N/A Splanchnic nerve block with neurolytics has been used to control the upper abdominal cancer pain. This gastric cancer case with severe chronic obstructive pulmonary disease complained of upper abdominal pain, severe dyspnea and orthopnea. He maintained a sitting position most of the time with nasal oxygen inhalation because he could not remain in a supine or prone position. We performed the unilateral splanchnic nerve alcohol block under right lateral position at the T12 and Ll vertebral level. For a short time after the block, he required oxygen inhalation therapy, Three months after unilateral alcohol block, he is still alive without severe abdominal pain and severe dyspnea.

전래동요 놀이를 통한 자폐장애 유아의 규칙 지키기 행동 변화

본 연구는 유아의 언어 및 사회성 발달을 촉진하는 것으로 알려진 전래동요 놀이가 사회성 발달에 심각한 지체를 나타내는 자폐장애 유아의 사회적 행동 발달도 촉진하는지를 알아보고자 한 것이다. 이를 위하여 2명의 자폐장애 유아를 선정하였고, 사회적 행동을 규칙지키기 행동으로 제한하여 그것을 차례지키기, 자리이탈, 물건정리하기 등으로 구체화하였다. 행동 변화를 알아보기 위하여 AB설계를 이용하여 기초선, 중재, 유지단계로 나누고, 각각의 행동마다 3점 척도로 목표행동을 측정하였다. 행동변화는 시각적 분석으로서 그래프로 나타내고 단계별 평균과 경향선을 제시하였으며, 기초선과 중재단계의 자료점 중복비율도 산출하였다. 연구결과, 2명의 유아 모두가 단계별로 평균의 상승이 나타났으며, 경향선도 꾸준한 상승선으로 나타나 행동상의 변화가 있는 것으로 나타났다. 그러나 단계별 평균이 모두 6을 전후한 수준으로서 최고수준인 9에는 미치지 못함으로써 자발적으로 목표행동을 실행하기보다는 촉진적 단서가 있어야 행동이 나타나는 수준으로 변화하였다고 볼 수 있다. 본 연구는 변인간의 기능적 관계를 밝히는 설계가 아니었으므로 행동의 변화가 중재프로그램에 기인한 것으로 단정할 수는 없다. 그러나 연구대상 유아 모두에게서 뚜렷한 행동변화가 나타났으므로 매개변인을 통제한 보다 구조화된 연구를 통하여 전래동요 놀이의 효과를 밝힐 필요가 있을 것으로 사료된다. The purpose of this study was to investigate the effect of teaching-learning activities which used a traditional children's song play program on rule obeying and personal relationships on social behavior development of the children with autistic disorder of five to six years old. The research process was composed of the following steps: baseline phase, intervention phase, and maintenance phase, and testing the effect of intervention using AB design. When we consider that most studies using traditional children's song play are for the non-handicapped, and few are childhood educational arbitration, this research is important because it expands the research to include the social behaviors of children with autistic disorders. The results were that traditional children's song play programs showed a positive effect toward improvement of social behaviors of young children with autistic disorders.

여성 돌봄서비스 노동자의 노동환경이 주관적 건강에 미치는 영향에 관한 연구

본 연구는 돌봄서비스 확대에도 불구하고 건강을 보장 받고 있지 못하는 돌봄서비스 노동자의 건강을 증진할 수 있는 방안모색을 위해 돌봄서비스 노동자의 주관적 건강에 영항을 미치는 노동환경 요인이 무엇인지 규명하는 것을 목적으로 한다. 이를 위해 부산광역시의 돌봄서비스 노동자에게 구조화된 자기기입식 설문지를 1,051부 배부, 863부 회수하여 불성실한 응답 등 연구에 적합하지 않은 43부를 제외하고 총 820부를 최종 분석하였다. 독립변수인 노동환경은 직무특성, 고용조건적 특성, 조직심리적 특성으로 나누어 살펴보았다. 연구의 분석을 위해 SPSS 21.0를 사용하여 빈도분석, 기술통계, 상관관계분석, 위계적 회귀분석을 실시하였고 주요 연구결과는 다음과 같다. 첫째, 돌봄서비스 노동자의 노동환경 중 직무특성인 ‘화가 난 고객응대’와 ‘이용자폭력’경험이 많을수록 돌봄서비스 노동자의 주관적 건강에 부정적인 영향을 미치는 것으로 나타났다. 둘째, 돌봄서비스 노동자의 노동환경 중 고용조건적 특성은 ‘고용형태’와 ‘교대근무형태’가 주관적 건강에 영향을 미치는 것으로 나타났다. 셋째, 돌봄서비스 노동자의 노동환경 중 조직심리적 특성인 ‘일터영성’은 돌봄서비스 노동자의 주관적 건강에 긍정적인 영향을 미치는 것으로 나타났다. 이러한 연구결과를 바탕으로 돌봄서비스 노동자의 건강을 증진하기 위한 노동환경을 조성할 수 있도록 실천적·정책적 방안을 제시하였다. This paper attempts to explain the effect of care worker’s working environments on Subjective health. Working environments consists of three environment characteristics : characteristics employment conditions , job characteristics, organizational psychological characteristics. This research surveyed a total of 820 female care workers in Busan. The descriptive statistic, reliability analysis, and regression analysis were performed on the collected data using SPSS 21.0. The results are the followings. First, Among the job characteristics, responding to angry customers and violence by users have a negative effect on the Subjective health of care workers. Second, among the characteristics of employment conditions, employment type and shift work were found to affect Subjective health of care workers. Finally, workplace spirituality, which is an organizational psychological characteristic, has a positive effect on the Subjective health of care workers. In conclusion, this study suggested to improve the quality of care service working environment to promote the health of female care workers.

하지 분절 각도에 따른 수의 등척성 수축(MVIC)시 근전도 비교

The purpose of this study was to quantify the maximum EMG levels and determine if there are differences in these EMG levels with respect to different knee flexion angles. Eight university students with no known musculoskeletal disorders were recruited as the participants. The maximum voluntary isometric knee extensions and flexions were taken from each participant sat on the isokinetic exercise machine (Cybex 340) at five different knee flexion angles (10?30?50?70? 90?. After surface electrodes were attached to rectus femoris, vastus medialis, vastus laterlis, biceps femoris, and semitendinosus, maximum EMG levels at five different knee flexion angles were measured. The results showed that there was no significant difference in maximum EMG levels among five different knee flexion angles. Although there was no significant difference in EMG levels and were some variations among different knee flexion angles, the EMG signals of quadriceps in extension and biceps femoris in flexion were the greatest at 30? It seems that different joint angles or relative locations of body segments might affect the magnitude of EMG levels. Because the maximum EMG levels could change with a different knee flexion angle, an attempt should be made to more accurately measure these values. If then, %MVIC measure provides more reliable data and is most appropriate for EMG normalization.

The incidence of postspinal headache is one of the well known complications of spinal anesthesia. Several factors such as needle size, bevel direction, multiple dural puncture and previous history of postspinal headache were thought to influence the incidence of postspinal headache. This studies were done to see the effect of needle size (22 and 25 gauge needle) and needle bevel direction (parallel, vertical, oblique insertion to the longitudinal dural fiber) on the incidence, duration, severity and location of spinal headache in the 548 patients underwent spinal anesthesia. The following results wre observed: 1) Neither needle size nor needle bevel direction had effect on the incidence of severity, duration and location of postspinal headache. 2) The ineidence of headache was 8.8% (48 cases), 3) The onset of headache was 1~2 day (67%) and duration of headache was 4~5 day (85%) in postanesthetic day. 4) The severity of headache was mild and moderate in 77% cases. 5) In the half cases, headache was relieved by means of bed rest alone.
