        Multimorbidity-adjusted YLD rates calculated through Monte-Carlo simulation

        OBJECTIVES: To efficiently utilize limited health and medical resources, it is necessary to accurately measure the level of health, which requires estimating the multimorbidity-corrected burden of disease. METHODS: This study used 2015 and 2016 data from the National Health Insurance Service, and employed the list of diseases defined in a Korean study of the burden of disease, the criteria for prevalence, and the “cause–sequelae–health state” disease system. When calculating the years lost to disability (YLD), multimorbidity was corrected using Monte-Carlo simulation. RESULTS: Correcting for multimorbidity changed YLD at all ages in Korea by -1.2% (95% CI, -24.1 to 3.6) in men and -12.4% (95% CI, -23.0 to 0.3) in women in 2015, and by -10.8% (95% CI, -24.1 to 4.6) in men and -11.1% (95% CI, -22.8 to 1.7) in women in 2016. The YLD rate for non-communicable diseases in men decreased more than that of other disease groups in both years, by -11.8% (95% CI, -19.5 to 3.6) and -11.5% (95% CI, -19.3 to -3.0), respectively. The overall YLD rate changed by -1.3% in the 5-year to 9-year age group, and the magnitude of this change remained similar until the 10- to 19-year age group, gradually decreased after 20 years of age, and steeply increased to more than 10% in those aged 60 and older. CONCLUSIONS: Calculations of YLD should adjust for multimorbidity, as the disease burden can otherwise be overestimated for the elderly, who tend to exhibit a high prevalence of multimorbidity

        Corrective Feedback Effects in University English Writing Classes

        This paper investigates the effects of corrective feedback on writing in beginner-level university English writing classes. The research questions were 1) What were the effects of the class on writing? 2) Which errors improved the most and the least? To answer these questions, a test consisting of 15 questions as well as free writing were conducted as pre-tests and post-tests at the beginning and end of the class, respectively. Forty six college students were the participants in this study. The findings are as follows: First, a writing class based on direct and indirect corrective feedback provided by the teacher and peers can be effective in reducing students' grammatical errors and improving their free writing ability; Second, among six grammatical categories (article and determiner, sentence structure, verb, preposition, singular and plural, and subject-verb agreement), subject-verb agreement errors improved the most, while singular and plural the least; Third, corrective feedback on the sentence level contributed to developing free writing ability, especially in the low and intermediate level students. The results imply that reflecting the importance of corrective feedback, it would be desirable to design small class sizes for writing courses.

        Developmental Research of a Collaborative Learning Support Tool

        This study used a developmental research method to systematically apply knowledge building and software design principles and scaffolding strategies to support the learners complex problem-solving process in a collaborative online learning tool. The purpose of developing the tool was to investigate how a scaffolding strategy based on a knowledge building framework could be used to induce productive interactions in sharing activities required different types of knowledge and skills. Development data were collected from six phases using both formative and summative evaluation methods. The final prototype can effectively promote learner interaction, from convergence and divergence to reflective interactions, as required in a collaborative problem-solving process. This study suggests the importance of applying a systematic methodological approach with a theoretical framework for developing collaborative learning support tools.

        Well-being Index Scores and Subjective Health Status of Korean Healthcare Workers

        Objectives: The aim of this study was to evaluate the subjective level of health-related quality of life of Korean healthcare workers using various quality-of-life instruments. Methods: This study included 992 participants, who were doctors and nurses. A survey was conducted between November 28 and December 4, 2019. Data from 954 participants divided into 3 groups (physicians, residents, and nurses) were analyzed. Four measurement tools (29 questions) were used in the survey to evaluate subjective health status and well-being. Results: In the Mayo Well-being Index, burnout during work (88.5%) and emotional difficulties caused by work (84.0%) were frequently cited by the respondents. Regarding questions on burnout and emotional difficulties, residents and nurses had the highest scores (91.0 and 89.6%, respectively). Emotional problems, such as anxiety, depression, and irritability, accounted for a high percentage (73.1%) of the total, while 82.2% of respondents reported that their work schedules interfered with their leisure and family time. There was no significant difference among the groups in subjective health status. However, 10.1% of the residents experienced very low quality of life, which was a higher proportion than that of physicians (2.7%) and nurses (5.2%). Conclusions: The level of well-being that Korean medical workers experienced in relation to work was lower than the results of the United States healthcare workers surveyed using the same tool. This study was unique in that it conducted a subjective quality-of-life survey on Korean healthcare workers.

        사티어모델 기반 의사소통 교육프로그램이 공공병원 간호사의 의사소통 능력, 정서지능, 우울 및 행복에 미치는 효과

        Purpose: This study was conducted to identify the effects of Satir"s model based communication education on communication competence, emotional intelligence and depression-happiness in regional public hospital nurses. Methods: A non-equivalent control group pretest-posttest quasi-experimental design was used. The subjects were 36 nurses of the experimental group and 36 nurses of the control group. Satir"s model based communication education was conducted for two days and the dependent variables were measured using the global interpersonal communication competence scale (GICC), emotional intelligence scale and short depression-happiness scale (SDHS). Data were analyzed by the Chi-squared test, Fisher"s exact test and ANCOVA using SPSS 21.0. Results: Communication competence (F=12.7, p<.001) was significantly increased in the experimental group, as was emotional intelligence (F=6.67, p=.01). Conclusion: The results of the study suggest that Satir"s model based communication education is an effective method to increase public hospital nurses" communication competence and emotional intelligence.

        컴퓨터기반 협력학습에서 공유지식 형성을 위한 표상도구설계

        컴퓨터기반 협력학습도구를 사용하여 한 공간에서 다양한 관점을 가진 사람들이 토의하고자 할 때,작성된 글이 과제내용 중 어느 부분에 해당하는 것인지를 파악하는 것이 어렵고 서로의 지식과 의견을 공유하는데 어려움이 따른다. 본 연구에서는 컴퓨터 기반 협력학습에서 공유지식 형성을 방해하는 요인을 문헌연구를 통해 분석하고 도출된 원리를 기반으로 협력표상도구를 설계하였다. 설계된 도구는 평가 준거에 따른 체크리스트와 F.G.I를 통해 교수자, 설계자, 학습자의 다양한 의견을 수렴함으로써 반복 조정되었다. 최종 조정된 도구는 복합 과제를 해결해야하는 컴퓨터 기반 협력학습상황에서 학습자 간 지식 및 의견을 공유하는 데 방해요소를 최소화하여 협의를 촉진하고 고차원의 해결책을 도출하는 데 기여할 것이라 기대한다. When using computer-supported collaborative learning(CSCL) tool where learners with different perspective are discussing in the same place, it is difficult to catch connection between specific position of learning content and written message by learners, share each other knowledge and opinion. This study investigated the phenomena responsible for causing "Un-shared knowledge" by literature research and designed collaborative representation tool based on described principle. A version of the Mediated annotation tool was developed for each problem identified and was subsequently tested by software evaluation standard and F.G.I. Final mediated tool will contribute to overcome knowledge sharing barriers, facilitate communication among students and describe high quality solution.

        공공보건의료인력 임상교육효과 평가: 지역거점공공병원 간호사 대상

        Background: The purpose of this study is to evaluate the outcomes of clinical education program for nurses in regional public hospital, utilizing the Kirkpatrick`s model. Methods: Kirkpatrik`s 4-level model was applied to this study. Trainees were asked to fill out questionnaires in the middle and at the end of the program. Also administrators of excellent trainees were asked to fill out the questionnaires regarding nursing management performance after 1-2 months from the end of the training course. Results: All trainees had positive reactions to the clinical education program. Not only the results of individual level (satisfaction and achievement scores, academic achievement scores, practical application rate, and educational transition factors) but also the scores of organization level (nursing management performance scores) are improved. Conclusion: By showing a correlation between the effectiveness factors we need to verify the relationship between these factors in a future study. In addition, development of quantitative and qualitative performance indicators are needed. To establish a long-term education system, it is required to applying the excellent trainee`s successful experiences.

      • Three-Dimensional Electroconductive Hyaluronic Acid Hydrogels Incorporated with Carbon Nanotubes and Polypyrrole by Catechol-Mediated Dispersion Enhance Neurogenesis of Human Neural Stem Cells

        <P>Electrically conductive hyaluronic acid (HA) hydrogels incorporated with single-walled carbon nanotubes (CNTs) and/or polypyrrole (PPy) were developed to promote differentiation of human neural stem/progenitor cells (hNSPCs). The CNT and PPy nanocomposites, which do not easily disperse in aqueous phases, dispersed well and were efficiently incorporated into catechol-functionalized HA (HA CA) hydrogels by the oxidative catechol chemistry used for hydrogel cross-linking. The prepared electroconductive HA hydrogels provided dynamic, electrically conductive threedimensional (3D) extracellular matrix environments that were biocompatible with hNSPCs. The HA CA hydrogels containing CNT and/or PPy significantly promoted neuronal differentiation of human fetal neural stem cells (hfNSCs) and human induced pluripotent stem cell-derived neural progenitor cells (hiPSC-NPCs) with improved electrophysiological functionality when compared to differentiation of these cells in a bare HA CA hydrogel without electroconductive motifs. Calcium channel expression was upregulated, depolarization was activated, and intracellular calcium influx was increased in hNSPCs that were differentiated in 3D electroconductive HA CA hydrogels; these data suggest a potential mechanism for stem cell neurogenesis. Overall, our bioinspired, electroconductive HA hydrogels provide a promising cell-culture platform and tissue-engineering scaffold to improve neuronal regeneration.</P>

        Cordyceps militaris Enhances MHC-restricted Antigen Presentation via the Induced Expression of MHC Molecules and Production of Cytokines

        Background: Cordyceps militarys water extract (CME) has been reported to exert antitumor and immunomodulatory activities in vivo and in vitro. However, the therapeutic mechanism has not yet been elucidated. In this study, we examined the effects of CME on the antigen presenting function of antigen presenting cells (APCs). Methods: Dendritic cells (DCs) were cultured in the presence of CME, and then allowed to phagocytose microspheres containing ovalbumin (OVA). After washing and fixing the efficacy of OVA, peptide presentation by DCs were evaluated using CD8 and CD4 T cells. Also, we confirmed the protein levels of proinflammatory cytokines through western blot analysis. Results: CME enhanced both MHC class I and class II-restricted presentation of OVA in DCs. In addition, the expression of both MHC class I and II molecules was enhanced, but there was no changes in the phagocytic activity of exogenous OVA. Furthermore, CME induced the protein levels of iNOS, COX-2, proinflammatory cytokines, and nuclear p65 in a concentration-dependent manner, as determined by western blot. Conclusion: These results provide an understanding of the mechanism of the immuno-enhancing activity of CME on the induction of MHC-restricted antigen presentation in relation to their actions on APCs.
