Occupational hazards of firefighting and rescue works include frequent exposure to emergencies and life-threatening situations. These stressful work conditions of being constantly under pressure and exposed to potentially traumatic events put them at higher risk of developing posttraumatic stress disorder (PTSD), compared to the general population. PTSD is a potentially debilitating mental disorder, due to persistent intrusive thoughts, negative alterations of mood and cognition, hypervigilance, avoidance of similar situations and reminders, and re-experiences of the traumatic event. Previous studies have shown a relatively high prevalence of PTSD among firefighters, indicating the need for a systematic approach of early detection and prevention. Therefore, a critical review of the current literature on PTSD in firefighters would provide valuable insights into developing effective prevention and intervention programs. Literature indicated that there are risk factors of PTSD in firefighters, such as pre-existing depression, anxiety, sleep disorders, occupational stress, physical symptoms, and binge drinking, whereas social support and adequate rewards are protective factors. Although there are differences in the prevalence of PTSD across studies, partly due to various assessment tools utilized, different sample sizes, and sample characteristics, over one tenth of the firefighters were estimated to have PTSD. The current review warrants further investigations to precisely assess PTSD and co-morbid mental disorders, functional outcomes, and associated factors, and to develop evidence-based preventive and interventional programs to help firefighters with PTSD.

Due to a Supreme Court ruling in April 2020, the Korean Industrial Accident Insurance Act (KIAIA) was revised, and starting in January 2023, industrial accident insurance compensation for a live-born child with health issues due to its mother’s work during pregnancy has been provided. Based on this, a bill to amend the Public Officials’ Accident Compensation Act (POACA) was announced to provide similar compensation to the children of public officials with health issues. This study began with the purpose of presenting problems and alternatives to the currently announced POACA. As a methodology, literature reviews and case studies are used. The proposed recommendations can be largely divided into a plan to expand only services and a plan to expand beyond services to cash compensation. By setting the time of disability determination at the time of discovery of disability, nursing benefits, vocational rehabilitation benefits, and two kinds of paid care leaves could be immediately provided. In addition, disability benefits could be provided at a lower amount to minors in order to cover additional costs due to disabilities. The proposed measures do not significantly modify the current legal framework, but are appropriate for satisfying the principles and needs of compensation.

Although acute occupational poisonings are very rare officially, it seems that there are a considerable number of unreported cases seen from non-intentional poisoning cases of the national injury and poisoning survey on discharged patients. Establishing a proper national surveillance system for acute occupational poisonings is needed. It is difficult for clinical physicians to diagnose acute occupational poisonings appropriately because the victims who usually are not mentally alert can not provide the information on the workplace hazard. Consulting a specialist of occupational and environmental medicine may help to reveal the cause of poisoning. Furthermore, reporting to the public organization such as workers health center and Korea occupational safety and health is helpful to prevent additional injuries.
