        Tinnitus is noise from ear or head without any external sound stimulation and can cause hearing difficulties, psychogenic disturbances or many difficulties in everyday life. The etiology has yet been discussed and the management is also very difficult. Our study was performed to evaluate the clinical characteristics of idiopathic tinnitus. Three hundred thirty five patients(417 ears) with idiopathic tinnitus were entered into our department's database between Nov. 1995 and Oct. 1997. The patients were fully questioned about their history, and then were physically examined and assessed audiometrically, including pure tone audiogram and tinnitogram. It was found that (1) there was no sexual difference, (2) average age was 48.5 years, (3) there was no laterality, (4) buzzing sound was the most common, (5) average symptom duration was 575.7 days, (6) the most common sensation level was within 5dB, (7) the most common loudness was less than 29dB, and (8) the most common frequency was over 8000Hz.

        Foreign body in the esophagus is common is otolaryngological field and is developed in all ages. Types, sites and degrees of the injury from foreign body are variable. Especially, rare esophageal foreign body may occur in psychiatric patients and can't be expected. Authors report our recent experiences of three cases of rare esophageal foreign body in psychiatric patients.

        兩側 및 單側 電氣衝擊療法後의 腦波變化에 對한 硏究

        Changes on EEG after bilateral and unilateral ECT was studied in 12 young adult schizophrenic patients who were admitted st Seoul Municipal Mental Hospital. Their EEC data were obtained during the interictal period of serial ECT sessions and subjected to global quantitative analysis by visual assessment. Some significant findings are summarizcol as follows: Repeated ECT resulted in increased EEG slow wave activity more see in the frontal and temporal leads which became gradually generalized in forms of occasional burst and run. No spike and focalized activities were seen. The degree of slowing and its amplitude was directly related to the number of ECT sessions. As the successive four to six treatments were administered, the slowing became more persistent and widespreaded with high voltage paroxysmal activities, while the alpha rhythm became disturbed and disappeared. After seven to eight serial treatment, the percent time delta remained constant in plateau or falled off. Fourteen days after the last 10th treatment, 34 percent time of slow wave activities was recorded in frontal lead of bilateral ECT group and 25 percent of unilateral ECT group. Minor lateralized asymmetries of frequancy and amplitude which were predominently right sided, appeared more in the right unilateral ECT group (4 in 6 cases) than the bilateral group (1 in 6 cases). The amount of post-ECT slow wave activity did not revealed major difference between these kinds of ECTs and was closely related to the number of ECT sessions and to the pretreatment EEG with individual variations. No relationship was found between the therapeutic response, degree of amnesia or confusion, and EEG findings after electroconvulsive therapy.

        In this paper, availability of deteriorated remicon due to delaying delivery time is discussed. According to test results, fluidity and air content of plain concrete exhibited a tendency to decline with a elapse of time. After 90 minutes since mixed, use of superplasticizer and AE admixture resulted in an increase in fluidity and air content. Adequate dosage of superplasticizer and AE adimixture is found to add with 0.4% and 0.002% respectively to restore the level of original slump and air content from loss one. Setting time of concrete exhibited an acceleration with an increase in leaving time of concrete. Compressive strength of plain concrete is enhanced with and increase of leaving time of concrete. Therefore, it is found that the use of superplasticizer with 0.4% and AE admixture with 0.002% has a desirable effect on restoring the deteriorated quality of concrete related to fluidity, air content and setting time without strength loss caused by delivery time delaying.

        Pyogenic granuloma is a well circumscribed, soft tissue tumor of inflammatory rather than neoplastic nature. It arises from the connective tissue of the skin or mucous membrance. Pyogenic granuloma arising from the nasal septum is an uncommon benign vascular tumor and it is at times difficult to differentiate from the hemangioma or angiofibroma histologically. Recently the authors had experienced a case of Pyogenic granuloma arising from right side of the nasal septum which was treated by local excision and electrocauterization. We report this case with review of the literatures.

      • 알레르기 환자에서 MAST-CLA 검사의 의의

        To evaluate usefulness of application of MAST-CLA to indetify allergens, authors conducted MAST-CLA allergen-specific IgE assay and collected allergy related symptoms in 118 allergic patients diagnosed at Soonchunhyang University Chunan Hospital during October 1998 through January 2000. The results were as followings; 1. The mean (SD) age was 23.1 (±12.17) years old and teenagers accounted for 33.1% of patients. 2. The frequent allergic symptoms were sneezing (59.3%), rhinorrhea (53.4%), nasal stiffness (44.1%), itching sensation (24.6%), cough or sputum (16.1%), and eye symptom (8.5%) in order. 3. Among these subjects, 62.7% of patients revealed at least more than one specific allergen and 70.3% showed elevated total IgE levels by MAST-CLA. The three most frequent specific allergens were Dermatophagoides farinae (47.5%), Dermatophagoides pteronyssinus (45.8%), and house dust (30.5%). 4. While age was not significantly related to positive of both specific allergens and total IgE (P>0.05), sneezing and nasal stiffness were significantly associated with positive of specific allergen and total IgE, respectively (both P<0.05). 5. Total IgE increased with the number of specific allergens, but both specific allergen and total IgE were not significantly related to increases of allergic symptoms (P>0.05). In conclusion, our data suggest that MAST-CLA allergen-specific IgE assay is useful measure for identification of causative allergens in allergic patients, however, it is recommended that more specific allergic antigens should be determined and replaced or added to Korean inhalent panel of MAST-CLA.

        Ultrastructure of Oocytes During Oogenesis and Oocyte Degeneration Associated with Follicle Cells in Female Sinonovacula constricta(BIVALVIA: PHARIDAE) in Western Korea

        The ultrastructure of oocytes during oogenesis and oocyte degeneration associated with follicle cells in female Sinonovacula constricta(Lamarck, 1818) were investigated by electron microscope observations. Ovarian follicles are surrounded by a matrix of vesicular connective tissue cells(VCT cells). VCT cells contain large quantities of glycogen particles and several lipid droplets in their cytoplasm. It is suggested that VCT cells act as a source of nutrients for vitellogenesis during oogenesis. In early vitellogenic oocytes, several coated vesicles, which appear at the basal region of the oocyte, lead to the formation of membrane-bound vesicles via endocytosis. The uptake of nutritive materials in coated vesicles formed by endocytosis appears through the formation of coated pits on the oolemma during vitellogenesis. During the late stage of oogenesis, yolk precursors(yolk granules), mitochondria and lipid droplets are present in the cytoplasm of late vitellogenic oocytes. In particular, proteinaceous yolk granules containing several different components are intermingles and form immature yolk granules. In the mature oocyte, small immature yolk granules are intermingled and form large mature yolk granules. Vitellogenesis occurs through a process of autosynthesis, involving combined activity of the Golgi complex, mitochondria and rough endoplasmic reticulum in the cytoplasm of vitellogenic oocytes. The process of heterosynthesis is where extraovarian precursors are incorporated into oocytes by endocytosis at the basal region of early vitellogenic oocytes before the formation of the vitelline coat. Follicle cells appear to play an important role in vitellogenesis and oocyte degeneration. The functions of attached follicle cells to the oocyte during oocyte degeneration are phagocytosis and digestion of phagosomes originating from oocyte degeneration. After digestion of phagosomes, it is assumed that the function of follicle cells can permit a transfer of yolk precursors necessary for vitellogenesis and allows for the accumulation of glycogen and lipid during oocyte degeneration, which can be employed by vitellogenic oocytes. Follicle cells of S. constricta may possess a lysosomal system for induction of oocyte breakdown and might resorb phagosomes in the cytoplasm for nutrient accumulation during oocyte degeneration.

        Enhanced Chondrogenic Responses of Human Articular Chondrocytes Onto Silk Fibroin/Wool Keratose Scaffolds Treated With Microwave-Induced Argon Plasma

        <P>Abstract</P><P>Silk fibroin (SF) is a natural, degradable, fibrous protein that is biocompatible, is easily processed, and possesses unique mechanical properties. Another natural material, wool keratose (WK), is a soluble derivative of wool keratin, containing amino acid sequences that induce cell adhesion. Here, we blended SF and WK to improve the poor electrospinability of WK and increase the adhesiveness of SF. We hypothesized that microwave-induced argon plasma treatment would improve chondrogenic cell growth and cartilage-specific extracellular matrix formation on a three-dimensional SF/WK scaffold. After argon plasma treatment, static water contact angle measurement revealed increased hydrophilicity of the SF/WK scaffold, and scanning electron microscopy showed that treated SF/WK scaffolds had deeper and more cylindrical pores than nontreated scaffolds. Attachment and proliferation of neonatal human knee articular chondrocytes on treated SF/WK scaffolds increased significantly, followed by increased glycosaminoglycan synthesis. Our results suggest that microwave-induced, plasma-treated SF/WK scaffolds have potential in cartilage tissue engineering.</P>

      • Intra- and inter-laboratory reproducibility and predictivity of the HaCaSens assay: A skin sensitization test using human keratinocytes, HaCaT

        <P><B>Abstract</B></P> <P>Due to considerable constraints in using animals for risk assessment, much effort has been directed at developing non-animal test methods. Developing assays for skin sensitization, the leading cause of contact dermatitis, is particularly important, but there are currently no <I>in vitro</I> skin sensitization tests that completely replace animal tests.</P> <P>HaCaSens, a simple skin sensitization test using non-transformed HaCaT cells, predicts keratinocyte activation by skin sensitizers with 75% sensitivity, 83% specificity and 77% accuracy in a previous study using 22 coded substances. Although the data show promising results, the number of tested substances is insufficient to prove predictive capacity. Moreover, reproducibility among different laboratories has not been studied.</P> <P>Here, three laboratories participated in a validation in order to assess HaCaSens feasibility for official validation. To examine transferability, intra- and inter-lab reproducibility and predictive capacity, HaCaSens was assessed on a set of 30 test substances coordinated by the Validation Management Team (VMT). The results showed satisfactory transferability as well as intra- and inter-laboratory reproducibility. Further assessment of its predictive capacity on 20 test substances demonstrated a sensitivity of 81.8% (18/22), specificity of 87.5% (7/8), and accuracy of 83.3% (25/30) in identifying skin sensitizers, which is comparable with presently validated assays, KeratinoSens™ and LuSens.</P> <P>This validation study shows that the HaCaSens assay is easily transferable, reproducible and highly predictable for identifying skin sensitizers.</P> <P><B>Highlights</B></P> <P> <UL> <LI> HaCaSens assay is performed in three laboratories with a set of 30 test substances. </LI> <LI> Intra- and inter-laboratory validation shows satisfactory accuracy over 83%. </LI> <LI> Compared with other similar assays, the HaCaSens acquired better accuracy. </LI> <LI> This study can be a reliable support for proposing HaCaSens as a promising assay. </LI> </UL> </P>

        Optimization of target, moderator, and collimator in the accelerator-based boron neutron capture therapy system: A Monte Carlo study

        The aim of this study was to optimize the target, moderator, and collimator (TMC) in a neutron beam generator for the accelerator-based BNCT (A-BNCT) system. The optimization employed the Monte Carlo Neutron and Photon (MCNP) simulation. The optimal geometry for the target was decided as the one with the highest neutron flux among nominates, which were called as angled, rib, and tube in this study. The moderator was optimized in terms of consisting material to produce appropriate neutron energy distribution for the treatment. The optimization of the collimator, which wrapped around the target, was carried out by deciding the material to effectively prevent the leakage radiations. As results, characteristic of the neutron beam from the optimized TMC was compared to the recommendation by the International Atomic Energy Agent (IAEA). The tube type target showed the highest neutron flux among nominates. The optimal material for the moderator and collimator were combination of Fluental (Al<sub>2</sub>0<sub>3</sub>+AlF<sub>3</sub>) with <sup>60</sup>Ni filter and lead, respectively. The optimized TMC satisfied the IAEA recommendations such as the minimum production rate of epithermal neutrons from thermal neutrons: that was 2.5 times higher. The results can be used as source terms for shielding designs of treatment rooms.
