      • Therapeutic physical exercise in neural injury: friend or foe?

        <P>[Purpose] The intensity of therapeutic physical exercise is complex and sometimes controversial in patients with neural injuries. This review assessed whether therapeutic physical exercise is beneficial according to the intensity of the physical exercise. [Methods] The authors identified clinically or scientifically relevant articles from PubMed that met the inclusion criteria. [Results] Exercise training can improve body strength and lead to the physiological adaptation of skeletal muscles and the nervous system after neural injuries. Furthermore, neurophysiological and neuropathological studies show differences in the beneficial effects of forced therapeutic exercise in patients with severe or mild neural injuries. Forced exercise alters the distribution of muscle fiber types in patients with neural injuries. Based on several animal studies, forced exercise may promote functional recovery following cerebral ischemia via signaling molecules in ischemic brain regions. [Conclusions] This review describes several types of therapeutic forced exercise and the controversy regarding the therapeutic effects in experimental animals versus humans with neural injuries. This review also provides a therapeutic strategy for physical therapists that grades the intensity of forced exercise according to the level of neural injury.</P>

      • Synergistic effect of melatonin on exercise-induced neuronal reconstruction and functional recovery in a spinal cord injury animal model

        <P>Abstract: </P><P>Nitric oxide (NO) may aggravate neuronal damage after spinal cord injury (SCI). We hypothesized that NO produced by inducible nitric oxide synthase (iNOS) accelerates secondary damage to spinal tissue, which may be reversed by the neuroprotectant, melatonin. This study investigated the effects of combination therapy with melatonin (10 mg/kg) and exercise (10 m/min) on recovery from SCI caused by contusion. We examined locomotor recovery, iNOS gene expression, autophagic and apoptotic signaling, including Beclin-1, LC3, p53 and IKK&agr; protein expression and histological alterations in the ventral horn of the spinal cord. Melatonin in combination with exercise resulted in significantly increased hindlimb movement (<I>P </I><<I> </I>0.05), a reduced level of iNOS mRNA (<I>P </I><<I> </I>0.05) and more motor neurons in the ventral horn, versus control SCI and SCI plus exercise alone, with no effect on the other signaling molecules examined. This study shows that combined therapy with melatonin and exercise reduces the degree of secondary damage associated with SCI in rats and supports the possible use of melatonin in combination with exercise to reduce the side effects related to exercise-induced fatigue and impairment.</P>

        Raynaud syndrome is a medical condition that causes pain, numbness, and changes in skin color at the distal extremities. Raynaud syndrome can be subdivided into primary Raynaud's and secondary Raynaud's. The former is diagnosed when the cause is unknown and the latter is caused by an underlying condition, such as connective tissue diseases, injury, smoking, or certain medications. Both cancer chemotherapy and β-blockers are relatively common causes of Raynaud syndrome but there are no reports of its association with methimazole administration. The authors encountered a 43-year old woman with hyperthyroidism who developed digital ulcers associated with Raynaud syndrome after a methimazole treatment. Her digital ulcers and Raynaud syndrome were improved after methimazole was replaced with propylthiouracil and conventional therapy. This paper reports this case along with a review of the relevant literature. (J Rheum Dis 2018;25:203-206)

        N. 스콧 모마데이의 『새벽으로 지은 집』 : 에이벌의 구전 전통 ‘기억하기’를 통한 외상 회복

        This paper examines Momaday’s idea of the Native’s survival in House Made of Dawn, paying attention to his representations of Abel (a Pueblo Jemez, war veteran, traumatized protagonist who has suffered from his loss of cultural identity). In this paper, I will argue that the writer highly suggests the reading public to be fully aware of the significance of storytelling, by exemplifying the psycho-somatically battered protagonist’s ordeal and recovery through his ‘remembering’ of the Native’s culture (language, songs, and stories). This paper focuses on the events of Abel’s consecutive meetings with three storytellers (Angela, Benally, and Francisco) who all support him to ‘recall’ his hereditary songs and stories. First, Abel hears from Angela the legend of the ‘bear’ of the Kiowa tribe, which symbolizes vitality and valor. Next, when Abel lives a hard life in a white man-centered city, he meets Benally who remembers the Navajo traditions and encounters the Navajo s traditional song, Night Chant. With this Night Chant, Abel gets closer to tribal traditions and feels the importance of traditional worldview and consciousness. Returning to his hometown, Abel takes care of his dying grandfather Francisco. Francisco tells him various traditional stories from his youth and stresses the importance of staying connected to Navajo’s traditions. Accepting Francisco’s lesson and wisdom, Abel finally participates in Jemez Pueblo s traditional ritual of ‘dawn running’ and sings Night Chant. This experience helps him to heal his cultural trauma and restores his bond with the earth.

        Monte-Carlo 모의실험을 통한 부분 인구집단별 벤젠 및 PM 10 의노출 및 위해성 평가

        Objectives: The Korea Ministry of Environment regulates concentrations of hazardous air pollutants (HAPs) through Atmosphere Environmental Standards to protect public health from HAPs. However, simply determining the exceedance of HAP concentrations has several limitations and more comprehensive assessment is required. In addition, integrated risk assessment is needed considering exposure in all microenvironments, including outdoor as well as indoor environments. The purpose of this study was to assess the differences in risk by sub-population groups according to time-activity patterns and reported concentrations, as well as the lifetime risk for Koreans. Methods: In this study, we calculated time-weighted average exposure concentrations for benzene and PM 10 among preschool-age children, students, housewifes, workers, and the elderly using residential time and concentrations for indoor (house, school or workplace, other), outdoor, and transport by the meta-analysis method. The risk assessments were conducted by excess cancer risk and disease death risk using 1,000,000 Monte-Carlo simulations for probabilistic analysis. Results: Preschool-age children, students, housewifes, workers, and the elderly spent 91.9, 86.0, 79.8, 82.2, and 77.3% of their day in their house, workplace, or school, respectively. The more than 99% excess cancer risk for benzene exceed 1.0E-06 in all sub-populations and lifetime. The acute disease death risk for PM 10 for housewifes and workers for lifetime were 3.35E-04 and 3.18E-04, and chronic disease death risks were 2.84E-03 and 2.70E-03, respectively. Conclusions: The risk of benzene and PM 10 by sub-population group and for the lifetime of housewifes and workers were assessed. Benzene showed risky results for this study. All disease death risks of PM 10 were higher than 1.0E-04 and showed different risks by sub-population. This study can be used as a basis for lifetime exposure and risk assessment to benzene and PM 10 .

        상태기반 정비를 위한 초기 성능저하 시점(First Prediction Time, FPT) 추정 연구

        장비의 고장 및 성능의 저하를 방지하기 위해서는 시스템의 상태를 모니터링 하고 장비의 고장을 예측하는 것이 필요하다. 이는 상태기반정비(Condition Based Maintenance +, CBM+)라고 불리며, 이를 위해서는 기계 혹은 장비의 상태를 실시간으로 진단하고, 초기 고장 감지 및 열화 상태를 예측하는 예측유지관리(Prognostics and Health Management, PHM)가 필수적이다. 본 논문에서는 무기체계의 냉매 누출의 문제에서 CBM+를 구현하기 위한 초기 냉매 누출시점(First Prediction Time, FPT) 추정방법론을 제시한다. 이는 냉매 누출 상태인 건강상태지표(Health Indicator, HI)를 보다 효과적으로 표현하여, 잔존수명(Remaining Useful Lifetime, RUL) 예측 성능에 큰 기여를 할 수 있다. 제안하는 방법론의 효율성은 현재 운용중인 무기체계에서 수집된 데이터를 통해 검증되었으며, FPT을 반영한 RUL 예측 성능이 FPT를 반영하지 않은 경우에 비해 우수한 성능을 보였다. 본 연구 방법론은 유사한 특성을 가진 하위부품에 대한 CBM+ 적용에 유용한 가이드라인으로 활용될 수 있을 것으로 보인다. To prevent equipment malfunction and degradation of performance, it is imperative to monitor the system status and anticipate equipment failures. This approach is referred to as Condition Based Maintenance Plus (CBM+), necessitating the real-time diagnostic of machinery or equipment status and the predictive maintenance via Prognostics and Health Management (PHM) to predict initial failures and degradation states. This paper presents a methodology for estimating the First Prediction Time (FPT) to implement CBM+ in addressing the issue of refrigerant leakage in weapon systems. This entails representing the Health Indicator (HI) for refrigerant leakage status more effectively, thereby significantly contributing to the predictive performance of the Remaining Useful Lifetime (RUL). The efficacy of the proposed methodology has been validated through data collected from currently operating weapon systems and has demonstrated superior RUL prediction performance when incorporating FPT compared to without its incorporation. The research methodology proposed herein appears to be a viable guideline for the application of CBM+ to subcomponents with similar characteristics.

        NAMPT enzyme activity regulates catabolic gene expression in gingival fibroblasts during periodontitis

        <P>Periodontal disease is one of the most prevalent chronic disorders worldwide. It is accompanied by inflammation of the gingiva and destruction of periodontal tissues, leading to alveolar bone loss. Here, we focused on the role of adipokines, which are locally expressed by periodontal tissues, in the regulation of catabolic gene expression leading to periodontal inflammation. The expression of the nicotinamide phosphoribosyltransferase (NAMPT) adipokine was dramatically increased in inflamed human and mouse gingival tissues. NAMPT expression was also increased in lipopolysaccharide- and proinflammatory cytokine-stimulated primary cultured human gingival fibroblasts (GF). Adenovirus-mediated NAMPT (Ad-<I>Nampt</I>) overexpression upregulated the expression and activity of COX-2, MMP1 and MMP3 in human GF. The upregulation of IL-1β- or Ad-<I>Nampt</I>-induced catabolic factors was significantly abrogated by the intracellular NAMPT (iNAMPT) inhibitor, FK866 or by the sirtuin (SIRT) inhibitor, nicotinamide (NIC). Recombinant NAMPT protein or extracellular NAMPT (eNAMPT) inhibition using a blocking antibody did not alter NAMPT target gene expression levels. Moreover, intragingival Ad-<I>Nampt</I> injection mediated periodontitis-like phenotypes including alveolar bone loss in mice. SIRT2, a part of the SIRT family, was positively associated with NAMPT actions in human GF. Furthermore, <I>in vivo</I> inhibition of the NAMPT-NAD<SUP>+</SUP>-SIRT axis by NIC injection in mice ameliorated the periodontal inflammation and alveolar bone erosion caused by intragingival injection of Ad-<I>Nampt</I>. Our findings indicate that NAMPT is highly upregulated in human GF, while its enzymatic activity acts as a crucial mediator of periodontal inflammation and alveolar bone destruction via regulation of COX-2, MMP1, and MMP3 levels.</P>
