        항암치료와 관련된 백질뇌증 1례

        Antineoplastic treatment-related leukoencephalopathy is defined as an entity in which acute or delayed changes in with matter, secondary to chemotherapy and/or radiation, produce cerebral, cerebellar, and/or brainstem dysfunction in the absence of demonstrable, causally microorganism. We experienced a case of antineoplastic treatment-related leukoencepholopathy in an 11 year-old girl who was diagnosed as having acute lymphoblastic leukemia when she was 8 years old. She had 3 episodes of isolated CNS relapse during maintenance chemotherapy. One month prior to admission, she had difficulties. in adaptation of new class and refused to go to school Six days prior to admission, she began to stammer over a few words and at that night, dysarthria became overt. Next morning, locomotor activities were slow and she was not able to salk without assistance and had right foot dragging. Brain MRI revealed diffuse demyelination in deep white matter with mild degree of diffuse ventricular dilatation and prominent subarachnoid space and sulcus. She had received craniospinal irradiation(whole brain 5,040 cGy, spine 1,350 cGy), intrathecal MTX 468mg, and intrathecal Ara-C 1,890mg. Supportive managements including L-tube feeding, steroid, and mannitolization were done without chemotherapy under the impression of antineoplastic treatment-related leukoencephalopathy. Bone marrow relapse was detected during admission. Her general conditions deteriorated and decorticate posture changed to decerebarte posture. She died at 54th hospital day but autopsy was not done.

        부신백질이영양증의 임상적, 생화학적 소견 및 뇌자기공명영상 소견

        Seven cases of adrenoleukodystrophy who were diagnosed in the Department of Pediatrics, Seoul National University Hospital from January 1985 to April 1993 were reviewed. The results were as follows: 1) All of the patients were male and the onset of symptoms was between 5 and 13 years of age. They were all classified as X-liked childhood adrenoleukodystrophy. 2) The initial symptoms were behavior change, visual disturbance, skin pigmentation poor school perfomance and gait disturbance. 3) Skin pigmentation was noticed in 3 cases which showed elevated plasma ACTH level on ACTH stimulation test, the cortisol levels were not stimulated. One case out of 4 cases having no hypoadrenal symptoms showed no response to ACTH stimulation test. 4) Elevated CSF IgG and protein levels were noticed in 4 cases of 7(57%) and 4 cases out of 6(67%), respectively. 5) There is no correlation between the evoked potentials and the subjective visual and auditory disturbance. 6) There is correlation between the involved sites on brain MRI and the clinical manifestations. 7) Brain MRI showed typically bilateral, symmetrical hyperintense areas in the periventricular white matter of parieto-occipital areas in 5 cases out of 7 cases, One case showed frontal lobe involvement only without occipital lobe and another case showed multiple patch involvement in the internal capsules and thalamus. 8) Management was including restriction of saturated VLCFA in diet, steroid therapy, physiotherapy, IV globulin, and Lorenzo oil. Clinical courses were not reversed. In conclusion, we could confirm adrenoleukodystrophy using plasma VLCFA assay without brain biopsy, and earlier diagnostic approach was possible owing to brain MIR.

        납 안정동위원소를 이용한 아연제련시설 인근지역의 납 오염 특성

        This study was conducted to evaluate lead(Pb) pollution near a zinc smelter using Pb stable isotopes as environmental tracers. Environmental samples such as air particles, precipitation, soils, conifer leaves, and tree rings were analyzed; input and output materials were also collected from the zinc smelter. The 207Pb/206Pb ratios of the output materials from the zinc smelter were in the range of 0.865-0.881, and plotted on a mixing line with zinc ores from South America and Australia as the two end-members. The 207Pb/206Pb ratios of air particles, precipitation, and conifer leaves were in the ranges of 0.822-0.887, 0.853-0.882, and 0.86 -0.877, respectively, and fall within the line attributed to the smelter. In contrast, the 207Pb/206Pb ratios of soils fell within a broader range of 0.694-0.893. Soils with a higher 207Pb/206Pb ratio of 0.850 were plotted on the smelter line and showed high Pb concentrations of 207.5±141.9 mg/kg. Soils with a lower 207Pb/206Pb ratio of 0.850 were plotted on the Korean ore line and showed low Pb concentrations of 60.5±44.2 mg/kg. The 207Pb/206Pb ratios of tree rings showed an increasing trend toward the isotopic composition of emission gases, implying that the source of Pb pollution changed from the local mine to the zinc smelter.

        Dose-dependent embryotrophic effect of recombinant granulocyte-macrophage colony-stimulating factor and brain-derived neurotrophic factor in culture medium for mouse preimplantation embryo

        Objective To evaluate the dose effect of recombinant mouse granulocyte-macrophage colony-stimulating factor (rmGM-CSF) or brain-derived neurotrophic factor (BDNF) in culture medium on the development of in vitro fertilized mouse embryos. Methods Mature oocytes were retrieved from superovulated female BDF1 mice and inseminated by sperm from male BDF1 mice. On day 1, two-cell stage embryos were divided and cultured until day 5 in the embryo maintenance medium supplemented with 0, 1, 2, 5, or 10 ng/mL of rmGM-CSF or supplemented with 0, 5, 10, or 20 ng/mL of BDNF. Blastocyst formation rate and their cell numbers were assessed. Results The blastocyst formation rate and the total cell count in blastocyst was similar in all the rmGM-CSF treatment groups when compared with the control. However, the blastocyst formation rate and the total cell count was significantly higher in the group supplemented with 10 ng/mL of BDNF compared with the control (63.9%, 45.8±11.5 vs. 52.3%, 38.0±6.8; P<0.05, respectively). Conclusion Supplementation of 10 ng/mL of BDNF enhanced the developmental potential of mouse preimplantation embryos, but supplementation of rmGM-CSF did not.

        Effect of Cisplatin on Sodium-Dependent Hexose Transport in LLC-PK<SUB>1</SUB> Renal Epithelial Cells

        <P> Cis-dichlorodiammine platinum II (Cisplatin), an effective chemotherapeutic agent, induces acute renal failure by unknown mechanisms. To investigate direct toxic effects of cisplatin on the renal proximal tubular transport system, LLC-PK<SUB>1 </SUB>cell line was selected as a cell model and the sugar transport activity was evaluated during a course of cisplatin treatment. Cells grown to confluence were treated with cisplatin for 60 min, washed, and then incubated for up to 5 days. At appropriate intervals, cells were tested for sugar transport activity using α-methyl-D-[<SUP>14</SUP>C]glucopyranoside (AMG) as a model substrate. In cells treated with 100 ㄍM cisplatin, the AMG uptake was progressively impaired after 3 days. The viability of cells was not substantially changed with cisplatin of less than 100 ㄍM, but it decreased markedly with 150 and 200 ㄍM. In cisplatin-treated cells, the Na<SUP>+</SUP>-dependent AMG uptake was drastically inhibited with no change in the Na<SUP>+</SUP>-independent uptake. Kinetic analysis indicated that Vmax was suppressed, but Km was not altered. The Na<SUP>+</SUP>-dependent phlorizin binding was also decreased in cisplatin-treated cells. However, the AMG efflux from preloaded cells was not apparently retarded by cisplatin treatment. These data indicate that the cisplatin treatment impairs Na<SUP>+</SUP>-hexose cotransporters in LLC-PK<SUB>1</SUB> cells and suggest strongly that defects in transporter function at the luminal plasma membrane of the proximal tubular cells constitute an important pathogenic mechanism of cisplatin nephrotoxicity.

        Original Article : Role of 15-hydroxyprostaglandin dehydrogenase down-regulation on the prognosis of hepatocellular carcinoma

        Background/Aims: The role of prostaglandin E2 (PGE2) in the modulation of cell growth is well established in colorectal cancer. The aim of this study was to elucidate the significance of 15-hydroxyprostaglandin dehydrogenase (15-PGDH) down-regulation on the prognosis of hepatocellular carcinoma (HCC) patients. Methods: The expression of 15-PGDH in HCC cell lines and resected HCC tissues was investigated, and the correlation between 15-PGDH expression and HCC cell-line proliferation and patient survival was explored. Results: The interleukin-1-β-induced suppression of 15-PGDH did not change the proliferation of PLC and Huh-7 cells in the MTS [3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide] assay. The induction of 15-PGDH by transfection in HepG2 cells without baseline 15-PGDH expression was suppressed at day 2 of proliferation compared with empty-vector transfection, but there was no difference at day 3. Among the 153 patients who received curative HCC resection between 2003 and 2004 at our institution, 15-PGDH expression was observed in resected HCC tissues in 56 (36.6%), but the 5-year survival rate did not differ from that of the remaining 97 non-15-PGDH-expressing patients (57.1% vs 59.8%; P=0.93). Among 50 patients who exhibited baseline 15-PGDH expression in adjacent nontumor liver tissues, 28 (56%) exhibited a reduction in 15-PGDH expression score in HCC tissues, and there was a trend toward fewer long-term survivors compared with the remaining 22 with the same or increment in their 15-PGDH expression score in HCC tissues. Conclusions: The prognostic significance of 15-PGDH down-regulation in HCC was not established in this study. However, maintenance of 15-PGDH expression could be a potential therapeutic target for a subgroup of HCC patients with baseline 15-PGDH expression in adjacent nontumor liver tissue.       • Heat Shock Protein Expression in Endotoxin Treated Renal Epithelial Cells

        Recent studies have reported that markedly increased concentrations of a number of different classes of protein are initially identified by the marked increase in their concentration in cells after various stresses including heat shock. In this study, putative interactions between apoptosis and heat-shock proteins disturbed by endotoxin as aseptic shock model were investigated. The direct cellular damages were assessed by the LDH release from the cytoplasm of the human tubular epithelial cells(HK-2 cells) after lipopolysaccharides(LPS) treatment. And the expressions of Bcl2 and BAX mRNA that had been employed as an index of apoptosis to assess by what extent apoptosis contributed to tubular cell damage and the expression of heat shock protein(HSP)90, 72 and 27 with the Bcl2/BAX ratio in septic shock model were studied also. LPS treatment let HK-2 cells release SDH markely(14.33+/-1.55 vs. 61.00+/- 2.65, p<0.01). LDH release was greater significantly from the cells that had not been heat preconditioned than that of heat preconditioned(61.00+/- 2.65vs. 41.33 +/- 3.21,p<0.05) LPS treatment itself did not up-regulated Bcl2 significantly. But the expression of Bcl2 was marked with heat preconditioning. The expression of BAX was increased with LPS treatment but did not increase significantly with heat preconditioning(165% vs. 1496%). We did not see any contribution of heat preconditioning on the expression of HSP90. HSP72 expression was slightly increased with LPS treatment. But the expression of HSP27 was more evident after preconditioning with LPS treatment HSP27 expression was evident after LPS treatment. This study showed that the heat preconditioning was able to provide protection on the human tubular epithlial cells from LPS treatment extensively. The protection from cytolytic damage and increase of Bcl2/BAX ratio was closely related with the increase of HSP72 and HSP27, especially with LPS treatment.
