        부갑상선호르몬 측정을 위한 두 가지 2세대 검사법의 비교

        배경: 부갑상선호르몬(parathyroid hormone, PTH)의 측정은 일/이차성 부갑상선 항진증 및 저하증의 진단 및 치료 방향의 결정에 중요하다. PTH는 혈액 내에 다양한 분자의 형태로 존재하므로, 측정 방법에 따라 농도값에 상당한 차이가 있는 것으로 알려져 있다. 본 연구에서는 두 가지 서로 다른 방법의 2세대 PTH 검사를 비교 평가해 보았다. 방법: 두 검사법의 비교평가를 위해 PTH가 의뢰된 잔여 검체 56개를 이용하였다. 각 검체를 기존의 면역방사계수측정법(ELSA-PTH; Cisbio assays, France)과 새로 도입한 자동화 화학발광면역측정법(Alinity i Intact PTH; Abbott, USA)으로 검사를 시행한 후 상관성을 분석하였다. Alinity i Intact PTH 검사법의 분석능 검증을 위해 정밀도, 직선성, 참고범위 및 정량한계 검증이 수행되었다. 결과: ELSA-PTH와 Alinity i Intact PTH의 중앙값은 각각 69.0 pg/mL과 101.9 pg/mL였고 Alinity i Intact PTH의 측정값이 유의하게 높았다(P<0.0001). Pearson 상관계수는 0.959였고, 두 검사법의 결과값 차이의 평균은 78.57 pg/mL였다. Alinity i Intact PTH 분석능 평가로 3가지 농도에서 구한 변이계수는 반복정밀도 1.9%, 2.4%, 1.8%와 검사실내정밀도 2.0%, 2.7%, 2.7%였다. 평가된 3–3,000 pg/mL 범위에서 직선성을 확인하였으며 제조사가 제시한 참고범위 15–68.3 pg/mL와 정량한계 3 pg/mL를 검증하였다. 결론: ELSA-PTH와 Alinity i Intact PTH의 측정값은 높은 상관관계를 보였고, Alinity i Intact PTH 검사는 검사실 도입에 적합한 분석능을 보였다. Background: Accurate measurement of parathyroid hormone (PTH) levels is crucial for the diagnosis and treatment of hyperparathyroidism and hypoparathyroidism. Given the existence of various PTH forms in the bloodstream, it is well-known that there are variations in PTH levels depending on the measurement method. This study aimed to evaluate two different second-generation PTH assays. Methods: A total of 56 residual samples originally requested for PTH testing were utilized for comparing the two methods. Each sample was subjected to analysis using an immunoradiometric assay (ELSA-PTH; Cisbio Bioassays, France) and a chemiluminescence microparticle immunoassay (Alinity i Intact PTH; Abbott, USA). Precision and linearity were analyzed, and reference interval and limit of quantitation (LoQ) were verified to investigate the performance of the Alinity i Intact PTH assay. Results: The median PTH values of ELSA-PTH and Alinity i Intact PTH were 69.0 pg/mL and 101.9 pg/mL, respectively, with significantly higher PTH values for Alinity i Intact PTH (P<0.0001). The Pearson correlation coefficient was 0.959, and the average difference between the two tests was 78.57 pg/mL. For Alinity i Intact PTH, the coefficient of variation obtained at three concentrations was 1.9%, 2.4%, 1.8% for repeatability, and 2.0%, 2.7%, 2.7% for within-laboratory precision. Linearity was observed in the range of 3–3,000 pg/mL. The reference interval and LoQ reported by the manufacturer were verified. Conclusions: The PTH results of the ELSA-PTH and Alinity i Intact PTH assays showed a strong correlation, and the Alinity i Intact PTH assay demonstrated acceptable analytical performance.

        코로나바이러스감염증-19 신속항원검사의 진단적 성능: 민감도와 그 이상의 고려사항

        Background: The rapid antigen test (RAT) is generally used to diagnose coronavirus disease 2019 (COVID-19) because of its ease of use and quick results. We compared a novel RAT with an approved one and interpreted their clinical usefulness based on their diagnostic performance. Methods: Diagnostic performances of the novel RADI COVID-19 Ag Rapid Test (RADI) and the STANDARD Q COVID-19 Ag Test (SDQ) were assessed using a total of 804 residual nasopharyngeal swab samples and confirmed by real-time reverse transcriptase-PCR (rRT-PCR). The positive/negative likelihood ratio (LR+/-) was applied to Fagan’s nomogram to estimate pre- and post-test probability according to the test results. The prevalence of COVID-19 was counted as 5.7%, based on the rRT-PCR positive rate achieved in the laboratory in 2022. Results: The RADI and SDQ had sensitivities of 82.2% and 62.8%, respectively, and specificities of 98.7% and 99.6%, respectively. The LR+ was 64.1 for RADI and 171.4 for SDQ. The LR- was 0.2 for RADI and 0.4 for SDQ. After applying the LR+ to Fagan’s nomogram, the RADI raised the post-test probability to 79.5%, and the SDQ raised it to 91.2%. Conclusions: The RADI was found to be more sensitive than the SDQ. However, when the likelihood ratio and prevalence were taken into consideration, the SDQ showed a higher post-test probability. In addition to the sensitivity and specificity of the tests, it is necessary to provide information on the interpretation of the results, such as the probability based on the test results or whether further tests are necessary. 배경: 최근 코로나바이러스감염증-19 (coronavirus disease-2019, COVID-19) 진단에 사용이 간단하고 결과를 빠르게 알 수 있는 신속항원검사(rapid antigen test, RAT)가 많이 이용되고 있다. 본 연구에서는 새로 개발된 RAT 제품을 기승인된 제품과 비교 평가하고, 추가적으로 진단 성능을 바탕으로 임상적 유용성을 해석하였다. 방법: 기존 검사실에서 실시간 역전사 중합효소연쇄반응법(real-time reverse transcription PCR, rRT-PCR)으로 확인된 상기도 검체 총 804개(양성 258개, 음성 546개)를 이용하여 RADI COVID-19 Ag Rapid Test (RADI)와 STANDARD Q COVID-19 Ag Test (SDQ)의 진단적 성능을 평가하고 분석하였다. 양/음 우도비(positive/negative likelihood ratio, LR+/-)를 Fagan의 노모그램에 적용하여 검사 결과에 따른 질병확률의 변화를 확인하였다. 검사 전 질병확률은 지역 내 2022년도 rRT-PCR 양성률 5.7%를 유병률로 간주하였다. 결과: RADI와 SDQ의 민감도는 각각 82.2%와 62.8%였으며, 특이도는 각각 98.7%와 99.6%였다. RADI와 SDQ의 LR+는 각각 64.1와 171.4였으며, LR-는 각각 0.2와 0.4였다. 이를 Fagan의 노모그램에 적용시, RADI 양성 결과는 검사 후 질병확률을 79.5%, SDQ 양성 결과는 91.2%로 증가시켰다. 결론: RADI의 민감도가 SDQ보다 더 높았으나, 우도비와 유병률을 바탕으로 양성 결과 시의 질병확률을 구했을 때, 오히려 SDQ가 검사 후 질병확률이 더 높았다. 이처럼 검사의 민감도나 특이도 등의 성능과 더불어 검사 결과에 따른 질병확률이나 다음 단계 검사에 대한 진행 필요성 등과 같은 결과 해석에 대한 정보 제공이 필요하다.

        Work-Related Upper Extremity Musculoskeletal Pain Korean Baristas

        The purpose of this study was to investigate the current status of work-related upper extremity musculoskeletal pain in baristas and to identify the elements that are related to pain. The questionnaires were distributed to 100 workers in 63 cafes on Jeonju city in Korea. As a result of investigating work related pain, 65.3% (n=64) answered "yes" to the question that had pain at least once a week, month, or year, or 34.7% (n=34) answered "no". Most of the workers were right-handed, and when they felt pain, they felt 25 to 50% of time per day. Especially, baristas reported that the use of porter filters during work and the tamping operation were the most painful. In conclusion, our study indicated that necessary to introduce and develop a program to prevent cafe worker disease, as well as need to future research to improve work environment and posture according to the characteristics of the cafe works.

        리튬이차전지 기반 에너지저장장치(ESS)의 화재와 열폭주 억제 기술에 대한 고찰

        A lithium ion battery is an energy storage device that the electrical energy into chemical energy through a charging reaction and then uses electricity through a discharging reaction. Although this technology has been commercialized as a power source for portable electronic devices, its range of use is expanding from several tens of kWh to MWh-class power sources such as electric vehicles(EVs) and energy storage systems (ESS). Currently, it is widely used in battery energy storage systems (BESS) can be used for a variety of applications, including frequency regulation, demand response, integration of renewable energy, and microgrids, respectively. The ESS industry has rapidly expanded its supply through various support policies such as special electricity rate discounts(KEPCO) and renewable energy supply certificates(REC). Simultaneously with the expansion of ESS installation, a total of 29 fires have occurred since the first ESS fire in August 2017. Therefore, in this paper, the ignition mechanism of lithium-ion batteries was analyzed based on the current status of ESS fires in Korea and the results of the investigation. In addition, it is intended to present a method for securing cell-level safety by securing thermal runaway suppression technology by improving the thermal stability of the electrolyte and separator constituting the lithium-ion battery.

        공공하수처리시설의 에너지절감 및 폐슬러지 저감에 관한 실증연구

        An empirical study on the energy saving and waste-sludge reduction by using ultra-fine bubbles was demonstrated in a public sewage treatment plant. The test-bed of 100m3/day capacity was installed and operated with the ultra-fine bubbles of μm-size for aeration and sludge reduction tools. The rate of aeration-energy saving by using ultra-fine bubbles was 37% compared to that of conventional diffuser, and the rate of excess-sludge reduction by using the ultra-fine bubbles of air+10%O₃ was 31% for 1-day operation and was 45% for 2-day operation. We could conclude that the ultra-fine bubbles might have a powerful potential for the energy saving and sludge reduction in sewage and waste-water treatment processes.
