        Purpose. This study was to correlations between oral health education experience and subjective oral health level of elderly in Yeongnam region. Methods. The data were collected from 254 elderly in Yeongnam region. Data analysis was performed using cross Analysis, t-test(One-way ANOVA), logistic regression by SPSS WIN 17.0 program. Results. According to the general characteristics of the study subjects, 42.8% of those under 75 years and 57.2% of those over 75 years of age had never been educated, 50.7% of living expenses less than 1 million won and 37.3% of those below 2 million won had never received oral health education (p<.05). The differences in subjective oral health levels with or without oral health education experience showed higher limitations of function and impairment of social psychic function in the elderly with no experience than those with oral health education experience(p<.05). Correlation between oral health education experience and subjective health level of the elderly showed a positive relationship with each factor, meaning that the subjective oral health level was higher with oral health education(p<.05). Subjective oral health levels for functional limitations or social and psychological disorders were high in the elderly without oral health education(p<.05). Conclusion. Considering the impact of oral health education experience on the subjective oral health level of the elderly as above, we should develop various continuous and systematic programs that can increase prevention and post-education practices for the increase of education beneficiaries through diverse approaches to enhance their usability.

      • 치위생과 학생들의 전공만족도가 진로결정 자기효능감 및 진로준비행동에 미치는 영향

        Purpose. This study was to effects of major satisfaction of dental hygiene students and students on self-efficacy and career preparation behavior by investigation to provide basic data for developing a systematic career customized employment support program. Methods. The data were collected from 249 students from December 1 to 10 day 2016 in Yeongnam region. Data analysis was performed using descriptive statistics frequency, t-test(One-way ANOVA), logistic regression by SPSS WIN 18.0 program. Results. According to the general characteristics department dental hygiene students were major satisfied with 59.8%. Satisfaction with career decision self-efficacy and career preparation self-efficacy was found to $68.30{\pm}10.21$ statistically significant(p<.001). Satisfaction was also found to statistically significant ($32.65{\pm}6.05$) in career preparation behavior(p<.05). Satisfactory scores were found to 1.306 times higher than the dissatisfied students in the career decision self-efficacy goal setting. It was found that 8.423 times in satisfaction of career planning in career preparation. Conclusion. Considering the effects of dental hygiene students' major satisfaction on career decision self - efficacy and career preparation behavior, it would be good if we could develop and use various programs to improve the satisfaction of students in their majors.

        성별에 따른 치과위생사의 직무만족 감정노동 이직의도와의 융합적 관련성

        본 연구는 치과 의료기관의 성별에 따른 치과위생사를 대상으로 직무만족, 감정노동, 이직의도의 상관성를 자기기입식 설문조사를 실시하였다. 남성의 경우 직무만족은 금연에서 높고, 감정노동은 높은 연령이 낮았다. 이직의도는 35세 이상에서는 낮고 직원수가 11-20명, 흡연에서는 높았다. 여성인 경우 직무만족은 10년 이상 경력, 대학졸, 직원 수 20명 초과가 높았으며 감정노동은 높은 연령이 낮고 음주에서 높았다. 이직의도는 전문대졸, 미혼, 음주가 높았다. 남성이 여성에 비해 감정노동이-0.22배 적었으나 이직 의도는 3.9배로 높았다. 직원 수는 10명 이상이 2.7배로 높았다. 따라서 성별에 따른 차이점을 고려하여 직무만족을 높이고 감정노동과 이직의도를 줄이는 노력이 체계적으로 이루어져야 것이다. This study conducted a self-written questionnaire survey on the correlation between job satisfaction, emotional labor, and turnover intention of dental hygienists according to the gender of dental medical institutions. Job satisfaction was high smoking cessation, emotional labor was low age. Turnover intention was low in over 35 years old, number of employees was 11-20, was high in smoking. For women, job satisfaction was high 10 years experience, university, 20 employees. Emotional labor was low age, drinking. Intention to turnover was high in college, single, drinking. Men's emotional labor was 0.22 times less than women's, but their turnover intention was 3.9 times higher. The number of employees was 2.7 times higher than 10 employees. Therefore, efforts to raise job satisfaction and reduce emotional labor and turnover intentions should be made systematically in consideration of gender differences.

        일부지역 노인들의 만성질환 경험에 따른 구강상태와 구강건강관리 효능감 및 주관적 구강건강수준에 미치는 영향

        Objectives: The purpose or this study was to general characteristics and body oral condition, effects of chronic diseases experience according to oral condition, self-efficacy scale for self-care and subjective oral health level elderly in Daegu-Kyeongbuk region. Methods: The data were collected from 250 Daegu-Kyeongbuk elderly in region. Data analysis was performed using cross analysis, t-test, logistic regression by SPSS WIN 18.0 program. Results: The statistically significant differences chronic disease experience high level of using dentures elderly (ρ<.05). There were many elderly people who had a chronic illness but thought that their own health was more than normal, and there was a statistically significant difference (ρ<.01). The statistically significant difference chronic disease showed high self-oral management efficacy and group did not experience(ρ<.001). Conclusions: A lot of research is needed to establish various programs related to considering the close relationship between oral health and chronic disease

        칼슘포스페이트 나노-크리스탈이 코팅된 골이식재와 자가골을 병행 이용한 상악동 거상술

        Purpose: Rehabilitation of the edentulous posterior maxilla with dental implants often poses difficulty because of insufficient bone volume caused by pneumatization of the maxillary sinus and by crestal bone resorption. Sinus grafting technique was developed to increase the vertical height to overcome this problem. The present study was designed to evaluate the sinus floor augmentation with anorganic bovine bone (Bio-$cera^{TM}$) using histomorphometric and clinical measures. Patients and methods: Thirteen patients were involved in this study and underwent total 14 sinus lift procedures. Residual bone height was ${\geq}2mm$ and ${\leq}6mm$. Lateral window approach was used, with grafting using Bio-$cera^{TM}$ only(n=1) or mixed with autogenous bone from ramus and/or maxillary tuberosity(n=13). After 6 months of healing, implant sites were created with 3mm diameter trephine and biopsies taken for histomorphometric analysis. The parameters assessed were area fraction of new bone, graft material and connective tissue. Immediate and 6 months after grafting surgery, and 6 months after implantation, computed tomography (CT) was taken and the sinus graft was evaluated morphometric analysis. After implant installation at the grafted area, the clinical outcome was checked. Results: Histomorphometry was done in ten patients.Bio-$cera^{TM}$ particles were surrounded by newly formed bone. The graft particles and newly formed bone were surrounded by connective tissue including small capillaries in some fields. Imaging processing revealed $24.86{\pm}7.59%$ of new bone, $38.20{\pm}13.19%$ connective tissue, and $36.92{\pm}14.51%$ of remaining Bio-$cera^{TM}$ particles. All grafted sites received an implant, and in all cases sufficient bone height was achieved to install implants. The increase in ridge height was about $15.9{\pm}1.8mm$ immediately after operation (from 13mm to 19mm). After 6 months operation, ridge height was reduced about $11.5{\pm}13.5%$. After implant installation, average marginal bone loss after 6 months was $0.3{\pm}0.15mm$. Conclusion: Bio-$cera^{TM}$ showed new bone formation similar with Bio-$Oss^{(R)}$ histomorphometrically and appeared to be an effective bone substitute in maxillary sinus augmentation procedure with the residual bone height from 2 to 6mm.

        가토모델에서 배양 구강상피를 이용한 근-점막 피판의 형성에 관한 연구

        Purpose : Extensive defect of oral and maxillofacial area is usually reconstructed with composite flap including skin paddle. However, if the defects are lined with only skin components, the mucosa's role in mastication and texture are not restored. Furthermore, stiffness and hair-growing prevent denture rehabilitation and good oral hygiene. This study was performed to overcome the disadvantages of composite soft tissue flaps including the skin and to make a model for myo-mucosal flaps. Materials and methods : Buccal mucosa sized $0.5\times1.0\;cm^2$ from New Zealand rabbit (around 1.5kg) was harvested and cultivated by the modification of Rheinwald and Green's keratinocyte culture method. Cultured mucosa was grafted on the fascia of latismus dorsi as form of mucosal sheet. After 7, 10, 14 days, the myomucosal flap was excised and evaluated under light microscope with H & E and immunohistochemical staining. As control group, harvested buccal mucosa from rabbit was transplanted to gracilis muscle(n=6). Results : From 7 days after prelamination, the basal layer of the grafted mucosa resembled that of normal mucosa. As control group, transplanted mucosa had original shape but there's slight inflammatory reaction. Prelaminated mucosa has 19.8$\pm$4.59 cell layers and some samples have more than 20 layers. The expression rate of PCNA was relatively strong (42.9%$\pm$14.1) at the basal layer of grafted mucosa and the laminin was found at the basal layer. On the contrary, prelaminated mucosa at 10 days showed moderate expression rate of PCNA(32.4%$\pm$4.62). We found the mucosal layer was somehow disappeared and there is strong inflammatory reaction. After 14 days prelamination, the grafted oral keratinocytes were almost disappeared and expression of PCNA was not observed. Conclusion : We can make 75 fold large mucosal($3850mm^2$) sheet from small samples of mucosa $(50mm^2)$. Epithelial sheet that grafted on the fascia of muscle underwent differentiation and proliferation. But after 10, 14 days, there was strong inflammatory reaction and the grafted mucosa was destroyed from surface layer. In rabbit model, transfer of fascio-mucosal flap should be done from 7 to 10 days after prelamination.

        The purpose of this study is to assess the mechanical properties of the ceramic abutment with washer. In this study, ceramic abutment were used, tested with 30° compression load, shear fatigue, adaptation accuracy test(rotation angle, contact interval), removal torque force test, torsional breaking force test. The 30° compression load was 729 N, the shear fatigue load was 275 N, adaptation accuracy test of rotation angle was within 3°, contact interval within 10 μm, and removal torque force test value is 18.88 N·cm, torsional breaking force test value is 35.52 N·cm. Ceramic abutment with a washer fitted have sufficient mechanical strength and may be substituted for titanium abutment.

        인간 무세포성 진피기질 위에 배양한 가토 구강각화상피세포의 중충화와 기저막 형성에 관한 연구

        To assess the clinical applicability of bio-artificial mucosa which was made with autologous oral keratinocytes and human acellular dermal matrix, the formation of basement membrane and stratification of oral keratinocytes were evaluated. Six New Zealand white rabbits (around 2kg in weight) were anesthetized and its buccal mucosa was harvested (1.0 $\times$ 0.5cm size). Oral keratinicytes were extracted and cultured primarily with the feeder layer of pretreated NIH J2 3T3 fibroblast. These confluent cells were innoculated on the human acellular dermal matrix and cultured in multiple layer by air-rafting method. After 3, 5, 7, 10, 14 days of culture, each cultured bio-artificial mucosa was investigated the number of epthelial layer of by H&E stain and toluidine blue stain. The immuhohistochemical methods were used to evaluate the cell division capacity, the formation of basement membrane, and it's property of specific cells (PCNA, cytokeratin 14, laminin). Transmission electromicroscopy was used for the attachment between cells and matrix with the number of hemidesmosome. In result, the numbers of layer of stratified growth of oral keratinocyte cultured on the human acellular dermal matrix and the number of hemidesomal attachment between epithelial cells and human acellular dermal matrix were similar to the layers of normal oral mucosa after 10 days of culture. The cell division rate, basement membrane formation and proliferation rate increased as culture period increased. With these results, bio-artificial mucosa with autologous oral epithelial cells cultured on the acellular dermal matrix had clinically adaptable properties after 10 days' culture and this new bio-artificial mucosa model with relatively short culture time can be expected clinical applicability.

        말초신경재생을 위한 hNGF-$\beta$ recombinant Adenovirus의 제작 및 수종세포주에서 신경성장인자의 발현

        Nerve growth factor(NGF) has a critical role in peripheral nerve regeneration. The aim of this study is to construct a well-functioning hNGF-$\beta$ recombinat adenovirus for the ultimate development of improved method to promote peripheral nerve regeneration with adenovirus mediated hNGF-$\beta$ gene transfection into Schwann cells. First PCR associated cloning of GFP-tagged hNGF-$\beta$ which was ligated into E1/E3 deleted adenoviral vector was performed and tranfected into E. coli to construct hNGF-$\beta$ recombinant adenovirus. After production of recombinat adenovirus in a large scale, its transfection efficiency, expression, and function were evaluated using cell lines or primarily cultured cells of HEK293 cells, Schwann cells, fibroblast(NIH3T3) and myocyte(CRH cells). GFP expression was observed in 90% of infected cells compared to uninfected cells. Total mRNA isolated from hNGF-$\beta$ recombinat adenoviru infected cells showed strong RT-PCR band, however, LacZ recombinant adenovirus infected or uninfected cells did not. NGF quantification by ELISA showed a maximal release of 18.865 +/- 0.31ng/mL at 4th day. PC-12 cells exposed to media with hNGF-$\beta$ recombinant adenovirus infected Schwann cell demonstrated higher levels of differentiation compared with controls. We generated hNGF-$\beta$ recombinant adenovirus and induced over expression of NGF successfully in nonneuronal and neuronal cells. Following these result, it is expected to develop an improved treatment strategy peripheral nerve regeneration using the hNGF-$\beta$ gene transfected cells.
