      • 미세알부민뇨를 동반한 인슐린 비의존성 당뇨병 환자에서 혈장 Endothelin의 변화

        목적 : 혈관내피세포에서 생성되는 endothelin(ET)는 강력한 혈관수축작용을 가진 펩타이드로 당뇨병 환자에서 혈관내피세포의 손상여부에 따라 혈중 농도가 증가할 수 있으며 혈관합병증과 관련이 있다는 보고가 있다. 그러나. 혈중 ET치의 상승과 당뇨병성 합병증의 상관성에 대해서는 논란이 많다. 이에 저자들은 인슐린 비의존성 당뇨병 환자에서 미세알부민뇨의 동반유무에 따른 혈중 ET치의 변화 및 혈장 ET치와 요중 알부민 배설률과의 상관성을 검토하였다. 대상 및 방법 : 인슐린 비의존성 당뇨병 환자중 미세알부민뇨를 보인 22명과 정상알부민뇨를 보인 43명을 대상으로 하였으며 혈장 ET를 측정하여 요중 알부민 배설률, 당뇨병성 망막증 및 당뇨병 관련인자들과의 관계를 비교하였다. 결과 : 당뇨병성 망막증이 합병된 군에서 망막증이 합병되지 않은 군에 비하여 혈장 ET치가 유의하게 높았던 반면 (각각 1.9±0.66, 1.5±0.76pg/㎖, p<0.05) 미세알부민뇨를 보인 군과 정상알부민뇨를 보인 군사이에 혈장 ET치는 통계학적으로 유의한 차이가 없었다(각각 1.9±1.14, 1.7±0.69pg/㎖). 또한 요중 알부민 배설률과 혈장 ET치 사이에 유의한 상관관계는 없었으며 미세알부민뇨를 동반한 인슐린 비의존성 당뇨병 환자군에서 혈장 ET치는 당뇨병의 관련인자들과도 유의한 상관관계가 없었다. 결론 : 인슐린 비의존 당뇨병 환자에서 혈장 ET치가 초기 당뇨병성 신증의 합병여부나 미세알부민뇨의 정도를 반영하기는 어려울 것임을 시사한다. Background : Endothelin(ET), a potent vasoconstritor peptide released from vascular endothelial cells, is known to be increased in diabetic patients with vascular damage and related with diabetic angiopathy. However, whether plasma ET levels may be related to diabetic complications is controversial. We studied the change of ET levels in non-insulin dependent diabetics with microalbuminuria and relation between ET and urinary albumin excretion rate. Methods : Plasma ET levels of 22 non-insulin dependent diabetics with microalbuminuria and 43 patients with normoalbuminuria were measured and relations of plasma ET to diabetic complications such as urinary albumin excretion rate, retinopathy and other associated factors were studied. Results : Plasma ET levels were significantly higher in patients with diabetic retinopathy than those without retinopathy(1.9 ± 0.66 vs 1.5 ± 0.76 pg/㎖. p<0.05). The difference, however, between diabetics with and without microalbuminuria was not significant(1.9 ± 1.14 vs 1.7 ± 0.69 pg/㎖). There were no correlations of plasma ET to urinary albumin excretion rate and to the other associated factors in non-insulin dependent diabetics. Conclusion : These results suggest that plasma ET levels may not seem to act as a predictor of diabetic nephropathy or microalbuminuria in non-insulin dependent diabetics.

      • Necessity of Protective Coordination Systems According to Battery Capacity

        As the capacity of lithium-based batteries rapidly is increasing, the number of applications using them is continuously increasing. However, this leads to the risk of fire and explosion, and thus, the number of accidents is increasing. To analyze the magnitude of short circuit currents according to the battery capacity and external circuit impedance and the necessity of protective coordination, this study conducted experiments using cylindrical batteries with capacities of 800, 2200, 3000, and 5000 mAh and cables with an impedance of 0.1Ω/m. Results indicated that as the battery capacity increased, the short circuit current increased and circuit-opening time decreased. However, as the circuit impedance increased, the short circuit current decreased and circuit-opening time increased. Based on these results, we reviewed protective systems applied to batteries and analyzed their problems. For protective systems designed considering the maximum short circuit current, if the state of charge is low or the impedance is high at the point of short circuit, the protective system does not operate when short circuit occurs, thus failing to block the circuit and potentially leading to fire. Therefore, while designing protective systems, it is appropriate to use multiple circuit breakers and fuses, considering the short circuit current according to the battery capacity and circuit impedance to enable protective coordination between devices.

      • 2016년도 전북지역 농산물의 중금속 오염실태조사

        In domestic agricultural products, heavy metals are one of the most important harmful substances together with residual pesticides. The purpose of this study was to investigate the contamination status of 17 agricultural products produced, distributed and sold in Jeonbuk Province in 2016 by investigating the contents of four heavy metals (Pb, Cd, As, Inogarnic As). Agricultural products were analyzed for 248 crops and 17 vegetables. For agricultur80al products, 17 items of cereals and vegetables were analyzed by 248 cases. Microwave method was used for preprocessing and ICP-MS was used. Heavy metals were detected in all samples. In the case of plumbumb(Pb), the mean detectable value was 0.032 for dried bracken, 0.094 for spinach, 0.050 for chestnut sweet potato, 0.023 for soybeans, 0.002 for green chilli, and 0.394 mg/kg for brown rice. Cadmium(Cd) was found to be 0.035 for dried bracken, 0.058 for spinach, 0.041 for potato, 0.024 for red pepper, 0.015 for chinese chive, 0.057 for nonglutinous rice, 0.022 in medicinal plant, 0.077 in brown rice and 0.071 mg/kg in chestnut sweet potato. As for arsenic, nonglutinous rice was 0.041, glutinous rice 0.047, brown rice 0.069, brown rice glutinous rice 0.067 mg/kg. In the case of inorganic arsenic was of 0.033 for nonglutinous rice, 0.070 for brown rice, 0.067 for brown glutinous rice, 0.039 for glutinous rice and 0.090 mg/kg for germinated brown rice showed the results. The samples that exceeded the residual limit of heavy metals (Cd 0.1, Pb 0.2 mg/kg) were 3 brown rice, 0.674 cadmium, 0.337 lead and 0.394 mg/kg lead. All 248 cases were analyzed for harmful heavy metals. Therefore, continuous safety management is required.

      • Prevalence of Depression and Its Associated Factors in Bronchiectasis: Findings from KMBARC Registry

        Background With the emergence of bronchiectasis as a common respiratory disease, epidemiological data have accumulated. However, the prevalence and impact of psychological comorbidities were not sufficiently evaluated. The present study examined the prevalence of depression and its associated factors in patients with bronchiectasis. Methods This study involved a multicenter cohort of bronchiectasis patients recruited from 33 pulmonary specialist hospitals. The baseline characteristics and bronchiectasis-related factors at enrollment were analyzed. Depressive symptoms were assessed using the Patient Health Questionnaire (PHQ-9). Results Of the 810 patients enrolled in the study, 168 (20.7%) patients had relevant depression (PHQ-9 score ≥ 10), and only 20 (11.9%) patients had a diagnostic history of depression. Significant differences were noted in the depressive symptoms with disease severity, which was assessed using the Bronchiectasis Severity Index and E-FACED (all p <0.001). Depressive symptoms inversely correlated with quality-of-life (r = - 0.704, p <0.001) and positively correlated with fatigue severity score (r = 0.712, p <0.001). Multivariate analysis showed that depression was significantly associated with the modified Medical Research Council dyspnea scale ≥ 2 (OR 2.960, 95% CI: 1.907-4.588, p = <0.001) and high number of exacerbations (≥ 3) in the previous year (OR 1.596, 95% CI: 1.012-2.482, p = 0.041). Conclusions Depression is common, but its association with bronchiectasis was underrecognized. It negatively affected quality-of-life and presented with fatigue symptoms. Among the bronchiectasis-related factors, dyspnea and exacerbation were closely associated with depression. Therefore, active screening for depression is necessary to optimize the treatment of bronchiectasis.

        Nesidioblastosis is characterized by a diffuse proliferation of islet cells arising from pancreatic ducts and is the most common cause of hyperinsulinemic hypoglycemia in newborns and infantile. It is exceedingly rare in adults and no concensus regarding its diagnosis and management is available. We herein describe an elderly man with fasting hypoglycemia, inappropriate insulin hypersecre- tion. And pathologic examination of his pancreas revealed the characteristic finding of nesidioblastosis confirmed by immunohistochemical stain. (J Kor Soc Endocrinol 12:485-490, 1997)

        PET/CT SUV Ratios in an Anthropomorphic Torso Phantom

        표준섭취계수(SUV) PET과 영상 재구성 방법에 따라 크게 달라진다. 삼성 서울병원 핵의학과에 설치된 GE사의 Discovery MIDR와 Discovery Ste의 영상 재구성 알고리즘을 이용하여 의인화몸통팬텀 내의 심장, 간과 배경영역에서 표준섭취계수를 측정하여 실제 SUV와 비교하였다. 영상 재구성 알고리즘은 MIDR에서는 VPFX-S (TOF+PSF), QCFX-S-350 (Q.Clear+TOF+PSF), QCFX-S-50와 VPHD-S (OSEM+PSF), Ste에서는 VUE Point (OSEM)와 FORE-FBP를 사용하였다. 방사선사의 방사선 피폭을 감소시키기 위하여 소량의 ¹⁸F-FDG 선원을 물과 혼합하였다: 52.5 ml 심장에는 2.28 MBq, 1,290 ml 간에는 20.3 MBq와 9,590 ml 배경영역에는 45.7 MBq을 주입하였다. 심장에서의 표준섭취계수는 MIDR의 VPFX-S, QCFX-S-350, QCFX-S-50, VPHD-S와 Ste의 VUE Point (OSEM)와 FOR-FBP 알고리즘에서 각각 27.1, 28.0, 27.1, 26.5, 8.0와 7.4 이었으며, 기대치는 5.9이었다. 배경역역에서는 4.2, 4.1, 4.2, 4.1, 1.1와 1.2 이었으며, 기대치는 0.8이었다. 각 영역에서 표준섭취계수는 PET과 알고리즘에 따라 크게 차이가 있었지만, 심장과 배경영역의 SUV 비율은 비교적 일정하여 6개 영상 재구성 알고리즘에 대하여 6.5, 6.8, 6.5, 6.5, 7.3와 6.2 이었으며 기대치는 7.8이었다. 심장에서의 평균 신호 대 잡음비(SNR)는 각각 6개 알고리즘에 대하여 8.3, 12.8, 8.3, 8.4, 17.2와 16.6이었다. 결론적으로 PET 성능 평가는 각 영역에서의 절댓값 보다는 두 영역 사이의 SUV 비율이 적절하며, 비교적 소량의 방사능으로도 확인할 수 있는 가능성이 있다. The standard uptake values (SUVs) strongly depend on positron emission tomographs (PETs) and image reconstruction methods. Various image reconstruction algorithms in GE Discovery MIDR (DMIDR) and Discovery Ste (DSte) installed at Department of Nuclear Medicine, Seoul Samsung Medical Center were applied to measure the SUVs in an anthropomorphic torso phantom. The measured SUVs in the heart, liver, and background were compared to the actual SUVs. Applied image reconstruction algorithms were VPFX-S (TOF+PSF), QCFX-S-350 (Q.Clear+TOF+PSF), QCFX-S-50, VPHD-S (OSEM+PSF) for DMIDR, and VUE Point (OSEM) and FORE-FBP for DSte. To reduce the radiation exposure to radiation technologists, only the small amount of radiation source ¹⁸F-FDG was mixed with the distilled water: 2.28 MBq in the 52.5 ml heart, 20.3 MBq in the 1,290 ml liver and 45.7 MBq for the 9,590 ml in the background region. SUV values in the heart with the algorithms of VPFX-S, QCFX-S-350, QCFX-S-50, VPHD-S, VUE Point, and FOR-FBP were 27.1, 28.0, 27.1, 26.5, 8.0, and 7.4 with the expected SUV of 5.9, and in the background 4.2, 4.1, 4.2, 4.1, 1.1, and 1.2 with the expected SUV of 0.8, respectively. Although the SUVs in each region were different for the six reconstruction algorithms in two PET/CTs, the SUV ratios between heart and background were found to be relatively consistent; 6.5, 6.8, 6.5, 6.5, 7.3, and 6.2 for the six reconstruction algorithms with the expected ratio of 7.8, respectively. Mean SNRs (Signal to Noise Ratios) in the heart were 8.3, 12.8, 8.3, 8.4, 17.2, and 16.6, respectively. In conclusion, the performance of PETs may be checked by using with the SUV ratios between two regions and a relatively small amount of radioactivity.

        3D 프린팅을 위한 수리미 제조 시 다당류 알긴산 첨가에 따른 물성 변화

        When manufacturing Alaska pollack Gadus chalcogrammus surimi for 3D printing, it examined the characteristics of physical properties and sensuality by adding polysaccharides alginic acid [0, 1, 2, 3% (w/w)]. As a result of the color value of surimi by adding alginic acid, it showed that the lightness of heated surimi containing 2% (w/w) alginic acid was the highest and ΔE value was the lowest. In the changes in physical properties, the heated surimi of 3% (w/w) alginic acid content showed the lowest values of hardness, springiness, cohesiveness, gumminess, chewiness and resilience, but the highest adhesiveness. In the case of fried surimi, its hardness, cohesiveness, gumminess, chewiness and resilience were the lowest at the content of 3% (w/w) alginic acid. After 7 days of cold storage, the hardness, gumminess, and chewiness of heated surimi with an alginic acid content of 2% (w/w) were significantly higher, and fried surimi was also the same. In the results of the sensory evaluation, there were significant differences according to the alginic acid content in hardness and fishy smell, and as the alginic acid content increased, it felt more fishy smell, resulting in poor preference.

      • 실시간 연관규칙 탐사를 위한 능동적 후보항목 관리 모델

        미디어의 발달과 생활 패턴의 변화를 토대로 새롭게 나타나고 있는 다양한 판매 패턴들을 분석하는데 있어 단일한 분석 방법을 적용하는 것은 효과적이지 못하다. 특히 신선 식품이나 기념일 주변에서 집중적인 매출이 발생하는 품목들은 제한된 시간 내에 판매를 최대로 해야 하는 시간적 제약을 갖는다. 그러나 기존의 연관규칙 탐사 기법은 대규모 거래 데이터베이스로부터 반복적 스캔 연산을 통해 연관규칙 탐사를 수행하기 때문에 제한된 시간안에서 빈번히 필요로 하는 패턴을 분석하기에는 비효율적이기 때문이다. 따라서 이 논문에서는 시간 제약을 갖는 특수한 판매 패턴에 대한 실시간 연관규칙 탐사가 가능하도록 하기 위해 트리거와 저장 프로시져를 이용한 점진적 후보항목 관리 모델을 제안한다. 아울러 이 논문에서는 제안 모델의 구현 및 실험을 통해 그 성능 특성의 분석도 수행한다. 특히 이 논문에서 제안하는 방법은 이중 해쉬 기법을 이용함으로써 연산의 성능을 향상시킨다. Considering the rapid process of media's breakthrough and diverse patterns of consumptions's analysis, a uniform analysis might be much rooms to be desired for interpretation of new phenomena. In special, the products happening intensive sails on around an anniversary or fresh food have the restricted marketing hours. Moreover, traditional association rule discovery algorithms might not be appropriate for analysis of sales pattern given in a specific time because existing approaches require iterative scan operation to find association rule in large scale transaction databases. in this paper, we propose an incremental candidate set management model based on twin-hashing technique to find association rule in special sales pattern using database trigger and stored procedure. We also prove performance of the proposed model through implementation and experiment.
