        Lupus erythematous panniculitis is a chronic inflammatory disease of subcutaneous fat that occurs in 2-3% of patients with systemic lupus erythematosus. Lupus erythematous panniculitis typically presents with firm, indurated subcutaneous nodules or plaques commonly developed in proximal extremities, trunks, head and neck. Face, breast and upper extremities involvement with lupus erythematous panniculitis has been infrequently reported. We report a case of lupus erythematous panniculitis developed in a 27-year-old female confirmed by histologic features and laboratory findings.

        Heterologous Expression of Phanerochaete chrysoporium Glyoxal Oxidase and its Application for the Coupled Reaction with Manganese Peroxidase to Decolorize Malachite Green

        CDNA of the glx1 gene encoding glyoxal oxidase (GLX) from Phanerochaete chrysosporium was isolated and expressed in Pichia pastoris. The recombinant GLX (rGLX) produces H2O2 over 7.0 nmol/min/mL using methyl glyoxal as a substrate. Use of rGLX as a generator of H2O2 improved the coupled reaction with recombinant manganese peroxidase resulting in decolorization of malachite green up to 150uM within 90 min.

        간호간병통합병동 간호사의 전문직 자율성, 역할갈등이 임상적 의사결정능력에 미치는 영향

        본 연구는 간호간병통합서비스 병동간호사를 대상으로 전문직 자율성, 역할갈등, 임상적 의사결정능력과의 관계와 함께 임상적 의사결정능력에 영향을 미치는 영향요인 파악을 목적으로 한 서술적 조사연구이다. 본 연구에서는 D시와 G시에 근로간호사 234명을 편의 표집하여 조사하였다. 자료수집은 2017년 10월 12일부터 11월 30일 사이에 D시와 G시에 소재하는 4개의 종합병원의 234명을 대상으로 연구 참여에 서면 동의한 간호사에게 자가 보고식 설문조사지를 사용하여 자료를 수집하였다. t-test, ANOVA, Pearson’s coefficients와 단계적 회귀분석으로 자료를 분석하였다. 연구결과에서 임상적 의사결정능력은 평균평점 3.38±0.78점으로 중간수준으로 나타났다. 임상적 의사결정능력은 전문직 자율성과 양의 상관관계(r=.40, p<.001)를 보였으나, 역할갈등과는 음의 상관관계(r=-.21, p<.001)가 있는 것으로 나타났다. 임상적 의사결정능력에 미치는 영향은 근무경력, 교육수준, 전문직 자율성과 역할갈등이었으며, 임상적 의사결정능력에 대한 설명력은 38.7%였다. 임상적 의사결정능력을 개선하기 위해 중요한 변수인 전문직 자율성과 역할갈등을 통해 간호사의 직무를 결정할 수 있는 임상적 의사결정능력을 향상시킬 필요가 있다. 그러므로, 본 연구결과가 간호사의 임상적 의사결정능력 향상프로그램 개발의 근거자료로 활용될 수 있을 것으로 기대하며, 개발된 교육프로그램의 타당성과 효과성을 분석하는 중재연구의 필요성을 제안하는 바이다. This investigation was a descriptive study that examined the relationship among factors including professional autonomy, role conflict, and clinical decision-making ability of nurses on comprehensive nursing service wards. To accomplish this, a survey of 234 nurses working at D city and G city who were selected based on convenience sampling was conducted. Data were collected using a self-reported questionnaire completed by 234 nurses at four clinical hospitals from October 12, 2017 to November 30, 2017. The collected data were evaluated by t-tests, ANOVA, Pearson"s correlation and stepwise multiple regression analysis. The mean clinical decision-making ability score was 3.38±0.78, which corresponded to an intermediate level of clinical decision-making ability. Clinical decision making ability was positively correlated with professional autonomy, but role conflict showed a negative correlation. The factors having the greatest impact on clinical decision-making ability were found to be, in order, length of work, education level, professional autonomy and role conflict, which together accounted for 38.7% of the total impact. To improve clinical decision making ability, it is important to improve autonomy and role conflict. Additionally, to improve clinical decision-making ability, it is necessary to improve clinical decision-making ability to determine nurses tasks through professional autonomy and role conflict. Overall, the results of this study can be utilized as baseline data when developing a program to enhance nurses" clinical decision making ability. Moreover, it is suggested that an interventional study analyzing validity and effectiveness of developed education programs be conducted.

        종합병원 간호사의 환자안전문화에 대한 인식이 안전간호활동에 미치는 영향

        본 연구는 종합병원 간호사를 대상으로 환자안전문화에 대한 인식과 안전간호활동에 미치는 영향을 파악하고 변인을 규명하기 위함이다. D시와 G시에 위치한 종합병원의 간호사 178명을 대상으로 하였다. 자료는2015년 6월 20일부터 7월 30일까지 수집하였으며, Kim 등이 수정․보완 한 환자안전문화에 대한 인식과 Lee가 개발한 안전간호활동을 연구도구로 사용하였다. 수집된 자료는 t-test, One-way ANOVA, Pearson correlation coefficients와 단계적 중다회귀분석하였다. 환자안전문화에 대한 인식은 환자안전간호활동과 양의 상관관계가 있었다(r=.407, p<.001) 본 연구 결과 대상자의 병원 근무 환경은 안전간호활동에 43.3%의 영향력이 있는 것으로 나타났으며, 병원간호사의 안전간호활동에 병원근무환경이 가장 중요한 변인으로 볼 수 있다. 안전간호의 질 향상을 위하여 환자안전문화에 대한 인식을 높일 수 있는 구체적인 방안과 안전간호 지침이 필요하다. This study examined the influence of patient safety culture and safety care activities of general hospital nurses. The participants were 178 nurses working in a hospital in D and G cities. Data were collected from June to July, 2015 through Questionnaires that included the Measure of Patient Safety Culture by Kim et al.(2007), and the Measure of Safety Care Activities by Lee(2009). The collected data were analyzed by descriptive analysis, t-test, One-way ANOVA, Pearson correlation coefficients, and stepwise Multiple regression. A statistically significant positive relationship was observed between the nurses perception of the patient safety culture and their safety care activities (r=.407, p<.001). The findings show the patient safety culture accounted for 43.3% of the variance in the safety care activities for hospital nurses followed by the importance of hospital work environment among nurses to improve the safety care activities. These findings highlight the need to develop effective programs to improve the perception of patient safety culture and safety nursing guideline.

      • Bias-reduced ℓ_1-trend filtering

        The ℓ_1-trend filtering method is one of the most widely used methods for extracting underlying trends from noisy observations. Contrary to the Hodrick-Prescott filtering, the ℓ_1-trend filtering gives piecewise linear trends. One of the advantages of the ℓ_1-trend filtering is that it can be used for identifying change points in piecewise linear trends. However, since the ℓ_1-trend filtering employs total variation as a penalty term, estimated piecewise linear trends tend to be biased. In this study, we demonstrate the biasedness of the ℓ_1-trend filtering in trend level estimation and propose a two-stage bias-reduction procedure. The newly suggested estimator is based on the estimated change points of the ℓ_1-trend filtering. Numerical examples illustrate that the proposed method yields less biased estimates for piecewise linear trends.

        Effect of Pilates Gymball Exercises on the Electroencephalogram and Cognitive Function in Mentally Disabled Persons

        Purpose: The aim of this study was to determine if Pilates gymball exercise can change the electroencephalogram and cognitive function of mentally disabled people. Methods: Twenty-one mentally disabled people were enrolled in this study. They were assigned randomly to one of two groups: Pilates gymball exercise group (PGEG, n=11), and control group (CG, n=10). The subjects in the PGEG group performed the exercises for 50 minutes a day, three days per week for 6 weeks. The PGEG program consisted of warm up (10 minutes), main workout (30 minutes), and cool down (10 minutes). The main workout consisted of 10 exercise programs. The electroencephalogram (EEG) of Fp1, Fp2, F3, F4, C3, C4, O1, and O2 were measured using an PolyG-I system. The cognitive function was evaluated using a mini-mental state examination (MMSE). The measurements were performed before exercise, and 6 weeks after exercise. Covariance analysis (ANCOVA) was performed to determine the difference between the two groups Results: A significant difference in Fp1, Fp2, and F3 on the relative alpha power was observed between the PGEG and CG groups (p<0.05). A significant difference in Fp1 on the relative beta power was observed between the PGEG and CG groups (p<0.05). No significant difference in the MMSE score was observed between the PGEG and CG groups. Conclusion: Pilates gymball exercise did positively change the EEG in the frontal lobe. On the other hand, the effect related to cognitive was limited. Pilates gymball exercise appears to be more effective in facilitating brain stimulation related to cognition.

      • Three-dimensionally crossing manifold micro-mixer for fast mixing in a short channel length

        <P>In this study, we report a neo-conceptive three-dimensionally (3D) crossing manifold micromixer (CMM) embedded in microchannel. Fabricated by sequential processes of photolithography and two photon absorption stereolithography, this leads to a microfluidic system with a built-in micromixer in a site controlled manner. The effectiveness of CMM is investigated numerically and experimentally. Through the numerical simulation, it is estimated that a high mixing ratio of 90% can be obtained even in a channel length shorter than five times the channel width. This compares well with the conventional passive type of micromixers that have a gradual increase in mixing efficiency with the length of the channel.         Significance of chitinase-3-like protein 1 in the pathogenesis of inflammatory diseases and cancer

        Chitinase-3-like protein 1 (CHI3L1) is a secreted glycoprotein that mediates inflammation, macrophage polarization, apoptosis, and carcinogenesis. The expression of CHI3L1 is strongly upregulated by various inflammatory and immunological diseases, including several cancers, Alzheimer’s disease, and atherosclerosis. Several studies have shown that CHI3L1 can be considered as a marker of disease diagnosis, prognosis, disease activity, and severity. In addition, the proinflammatory action of CHI3L1 may be mediated via responses to various proinflammatory cytokines, including tumor necrosis factor-α, interleukin-1β, interleukin-6, and interferon-γ. Therefore, CHI3L1 may contribute to a vast array of inflammatory diseases. However, its pathophysiological and pharmacological roles in the development of inflammatory diseases remain unclear. In this article, we review recent findings regarding the roles of CHI3L1 in the development of inflammatory diseases and suggest therapeutic approaches that target CHI3L1.
