        만성 두부외상 환자에서 99mTc - HMPAO Brain SPECT의 임상적 유용성

        N/A Minima1 deterioration of cerebral perfusion or microanatomical changes were undetectable on conventional Brain CT or MRI. So evaluation of focal functional changes of the brain parenchyme is essential in chronic head injury patients, who did not show focal anatomical changes on these radiological studies. However, the patients who had longstanding neurologic sequelae following head injury, there had been no available imaging modalities for evaluating these patients precisely. Therefore we tried to detect the focal functional changes on the brain parenchyme using Tc-99m-HMPAO Brain SPECT on the patients of chronic head injuries. Twenty three patients who had suffered from headache, memory dysfunction, personality change and insomnia lasting more than six months following head injury were included in our cases, which showed no anatomical abnormalities on Brain CT or MRI. At first they underwent psychological test whether the symptoms were organic or not. Also we were able to evaluate the cerebral perfusion changes with Tc-99m-HMPAO Brain SPECT in 22 patients among the 23, which five patients were focal and 17 patients were nonfocally diffuse perfusion changes. Thus we can predict the perfusion changes such as local vascular deterioration or functional defects using Tc-99m-HMPAO Brain SPECT in the patients who had suffered from post-traumatic sequelae, which changes were undetectable on Brain CT or MRI.

        To detect the numerical changes of the chromosome 13 and 17, containing tumor suppressor genes (retinoblastoma gene and p53) in normal tissue, intraepithelial neoplasm and carcinoma of the prostate, the Fluorescence In Situ Hybridization technique with paraffin embedded tissue was done. Materials observed in this study consisted of 14 cases of lacalized carcinoma(TB1NOMO) and 1 case of locally infiltrating cancer(TC1NOMO)> The chromosomal number changes were counted at normal tissue, intraepithelial neoplasm and neoplastic gland at the same radical prostatectomy specimen. From the 4 patients, studied prospectively, fresh cancer tissue and peripheral whole blood were collected for the primary culture of the cancer cells and lymphocytic culture. There cultured cells were used fro karyotyping. The results were as follows : 1. There were no significant chromosomal number changes between 13 and 17 in each group. 2. The normal non-neoplastice duct, intraepithelial neoplasm and neoplastic gland of experimental group showed no significant difference in chromosomal change. 3. There were significant changes more than 30% in chromosomal numbers between control Y chromosome and experimental chromosome 13 and 17 in each group. 4. The karyotyping on primary cultured prostate cancer cells and lymphocyte showed significant difference in chromosomal number in spite of the same host, representing local chromosomal changes.

        In diabetic patients who were being treated with hemodialysis, coma and other neurological deficits did not occur in spite of extremely elevated serum glucose levels. In this report, we compared diabetic patients with renal failure in severe hyperglycemia with nonketotic hyperosmolar coma patients with normal renal function to know what affect mental changes. Mental changes were not present in diabetic patients with renal failure. These patients with renal failure showed more severe hyperglycemia, but corrected serum sodium concentration and calculated effective serum osmolality were low. So it is suggested that corrected serum sodium concentration and effective serum osmolality are more important factor affecting mental changes than high blood glucose levels in diabetic patients with renal failure, due to absence of osmotic diuresis.

      • Age-Associated Changes in the Vascular Renin-Angiotensin System in Mice

        <P><I>Background</I>. This study evaluated whether the change in the renin-angiotensin system (RAS) is associated with arterial aging in mice.<I> Methods</I>. Histologic changes and expressions of transforming growth factor-<I>β</I> (TGF-<I>β</I>), collagen IV,<I> fibronectin</I>, angiotensin II (Ang II), angiotensin-converting enzyme (ACE), angiotensin-converting enzyme 2 (ACE2), angiotensin II type 1 receptor (AT1R), angiotensin II type 2 receptor (AT2R), prorenin receptor (PRR), Mas receptor (MasR), endothelial nitric oxide synthase (eNOS), NADPH oxidase 2 and oxidase 4 (Nox2 and Nox4), 8-hydroxy-2′-deoxyguanosine (8-OHdG),<I> 3-nitrotyrosine</I>, and superoxide dismutase 1 and dismutase 2 (SOD1 and SOD2) were measured in the thoracic aortas from 2-month-old, 12-month-old, and 24-month-old C57/BL6 mice.<I> Results</I>. Twenty-four-month-old mice showed significantly increased aortic media thickness and expressions of TGF-<I>β</I>, collagen IV, and fibronectin, compared to 2-month-old and 12-month-old mice. The expressions of PRR, ACE, and Ang II, and AT1R-positive area significantly increased, whereas expressions of ACE2 and MasR and AT2R-positive area decreased with age. The expressions of phosphorylated serine<SUP>1177</SUP>-eNOS, SOD1, and SOD2 decreased, and the 8-OHdG-positive area and the 3-nitrotyrosine-positive area increased with age. The expression of Nox2 significantly increased with age, but that of Nox4 did not change.<I> Conclusions</I>. The enhanced PRR-ACE-Ang II-AT1R axis and reduced ACE2-MasR axis were associated with arterial aging in mice. </P>

      • 점증적 최대부하 운동이 호흡순환계 및 젖산변화에 미치는 영향

        This study was to investigate a cardiorespiratory response and the chang of blood lactate concentration between untrained men and judo fellows after maximal exercise. The result of this study was as follows: All of the variables(HR, VE, and VO2max) were non significant as well as RER in a cardiorespiratory response. In addition, both between groups and interaction were non significant the change of a blood lactate concentration. However, not only VO2 was sigpificant(p<.05) in the comparison of between groups in a cardiorespiratory response but the chang blood lactate concentration was significant(p<.01) in measuring time. As a result of this study, the study of a cardiorespiratory variables and the reduction of blood lactate concentration related to recovery on fourties subjects after maximal exercise didn't get particular result but it is showed the condition of health of middle aged men indirectly as well as the difference of between untrained men and judo fellows. Therefore, it is showed that both untrained men and judo fellows need a exorcise, which is suggested a objective and scientific the method and the need of motivation for health.

        Numerical analysis to determine the performance of combined variable ceiling and floor-based air distribution systems in an office room

        <P>This study analyses the performance characteristics of combined variable ceiling and floor-based air distribution systems in an office room using a numerical method to optimize the level of thermal comfort and indoor air quality. To validate the computational fluid dynamics simulations, temperature, air velocity and CO<SUB>2</SUB> concentration were measured and compared. The simulation results of various combinations of ceiling and floor-based air distribution systems show that the exhaust should be located above the occupant and the window in summer; due to the large thermal plume as heat sources that could be generated by the window and the occupant. Whereas in winter, the supply should be located on the ceiling far from the window and the exhaust should be positioned on the floor near the occupant; as the downward airflow around the cold window surface would become more dominant when compared to the upward airflow associated with any other heat sources. The results have indicated that having both ceiling and floor-based air conditioning systems would allow the flexibility to change supply and exhaust air diffuser locations to optimize thermal and ventilation performance more efficiently in countries that are subject to distinct seasonal changes.</P>

        전기통신금융사기의 방지를 위한 법정책적 고찰

        Telecommunications-based financial fraud has caused tremendous damage to the financial life of Korea since the early 2000s. The initial method of crime of telecommunications-based financial fraud criminals was voice Phishing, but later developed into Pharming, Memory Hacking and Smishing. Recently, it has become difficult to commit such crimes because of the government’s and financial institutions’ efforts to promote crime prevention. Scammers have developed their own techniques and used Qshing and the Obscene Video Chat Phishing, in which the criminal seduces others to find their flaws and exploit them to steal money. According to the National Police Agency statistics, a total of 6,227 telecommunication-based financial fraud (Phishing, Pharming, Smishing, Memory Hacking, and Obscene Video Chat Phishing) incidents occurred solely in the year of 2016, and the total amount of financial damage was more than 27.4 billion won. However, considering the cases of undeclared crimes, such as incidents or financial damage that are not reported in the statistics of the National Police Agency, it is estimated that the extent of damage will be much greater than stated by the police statistics. The Financial Services Commission, the National Police Agency, and the Financial Company have continually worked to minimize these damages. The government and the National Assembly have adopted 「Digital Signature Act」, 「Electronic Financial Transaction Act」, 「Act on the Promotion of Information and Communications Network Utilization and Information Protection, etc」, 「Personal Information Protection Act」, and 「Special Act on the Prevention of Losses Caused by Telecommunications-Based Financial Fraud and Refund for Loss」. They have tried to prevent telecommunications-based financial fraud by utilizing these legal safeguards. The police have operated 112 centers to try to help victims of voice phishing. Financial institutions and companies have strengthened the security regarding the issuance and reissuance of public key certificates. They used a system that delayed ATM withdrawals for a certain amount of time. They also tried to prevent the occurrence of borrowed or stolen accounts that are being exploited as a major means of committing telecommunications-based financial fraud. Despite these efforts, telecommunications-based financial fraud has not been blocked, and the damage still remains. Therefore, fundamental examination and revision of the countermeasures against telecommunications-based financial fraud is necessary. The IP address tampering through borrowed or stolen accounts or VPNs should be continuously regulated to prevent telecommunications-based financial frauds. The laws related to telecommunication-based financial fraud should be revised to establish systematic regulatory bases. In addition, it is necessary to create a system that can detect the occurrence of such crime in advance. And it is necessary to create a unified organization responsible for telecommunication-based financial fraud that can effectively carry out such fraud prevention tasks. As a legal device to aid the victims in the aftermath, it is also necessary to create funds for the victim`s relief and to activate insurance systems. These policies are not perfect countermeasures against telecommunications-based financial fraud. This is because, based on how the methods of telecommunications-based financial fraud has evolved over the past decade, no one can predict how such methods will evolve and how the degree of damage will change.

        출산에 따른 그레이브스병의 재발 및 관련이자

        Background: Pregnancy affects the course of Graves' Disease (GD), and patients who initially maintain euthyroid function into their middle trimester with minimum doses of antithyroid drugs become exacerbated after delivery. Even patients who are completely cured, requiring no treatment during pregnancy, can relapse after delivery. In this study, we examined the postpartum changes in the thyroid functions of patients with GD, and attempted to determine the factors contributing to these changes. Methods: The study subjects were recruited from pregnant women visiting our outpatient clinic for routine prenatal evaluations. 45 women previously diagnosed with GD, who had been treated and cured with hyperthyroidism, and were no longer taking any thyroid medications, were evaluated for 1 year post delivery. Results: Among 45 patients, 20 (44.4%) developed thyroid disorders following delivery. Postpartum thyroiditis (PPT) developed in 8 patients (17.8%), and GD developed in 12 (26.0%). The onset of the PPT disease 3.1 ± 1.4 months following delivery, which was significantly earlier than the 6.7 ± 2.7 months required for the post delivery onset of GD (p=0.003). The TBII values, measured during the thyrotoxic state in each womaen, were negative in women with PPT and positive in 71.4% of women with GD (p=0.030). The duration of treatment for hyperthyroidism prior or pregnancy, the number of recurrences, and the time interval without treatment, were not associated with the development of postpartum thyroid disorders. Whereas, the mean number of past pregnancies for women who developed PPT was 3.9 ± 2.1, and was significantly higher than the 2.2 ± 1.7 for women developing no thyroid dysfunctions (p=0.044). In 13 women their initial onset of GD occurred within one year postpartum, 7 (53.8%) having had a recurrence, which was significantly higher than in women whose disease onset occurred unrelated to delivery (5 of 32 women: 15.6%). Conclusion: Women with GD developed postpartum thyroid dysfunctions in 44.4% of cases. Women whose initial disease onset occurred within one year postpartum had higher recurrences of GD, and women who developed PPT had a history of higher gravidity compared to the euthyroid women postpartum. Therefore, if women with GD develop postpartum thyroid dysfunctions, the diagnosis should be made, and a treatment modality planned, following careful considerations of the patients' past obstetric history, changes in clinical manifestations and the TBII values
