        The nationally-governed examinations for certification of allied health professions in Korea have been continued for thirty three years. During that time, there were a lot of managerial improvements in carrying out the examinations, for example, the looking-over the papers converted from manual method to computerized one. Nevertheless, the overall aspects of item management in the national examinations are still remained as obsolete style. In some developed countries they have already tried or adopted computerized system in making questions, executing item analysis, developing item banking, and in overall management of examinations, looking over papers for their national licensure examinations, and have established good reputations. Since the National Health Personnel Licensing Examination Board was established in 1998, now we can expect there would be a lots of improvements in the managerial systems and organizational structures associated with the national licensing examinations suggesting followings. 1. The contents and scope of the licensing examinations based on job analysis should be announced publicaly. 2. Items should be developed based on the scopes and contents of job characteristics. 3. All the developed items for the licensing examinations should be tested quantitatively prior to banking items. 4. All the parameters of the stored items should be fully estimated through item response theory. 5. All the efforts should be given in order to execute the examinations in individual residential areas on behalf of every examinee. 6. To the licensing examination system the Computerized Adaptive Testing system should be introduced in order to enhance the efficiencies. 7. Security enhancement on the stored items should be reminded in order to prevent leaking out the banking items. 8. Much more improvements should be made for the proper job evaluations. 9. Every efforts should be given to prevent cheatings encountable during the examinations. 10. Make the best of professional volunteers from the various fields in the execution of licensing examinations. 11. Consider a new system in that examinations can be executed twice or more in a year. 12. The current methods of presiding over the examinations should be replaced by a more reasonable one. 13. The results of the examinations should be announced as soon as possible in order not to prolong the examinees' unemployment periods. 14. The National Health Personnel Licensing Examination Board should try to rationalize the management keeping step with the information-oriented society.

      • Effect of rotational speed on rapid separation of primary silicon from an Al-Si alloy by novel centrifugation

        The optimum rotational speed (RS) of a novel centrifugation method for obtaining high-purity Si separated from Al-Si melts was evaluated numerically and experimentally. Numerical calculations showed that the separation efficiency (Si to Al ratio in the separated solid) increases and finally reaches a plateau at a high RS. The calculated relationship between RS and the separation efficiency was consistent with experimentally measured data except for the removal of primary Si crystallites, which happened at centrifugation at 800 revolutions per minute (RPM). Based on the acquired data, it was established that 700RPM is the optimal RS for our centrifugation apparatus (developed in a previous study) to maximize the separation efficiency.

        During the autumn of 1990 , we had investigated the clinical featuresw and followed the change in the direct immunofluorescent antibody(IFA) titers in 28 children with scrub typhus who resided in or near Chinju, Korea. All the children except one were diagnosed by the peak IFA titers of 1:160 or higher. The results were as follows: 1) The clinical features of our children were not different, to a large degree, from those of adults ever described in Korea literatures. But the frequence rates of skin rash, eschar and conjunctival injection were higher, those of proteinuria and hematuria were lower than those in adults and the characters of rash in our children were also quite different(faces were frequently the first site of eruption and even palms and soles were involved occasionally, initial bright red color of rash was changed into brown color during the acute phase). 2) IFA titers rose rapidly during the 2nd or 3rd week of illness and then formed plateau. When we suggest IFA titers of 1:180 or higher as the diagnostic criteria, the seropositivity of our children was 5/7(75%) in the 1st week and over 95% in the 2nd through 5th week of illness, and suggest IFA titers of 1:160 or higher,then the seropositivityin the 1st, 2nd and 3rd through 5th week were 3/7(43%), 23/31(74%) and over 95%,respectively. 3) IFA titers in 6-8 months after disease onset were 1:80(13 children) or 1:20( 7 children), and in one and half year (our longest follow-up period), 1:20 (3 children) showing that IFA lasts longer than one and half year after sickness. 4) From the above data (3) and the fact that majority of cases of scrub typhus occur in the autumn season in Korea, IFA titers of 1:40-1:160 should be used as the diagnostic criteria in order to distinguish between the present and past infections.

        Osteomalacia is characterized by the accumulation of an increased amount of unmineralized bone matrix. Of the several possible causes, hypophosphatemia is one of the major and mainly results from intestinal malabsorption of phosphate or from renal tubular phosphate loss. Oral phosphate supplementation is the mainstay of therapy for hypophosphatemic osteomalacia of diverse causes, but secondary hyperparathyroidism may be induced by such treatment. Although concomitant therapy with vitamin D is recommended for the prevention of this complication, it can nevertheless occur and sometimes results in tertiary hyperparathyroidism. We report the development of tertiary hyperparathyroidism in a patient with sporadic non-familial hypophosphatemic osteomalacia who had been treated with long-term phosphate therapy despite undergoing concomitant vitamin D therapy

        pH-Responsive globular poly(ethylene glycol) for photodynamic tumor therapy

        <P>In this study, we report the development of extremely small-sized globular poly(ethylene glycol) (gPEG) that can specifically recognize tumor acidic pH. gPEG coupled with chlorin e6 (Ce6, a photosensitizing drug) and 2,3-dimethylmaleic acid (DMA, as a pH-responsive moiety) (gPEG-Ce6-DMA, particle size: 3-4 nm in diameter) was easily dispersed in phosphate buffered saline (PBS) without any of the nanoparticle fabrication steps. We observed that gPEG-Ce6-DMA displayed pH-dependent zeta-potential changes due to coupling (at pH 7.4) or decoupling (at pH 6.8-6.0) of DMA. As a result, the uptake of gPEG-Ce6-DMA was significantly increased in tumors at acidic pH, likely due to the decoupling of DMA (backing cationic primary amines). As a result, the preferential cellular uptake of gPEG-Ce6-DMA at acidic pH allowed for a significant enhancement of in vitro/in vivo photodynamic tumor cell ablation under light illumination.         Cationic drug-based self-assembled polyelectrolyte complex micelles: Physicochemical, pharmacokinetic, and anticancer activity analysis

        <P>Nanofabrication of polymeric micelles through self-assembly of an ionic block copolymer and oppositely charged small molecules has recently emerged as a promising method of formulating delivery systems. The present study therefore aimed to investigate the interaction of cationic drugs doxorubicin (DOX) and mitoxantrone (MTX) with the anionic block polymer poly(ethylene oxide)-block-poly(acrylic acid) (PEO-b-PAA) and to study the influence of these interactions on the pharmacokinetic stability and antitumor potential of the formulated micelles in clinically relevant animal models. To this end, individual DOX and MTX-loaded polyelectrolyte complex micelles (PCM) were prepared, and their physicochemical properties and pH-responsive release profiles were studied. MTX-PCM and DOX-PCM exhibited a different release profile under all pH conditions tested. MTX-PCM exhibited a monophasic release profile with no initial burst, while DOX-PCM exhibited a biphasic release. DOX-PCM showed a higher cellular uptake than that shown by MTX-PCM in A-549 cancer cells. Furthermore, DOX-PCM induced higher apoptosis of cancer cells than that induced by MTX-PCM. Importantly, both MTX-PCM and DOX-PCM showed prolonged blood circulation. MTX-PCM improved the AUC(all) of MTX 4-fold compared to a 3-fold increase by DOX-PCM for DOX. While a definite difference in blood circulation was observed between MTX-PCM and DOX-PCM in the pharmacokinetic study, both MTX-PCM and DOX-PCM suppressed tumor growth to the same level as the respective free drugs, indicating the potential of PEGylated polymeric micelles as effective delivery systems. Taken together, our results show that the nature of interactions of cationic drugs with the polyionic copolymer can have a tremendous influence on the biological performance of a delivery system.         An albumin nanocomplex-based endosomal pH-activatable on/off probe system

        <P>Albumin has gained considerable interest as a material for fabricating nanoparticulate systems due to its biomedical advantages, such as biocompatibility and chemical functionality. Here, we report a new pH-sensitive albumin nanocomplex prototype with a zinc-imidazole coordination bond. Albumin was conjugated with 1-(3-aminopropyl)imidazole and mPEG(10kDa)-NHS, and the resulting albumin conjugate (PBI) was then modified with either Cy5.5 or BHQ-3. The newly formed albumin nanocomplex (C/BQ-PBI Zn NCs: similar to 116 nm) system was facilely self-assembled around pH 7.4 in the presence of Zn2+, but it quickly disassembled in an acidic environment (similar to pH 5.0). Based on this pH-sensitivity, C/BQ-PBI Zn NCs emitted strong near-infrared fluorescence and released Zn2+, turning 'off' at pH similar to 7.4 (e.g., plasma) and 'on' at pH similar to 5.0 (e.g., endo/lysosomes in tumor cells) on account of fluorescence resonance energy transfer. C/BQ-PBI Zn NCs displayed significant cytotoxicity due to an increase in cellular. Zn2+ in response to endosomal pH (similar to 5.0) in breast cancer MCF-7 cells and lung adenocarcinoma A549 cells. Particularly, confocal laser scanning microscopic images showed a strong fluorescence signal caused by the disassembly of C/BQ-PBI Zn NCs in the endosomal region of MCF-7 cells. Based on these results, we believe that this albumin nanocomplex is an attractive biocompatible tumor targeting probe carrier for the theranostic purpose. (C) 2016 Elsevier B.V.
