전신마취 시 사용한 열선내장형 가온 가습 호흡회로의 비교평가

Background: Dry and cold anesthetic gas deteriorates patient`s respiratory function and body heat balance. We examined whether a humidifier with heated wire circuit might maintain core temperature and humidity of inspired gas in patient undergoing general anesthesia. Methods: We enrolled forty ASA physical status I, II patients under general anesthesia for this study. We allocated the patients randomly into two groups with (experimental group) or without (control group) Humitube(R) anesthesia circuit, which delivered heated and humidified inspired anesthetic gases. We recorded the temperatures and humidity of the inspired gases throughout the surgery. Results: The temperatures and relative humidity of the inspired gases in experimental group were significantly greater compared to control group (36.2±0.9℃, 89.5±4.8% vs. 30.4±1.8℃, 37.9±5.9%, P<0.05) during anesthesia. The core temperatures in experimental group were significantly greater compared to control group (36.1±0.3℃ vs. 35.7±0.1℃, P<0.05) during anesthesia. Conclusions: A humidifier with heated wire system for anesthesia breathing circuit is helpful to maintain core temperature and adequate humidity. (Korean J Anesthesiol 2009; 57: 32~7)

Background: The development of total intravenous anesthesia in coronary artery bypass graft (CABG) surgery has led to increased interest in the use of combination of propofol and remifentanil. Early extubation in post-cardiac surgery reduces the length of stay in intensive care unit and hospital and costs. The purpose of this study is to evaluate which anesthetic factors affect awakening time after anesthesia. Methods: We enrolled twenty patients of ASA physical status II or III, scheduled for CABG in this study. All patients received a standardized propofol/remifentanil anesthesia as an effect site target controlled infusion. We recorded times to awakening and tracheal extubation, duration of cardiopulmonary bypass (CPB) period, total time of anesthesia and operation. Also, we recorded dose of propofol, remifentanil, fentanyl and minimum body temperature during CPB. To predict the factors that affect awakening time as a dependent variable, we considered all measured parameters as independent variables, and analyzed multiple linear regressions. Results: The mean time responded to verbal command was 216.5±124.8 minutes after end of surgery. Among several parameters, minimum body temperature during CPB (P=0.001) and total time of anesthesia (P=0.003) were considered as significant factors that influence awakening time after CABG. Conclusions: Minimum body temperature during CPB and the duration of anesthesia influence awakening time after CABG, significantly. (Korean J Anesthesiol 2009;56:502~6)
과학영재 학생의 학습 특성에 관한 사례연구 -소집단에서의 언어적, 비언어적 상호작용을 중심으로-

The purpose of this study was to investigate gifted students` verbal-nonverbal interactions in small grouped classes and to classify them into characteristics. For this study, an instrument to measure gifted student`s verbal- nonverbal interactions was developed. The subjects were 16 students of 5th and 6th grades at H-area gifted center in rural area. Video recorded was classified each 5minutes by researchers. The results were as follows; First, verbal interactions were about 27% and non-verbal interactions were about 73%. Activities unrelated to learning were about 20%. Second, high achievement gifted students enjoyed verbal interactions including discussions. Third, gifted students can be classified 3 types(Spoken-centered, Written-centered and Played-centered) by interactions. The results revealed that gifted student`s verbal-noverbal interactions played an important role in communication between students and teacher as well as students. Small-grouped instructions according to characteristic and levels of gifted students are needed in gifted science education.

A vibration monitoring is performed on a 1kW class stand alone wind turbine(W/T). When a W/T model is developed, general performance under various wind condition should be verified to introduce the product in the market. Especially, vibration characteristics within operating speed range are very important in the aspect of structural stability as well as generator's electrical efficiency. This paper examines the vibration performance of a home made 1kW W/T. Various data of the W/T model are acquired in real time using a remote vibration monitoring system installed in Daekwanryung test site. Vibration stability of the W/T structure is diagnosed based upon the data and the result is used to estimate the applicability of the W/T model.
