Purpose: There are researches indicating that in human skin, the cannabinoid receptors exist, which take a part of endocannabinoid system. Nonetheless, the possible association of cannabinoid receptors with the pathogenesis of psoriasis has not yet been fully elucidated. Through this research, we tried to document the association between cannabinoid system, epidermal differentiation and psoriasis. Methods: Using human keratinocyte (KC), the expression of cannabinoid receptor-1 (CB1R) and -2 (CB2R) was analyzed according to the degree of differentiation. Also we examined for changes in differentiation marker of KC after application of CBR agonist. In addition, murine model applied with imiquimod to induce psoriasis symptoms was used to evaluate the effect of topical CBR agonist on inflammation and skin barrier function. Results: Compared to normal human skin, CBR expression was reduced in epidermis of psoriasis skin. Western blotting revealed the expression of both CB1R and CB2R in undifferentiated KC. The expression level of CB1R increased as the differentiation progressed in KC as with involucrin, K1 and K10. On the other hand, there was no change of CB2R in the process of differentiation. Furthermore, CB1R agonist partly increased expression of proteins associated with epidermal differentiation. Through this we could speculate that the CB system may be associated with proteins related to epidermal differentiation. Moreover, inflammation and barrier function in murine models with psoriasis symptoms were improved after 3 days-application of topical CB1R agonist. Conclusion: Cannabinoid receptors in human skin might be related to the differentiation of KC, and since its activation could improve skin barrier condition in murine psoriasis model, it can be expected to be a new therapeutic target for treatment of psoriasis.

In this paper, we study the signed- DLMS adaptive algorithm of wireless repeater for solving shadow region due to propagation between base station and mobile station. The the signed-DLMS algorithm reduces interference signals from multipath and solves the oscillation problem of repeater by estimation and cancellation. To efficiently reject interference signal. the signed-DLMS adaptive algorithm is applied. The computational complexities of the signed-DLMS are reduced verse standard LMS algorithm. Wireless ICS repeater based on signed-DLMS reduces the cost and is able to increase channel capacities.
Li-Fraumeni syndrome that presented as cutaneous melanoma

Li fraumeni syndrome is an autosomal dominant syndrome with mutations in the well-known tumor suppressor gene, the TP53 gene. Many studies have shown an association with a wide variety of tumors, but their association with melanoma is still controversial as the absolute number of melanoma cases is limited. A 16-year-old boy presented with asymmetric black to brown colored plaque of 1.2x0.9 cm on his right lateral thigh which had been 6 to 7 years ago. He had no significant medical history and had no surgical history. Punch biopsy was done on the lesion, and histopathological finding showed atypical melanocytic proliferation in epidermis with no dermal invasion. Melanoma in situ was diagnosed and wide excision with 1.0 cm safety margin was performed. Since his diagnosis was atypical for his young age, detailed questions about family history were made. His grandfather died of sarcoma and his father had a history of thyroid cancer. In addition, his 25-year-old sister was diagnosed with adrenocortical carcinoma with liver and lung metastasis and undergoing chemotherapy. Considering his unique family history and the onset of cancer at a young age, genetic tests were performed to exclude Li-fraumeni syndrome in his living family members. Mutation of the P53 gene was found in the genes of him, his sister and his father. We have confirmed that there is no cancer in other parts of the patient’s body and are under constant observation..
국소 진동자극이 편마비 뇌졸중 환자의 팔 뻗기 수행에 미치는 영향에 대한 운동학적 분석

목적 : 본 연구의 목적은 뇌졸중 환자를 대상으로 환측 상지에 진동자극을 적용했을 때, 팔 뻗기 수행 에서 나타나는 팔꿈치 움직임의 운동학적 변화를 관찰하기 위함이다. 연구방법 : 연구 설계는 단일집단 교차실험설계(one-group cross-over trial design)를 사용하였으 며, 10명의 만성 뇌졸중 환자를 대상으로 하였다. 대상자의 환측 위팔두갈래근(biceps brachii)에 국소 진동자극을 5분, 10분, 20분 동안 무작위로 70Hz로 적용한 후, 3차원 동작분석 시스템을 통해 팔 뻗기 수행의 운동학적 움직임을 분석하였다. 종속변수에는 팔꿈치 움직임에서 나타나는 최대 각 속도, 최대 각속도까지의 시간, 움직임 단위를 포함하였다. 결과 : 팔 뻗기를 수행함에 있어서 팔꿈치의 움직임은 20분 동안 국소 진동자극을 적용하였을 때 보다 빠르고 부드러워졌으며, 효율적으로 나타났다. 팔꿈치 움직임의 최대 각속도는 증가하였고 (p<0.05), 최대 각속도까지의 시간과 운동단위는 유의하게 감소하였다(p<0.05). 결론 : 국소 진동자극은 편마비 뇌졸중 환자가 팔 뻗기 움직임을 수행함에 있어 발생하는 운동학적 구 성요소를 향상시킬 수 있는 효과적인 방법일 될 수 있다. Objective : Localized vibration has been shown to have a positive effect on recovery of upper- limb motor function in patients with hemiparetic stroke, but there has been little research on kinematic analysis for qualitative changes in movement. This study investigated kinematic changes in elbow motion during reaching after localized vibration in persons with hemiparetic stroke. Methods : This study used a one-group, cross-over trial design. Ten chronic stroke patients randomly received localized vibrations on the affected biceps brachii for 5, 10, or 20 min, at 70 Hz. Kinematic analysis of reaching was measured using a 3-D motion analysis system. Variables included peak angular velocity, time to peak angular velocity, and movement units during elbow motion. Result : Affected side elbow motion during reaching was faster, smoother, and more efficient after 20 min localized vibration. Peak angular velocity increased (p<0.05), and time to peak angular velocity (p<0.05) and the movement unit were significantly decreased (p<0.05) during elbow motion for reaching. Conclusion : Localized vibration can improve kinematic components during reaching motion in persons with hemiparetic stroke.

Objective To analyze the changes in the clinical characteristics and perinatal outcomes of twin pregnancies delivered at a tertiary referral center in Korea during a 24-year period. Methods This was a retrospective cohort study of twin pregnancies delivered at 24-40 weeks of gestation, from 1995 to 2018. The subjects were divided into 4 groups according to the year of delivery: 1995-2000, 2001-2006, 2007-2012, and 2013-2018. The trends in the changes in the twin birth rate, maternal age, assisted reproductive technology (ART) pregnancy rate, chorionicity, obstetric complications, delivery outcomes, and neonatal outcomes over the periods were analyzed. Results A total of 2,133 twin pregnancies were included in the study. The twin birth rate increased from 16.7/1,000 in 1995-2000 to 42.2/1,000 in 2001-2006, 49.5/1,000 in 2007-2012, and 61.8/1,000 in 2013-2018. The maternal age and ART pregnancy and dichorionic twin rates increased, while the monochorionic twin rate decreased over the periods. The incidence of fetal congenital anomalies, cervical incompetence, gestational diabetes mellitus, preeclampsia, and placental abruption increased over the periods. The preterm birth (PTB) rate significantly decreased owing to the decreasing elective late-PTB rate; however, the early-PTB rate significantly increased. Conclusion This study found that twin pregnancies increased steadily over the last 24 years and that the increase was related to increased maternal age and ART pregnancy rate. The incidence of obstetric complications increased over the periods; however, the neonatal intensive care unit admission rate decreased, along with decreases in the elective late-PTB rate.

<P>Although proteasome inhibitors (PIs) are used as anticancer drugs to treat various cancers, their relative therapeutic efficacy on stem cells vs. bulk cancers remains unknown. Here, we show that stem cells derived from gliomas, GSCs, are up to 1,000-fold more sensitive to PIs (IC50, 27-70 nM) compared with their differentiated controls (IC50, 47 to >> 100 mu M). The stemness of GSCs correlates to increased ubiquitination, whose misregulation readily triggers apoptosis. PI-induced apoptosis of GSCs is independent of NF-jB but involves the phosphorylation of c-Jun N-terminal kinase as well as the transcriptional activation of endoplasmic reticulum (ER) stress-associated proapoptotic mediators. In contrast to the general notion that ER stress-associated apoptosis is signaled by prolonged unfolded protein response (UPR), GSC-selective apoptosis is instead counteracted by the UPR. ATF3 is a key mediator in GSC-selective apoptosis. Pharmaceutical uncoupling of the UPR from its downstream apoptosis sensitizes GSCs to PIs in vitro and during tumorigenesis in mice. Thus, a combinational treatment of a PI with an inhibitor of UPR-coupled apoptosis may enhance targeting of stem cells in gliomas.</P>
