이 연구는 현재 태권도장에서 이루어지고 있는 품새지도의 방법에 대한 문제점을 분석함으로써 이를 통해 품새지도의 방향성을 탐색하고자 하였다. 이 연구의 연구방법은 질적 연구방법을 활용하였으며, 이에 필요한 연구참여자는 4년제 대학에서 태권도학을 전공한 품새경기 입상자로 하였다. 이러한 조건에 의해 선정된 연구참여자들에게 개방형설문과 심층면담을 실시 하였고, 이에 대한 종합적인 결론은 다음과 같다. 첫째로 태권도장의 품새지도에 대한 인식은 승급, 승품단을 위하여 지루하지만 불가피하게 동작을 외워야하는 과정으로 인식되고 있다. 둘째, 태권도장의 품새지도 방법은 이론과 동작이 서로 조화를 이루어 진행되어야 한다는 것에 모두 의견을 모으고 있지만 사실상 동작 위주로 편중되어 있다는 문제점이 도출되었다. 셋째, 태권도장에서의 품새수련을 통해 도달할 수 있는 교육적 가치인 품새선, 품새의 의미, 신체기능의 향상 요소, 집중력, 인내심 등을 체계적으로 설명하면서 지도할 수 있는 지도법과 기준점 제시가 요구되고 있다. 따라서 태권도장에서는 이와 같은 문제점을 인식하고 태권도 품새지도를 함에 있어서 지도자들은 태권도의 전반적인 교육적 가치를 지도할 수 있는 방향으로 개선되어야 할 것이다. The purpose of this study is to draw out current problems and future directions concerning the method of instructing Poomsae at Taekwondo Academy through exploration on intrinsic meaning of Poomsae instruction. The subjects of this study were people who both graduated with a bachelor``s degree in Taekwondo from four-year colleges and won in contests. Based on such conditions, an open survey was conducted. First collective conclusion from this survey is that recognition of Poomsae instruction could be boring although unavoidable for a promotion. It should be improved from memorizing movements to recognizing the possibility of teaching educational value of Taekwondo. Second, although the consensus is that method and theory of Poomsae instruction should be in harmony, but in reality, Poomsae instruction is more focused on movements. Third, presentation of standard and instruction method is required to systematically describe educational values such as Poomsae line, meaning of Poomsae, elements of improving physical abilities, concentration, and patience that can be obtained through Poomsae practice.

In this study, Integrity evaluation of offshore wind turbine substructure was carried out. The strain values obtained by strain gauges circularly installed on upper part of the substructure was converted into vertical and moment load. Considering directions of the moment load through FEM, the most conservative P-M interaction diagram of the model was graphed. Obtained load values were evaluated by utilizing P-M interaction diagram.

In this paper, we research about integrated jacket support structure. The connection of the offshore wind support structure was designed based on the DNV-RP-C201 standard and the design of the appropriate type was derived through numerical calculation and finite element analysis of the stress paths of various connecting part type.

The transition piece of the offshore wind power support structure transmits the load of the tower stably to the support structure on the lower side. The transition piece of the offshore wind power support structure should solve the stress concentration problem in design. In this paper, in order to solve the stress concentration problem occurring at the transition piece of the offshore wind power support structure, the location and the mitigation of the stress concentration have been studied.

Transition piece of offshore wind turbine tower is one of the main parts which takes the direct load and pass on to the lower support structure. In this study, a new design method for the transition piece has been developed. The trasition piece was designed based on the the load flow which was analyzed by approximation and was evaluated by FEM.
증례 : 지역사회에서 획득된 Acinetobacter baumannii에 의한 감염성 심내막염 1예

기저 질환이 없는 젊은 여성이 급성 뇌경색이 발생하여 내원하였으며 A. baumannii 균혈증이 확인되었다. 경흉부 심초음파에서 우식증이 관찰되어 심내막염 진단과 함께 cefoperazon/sulbactam을 총 5주 동안 사용한 결과 추가적인 합병증이 발생하지 않고 심내막염이 완치되었다. 지역사회 획득 A. baumannii에 의하여 발생한 감염성 심내막염의 국내 첫 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Acinetobacter baumannii has become an important pathogen that causes healthcare-associated infections, including pneumonia, bacteremia, and urinary tract infection. Only sporadic cases of infective endocarditis (IE) have been reported worldwide, and in particular, community-acquired IE caused by A. baumannii is very rare. To our knowledge, no case has ever been reported in Korea. We report a 30-year-old woman who recovered from native-valve community-acquired IE caused by A. baumannii. She did not have any underlying diseases and presented with left-sided weakness, dysarthria, and fever. On investigation of acute cerebral infarction, a vegetation on her anterior mitral leaflet was found in echocardiography, and A. baumannii was cultured from the blood. In this case, A. baumannii showed intermediate resistance to cefotaxime and was sensitive to all other antibiotics. She underwent antibiotic therapy with cefoperazone/sulbactam for 5 weeks, without surgery. Consequently, the vegetation was absent in follow-up echocardiography, and no complications related to IE developed. (Korean J Med 2013;84:313-317)

폐흡충증은 덜 조리된 민물게나 가재에 있는 폐흡충증의 피낭 유충을 섭취하여 생기는 기생충 질환이다. 이는 감소하는 경향이지만 한국과 일본 등 유행하는 지역에서는 여전히 폐와 흉막 질환을 일으키는 중요한 원인으로 고려되어야 한다. 그리고 흔하지는 않지만 뇌, 척수, 복강 및 골반 장기나 피하조직에 이소성 병소를 유발할 수 있다. 저자들은 흉막염을 동반한 폐결핵과 별개의 질환으로서 병발한 유방 농양이라고 오인했다가 이소성 유방 농양과 흉막염을 동반한 폐흡충증으로 진단된 증례를 보고한다. Paragonimiasis is an infectious disease caused by consumption of raw or improperly cooked freshwater crab or crayfish contaminated with Paragonimus metacercariae. The incidence of the disease has markedly decreased, but it is still a lung disease that requires a differential diagnosis in endemic areas such as Korea and Japan. It is commonly found in the lung but has also been found as extrapulmonary infestations, such as cerebral, spinal, subcutaneous, abdominal, urinary, and gynecological infestations. We report a rare case of ectopic paragonimiasis involving the breast with pleural effusion that was initially misdiagnosed as tuberculous pleurisy and a breast abscess. (Korean J Med 2011;81:502-507)

Exogenous lipoid pneumonia occurs rarely in healthy people. In most cases, exogenous lipoid pneumonia is usually caused by aspiration of mineral, vegetable, or animal oil. We report the case of 42-year-old woman, who have experienced lipoid pneumonia after silicon injection into her breast for cosmetic purposes. The patient experienced fever, dyspnea, sputum, and hemoptysis after silicon injection into her breast. Chest computed tomography demonstrated non- segmental distribution of bilateral consolidation in both lung fields. A transbronchial lung biopsy specimen shows foamy microphages in alveolar spaces. Papanicolaous staining of bronchoalveolar lavage fluid showed abundant foamy marcrophages and many neutrophils. With these results, we confirmed lipoid pneumonia was associated with silicon oil injection into breast.

Background: In the present study, we examined the effect of morphine on NO- and peroxynitriteinduced cell death using a human neuroblastoma SH-SYSY cell line which abundantly expresses μ, δ and κ-opioid receptors. Methods: The cultured cells were pretreated with morphine (100 μM) and exposed to 3-morpholino- sydnonimine (SIN-I, ImM). Agarose gel electrophoresis of DNA was done with the extracts from SH-SYSY cells. The cells were treated with selective ligands for opioid receptor subtypes and with PI3-kinase inhibitors. Cell damage was assessed by using an MTT assay. Spectrophotometric absorption spectra were measured from the mixture of morphine (100 μM) plus peroxynitrite (I mM) at room temperature. Results: SIN-1 treated cells showed the occurrence of a specific form of chromosomal DNA fragmentation which pretreatment with morphine inhibited. We selective ligands for opioid receptor subtypes, [D-Ala^2, N-Me-Phe^4, Gly-ol5]enkephalin (DAMGO, μ-opioid receptor agonist), [D-Pen] enkephalin (DPDPE, δ-opitor agonist) and U-69593 ( κ-opioid receptor agonist) at a con- centration of 10 pM did not prevent the cell death induced by SIN-1. Naloxone (20 μM) hardly antagonized the effect of morphine in SIN-1-induced cell death. The PI3-kinase inhibitors Wortmannin and LY294002 did not inhibit the action of morphine on apoptotic cell death. In the measurements of spectrophotometric absorption spectra, the peak of the absorbance of the mixture of morphine plus peroxynitrite at 295 - 300 nm disappeared three minutes after mixing. Conclusions: The present study showed that morphine protected the human neuroblastoma cell line, SH-SY5Y, from peroxynitrite-induced apoptotic cell death. However, it is suggested that the protective action of morphine is not via the activation of opioid receptors and/or the PI3-kinase pathway but possibly via direct chemical reaction.
