Background/Aims: Acute-on-chronic liver failure (ACLF) is a catastrophic illness. Few studies investigated the prognostic value of vitamin D-binding protein (VDBP) for hepatitis B virus (HBV)-related ACLF (HBV-ACLF) resulted in conflicting results. Methods: Two prospective HBV-ACLF cohorts (n=287 and n=119) were enrolled to assess and validate the prognostic performance of VDBP. Results: VDBP levels in the non-survivors were significantly lower than in the survivors (P<0.001). Multivariate Cox regression demonstrated that VDBP was an independent prognostic factor for HBV-ACLF. The VDBP level at admission gradually decreased as the number of failed organs increased (P<0.001), and it was closely related to coagulation failure. The areas under the receiver operating characteristic curve (AUCs) of the Child-Pugh-VDBP and chronic liver failuresequential organ failure assessment (CLIF–SOFA)-VDBP scores were significantly higher than those of Child-Pugh (P<0.001) and CLIF-SOFA (P=0.0013). The AUCs of model for end-stage liver disease (MELD)-VDBP were significantly higher than those of MELD (P= 0.0384) only in the case of cirrhotic HBV-ACLF patients. Similar results were validated using an external multicenter HBV-ACLF cohort. By longitudinal observation, the VDBP levels gradually increased in survivors (P=0.026) and gradually decreased in non-survivors (P<0.001). Additionally, the VDBP levels were found to be significantly decreased in the deterioration group (P=0.012) and tended to be decreased in the fluctuation group (P=0.055). In contrast, they showed a significant increase in the improvement group (P=0.036). Conclusions: The VDBP was a promising prognostic biomarker for HBV-ACLF. Sequential measurement of circulating VDBP shows value for the monitoring of ACLF progression.
Efficiently targeted therapy of glioblastoma xenograft via multifunctional biomimetic nanodrugs

Background: Glioblastoma multiforme (GBM) is a fatal malignant primary brain tumor in adults. The therapeutic efficacy of chemotherapeutic drugs is limited due to the blood-brain barrier (BBB), poor drug targeting, and short biological half-lives. Multifunctional biomimetic nanodrugs have great potential to overcome these limitations of chemotherapeutic drugs. Methods: We synthesized and characterized a biomimetic nanodrug CMS/PEG-DOX-M. The CMS/PEG-DOX-M effectively and rapidly released DOX in U87 MG cells. Cell proliferation and apoptosis assays were examined by the MTT and TUNEL assays. The penetration of nanodrugs through the BBB and anti-tumor efficacy were investigated in the orthotopic glioblastoma xenograft models. Results: We showed that CMS/PEG-DOX-M inhibited cell proliferation of U87 MG cells and effectively induced cell apoptosis of U87 MG cells. Intracranial antitumor experiments showed that free DOX hardly penetrated the BBB, but CMS/PEG-DOX-M effectively reached the orthotopic ntracranial tumor through the BBB and significantly inhibited tumor growth. Immunofluorescence staining of orthotopic tumor tissue sections confirmed that nanodrugs promoted apoptosis of tumor cells. This study developed a multimodal nanodrug treatment system with the enhanced abilities of tumor-targeting, BBB penetration, and cancer-specific accumulation of chemotherapeutic drugs by combining chemotherapy and photothermal therapy. It can be used as a flexible and effective GBM treatment system and it may also be used for the treatment of other central nervous systems (CNS) tumors and extracranial tumors.
Current Controlled with Valley Voltage Detection Three-port Converter with Current-Pulsed Load

In small power supply systems such as aircraft and ships, some loads present the characteristics of current pulse. After the pulse load is directly connected to the power supply bus, the instantaneous power of its load presents the pulse characteristic as the load current. In order to balance the difference of instantaneous power, this paper proposes a bi-directional converter to compensate for the current fluctuation caused by the pulse load characteristic, which makes the power supply bus in the state of constant voltage and constant current, and solves the problem of adaptability of pulse load and power supply. In the meanwhile, a current feedback control method with valley voltage loop is proposed to control the bi-directional converter, which is also applicable when there is any change in frequency or duty cycle of the pulse load, even the constant power load. Finally, an experimental prototype is built to verify the correctness of the theoretical analysis.
Similar Hand Gesture Recognition by Automatically Extracting Distinctive Features

With the flourish development of computer vision technology, hand gesture recognition plays a moreand more vital role in human-computer interaction for its convenient and nonverbal communication. However,confusion caused by similar gestures brings inherent errors when considering enough meaningful gestures in thedatabase. In this paper, an automatic feature extraction for similar gesture recognition is proposed with respect toconfusion arising in similar gestures. Except the orientation feature, four additional innovative features are extractedto distinguish all the similar gestures remarkably in the experimental database containing 10 numbers and 26 letters. Compared with the conventional method that a couple of similar gestures are extracted as a specific feature, theproposed method distinguishes similar gestures with automatic distinctive feature extraction. Experimental resultsshow high recognition rate and versatility of the proposed method.
