        미중패권경쟁에서 아세안을 통한 한국의 경제개발 전략에 관한 연구

        미국과 중국의 무역 경쟁은 관련 동맹국에게도 영향을 미칠 정도로 치열하다. 반도체는 첨단산업의 쌀이라고 불릴 정도로 최첨단 기술에서 중요한 역할을 하며 자동차, 휴대폰, 디스플레이 등에 반드시 들어가기 때문에 산업에서도 반드시 안전한 공급망이 확보될 필요가 있다. 본 연구는 미중패권경쟁 속에서 한국의 국익을 높이기 위하여 아세안을 활용한 경제외교를 강화하는 것이 필요하다는 것에 주목하여 연구를 하였다. 아세안 국가들은 헤징(Hedging)전략을 통하여 실리를 추구한다. 우크라이나 전쟁이 계속되고 이스라엘과 하마스가 이끄는 팔레스타인 급진 무장 단체가 전쟁을 치르면서 국제적으로 안보 위기가 나타나고 아세안 국가들은 전략적으로 협력하는 것이 필요하다는 데 의견을 모으고 있다. 미국은 아시아로의 회귀((Pivot to Asia)를 강조하면서 관계를 개선하려는 노력을 꾸준하게 하였다. 중국은 중국은 동남아시아 지역에서 중국의 영향력을 확보하는 것이 필요하다고 보고 아세안에 적극 참여하고 있다. 한국이 전략적으로 이러한 어려운 상황을 타개하기 위해서는 아세안 국가들과의 협력을 강화하고 경제외교를 추진하여 실리적인 이익을 확보하여야 한다. 한국은 지리적으로 아세안 국가들과 가까운 편에 속하기 때문에 수출 다변화와 수입 다변화의 측면에서도 아세안을 통하여 협력하는 것이 필요하다. 한국은 아세안과의 경제외교를 강화하여 향후 아세안 국가들이 빠르게 경제 성장을 하였을 때 상생하는 관계가 되도록 전략적인 노력을 하여야 한다. 한국은 베트남, 라오스, 태국, 캄보디아, 미얀마, 필리핀, 브루나이, 말레이시아, 싱가포르, 인도네시아와 협력하고 경제외교를 강화하여 한국의 국익을 확보하여야 한다. The trade competition between the United States and China is fierce enough to affect relevant allies. Semiconductors play an important role in cutting-edge technology, so much so that they are called the rice of the high-tech industry. Since they are essential for automobiles, mobile phones, displays, etc., there is a need to ensure a safe supply chain in the industry. This study focused on the need to strengthen economic diplomacy using ASEAN to enhance Korea's national interests amid the US-China hegemony competition. ASEAN countries pursue practical benefits through hedging strategies. As the war in Ukraine continues and Israel and the Palestinian radical armed group led by Hamas are at war, an international security crisis is emerging, and ASEAN countries are converging on the need for strategic cooperation. The United States has consistently made efforts to improve relations, emphasizing a pivot to Asia. China believes that it is necessary to secure Chinese influence in the Southeast Asian region and is actively participating in ASEAN. Korea In order to strategically overcome this difficult situation, we must strengthen cooperation with ASEAN countries and promote economic diplomacy to secure practical benefits. Korea should strengthen economic diplomacy with ASEAN and make strategic efforts to create a win-win relationship when ASEAN countries achieve rapid economic growth in the future. Korea should secure its national interests by cooperating with Vietnam, Laos, Thailand, Cambodia, Myanmar, the Philippines, Brunei, Malaysia, Singapore, and Indonesia and strengthening economic diplomacy.

        Catamenial hemoptysis accompanied by subcutaneous endometriosis treated with combination therapy

        Extra pelvic endometriosis is considered to be rare. This paper reports a case of catamenial hemoptysis accompanied by subcutaneous endometriosis in 26-year-old woman. A computed tomography scan of the chest revealed a focal ground-glass opacity lesion in the posterior segment of the right upper lobe. Histopathology confirmed the diagnosis of endometriosis of right lung and concurrent subcutaneous endometriosis. She was treated with surgical resection of the endometriosis lesions on two different sites and perioperative gonadotropin-releasing hormone agonist therapy. The 6-month follow-up after combination treatment showed no recurrence. Though long-term follow-up result is needed, aggressive treatment using combination treatment (surgery and perioperative medication) should be considered for symptomatic extra pelvic endometriosis.

        Crepidiastrum denticulatum Extract Protects the Liver Against Chronic Alcohol-Induced Damage and Fat Accumulation in Rats

        Alcohol is a severe hepatotoxicant that causes liver abnormalities such as steatosis, cirrhosis, and hepato-carcinoma. Crepidiastrum denticulatum (CD) is a well-known, traditionally consumed vegetable in Korea, which was recently reported to have bioactive compounds with detoxification and antioxidant properties. In this study, we report the hepato-protective effect of CD extract against chronic alcohol-induced liver damage in vivo. The rats that were given CD extract exhibited decreased alanine aminotransferase, aspartate aminotransferase, and ${\gamma}$-glutamyl transpeptidase activities, which are liver damage markers that are typically elevated by alcohol consumption. The results were confirmed by histopathology with hematoxylin and eosin staining. Chronic alcohol consumption induced the formation of alcoholic fatty liver. However, treatment with CD extract dramatically decreased the hepatic lipid droplets. Treatment with CD extract also restored the antioxidative capacity and lipid peroxidation of the liver that had been changed by alcohol consumption. Furthermore, treatment with CD extract normalized the activities of the antioxidative enzymes superoxide dismutase, catalase, glutathione reductase, and glutathione peroxidase, which had been decreased by alcohol consumption. The results indicate that CD extract has protective effects against chronic alcohol hepatotoxicity in rats by increasing the liver's antioxidant capacity, and has potential as a dietary supplement intervention for patients with alcohol-induced liver damage.

        Dependence Potential of Tramadol: Behavioral Pharmacology in Rodents

        Tramadol is an opioid analgesic agent that has been the subject of a series of case reports suggesting potential for misuse or abuse. However, it is not a controlled substance and is not generally considered addictive in Korea. In this study, we examined the dependence potential and abuse liability of tramadol as well as its effect on the dopaminergic and serotonergic systems in rodents. In animal behavioral tests, tramadol did not show any positive effects on the experimental animals in climbing, jumping, and head twitch tests. However, in the conditioned place preference and self-administration tests, the experimental animals showed significant positive responses. Taken together, tramadol affected the neurological systems related to abuse liability and has the potential to lead psychological dependence.

        Inhibitory effect of fermented Arctium lappa fruit extract on the IgE-mediated allergic response in RBL-2H3 cells

        <P>Arctium lappa fruit has been used in traditional medicine, and it is known to exert beneficial effects, such as antioxidant, anti-inflammatory and anticancer effects. However, the effects of the Arctium lappa fruit on the allergic response remain unknown. In this study, we evaluated the anti-allergic effects of Arctium lappa fruit extract (AFE) and its fermented form (F-AFE) using immunoglobulin E (IgE)-activated RBL-2H3 cells. To investigate the anti-allergic effects of AFE or F-AFE, we examined the release of beta-hexosaminidase, a key biomarker of degranulation during an allergic reaction, and the production of pro-inflammatory mediators, such as tumor necrosis factor-alpha (TNF-alpha) and prostaglandin E-2 (PGE(2)) in the cells treated with or without the above-mentioned extracts. AFE weakly inhibited the release of beta-hexosaminidase, whereas F-AFE significantly suppressed the release of beta-hexosaminidase in a dose-dependent manner. Consistently, F-AFE suppressed the production of TNF-a and PGE2 in a dose-dependent manner. F-AFE exerted an inhibitory effect on the production of beta-hexosaminidase, TNF-a and PGE2 with an IC50 value of 30.73, 46.96 and 36.27 mu g/ml, respectively. Furthermore, F-AFE inhibited the phosphorylation of Lyn, Fyn and Syk, which are involved in the FceRI signaling pathway, that of phosphoinositide phospholipase C (PLC)gamma 1/2 and protein kinase C (PKC)delta, which are associated with the degranulation process, as well as that of extracellular signal-regulated kinase 1/2 (ERK1/2), c-Jun N-terminal kinase 1/2 (JNK), p38 and Akt, which are associated with cytokine expression. In the late phase, F-AFE partially suppressed the phosphorylation of cytosolic phospholipase A(2) (cPLA(2)), but not the expression of cyclooxygenase (COX)-2. To compare and identify the major components of the two extracts, we used high-performance liquid chromatography. The levels of arctigenin, one of the major compounds, were elevated 6-fold in F-AFE compared with AFE, whereas the levels of arctiin, an arctigenin glycoside, were decreased in F-AFE by approximately 57.40%. These results suggest that arctigenin plays an important role in the anti-allergic effects of F-AFE. Taken together, F-AFE containing anti-allergic phytochemicals, including arctigenin, inhibited the activation of the FceRI receptor induced by the antigen-IgE complex. Such effects may provide further information for the development of a phytomedicine for allergic diseases.</P>

        Perioperative administration of propranolol to women undergoing ovarian cancer surgery: A pilot study

        Objective This study was done to evaluate whether perioperative propranolol (ß-blocker) in ovarian cancer patients undergoing debulking surgery reduced perioperative tumor growth induced by surgical stress. Methods This was a prospective randomized single institution analysis. The primary objective was to compare the changes in CA 125 level (changes between preoperation day 2 and postoperative day 7). As a study arm, patients received a low dose of propranolol 40 mg/day (4×10 mg) starting two days before surgery and 40 mg twice daily for three days following surgery. Results Twenty-two patients were enrolled and 16 were evaluable for efficacy. The drug was well tolerated. The mean decrease of CA 125 during the seven perioperative days was 83.1±8.9% in the propranolol group and 72.4±14.7% in the placebo group. The difference was statistically significant (P=0.044). The change of C-reactive protein, cortisol, and anxiety score (State-Trait Anxiety Inventory-X1) were not different between the two groups. Conclusion This preliminary result is the first to directly test the role of perioperative propranolol on tumor growth. Even with the small sample size and short term use of the drug, perioperative propranolol was effective in reducing tumor burden (as measured by CA 125) suggesting its potential benefits in decreasing perioperative tumor growth.

        The Cytotoxic Saponin from Heat-Processed <i>Achyranthes fauriei</i> Roots

        <P>An extract of heat-processed <I>Achyranthes fauriei</I> roots showed more potent cytotoxic activity against human hepatoma SK-Hep-1 cells than that of the crude plant. Employing a bioassay-linked HPLC-ELSD (high performance liquid chromatography-evaporative light scattering detector) method, followed by silica gel column chromatography, a cytotoxic triterpenoid saponin, chikusetsusaponin IVa was isolated. It is contributive to the increased cytotoxic activity of the heat-processed. The amount of chikusetsusaponin IVa from the roots of <I>Achyranthes fauriei</I> was significantly increased after heat-processing.</P>

      • Effect of Proton Pump Inhibitor on Drug-resistant Epithelial Ovarian Carcinoma

        Acidification of the extracellular space in tumors plays a key role contributing to tumor progression, chemoresistance, and metastatic behavior. A key mechanism controlling cellular pH is mediated by vacuolar-type H+ ATPases, ATP-dependent proton transporters that are normally expressed in intracellular compartments like lysosomes, endosomes, and secretory vesicles and on plasma membrane as well. The up-regulated V-ATPase activity and increased membrane V-ATPase expression detected in some human tumors. And the acidic pH of the tumor microenvironment may impair the uptake of weakly basic chemotherapeutic drugs. Proton pump inhibitors (PPIs), inhibitor of V-ATPase, could inhibit the acidification of extracellular space and increase the sensitivity of tumor cells to anti-cancer agents. We evaluated the sensitivity to taxane after omeprazole and V-ATPase siRNA pretreatment in epithelial ovarian cancer cell lines. And to test the effect of omeprazole on tumor development and growth in ovarian cancer model, we performed in vivo experiments in an animal model system by BALB/c-nude mice. At least 10 mice were used for each treatment group. Each mouse was injected intraperitoneally with HeyA8, HeyA8-MDR and ES2 cells (1x106/mouse). Taxan was administered by i.p. injection, 24 hours after omeprazole i.p. injection. To confirm the expression of MMP-9, VEGF and active caspase-3 after omeprazole treatment, ELISA assay was performed. Omeprazole pretreatment enhanced chemosensitivity of HeyA8-MDR cells by nearly 20% (p<0.01). Similar results were obtained with other tumor cell lines, such as SKOV3TR (30%, p<0.01) and ES2 (20%, p<0.01). V-ATPase siRNA treatment also enhanced chemosensitivity of HeyA8-MDR (20%, p<0.05) and SKOV3TR (20%, p<0.01). In vivo animal model, Omeprazole pretreatment decreased mouse tumor weight than mouse treated with taxan alone (HeyA8-MDR, p<0.01, ES2, p<0.05). After omeprazole treatment, the expression of MMP-9 decreased by nearly 50% in taxane-resistant cell (HeyA8-MDR, p<0.05). VEGF secretion decreased by nearly 30~40% and apoptosis increased by omeprazole pretreatment in HeyA8, HeyA8-MDR and ES2 cell. Vacuolar-type H+ ATPase regulates the pH gradient of within tumor microenvironment and chemosensitivity to anti-cancer agents. Because the low extracellular pH may induce secretion and activation of matrix metalloproteinases (MMP) and VEGF, cancer cells with omeprazole treatment inhibit invasion and angiogenesis in vitro. Our results suggest that V-ATPase as a key target for new strategies in chemo-resistant cancer treatment.

        Original Article : The usefulness of non-invasive liver stiffness measurements in predicting clinically significant portal hypertension in cirrhotic patients: Korean data

        Background/Aims: Liver stiffness measurement (LSM) has been proposed as a non-invasive method for estimating the severity of fibrosis and the complications of cirrhosis. Measurement of the hepatic venous pressure gradient (HVPG) is the gold standard for assessing the presence of portal hypertension, but its invasiveness limits its clinical application. In this study we evaluated the relationship between LSM and HVPG, and the predictive value of LSM for clinically significant portal hypertension (CSPH) and severe portal hypertension in cirrhosis. Methods: LSM was performed with transient elastography in 59 consecutive cirrhotic patients who underwent hemodynamic HVPG investigations. CSPH and severe portal hypertension were defined as HVPG ≥10 and ≥12 mmHg, respectively. Linear regression analysis was performed to evaluate the relationship between LSM and HVPG. Diagnostic values were analyzed based on receiver operating characteristic (ROC) curves. Results: A strong positive correlation between LSM and HVPG was observed in the overall population (r2=0.496, P<0.0001). The area under the ROC curve (AUROC) for the prediction of CSPH (HVPG ≥10 mmHg) was 0.851, and the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for an LSM cutoff value of 21.95 kPa were 82.5%, 73.7%, 86.8%, and 66.7%, respectively. The AUROC at prediction of severe portal hypertension (HVPG ≥12 mmHg) was 0.877, and the sensitivity, specificity, PPV, and NPV at LSM cutoff value of 24.25 kPa were 82.9%, 70.8%, 80.6%, and 73.9%, respectively. Conclusions: LSM exhibited a significant correlation with HVPG in patients with cirrhosis. LSM could be a non-invasive method for predicting CSPH and severe portal hypertension in Korean patients with liver cirrhosis.
