Background/Aims: There is a paucity of data on the longterm efficacy of combination lamivudine (LMV) plus adefovir (ADV) combination or entecavir (ETV) rescue therapy in patients with LMV-resistant chronic hepatitis B (LMV-r CHB). Methods: 51 patients with LMV-r CHB underwent rescue therapy with LAM [100 mg (qd)]+ADV [10 mg (qd)] (n=32, ADV combination group) and ETV [1mg(qd)] (n=19, ETV group). We determined the duration of rescue therapy, timing and type of mutation, undetectable serum HBV DNA by PCR (lower limitation of detection, <140 copies/mL), biochemical response (alanine aminotransferase (ALT) <40 IU/mL), and hepatitis B virus e antigen (HBeAg) seroconversion and virologic breakthrough. Results: Baseline characteristics were not different between the two groups. The duration of rescue therapy in ADV combination and ETV group was 55.7 months (range 22-91 months) vs 43.6 months (range 13-71 months)(P=0.036). The rate of sustained treatment was 62.5% vs. 31.6% (P=0.033). In ADV combination group, the cumulative rates of HBV-DNA undetectability, ALT normalization, and HBeAg seroconversion up to 5 years were 62.5%. 100%, and 33.3%, respectively. Three patients had genotypic resistance (A181T/S/V, median period, 12.7 months). In ETV group, five patients had genotypic resistance (S202I, T184G, median period, 22.8 months). The rates of virologic breakthrough in each group were 31.3%(10/32) vs 63.1%(13/19). The rates of cumulative virologic breakthrough at 12, 24, 36, 48, and 60 months in each group were 3%, 16%, 22%, 25%, and 28%, vs 16%, 26%, 47%, 58%, and 68%, respectively. Conclusions: Combination LAM plus ADV therapy for up to 5 years achieved modest rates of virological suppression, but resistance developed in only 9.4% of patients. ETV is not an optimal therapy because of increasing risk of viral breakthrough to ETV over time.
A case of remnant cystic duct carcinoma

Carcinoma of the remnant cystic duct is extremely rare, and only eight cases have been reported in worldwide. We experienced a case of incidentally detected remnant cystic duct carcinoma in a patient who had undergone cholecystectomy. A 74-year-old woman visited our hospital with pain of right upper quadrant of the abdomen. Her past history included cholecystectomy for acute cholecystitis 10 years ago. Both computer tomography (CT) scan and magnetic resonance imaging of the abdomen suspected acute cholecystitis in remnant gallbladder (GB) or remnant cystic duct with impacted multiple stones. And there was focal enhanced wall thickening in cystic duct adjacent common bile duct (CBD). Endoscopic retrograde cholangiopancreatoscopy was performed, but there was no filling defect or dilatation in CBD. Resection of remnant GB and cystic duct was performed. The resected specimen revealed a chronic active inflammation with abscess formation, and adenocarcinoma invading into the wall (pT2). The diagnosis of carcinoma of the cystic duct was made using Ozden`s new criteria. A positron emission tomography-CT scan performed after cholecystectomy showed an hypermetabolic lesion in portal mass (standard uptake value 4.8), and residual malignancy could not be ruled out. Resection of remnant cystic duct, wedge resection of liver, and lymphadenectomy were performed. There was no residual cancer cells in cystic duct, liver, or lymph nodes. The final diagnosis was remnant cystic duct carcinoma.

Cryptococcosis is an invasive fungal infection which primarily involves lungs of immunocompromised patients. Usually, cutaneous involvement occurs secondarily to the systemic dissemination of Cryptococcus from the infected lungs. The primary involvement of skin, primary cutaneous cryptococcosis (PCC), is a rare clinical event. Recently, we have experienced a 75-year-old woman who had cryptococcal cellulitis at her right lower leg, with no evidence of systemic cryptococcal dissemination. She received long-term low-dose glucocorticoids after adrenalectomy for adrenal carcinoma.
Synthesis of Psoralen Derivatives and Their Blocking Effect of hKv1.5 Channel

Previously, we found that a furocoumarin derivative, psoralen (7H-furo[3,2-g][1 ]benzopyran-7-one), blocked a human Kv1.5 potassium channel (hKv1.5) and has a potential antiarrhythmic effect. In the present study, to develop more potent hKv1.5 blockers or antiarrhythmic drugs, we synthesized ten psoralen derivatives and examined their blocking effects or hKv1.5 stably expressed in Ltk cells. Among the newly synthesized psoralen derivatives, three derivatives (Compounds 5,9 and 10) showed the open channel-blocking effect. Compound 9 among them was the most potent In blocking hKv1.5. We found that compound 9, one of the psoralen derivatives, inhibited the hKv1.5 current in a concentration-, use- and voltage-dependent manner with an IC$_{50}$ value of 27.4 ${\pm}$ 5.1 nM at +60 mV. Compound 9 accelerated the inactivation kinetics of the hKv1.5 channel, slowed the deactivation kinetics of hKv1.5 current resulting in a tail crossover phenomenon. Compound 9 inhibited hKv1.5 current in a use-dependent manner. These results indicate that compound 9, one of psoralen derivatives, acts on hKv1.5 channel as an open charnel blocker and is much more potent than psoralen in blocking hKv1.5 channel. If further studios were done, compound 9 might be an ideal antiarrhythmic drug for atrial fibrillation.

Though there is much improvement of reduction of BSR (Buzz, Squeak, and Rattle) noise inside of vehicle, still there are a lot of shortcomings not considering the friction characteristics of material pair used in vehicle. In this paper, we proceeded fretting test using material pair used in a vehicle or excavators to find relationship between noise and estimated values such as surface roughness, wear area, hardness of each material. First, we find relationship between roughness and wear area by comparing measured surface roughness and calculated wear area. Then, we also find tendency between hardness and noise by comparing measured surface hardness of each material and recorded noise.
The effect of plateau honing on the friction and wear of cylinder liners

<P>The friction and wear tests with reciprocating motion were performed to compare the lubricity of sliding cylinder liner surfaces with each different mark of plateau honing. The friction and wear of surfaces with each different depth of profiles, which were used as the honing mark of the marine diesel engine, were compared with those of randomly ground surfaces. From the tests with the deep-grooved honing marks, it was found that the severe interactions due to asperity contacts and the formation of relatively thin films produced larger amounts of wear volumes than the test with the shallow-grooved ones.</P>
Comparative Analysis of Serum Proteomes of Moyamoya Disease and Normal Controls

Objective : The etiology and pathogenesis of moyamoya disease remain unclear. Furthermore, the definitive diagnostic protein-biomarkers for moyamoya disease are still unknown. The present study analyzed serum proteomes from normal controls and moyamoya patients to identify novel serological biomarkers for diagnosing moyamoya disease. Methods : We compared the two-dimensional electrophoresis patterns of sera from moyamoya disease patients and normal controls and identified the differentially-expressed spots by matrix-assisted laser desorption/ionization-time-of flight mass spectrometry and electrospray ionization quadruple time-of-flight mass spectrometry. Results : We found and analyzed 22 differently-expressed proteomes. Two proteins were up-regulated. Twenty proteins were down-regulated. Complement C1 inhibitor protein and apolipoprotein C-III showed predominantly changed expressions (complement C1 inhibitor protein averaged a 7.23-fold expression in moyamoya patients as compared to controls, while apolipoprotein C-III averaged a 0.066-fold expression). Conclusion : Although our study had a small sample size, our proteomic data provide serologic clue proteins for understanding moyamoya disease.
DEVELOPMENT OF THE WEAR RESISTANT BALL NUT ASSEMBLY IN THE RACK-BASED ELECTRIC POWER STEERING

Electric Power Steering (EPS) is rapidly evolving in terms of performance. However, the abnormal noise from steering, which is experienced by the drivers, reduces the perfection of performance. In this study, the deterioration of Rackbased Electric Power Steering (R-EPS) samples by mileage was analyzed to investigate the cause of steering abnormal noise in the R-EPS applied to a large-sized luxury vehicle that is more sensitive to noise. The results from our analysis showed that the ball nut and screw in the ball nut assembly of R-EPS were in the mode of normal wear. However, the steel ball between the ball nut and screw showed irregular wear mode. It was found that the ball Φ5 has a larger contact area than the ball Φ4, which is advantageous for abrasion and irregular wear because stress is dispersed in the load. Moreover, the ball with Φ5 was superior to the ball with Φ4 in the results of abrasive and adhesive wear tests verified by the ball-on-disk test. Therefore, in the future R-EPS development, the ball with Φ5 compare to the ball with Φ4 should be proposed for the ball nut assembly. This is in order to eliminate the potential cause of steering abnormal noise.

Background/Aims: Bacteria-derived outer membrane vesicles (OMVs) are commonly associated with various biological activities and functions. Helicobacter pylori-derived OMVs are thought to contribute to pathogenesis. This study aimed to investigate the effects of H. pylori-derived OMVs. Methods: H. pylori strains were isolated from patients with gastritis, gastric ulcer, or gastric cancer using endoscopic biopsy. The U-937, AGS, and MKN-45 cell lines were exposed to H. pylori and H. pylori-derived OMVs. The expression of interleukin 8 (IL-8) messenger RNA (mRNA) was assessed using reverse transcription-polymerase chain reaction (RT-PCR) and real-time RT-PCR, and IL-8 secretion was analyzed using enzyme-linked immunosorbent assay. Nuclear factor kappa B (NF-κB) activation was evaluated by Western blotting. Results: H. pylori and H. pylori-derived OMVs induced the expression of IL-8 mRNA and protein. Importantly, the bacteria induced higher IL-8 mRNA and protein expression than the OMVs. IL-8 expression was induced to different levels in response to H. pylori-derived OMVs from hosts with different gastric diseases. Western blotting revealed the increased phosphorylation and reduced degradation of inhibitor of NF-κB alpha in cells exposed to OMVs. Conclusions: H. pylori-derived OMVs may aid the development of various gastric diseases by inducing IL-8 production and NF-κB activation.
