largest, but the degree of injury severity was the highest among infant (0-2 years), according to ISS 7.95(±6.85). The frequency of accidents was highest on sunny days, and most accidents occurred from 16:00 to 20:00. The cause of multiple trauma for children was the greatest in the traffic accident, (95 patients, 49.0%). In addition, the trauma caused by traffic accidents showed the highest ISS value (9.02± 6.42) and the most serious degree (P=0.004). The ISS level of injury (8.40±6.64) for patients moved from a secondary hospital was higher than that (6.49±5.57) for patients who visited the medical center directly. The severity of injury was highest for patients who used a 119 ambulance (8.84±5.80). According to the injured parts of body, Injuries to the arms and the feet most frequent (79 patients, 40.7%), but the level of injury was the highest for internal organs and chest, 16.42± 8.56 and 11.23± 6.97, respectively. Conclusion: We used Abbreviated Injury Scale (AIS) in order to examine the characteristics by injured body part for pediatric multiple trauma patients. Because the degree of injury was the highest for internal organs or the chest, we need to more seriously examine and provide for patients who are suspected of having injuries to the internal organs or the chest. We need to quickly determine the need for surgery in patients with serious injuries to the arms and the feet, which is the greatest in frequency. In particular, we need to consider the surgical care of patients who are not very high in the severity of injury to their brains. (J Korean Soc Traumatol 2010;23:68-74)

N/A To investigate cell mediated immunity in chronic renal failure, delayed cutaneous hypersensitivity responses to 7 microbial antigens and peripheral blood T cell subsets using monoclonal antibodies against T helper-inducer, T cytotoxic-suppressor and total T cell subsets were measured in 23 uremic patients and 17 healthy controls. IgG production by peripheral blood lymphocytes in response to PWM triggering was evaluated. The results were summarized as follows: 1) Total peripheral blood lymphocyte counts were significantly decreased in patients with chronic renal failure. 2) T cell subsets in uremic patients were not different from those in healthy controls. 3) Delayed cutaneous hypersensitivity was significantly depressed in uremic patients, 4) IgG production by peripheral blood lymphocyte culture in response to PWM triggering was significantly decreased in patients with chronic renal failure. In conclusion, decreased peripheral blood lymphocyte counts and impairment of T cell function other than T cell subsets are thought to be involved in depressed cell mediated immunity in chronic renal failure.
고전적 및 삼각조각 기법을 이용한 단-측면 미세동맥 접합술의 실험적 연구

Problems of composite tissue transfer commonly arise when a single indispensable recipient vessel receives the graft vssel, and the graft vessel must be sutured in end-to-side fashion so as not todisturb the vascularity of the recipient vessel. The triangular flap in the recipient vessel wall gives an intact endothelial surface when the flow of blood stream is presented and may reduce the chance of anastomosis. We selected mature Wistar rats weighing over 450 grams to compare the conventional longitudinal slit from the triangular flap in the recipient carotid artery over bloood pressure and blood flow when the donor carotid artery was anastomosed in end-to-side fashion. In 30 minutes after anastomosis, maximum blood pressure measured in the donor carotid arterial side when the recipient arterial wall was fasioned with the longitudinal slit was recorded 114 mmHg and with the triangular flap 100mmHg. Minimum blood pressure with the longitudinal slit was 98mmHg and with the triangular flap 88mmHg. The amount of blood collected for 30 seconds in the conventional longitudinal slit was 1.18mg and in the triangular flap 0.78mg. Histology study in 30 minutes, the conventional longitudinal slit demonstrated the more hemorrhagic features around the suture material compared to that of the triangular. flap and, in the 7th day, the conventional longitudinal slit demonstrated the more prominent granulomatous reactions and vascular proliferations around the suture material compared to that of the triangular flap.

Free tissue transplantation is commonly performed with the brilliant achievement in microsurgery and anticoagulants and antithrombotic agents have been prescribed in the procedures. However, there is no clean-cut indication as to which agents would be more effective in every steps and final consequences. Low molecular weight heparins inhibiting coagulation in plateletrich plasma and acting on the vascular endothelium have antithrombotic and fibrinolysis action. The experiment with rat groin free flap transplantation after 6-hour ischemia and injection of the low molecular weight heparin was performed and the results between the injection and non-injection group were analysed as follows, 1. Both of the 24-hour groups, vessel patency was not proportional to color change of the groin flap. 2. On the second day after anastomois, heparin-injection group showed intact intima, patent lumen without thrombus, and mild granulomatous inflammation around the suture material and control group with doubtful patency revealed intimal loss and thrombus formation. 3. On the 5th, 7th, and 9th postoperative day, heparin group was patent in anastomosis and showed acute inflammatory cells. 4. The 7th-week period, heparin-injection group showed intact flap color, patent lumen with intact intima and persistent foreign body granuloma.

The effect of isoparaffin on the rheological properties of polytetrafluoroethylene (PTFE) was investigated. PTFE/isoparaffin paste was prepared for two different types of isoparaffin, that is, Isopars G and M, which can be used as PTFE lubricants. Molecular dynamics (MD) simulations were applied to investigate the effect of the lubricants on the zero-shear viscosity of the PTFE/isoparaffin system. The Green-Kubo relationship was used to obtain the zero-shear viscosity through the MD simulations. In the relationship, the autocorrelation function of the shear stress was calculated and integrated over time. The shear viscosity of PTFE/Isopar G was lower than that of PTFE/Isopar M, corresponding to the experimental results.
