      • 만성 C형 간염 환자에서 페그인터페론 알파2a와 리바비린 병합 치료중 발생한 벨마비 1예

        페그인터페론과 리바비린 병합요법은 만성 C형 간염의 일차 치료법이다. 저자들은 만성 C형 간염 환자에서 페그인터페론 과 리바비린 병합 요법 중에 발생한 벨마비 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. 환자는 5년 전부터 만성 C형 간염을 앓아온 48세 남자이며, PEG-IFN α-2a 135μgm 피하주사 주1회와 하루 1200㎎의 리바비린을 투여하였다. 치료시작 후 9개월째 환자는 오른쪽 안면의 근력약화를 호소하였으며 벨마비로 진단되었다. 페그인터페론과 리바비린 병합요법을 지속하면서 관찰하였다. 환자의 벨마비는 페그인터페론 치료를 중단하지 않았음에도 3개월후 증상이 회복되고 이후 벨마비 재발 없이 현재 경과관찰 중이다. 만성 C형 간염에서 페그인터페론과 리바비린 병합 요법시 벨마비의 발생 가능성을 염두에 두어야 하겠다. A Case of Bell's Palsy Associated with Combination Therapy of Pegylated Interferon Alfa-2a (PEG-IFN) and Ribavirin for Chronic Hepatitis C Virus Infection Pegylated interferon alfa(PEG-IFN α) and ribavirin therapy is the first line treatment for chronic hepatitis C. Mild complications of the therapy are common, but more serious complications are rare. We report here a case of Bell's palsy that occurred in a patient with chronic hepatitis C virus infection during combination therapy of PEG-IFN α-2a and ribavirin. The patient was 49-year-old man with chronic hepatitis C (genotype 1b) for 8 years. He had compensated liver cirrhosis with splenomegaly. Therapy with PEG-IFN α- 2a 135mcg/week and ribavirin 1200mg/day was initiated. After 9 months of the therapy, the patient showed a loss of muscular tone on the right side of his face. A diagnosis of Bell's palsy was made. The Bell's palsy resolved over 3 months despite continuation of the combination therapy.

        저자들은 만성골수성백혈병으로 동종 골수이식과 이식편 부전으로 인하여 추가적인 말초혈액 조혈모세포이식을 시행 받은 환자에서 만성 이식편대숙주질환과 그 폐 합병증인 폐색성 세기관지염에 동반된 자발성 기종격동과 피하 기종이 병발한 1예를 경험하였기에 이를 보고하는 바이다. Obstructive lung disorders following after allogeneic bone marrow transplantation (BMT) in association with graft- versus-host disease (GVHD) contribute significant morbidity and mortality. We report a case of a 28-year-old man who developed spontaneous pneumomediatinum and subcutaneous emphysema complicating bronchiolitis obliterans after allogeneic BMT. He received an allogeneic BMT for chronic phase of chronic myeloid leukemia. Five months after BMT, he was boostered by allogeneic peripheral blood stem cells from the same donor due to graft failure. One month after the boostering, chronic GVHD developed and were treated with cyclosporine and steroid. The patients developed spontaneous pneumomediatinum and subcutaneous emphysema secondary to severe bronchiolitis obliterans 4 months after boostering donor cells. The air-leak syndromes were recovered by conservative management, including high-flow oxygen.

        Afferent loop syndrome is an uncommon complication of a gastrectomy and Billroth Ⅱ reconstruction. It may cause symtoms at any time from the first postoperative day to many years after the gastrectomy. Afferent loop syndrome is characterized by abdominal pain, vomiting and elevation of serum amylase. Thus, it is difficult to differentiate afferent loop syndrome from other cause of acute pancreatitis. However, the history of gastrectomy can be an important clue for diagnosing afferent loop syndrome. We experienced one case of chronic afferent loop syndrome with acute pancreatitis. After appropriate management, the abdominal pain disappeared and serum amylase level decreased. We report this case with a review of relevant literatures.

      • Luteimonas lutimaris sp. nov., isolated from a tidal flat.

        <P>A Gram-staining-negative, strictly aerobic bacterium, designated strain G3(T), was isolated from a tidal flat of the Taean coast in South Korea. Cells were moderately halotolerant and non-motile rods showing catalase- and oxidase-positive reactions. Growth of strain G3(T) was observed between 15 and 40 C (optimum 30 C) and between pH 5.5 and 9.0 (optimum pH 6.5-7.5). Strain G3(T) contained Q-8 as the predominant lipoquinone and iso-C(15 : 0), iso-C(17 : 1)ω9c, iso-C(16 : 0) and iso-C(11 : 0) as the major fatty acids. The G+C content of the genomic DNA was 69.6 mol%. Phylogenetic analyses based on 16S rRNA gene sequences showed that strain G3(T) formed a tight phylogenetic lineage with Luteimonas mephitis B1953/27.1(T) within the genus Luteimonas and was most closely related to L. mephitis B1953/27.1(T) with 98.0 % 16S rRNA gene sequence similarity. The DNA-DNA relatedness between strain G3(T) and L. mephitis B1953/27.1(T) was 35.2 3.3 %. On the basis of chemotaxonomic data and molecular properties, strain G3(T) represents a novel species of the genus Luteimonas, for which the name Luteimonas lutimaris sp. nov. is proposed. The type strain is G3(T) (= KACC 14929(T) = JCM 16916(T)).</P>

        Improved pre‐osteoblast response and mechanical compatibility of ultrafine‐grained Ti–13Nb–13Zr alloy

        <P><B>Abstract</B></P><P><B>Objective: </B> Metallic implantation materials having high yield strength, low elastic modulus, and non‐cytotoxic alloying elements would be advantageous for the long‐term stability of implants. This study assessed the surface and mechanical properties, and also <I>in vitro</I> osteoconductivity of ultrafine‐grained (UFG) Ti–13Nb–13Zr alloy produced by dynamic globularization without any severe deformation for future biomedical applications as an endosseous implant material.</P><P><B>Material and methods: </B> The surface characteristics and mechanical properties were investigated by orientation image microscopy, contact angle measurements, optical profilometry, and uniaxial tension tests. Mouse calvaria‐derived pre‐osteoblastic cell (MC3T3‐E1) attachment, spreading, viability, alkaline phosphatase (ALP) activity, and quantitative analysis of osteoblastic gene expression on UFG Ti–13Nb–13Zr alloy were compared with coarse‐grained (CG) Ti–13Nb–13Zr and CG Ti–6Al–4V alloys.</P><P><B>Results: </B> Dynamic globularized Ti–13Nb–13Zr alloy has an ultrafine grain size (0.3 μm) and an excellent combination of yield strength and elastic modulus compared with CG alloys, which displayed significantly lower water contact angles compared with CG alloys (<I>P</I><0.05). The UFG and CG Ti–13Nb–13Zr alloys displayed significantly increased cellular attachment compared with CG Ti–6Al–4V alloy (<I>P</I><0.05). The UFG Ti–13Nb–13Zr supported better cell spreading and more numerous focal adhesions. ALP activity (<I>P</I><0.05) and mRNA expressions of the osteoblast transcription factor genes (osterix, Runx2) and marker gene for osteoblast differentiation (osteocalcin) were markedly increased in cells grown on the UFG substrate compared with CG substrates at early incubation timepoints.</P><P><B>Conclusion: </B> Enhanced pre‐osteoblast response to UFG Ti–13Nb–13Zr substrate is attributable to the non‐cytotoxic alloying elements and the submicron scale grain size contributes to the superior surface hydrophilicity and abundant grain boundaries favorable for cell behavior. These findings indicate that dynamic globularized UFG Ti–13Nb–13Zr alloy is promising for load‐bearing endosseous implant material because of excellent mechanical and biological compatibilites.</P><P> <B>To cite this article:</B>

        Differences in Clinical and Laboratory Findings between Group D and Non-Group D Non-Typhoidal Salmonella Gastroenteritis in Children

        Purpose: To investigate the differences in clinical features and laboratory findings between group D and non-group D non-typhoidal Salmonella (NTS) gastroenteritis in children. Methods: A retrospective chart review of children diagnosed with NTS confirmed by culture study was performed. The clinical features and laboratory findings of group D and non-group D NTS were compared. Results: From 2003 to 2012, 75 cases were diagnosed as NTS at our center. The number of group D and non-group D patients was 45 and 30, respectively. The mean age was higher in group D than in non-group D patients (5.1 years vs. 3.4 years, p=0.038). Headaches were more frequently observed (p=0.046) and hematochezia was less frequently observed (p=0.017) in group D than in non-group D NTS gastroenteritis patients. A positive Widal test result was observed in 53.3% of group D and 6.7% of non-group D NTS cases (O-titer, p=0.030; H-titer, p=0.039). There were no differences in white blood cell counts, level of C-reactive protein and rate of antimicrobial resistance between group D and non-group D cases. Conclusion: The more severe clinical features such as headache, fever, and higher Widal titers were found to be indicative of group D NTS gastroenteritis. Additionally, group D NTS gastroenteritis was more commonly found in older patients. Therefore, old age, fever, headache, and a positive Widal test are more indicative of group D NTS than non-group D NTS gastroenteritis. Pathophysiological mechanisms may differ across serologic groups.

        통섭의 인문학을 접목한 정신교육 콘텐츠 개발 방향

        군대가 전쟁에서 승리하고, 군인들이 병영생활을 잘 하려면, 무엇보다 중요한 것이 정신전력의 강화이다. 정신전력의 강화를 위해서, 인문학 교육이 최신 과학기술과 통섭하는(부합하는) 교양교육이어야 한다. 군인들이 굳건한 신념을 가지려면 뜻밖의 상황에서도 흔들리지 않을 배경믿음을 가져야 하며, 위기의 상황에서 현명한 판단을 할 수 있으려면 신뢰할만한 배경지식을 풍부히 가져야 하기 때문이다. 서양의 지성사를 돌아보면, 인문학인 철학이 과학과 상호영향을 주고받으며 발전해온 것을 보여준다. 고대 철학자 플라톤과 아리스토텔레스가 그러했고, 근대의 철학자 칸트가 그러했으며, 현대 철학자 콰인도 그러했다. 그리고 생존하는 철학자 처칠랜드 부부는 최신 뇌과학과 인공지능의 연구에 기초하여 전통 철학적 의문들에 대답하는 신경철학이란 분야를 개척하였다. 그러한 신경철학 연구에 따르면, 뇌는 다양한 정보들을 서로 비교하고 수정할 수 있는 신경회로의 복잡한 네트워크를 가진다. 그러므로 독립적 영역처럼 보이는 정보들이 뇌 내부에서 상호 영향을 주고받으며, 수정 및 교체될 수 있다. 뇌 내부에서 자연과학 정보와 인문학 정보는 상호 간섭하며, 따라서 인문학은 최신의 자연과학과 통섭될 수 있다. 그렇게 통섭된 인문학에 의한 소양교육은, 장병들이 현명한 판단을 할 수 있을 굳은 믿음체계를 제공할 것이며, 쉽게 흔들리지 않을 확고한 가치관 및 신념을 갖게 해줄 것이다. Mental aptitude and conviction are critical components of a successful military. Education in humanities plays an integral role with emphasis placed on “consilience.”In order for military members to make wise judgements, soldiers must have a resolute knowledge of liberal arts. Strong character and self-confidence enables soldiers to maintain composure during the unpredictable situations of war. Reinforcement of faith, either religious or nationalistic, is attainable through liberal arts education with a general focus on recent scientific and technological progression. Looking back upon the history of western intelligence, we can observe that the philosophy of humanities have developed when science and technology interact. Examples of this happening are illustrated throughout history in the works of ancient philosophers Plato and Aristotle, modern philosopher Emmanuel Kant, contemporary philosopher Willard Quine, and recent philosophers Paul and Patricia Churchland. The Churchlands’ pioneering neurophilosophical research answers questions of traditional philosophy on the basis of brain science and connectionist AI studies. The information of diverse domains seems independent of each other; however, from the viewpoint of neurophilosophy, brains have networks of complex neural circuits comparing the diverse information of other areas and revising each other. The information of diverse domains can be consilient among them. Therefore, the humanities can be consistent with recent sciences and technology. This paper explains how liberal art education of the consilient humanities can enable the members of military forces to more consistently make wise judgements based upon concrete frameworks and attain confidence through knowledge.

        Monascus ruber의 액체배양을 통한 색소 생산 및 고체발효를 통한 홍국쌀의 monacolin K 생산 특성

        The growth characteristics and production of color pigments by Monascus strains were investigated during liquid culture, and production of monacolin K in red mold rice was carried out by solid state fermentation. Four different Monascus ruber strains were cultured in potato dextrose yeast extract broth (PDYB) media at 25℃ for 15 days. The high producing strain for red pigment was not corresponded to the strain for yellow pigment. Production of red pigment was high in the strain causing the fast pH change in culture broth. Production of monacolin K in red mold rice by solid state fermentation was influenced by a combination of wet cell weight and spore density in inoculum by liquid culture. Most strains showed the high production of monacolin K in red mold rice, when submerged fermentation was carried out for 5 days as inoculum for solid state fermentation. These results suggest that submerged fermentation period of inoculum have an effect on the production of monacolin K in red mold rice by solid state fermentation, and monacolin K in red mold rice could be increased by controlling the condition of submerged fermentation for inoculum.
