        Head-up CPR 시 처치자의 위치에 따른 심폐소생술 정확도 비교

        Purpose: This study aimed to identify the position for the most accurate head-up cardiopulmonary resuscitation (CPR) by comparing saddle position CPR (SPCPR) and traditional CPR (TCPR). Methods: Sixty certified persons who completed a basic life support provider course between May 1 and June 21, 2019 were enrolled in the study. The participants were asked to perform 2 minutes of CPR, and the depth of chest compression, rate, position, full release, and hands off time were assessed. Accuracy was evaluated based on data collected from a smart phone application connected to the manikin via bluetooth and analyzed using frequency, percentage, t-test, analysis of variance and ${\chi}^2$. Results: The accuracy of chest compression was statistically significantly higher for SPCPR, 63.03%[${\pm}8.75$] for SPCPR and 55.50%[${\pm}10.17$] for TCPR [t=3.074, p=.003]. The depth of chest compression was statistically significantly greater for SPCPR, 4.51cm[${\pm}0.45$] for SPCPR and 4.16cm[${\pm}0.61$] for TCPR [t=2.503, p=.015]. The rate of chest compression was statistically significantly higher for TCPR, 105/min[${\pm}10.79$] for SPCPR and 111/min[${\pm}11.57$] for TCPR [t=-2.008, p=.049]. Accuracy of position of chest compression was statistically significantly higher for SPCPR, 96.10%[${\pm}13.73$] for SPCPR and 79.93%[${\pm}30.34$] for TCPR [t=2.659, p=.011]. Accuracy of full release was higher with SPCPR, with 86.30%[${\pm}30.53$] for SPCPR and 71.10%[${\pm}36.05$] for TCPR, but the difference was not statistically significant [t=1.762, p=.083]. Conclusion: Saddle position CPR was found to be more accurate than TCPR in the performance of manual head-up CPR.

        응급구조사의 업무 특성에 따른 보수교육 만족도 및 교육요구도

        Purpose: To assess the satisfaction of continuing education and need assessment according to job characteristics in emergency medical technicians (EMTs). Methods: A self-reported questionnaire was completed by 222 EMTs from May 28 to July 6, 2018. It consisted of questions on the method and need of continuing education, and satisfaction regarding education time, assessed using a 5-point Likert scale. Descriptive statistics, a chi-square test, and ANOVA were used to analyze the data. Results: The responses to questions on the method of continuing education (${\rho}=.000$), satisfaction of regarding the duration of continuing education (${\rho}=.029$), method (${\rho}$< .001), and topic (${\rho}=.000$) varied according to the differences in job characteristics of EMTs. Assessment and management of patients with multiple trauma ($4.17{\pm}.735$) emerged as the most important while license examination protocol for EMTs ($3.33{\pm}.968$) was rated the least important among topics for continuing education. Separation of continuing education (${\rho}$< .001), education method (${\rho}$< .001), education topic (${\rho}$< .001), and necessity of quality management of continuing education (${\rho}$< .001) differed according to the job characteristics of EMTs. Conclusion: It is important to provide a suitable program catering to the diverse requirements of EMTs. The education programs must be tailored to the needs of level 1 and level 2 EMTs separately. To improve the quality of education, it is necessary to evaluate the continuing effects.

        보건계열 대학생의 그릿, 진로준비 행동이 학업 성취도에 미치는 영향

        이 연구는 보건계열 대학생의 그릿, 진로준비 행동을 살펴보고 학업 성취도에 미치는 요인을 확인하고자 하였다. D소재 일개 4년제 대학 보건계열 대학생 241명을 대상으로 구조화된 설문지를 이용하여 자료를 수집하였다. 연구결과 연구대상자의 그릿은 3.32점, 진로준비 행동은 2.87점, 학업 성취도는 3.44점이었다. 일반적 특성에 따른 학업 성취도는 학과선택 동기, 전공 만족도, 학교생활 만족도에 따라 통계적으로 유의한 차이가 있었다. 그릿, 진로준비 행동, 학업 성취도의 상관관계를 분석한 결과 학업 성취도는 그릿, 진로준비 행동과 유의한 정(+)의 상관관계를 보였다. 학업 성취도에 영향을 미치는 요인으로는 전공 만족도(β=.279 p<.001)와 그릿(β=.224 p=.001)이었으며 설명력은 23%였다. 전공 만족도와 그릿에 영향을 미치는 요인들을 확인하여 다양한 학습 전략을 모색하여 학업 성취도를 향상 시킬 수 있는 중재 프로그램이 마련되어야 할 것이다. The purpose of the study is to identify the grit and career preparation behaviors of healthcare major college students and to analyze factors associated with their academic achievements. A structured questionnaire survey was carried on 241 students at an university in D-city. The scores for grit, career preparation behavior, and academic achievements were 3.32, 2.87, and 3.44, respectively. The study results show statistical differences among academic achievements, motivation to choose major, and satisfaction with major and university. The strong and positive effects of grit and career preparation on academic achievements were found. The satisfaction with major(β=.279 p<.001) and grit (β=.224 p=.001) explained 23% of academic achievements. The results suggest various intervention programs based on multiple factors associated with grit and major satisfaction in order to enhance academic achievements.

        Purpose: To discuss the records and legal standards of emergency medical technician training institutions in Korea and abroad, to identify the problems, and to provide the basic resources for improving the EMT-basic training institutions. Methods: We received advice through an advisory meeting of experts (professors of department of emergency medical technicians) and interested parties (Korean Association of Emergency Medical Technicians, officials of emergency medical technician training institutions) and referred to various reports published by governments, official institutions, and other trustworthy organizations. Also, we communicated with the related experts abroad (3 countries) on the phone or by email for surveys. Results: Compared to the abroad, it is necessary to categorize the standards and procedures of designating the emergency medical technician training institutions in Korea and improve the management of training institutions to train competent emergency medical technicians. Conclusion: It is necessary to designate and manage continuously the emergency medical technician-basic training programs for the systematic primary healthcare service.

        심페소생술 시행 시에 휴대용 압박 센서 활용이 흉부압박의 질에 미치는 영향

        본 연구는 평균 이송시간에 따른 흉부압박을 심정지 환자에게 수기로만 이루어진 압박과 깊이 보조 장치를 사용한 압박을 적용하여 그 결과를 분석하고 그에 따른 영향을 규명하여 심정지 환자의 심폐소생술을 위한 기초자료 를 파악하고자 시도되었으며 2011년 9월부터 2012년 5월까지 청주시 소재 소방서에 근무하고 있는 1급 및 2급 응급 구조사 20명과 BLS provider를 이수한 응급구조과 학생 20명을 대상으로 수기로만 이루어진 흉부압박과 깊이 보조 장치를 사용한 흉부압박의 결과 기록지를 비교 분석하였다. 수기로만 이루어진 흉부압박은 시간이 지남에 따라 압박 깊이는 떨어지고 횟수는 증가했으며 깊이 보조 장치를 사용한 흉부압박은 압박깊이를 유지하고 횟수는 일정하게 유 지하는 기능을 나타내었고 두 그룹 모두 깊이 보조 장치를 사용하였을 때 압박 정확도는 의미 있는 차이를 보여 심 폐소생술 시행 시에 깊이 보조 장치를 사용하는 것이 심정지 환자에게 유용한 영역이라 사료된다. This study is to collect a basic data of how Cardiopulmonary Resuscitation (CPR) procedure can influence to cardiac arrest patient with and without the Depth Device during the average transport time period. The data has achieved by comparing result sheet of CPR procedure by hands only versus with Depth Device by twenty 1st and 2nd class Emergency Medical Technician (EMT) from five different fire stations in city of Chong-Ju, and twenty Emergency Rescue major students who completed the BLS provide course. The experiment participators experienced loss of compression depth and rate increase over time. However, the CPR procedure with Depth Device shows that both EMT and students to allow maintaining both the compression depth and rate. The experiment leaves a positive result for CPR operators and considers being valuable domain for cardiac arrest patient.

        119구급대원의 COVID-19감염관리에 대한 지식 및 교육 요구도

        본 연구의 목적은 119구급대원을 대상으로 COVID-19에 대한 지식 및 교육 요구도를 확인하여 신종감염병 대비 시뮬레이션 교육 프로그램 개발에 기초자료를 제공하고자 시도되었다. 186명의 119구급대원을 대상으로 2020년 11월 15일∼11월 30일까지 구조화된 설문지를 통해 자료를 수집하였다. 연구 결과 COVID-19 감염관리 교육을 받지 않은 경우가 98명(52.7%)이었고 COVID-19감염관리 지식은 25점 만점에 18.21±1.98점이었으며 환경관리 영역에 대한 정답률이 가장 낮았다. COVID-19 감염관리 교육 요구도는 총 평균은 3.81±.28(4점 척도)점으로 나타났으며 전파경로 및 확산 예방과 직원 안전관리 영역에 대한 교육요구도가 가장 높았다. 일반적 특성에 따른 지식은 성별(t=-1.999, p =.047)에 따라 통계적으로 유의한 차이가 있었으며 교육 요구도는 경력(t=-1.999, p =.047), 교육수준(t=2.336, p =.021)에 따라 유의한 차이가 있었다. 이에 따라 119구급대원을 대상으로 COVID 감염관리 교육 프로그램 개발 시 전파경로 및 확산 예방, 직원안전, 환경관리를 포함하는 것이 필요하다. This study aimed to provide the basic data for the development of a simulation training program for new infectious diseases by analyzing the knowledge and educational needs of 119 paramedics regarding COVID-19 infection control. Data was obtained through a structured questionnaire survey of 186 paramedics from November 15th to 30th 2020. The study showed that 98 of the 119 paramedic subjects (52.7%) had not been educated regarding COVID-19. The knowledge level was 18.21±1.98 out of 25 points, with environmental control securing the lowest correct answers. The highest need for education was in the areas of prevention of transmission and spread, and employee safety control. The total average for educational needs was 3.81±.28 (4 point scale) with the highest educational need in the area of prevention of the spread and dispersion of the disease and employee safety management. There was a statistically significant difference in the knowledge of the general characteristics according to gender (t=-1.999, p =.047) and the educational demand was related to career (t=-1.999, p =.047) and, education levels (t=2.336, p =.021). Accordingly, it is necessary to plan a new infectious disease simulation education program that addresses the low-scoring areas and items with high educational needs which include the propagation path and spread prevention, environmental management, and employee safety management as per the findings of this study.
