그림에서 표현된 조현병 환자의 증상 진행과정 고찰

조현병은 그 어느 때보다 현대사회에서 더욱 관심을 갖게 된 정신질환 중 하나이다. 다른 심리적 질병과는 달리 조현병을 앓는 환자는 현실감의 무너짐을 특징으로 하는 다양한 망상, 환각 증상들로 인하여 심각한 고통을 겪을 뿐 아니라 조현병에 이해에 대한 부족으로 사회 전반에 만연한 오해와 편견으로 힘듦을 경험한다. 본 고찰에서 구체적인 형태로 토의하기 위해서 정신분석과 미술심리치료라는 두 가지 모델을 가지고 이를 통해 증상의 원인, 증상의 진행과정, 증상의 발현, 치료를 포함한 다차원적인 방향에서 조현병을 검토해 보고자 한다. 또한, 현대정신의학이 규정하는 조현병의 핵심증상인 망상, 환각을 정신분석적 관점과 미술심리치료에서의 그림분석으로 검토해 보려고 한다. 조현병 환자의 그림사례를 제시하여 그림에서 표현된 요소들의 그림분석을 통하여 조현병으로의 진행과정에 대한 부분들을 살펴보면서 정신증에 해당하는 조현병 스펙트럼 장애에 대한 이해를 돕고자 한다. 따라서, 본 연구는 조현병 증상에 대한 정신분석적 미술심리치료 고찰의 시도이다. Schizophrenia is one of the mental disorders that have attracted more attention in modern society than ever before. Unlike other psychological diseases, patients suffering from schizophrenia not only suffer from various delusions and hallucinations characterized by a collapse of reality, but also suffer from widespread misunderstanding and prejudice throughout society due to lack of understanding of schizophrenia. to experience In order to discuss in a specific form in this review, we will take two models, psychoanalysis and art psychotherapy, and examine schizophrenia from a multidimensional direction including the cause of symptoms, progression of symptoms, manifestation of symptoms, and treatment. In addition, delusions and hallucinations, which are the core symptoms of schizophrenia defined by modern psychiatry, will be reviewed from a psychoanalytic perspective and picture analysis in art psychotherapy. By presenting examples of pictures of patients with schizophrenia, through picture analysis of the elements expressed in the pictures, we try to help understand the schizophrenia spectrum disorder corresponding to psychosis by examining the parts of the process of progressing to schizophrenia. Therefore, this study is an attempt to consider psychoanalytic art psychotherapy for symptoms of schizophrenia.

N/A Ascaris lumbricoides infection is usually asymptomatic, but when invasion of the ascaris to the human biliary tract is a prominent medical and surgical problems. Here we have two case reports of common bile duct ascariasis, which clinical manifestations were different each other, one is acute pancreatitis form and the other is acute cholangitis form. The management of biliary obstruction caused by ascaris lumbricoides has usually been surgical, but this report describes the endoscopic removal of the ascaris located in the common bile duct.

N/A To evaluate the efficacy and safety of recombinant interferon alpha-2b (INTRONA®), in eleven patients with chronic active hepatitis (CAH) type B, a course of 28-day Intron A® was given daily subcataneous injection at a dose of 1 milion unit (MU) on the first day, 3 MU on the second, 5 MU on the 3rd to 7th, 3 MU on the 8th to 28th each. In 10 patients with HBeAg positive CAH type B, DNA polymerase and transaminoferase activities were assessed and compared before, during and after INTRON A administration. The peripheral blood white blood cell (WBC), granulocyte and platelet counts were checked. The results were as follows: 1) during administration of INTRON A, DNA polymerase activities were decreased significantly within first week and sustained in spite of persistent HBe antigenemia in all patients (p<0.05). Serum alanine aminotransferase (ALT) and aspartate aminotransferase were decreased significantly from 2 and 3 weeks after therapy respectively (p<0.01). 2) After discontinuation of INTRON A, improved transaminase level lasted for 4 weeks but rebound phenomena of DNA polymerase activities were observed 7 cases (70%) within 2 weeks, 3) during follow up period, transient increase of serum ALT activities were observed 9 cases within 12 weeks. One case of seroconversion (Anti-HBe) and one case of transient loss of HBeAg were observed at 16 week during mean follow up period of 30.3 weeks (range 16-58 weeks). But transient increment of serum ALT activities were observed 9 cases (90%) within 16 weeks. 4) In all 11 patients, peripheral white blood cell, granulocyte, platelet counts were decreased significantly within first week but not decreased further during INTRON A administration and recovered within 2 weeks after discontinuation of INTRON A. 5) Myalgia and fever were noted in 10 cases (91%) and headache and high fever (>38'C) were noted in 5 cases (45%) within 4 days. Mild anorexia (36%), hair loss (27%), diarrhea (9%), nausea (9%) were noted after 2 weeks. It suggested that subcutaneous injection of 3 MU INTRON A was safe and effective in decrement of DNA polymerase and transaminase activities in patients with CAH type B transiently. Long term therapy will be nessessary for the eradication of HBe Antigenemia and clinical improvement.

β-Fluoroacetate 중독의 임상적 관찰

Intoxication of a kind of rodenticide, β-fluoroacetate is prevalent in rural area of our country. There are seversl reports concerning the clinical features of β -fluoroacetate intoxication. We experienced nonspecific manifestations of the intoxication which are not mentioned in such reports. Here we report the clinical features of the intoxication including nonspecific manifestations. We also investigated the prognostic factors. We reviewed 30 intoxicated patients who admitted to Shinchun General Hospital between November 1984 and September 1g85 and the results were as follows: l) Females were more frequently(10%) and patients of third R fourth decades were most frequently affected(30%). We expirenced 14 cases of mental deterioration(46.6%), 5 cases of convulsion(16.6%), 5 cases of nausea and vomiting(16.6%) and 4 cases of epigastric pain(13.3%), 3 cases of abnormal liver function(10.0%), 3 cases of increased RUN and creatinine(10.0%), and 13 cases of hypocalcemia(43.3%) were found. 2) Nonspecific manifestations which were not documonted in previous reports include petechiae in the chest and abdomen in 4 cases(13.3%), increased fasting blood sugar in 11 cases(36.6%), and increased serum amylase in 5 cases(16.6%). 3) Mental deterioration, leukocytosis, hematuria and proteinuria were more frequent in patients of worse prognosis.

Diverticular disease of the colon is frequently involved in Western countries, which in korea, it is regarding as rare disease. In Western the diverticulosis is mainly affected on left side and increasing in according to age and false type is more frequent than ture type. While in oriental countries, the diverticulosis of the colon is mainly affected on right side and it is more frequent in younger patients. Cecal diverticulitis is known to a very rare disease and very difficult to be differenciated from acute scopy has purnished another useful tool in the differential diagnosis of diverticular disease. We experienced a case of cecal diverticultitis which had been diagnosed by colonofiberscopy and reviewed literatures briefly.
