In order to observe the effects of freeze-dried saliva of Agkistrodon caliginosus and Agkistrodon saxatilis, the Korean poisonous snakes, on the contractility and action potential of frog ventricular muscle cells, the isometric tension in a vertical chamber and the action potential in horizontal chamber were recorded and analyzed. The following results were obtained; 1. ED_50, to reduce the contractility was 4-8ug/ml in the case of A. saxatilis and 8-16ug/ml in the case of A. caliginosus which all were higher than that of A. brevicaudus and the inhibitory effect on the contractility was greater in the case of A. saxatilis than that of A. caliginosus. 2. In the case of A. saxatilis, APD_50(time to repolarize 50% of action potential amplitude) decreased from 820±80 ms to 710±60 ms, the overshoot and the V max(Maximum depolarization rate) decreased from 34±8 mV to 18±4 mV, from 98±8 V/s to 89±4 V/s respectively. 3. In the case of A. caliginosus, ADP_50 decreased from 880±70 ms to 720±40 ms, the overshoot and the V max decreased from 28±6 mV to 20±4 mV, from 89±8V/s to 77±12V/s, respectively. 4. Inhibition of contractility and overshoot potential were more prominent in A. saxatilis and APD_50 was more decreased in A. caliginosus. The above result showed that Korean snake venoms would decrease the ionic current underlying the rapid upstroke phase and slow inward currents by calcium ions simultaneously in a frog ventricular muscle cells.

Background: Atopic dermatitis will persist into adult life and chronic severe atopic dermatitis may cause significant interference in social interaction. Despite the controversy concerning the existence of specific personality exhibited by patients with atopic dermatitis, psychological and social factors have long been considered important factors that have influence on the course of the disease. Objective : The purpose of this study was to investigate the charateristics of the personalities of adult patients with atopic dermatitis and their relationship to psychological factors and clinical characteristics. Methods : Eysenck Personahty Questionnaire(EPQ) was carried out on 54 adult patients with atopic dermatitis. Using the EPQ scores, a cluster analysis was done to identify the subgroup of psychologically disabled patients. The psychological and social aspects of the identified patients were then investigated using a designed questionnaire. Results : 1. A cluster analysis identified two patient subgroups. Psychoticism(P), extroversion-introversion(E), and neuroticism(N) scores were significantly different between the subgroups. The neuroticism scores were above the upper limit of the normal range in the higher EPQ scored subgroups. However, the total clinical severity score which was evaluated in 25 patients showed no significant difference between these subgroups. 2. The higher psychoticism, extroversion-introversion, and neuroticism scored patients tended to express severe disability due to atopic dermatitis and a high level of stress which was evaluated by the designed questionnaire. Conclusion : Eysenck personality questionnaire can be helpful in identifying a subgroup of patients who may need psychiatric intervention. (Kor J Dermatol 1996;34(3): 359-365)

The allopurinol hypersensicivity syndrome is a rare, but life thereaning immunologic reaction of allopurinol therapy, characterized by multiple abnormalities such as fever, rash, decreased renal function, hepatocellular injury, leukocytosis, and eosinophila. It may require prolonged hospitalization and occasionally involves residual morbidity. Three patients developed erythematous skin eruption three to five weeks after beginning therapy with allopurinol. The clinical, laboratory, and histologic findings of these patients were compatible with a allopurinol hyperensitivity syndrome. (Kor J Dermatol 1995; 33(1): 130-134)

Male breast cancer is a rare disease, accounting for approximately 1% of all breast cancer. We report a case of cutaneous, osteolytic, pulmonary, and hepatic metastases of male breast cancer in a 40-year-old male who presented 4 × 2.5 ㎝ erythematous plaque with ulceration and bloody discharge on the areolar area of the left anterior chest. The histopathologic findings of the lesion showed nests and strands of invasive carcinoma forming ductlike structures and cellular arrangement in single-row line, referred to as Indian filing, between collagen bundles. The tumor cell showed positive reaction for estrogen receptor, but negative reaction for progesterone receptor. He was treated with tamoxifen, 5-fluouracil, adriamycin, and cyclophosphamide. Six months after initiation of the treatment, skin lesion was improved. Also, number and size of the metastatic cancer nodules decreased in the computer tomographic imaging. Allopurinol is a xanthine oxidase inhibitor widely used to control plasma uric acid levels. Serious and life threatening adverse effects such as the allopurinol hypersensitivity syndrome(AHS) are rare. We describe a 32-year-old female patient who was treated with allopurinol and developed fever, rash, and renal and liver dysfunction.

Ramsay Hunt syndrome or herpes zoster oticus is a viral disease associated with otalgia, vesicuTar eruption, facial paralysis, hearing disturbance, tinnitus, vertigo and other symptoms. Rarely it can be associated with involvement of Ⅳ, Ⅴ, Ⅸ, Ⅹ, XI, XII cranial nerves, cervical plexus, and diffuse encephalitis. We report a case of Ramsay Hunt syndrome with vocal cord paralysis which developed prior to facial nerve paralysis. (Korean J Dermatol 2003;41(4) : 507~509)

N/A Background : The role of dietary factors in atopic dermatitis has been the subject of debate for several decades. Recently it has received convincing support from a double-blind controlled trial of an exclusion diet, showing benefit from careful avoidance of egg and milk in a group of atopic children. Object : The purpose of this study was to determine the prevalence of specific food allergies in Korean atopic dermatitis and the potential utility of the Pharmacia CAP-RAST FEIA in the diagnosis of food allergy. Methods : One hundred ten patients with atopic dermatitis were evaluated for food hypersensitivity by history, skin prick test, CAP-RAST FEIA, and open food challenge test(OFCT). Results : 1) The prevalence of specific food allergies in the study population was 28.2% in history and 18.2% in CAP-RAST FEIA and OFCT. 2) The frequent food allergens were milk, egg, peanut, and wheat. 3) There was no correlation between the clinical severity of atopic dermatitis and CAP-RAST FEIA and OFCT results. 4) Plasma eosinophil and IgE levels were correlated with CAP-RAST FEIA results, but not OFCT results. 5) According to the history, 31 atopic dermatitis patients answered that their dermatitis was related to food, and of the 31 patients, positive reactions to prick test and CAP-RAST FEIA were observed in 8 and 22 patients, respectively. Among the other 79 patients who answered that there was no relation between atopic dermatitis and food, 14 patients reacted positively to prick test and 47 patients showed positive result to CAP-RAST FEIA. Conclusion : The CAP-RAST FEIA provides standardized measurement of food specific IgE antibodies which may be useful to diagnose food allergy in atopic dermatitis.
