Orbital injuries are common with facial trauma. Direct injuries to the globe are not rare but it can result in complications such as chemosis, subconjunctival hemorrhage and hyphema. Periorbital trauma or injuries to the extraocular muscles and blow-out fracture may result in lid edema, ecchymosis & ptosis and diplopia or limitation of ocular motion respectively. Indirect injuries to the optic nerve come up without any injuries but its complication is irreversible and severe such as loss of vision. The aim of this study is to review the literature on blindness or ptosis following facial trauma and present the cases of blindness after facial trauma and ptosis after mandibular fracture without specific clinical findings.

The purpose of this study was to investigate and evaluate what proportion is the characteristics in Korean dental arches with normal occlusion. Many others have already indicated Golden proportion in normal dental arches, but have not considered any racial and sociocultural differences. So the author postulated (√2)<sup>n</sup> relations in Koreans. The materials were consisted of 134 dental casts with normal occlusion, which have never undergone orthodontic and prosthodontic procedures. Measurements were made on the arch dimensions using sliding caliper and data were computerized. The findings were as follows: 1. The width between the distal surfaces of the upper centrals, had (√2)<sup>³</sup> relation with the width between the buccal surfaces of the upper 1st premolars in Koreans. 2. The width between the distal surfaces of the lower laterals had (√2) relation with the width between the distal surfaces of the lower canines, and had (√2)<sup>²</sup> relation with the distal surfaces of the upper centrals. 3. The width between the distal surfaces of the lower centrals had (√2)<sup>²</sup> relation with the width between the distal surfaces of the lower laterals, and had (√2)<sup>³</sup> relation with the width between the distal surfaces of the upper centrals.

Aplastic anemia is a hematopoietic disorder characterized by marked reduction or absence of erythoid, granulocytic, and megakariocytic cells in the bone marrow with resultant pancytopenia. To control of infection & bleeding secondary to leukopenia and thrombocytopenia, the inflammatory lesions in oral & maxillofacial area should be removed. Material & Method: The extractions were performed on 21 patients with severe aplastic anemia. The initial, pre-operative and postoperative CBCs were checked up. Amount and kind of transfused platelet in each patient and increment of platelet level were recorded. The complications were documented. Result : A mean of 2.9 teeth were extracted from each patient(ranging between 1 and 13). Furthermore, surgical extractions including ostectomy and odontectomy of the third molar were performed on 11 patients. The preoperative WBC levels presented between $600/{\mu}L$ and $5000/{\mu}L$(mean $2376/{\mu}L$). The WBC values decreased by an average of $145/{\mu}L$ per patient after extractions had been performed. The teeth of 16 patients were extracted under 10.0g/dL, and the mean change in postoperative hemoglobin levels in comparison with preoperative hemoglobin levels was -0.06 per patient. The initial platelet levels were between 1000/(L and $81,000/{\mu}L$(mean $20,174/{\mu}L$). In five patients, extractions were performed with platelet levels less than $50,000/{\mu}L$. Conclusion : The results suggest that more active and preventive treatments in the oral and maxillofacial area are possible and are necessary to remove the infectious foci on the patients with severe aplastic anemia. We report the results of our experiences and literature reviews in treatment of the patients with severe aplastic anemia in our department.
30% 과산화수소를 함유한 전문가 미백제의 안전성 평가

30% 과산화수소를 함유한 전문가 미백제인 레메화이트(Remewhite, Remedent Inc., Deurle, Belgium)를 시험군 피험자 37명, 시험용 미백제에서 과산화수소를 제외한 동일한 제재를 대조약으로 대조군 피험자 34명에게 1일 3회씩, 2차례에 걸쳐서 시행하였다. 미백의 임상적 안전성은 전기치수검사, 치은염증지수, 지각과민증, 이상반응, 활력징후 등을 통하여 평가하였다. 미백시행 직후, 14주 후, 26주 후에 평가하여 다음과 같은 결론을 얻었다. 30% 과산화수소를 함유한 전문가 미백제를 임상적 적용시에 전기치수검사 결과에서는 미백 전후에 차이가 없었다. 치은염증지수, 지각과민증 평가 결과에서는 시험군에서 다소 경미한 통증 반응이 나타났으나 시간이 지나면서 사라지는 것을 확인하였다. 따라서 30% 과산화수소를 함유한 전문가 미백제를 사용하여 미백시에 임상적 안전성을 확인할 수 있었다. This study evaluated the safety of an office bleaching gel (RemeWhite, Remedent Inc., Deurle, Belgium) containing 30% hydrogen peroxide. 37 volunteers were received office bleaching with the RemeWhite for 3 times at one visit, total 2 visits. As control group, the same gel in which hydrogen peroxide was not included was applied to 34 volunteers with the same protocol. There was no difference between experimental group and control group using electric pulp test. In the result of gingival inflammation index and tooth sensitivity test, there was mild pain response in experimental group but it disappeared as time went by. Therefore, safety of the office bleaching gel containing 30% hydrogen peroxide was confirmed.
측두하악관절 환자의 전방재위치장치 치료 전후의 임상증상 및 자기공명영상을 이용한 관절원판 변화의 분석

Temporomandibular joint(TMJ) disorders have characteristic clinical findings such as pain, joint sound, and abnormal jaw function. With the rapid progress in TMJ imaging techniques, magnetic resonance imaging(MRI) especially provides the clinician and researcher with the ability to evaluate TMJ disorders(effusion, anterior disk displacement) and the hard and soft tissue of the TMJ. In order to manage the internal derangement of TMJ such as anterior disk displacement, the so-called appliance therapy is commonly used. This technique became the treatment of choice before surgical intervention by many practitioners. The purpose of the present study was to evaluate and determine the efficacy of anterior repositioning splint by means of the change of the position of the disk through pre- and post-treatment MRI. MRI and clinical symptom were carefully recorded in TMJ disorder patients. The relationship between TMJ symptoms such as pain, clicking sound, and disk displacement and effusion in MRI were analyzed. Then, splint therapy was applied for 3 months according to our protocol; at day 1, patients should wear 24 hours, in order to accomodate the splint and then, wearing time is reduced to 12 hours per day for one month. During next 2 months, it is tapered to 8 hours per day according to the evaluation of the clinical symptoms. Post-treatment MRI was obtained after 3 month protocol. This treatment strategy improved the clinical symptoms of TMJ disorder, and effusion and articular disk position showed a significant change. In conclusion, a treatment modality using an anterior repositioning splint therapy is an appropriate method for temporomandibular disorder patients.

Eagle's syndrome is the manifestation of elongated styloid process that is not a common entity in dental clinics. Without the knowledge of this syndrome misdiagnosis can be made and therefore differential diag-nosis with other diseases of oral and maxillofacial area is required. The symptoms of elongated styloid process are cervical pain, foreign body sensation, dysphagia and pharyngeal pain. A 27 years old male vis-ited our hospital. He had cervical pain with unilateral facial nerve palsy. Along with meticulous clinical examinations, 3D-CT, Plain radiographic studies were used to make the diagnosis. In computed tomogra-phy, Lt. styloid process was elongated and fractured. Patient have undergone surgical resection of elongat-ed styloid process and have been carefully observed. Characteristically, preoperative facial nerve palsy showed rapid postoperative recovery. The other clinical symptoms that were present in the initial visit were diminished. The purpose of this article is to report a case of Eagle's syndrome presenting unilateral facial nerve palsy that was treated with transoral surgical approach with literature review.
Cone Beam Computed Tomography의 두부계측분석을 통한 골격성 제3급 부정교합 환자에서 악교정 수술 후 입술과 주위 연조직의 수직적 변화

Purpose: The aim of this study is to evaluate the vertical changes of the lip and perioral soft tissue, following orthognathic surgery in skeletal class III patients by a cephalometric analysis of a cone beam computed tomography (CBCT). Methods: A total of 20 skeletal class III patients, who had bimaxillary surgery with Le Fort 1 osteotomy and bilateral sagittal split ramus osteotomy, were included in this study. The surgical plan for maxilla was posterosuperior impaction with the anterior nasal spine, as the rotation center. Further, the surgical plan for mandible was also posterosuperior movement. The soft tissue changes between lateral cephalogram and CBCT were compared. And the correlations between independent variables and dependent variables were evaluated. Results: There were no significant differences of the soft tissues changes between lateral cephalogram and CBCT. Upper lip philtrum length (SnLs), nasolabial angle increased and upper lip vermilion length (LsStms), lower lip length (StmiB'), lower lip vermilion length (StmiLi), lower lip philtrum length (LiB') and soft tissue lower facial height (SnMe') decreased after surgery. Change of SnLs (${\Delta}$SnLs) was influenced by vertical change of menton (${\Delta}$MeV), and change of LsStms (${\Delta}$LsStms) was influenced by upper lip thickness (ULT). Change of StmiLi' (${\Delta}$StmiLi') were influenced by preoperative overjet. Change of StmiB' (${\Delta}$StmiB') were influenced by preoperative overjet, vertical change of lower incisor (${\Delta}$L1V) and horizontal change of posterior nasal spine (${\Delta}$PNSH). Change of LiB' (${\Delta}$LiB') was influenced by ${\Delta}$L1V and ${\Delta}$PNSH. Change of SnMe' (${\Delta}$SnMe') was influenced by ${\Delta}$MeV, horizontal change of upper incisor (${\Delta}$U1H) and horizontal change of lower incisor (${\Delta}$L1H). ${\Delta}$Nasolabial angle was influenced by change of ULT (${\Delta}$ULT). Conclusion: Both soft tissues and hard tissues can be evaluated by CBCT. Posterosuperior rotation of maxillomandibular complex resulted in increase of upper lip philtrum length and nasolabial angle, while the upper lip vermilion length, lower lip philtrum length, lower lip vermilion length, and soft tissue lower facial height showed a decrease.
